Assessment of postoperative pain after single-visit root canal treatment using rotary and reciprocating file systems: an in vivo study

  • Background: Various instrument kinematics used in single-visit endodontics influence the occurrence of pain after endodontic therapy. This study aimed to evaluate the occurrence of pain after mechanical instrumentation with Hyflex EDM (HEDM) and WaveOne Gold (WOG) during single-visit endodontic therapy. Methods: Sixty patients diagnosed with asymptomatic irreversible pulpitis and normal apical tissues in mandibular premolar teeth were included in the study for single-visit root canal therapy. The patients were divided into two groups (n = 30) according to the rotary instrument used during root canal preparation (group A [HEDM] and group B [WOG]). Pain was evaluated after endodontic therapy at 8, 24, and 48 h intervals using the visual analog scale (VAS). Data obtained were analyzed using the chi-square test, independent t-test, MannWhitney U test, and Wilcoxon matched-pairs test. Results: Statistically significant differences were observed between the two groups (P < 0.001) at 8, 24, and 48 h, with WOG exhibiting less pain than HEDM files. Conclusion: Postoperative pain was lower in the WOG file system than in the HEDM file system after single-visit root canal therapy at 8, 24, and 48 h.

Intraoperative discomfort associated with the use of a rotary or reciprocating system: a prospective randomized clinical trial

  • Objectives: The aim of this randomized, controlled, prospective clinical study was to evaluate patients' intraoperative discomfort during root canal preparations in which either multi-file rotary (Mtwo) or single-file reciprocating (Reciproc) systems were used. Materials and Methods: Fifty-five adult patients, aged between 25 and 69 years old, with irreversible pulpitis or pulp necrosis participated in this study. Either the mesiobuccal or the distobuccal canals for maxillary molars and either the mesiobuccal or the mesiolingual canals for mandibular molars were randomly chosen to be instrumented with Mtwo multi-file rotary or Reciproc single-file reciprocating systems. Immediately after each canal instrumentation under anesthesia, patient discomfort was assessed using a 1 - 10 visual analog scale (VAS), ranging from 'least possible discomfort' (1) to 'greatest possible discomfort' (10). The Wilcoxon signed-rank test was used to determine significant differences at p < 0.05. Results: Little intraoperative discomfort was found in all cases. No statistically significant differences in intraoperative discomfort between the 2 systems were found (p = 0.660). Conclusions: Root canal preparation with multi-file rotary or single-file reciprocating systems had similar and minimal effects on patients' intraoperative discomfort.

근관세정 방법에 따른 수산화칼슘 제재의 제거 효율 비교 (A COMPARISON OF THE IRRIGATION SYSTEMS IN CALCIUM HYDROXIDE REMOVAL)

  • 이 실험의 목적은 근관 내에 충전된 수산화칼슘의 제거 시 치근단 1/3에서 기존의 근관세정법과 $EndoActivator^{(R)}$, $EndoVac^{(R)}$ system의 세정 효율을 비교하고, 근관 세정 중 치근단공 개방 확인의 영향에 대해 평가하고자 함이다. 60개의 단근치를 사용하였고 ISO #35까지 근관성형 후 수산화칼슘을 충전하였다. 근관 세정법과 치근단공 개방 확인 유무에 따라 6개 실험군으로 나누어 세정하였다. 실험 치아를 양분하여 치근단 3 mm에서 근관 면적에 대한 잔존 수산화칼슘 면적의 백분율을 측정하였다. 실험결과 $EndoActivator^{(R)}$, $EndoVac^{(R)}$군이 기존의 근관 세정법 군에 비해 유의하게 높은 제거효율을 나타냈으며 (p<0.05), 치근단공 개방 확인 유무에 따른 유의한 차이는 나타나지 않았다. 이 실험 결과에 따르면 기존의 근관세정법으로 효과적인 세정이 불가능했던 근관의 치근단 1/3부위에서 $EndoActivator^{(R)}$, $EndoVac^{(R)}$ system과 같은 근관 세정법이 효과적인 대안이 될 수 있다는 결론을 얻었다.

  • 서론: 근관 치료의 성공을 위해 근관계 내에 남아있는 조직 잔사와 박테리아를 제거하는 것은 매우 중요하다. 그러나 충분한 근관 형성과 근관 세척을 하더라도 근관의 복잡한 형태로 인하여 근관 내 박테리아의 biofilm이나 괴사된 치수 조직이 남아 있게 된다. 근래 보다 효과적으로 근관 세척을 하기 위한 여러 가지 방법과 근관 세척을 위한 기구들이 개발되었다. 본 종설에서는 근관 치료 영역에서 사용되는 근관 세척 방법과 기구에 대해 고찰하고자 한다. 본론: 아래와 같은 기구와 방법이 논의된다. - syringe-needle irrigation, manual dynamic irrigation, brushes - sonic and ultrasonic irrigation, passive ultrasonic irrigation, rotary brush, RinsEndo, EndoVac, Laser 결론: 최근 근관 세척을 위한 기구와 세정 방법이 개발되었으나, syringe와 needle을 이용한 근관 세척법과 ultrasonic을 이용한 근관 세척법을 제외하고는 새로 개발된 많은 기구들의 임상적 치료 결과의 향상에 대한 근거 중심 연구가 아직은 부족하며, 어떠한 기구나 장치도 근관 내 잔사를 완벽하게 제거할 수는 없다. 효과적인 근관 세척을 위해서는 적절한 근관 세척제의 선택과 함께, 근관 세척제를 근관장까지 충분한 양이 도달할 수 있도록 적절한 근관 세척 기구와 방법을 선택하여야 한다.

Impact of root canal curvature and instrument type on the amount of extruded debris during retreatment

  • Objectives: The aim of the current study was to assess whether the amount of extruded debris differs for straight and severely curved root canals during retreatment using H-files, R-Endo, Reciproc and ProTaper Universal Retreatment (PTU-R) files. Additionally, the area of residual filling material was evaluated. Materials and Methods: Severely curved (n = 104) and straight (n = 104) root canals of maxillary molar teeth were prepared with WaveOne Primary file and obturated with gutta-percha and AH Plus sealer. Root canal filling materials were removed with one of the preparation techniques: group 1: H-file; group 2: R-Endo; group 3: Reciproc; group 4: PTU-R (n = 26). The amount of extruded material and the area of the residual filling material was measured. The data were analyzed with 2-way analysis of variance (ANOVA) and 1-way ANOVA at the 0.05 significance level. Results: Except for Reciproc group (p > 0.05), PTU-R, R-Endo, and H-file systems extruded significantly more debris in severely curved canals (p < 0.05). Each file system caused more residual filling material in severely curved canals than in straight ones (p < 0.05). Conclusions: All instruments used in this study caused apical debris extrusion. Root canal curvature had an effect on extruded debris, except for Reciproc system. Clinicians should be aware that the difficult morphology of the severely curved root canals is a factor increasing the amount of extruded debris during the retreatment procedure.

Multivariate analysis of the cleaning efficacy of different final irrigation techniques in the canal and isthmus of mandibular posterior teeth

  • Objectives: The aim of this study was to compare the cleaning efficacy of different final irrigation regimens in canal and isthmus of mandibular molars, and to evaluate the influence of related variables on cleaning efficacy of the irrigation systems. Materials and Methods: Mesial root canals from 60 mandibular molars were prepared and divided into 4 experimental groups according to the final irrigation technique: Group C, syringe irrigation; Group U, ultrasonics activation; Group SC, VPro StreamClean irrigation; Group EV, EndoVac irrigation. Cross-sections at 1, 3 and 5 mm levels from the apex were examined to calculate remaining debris area in the canal and isthmus spaces. Statistical analysis was completed by using Kruskal-Wallis test and Mann-Whitney U test for comparison among groups, and multivariate linear analysis to identify the significant variables (regular replenishment of irrigant, vapor lock management, and ultrasonic activation of irrigant) affecting the cleaning efficacy of the experimental groups. Results: Group SC and EV showed significantly higher canal cleanliness values than group C and U at 1 mm level (p < 0.05), and higher isthmus cleanliness values than group U at 3 mm and all levels of group C (p < 0.05). Multivariate linear regression analysis demonstrated that all variables had independent positive correlation at 1 mm level of canal and at all levels of isthmus with statistical significances. Conclusions: Both VPro StreamClean and EndoVac system showed favorable result as final irrigation regimens for cleaning debris in the complicated root canal system having curved canal and/or isthmus. The debridement of the isthmi significantly depends on the variables rather than the canals.

감염근관에서 분리한 세균의 동정 및 항생제 감수성 검사 (INDENTIFICATION AND ANTIBIOTIC SUSCEPTIBILITY TEST OF MICROORGANISMS ISOLATED FROM INFECTED ROOT CANALS)

  • Bacteria have been regarded as major etiolgic factors in root canal infections. Infected root canal flora from thirteen patients who had visited to conservative department of Wonkwang dental hospital were cultured on blood agar plates. Cultued microorganisms were isolated and identified with Gram stain and biochemical tests using Vitek Systems(BioMeriux, MO, USA); Antibiotic susceptibillity was performed with disk diffusion and broth microdilution using Vitek Systems. Gram positive cocci(65 %) were predominant, which were composed of 6 Streptococcus viridans group, 5 Staph. spp., and 4 Enterococcus faecium, in the isolatd 23 strains. Gram negative rods (26 %) were the next common bacteria, which were composed of 5 non - fermentative Gram negative rods, and 1 Enterobacter cloacae. Most strains of S. viridans group and E. faecium were susceptible to antibiotics including penicillin. But strains of Staphylococcus spp. and non - fermentative Gram negative rods showed marked resistance to antibiotics except tetrancyclin and cefotaxime. Most results between disk diffusion and microdilution were all agreed, but the results of non - fermentative Gram negative rods were susceptible to cefotaxime in disk diffusion method but resistant in microdilution.

근관장 측정에 있어서 디지털 영상 처리기와 주파수 의존형 측정기의 정확도 (The Accuracy of the Digital Imaging System and the Frequency Dependent Type Apex Locator in Root Canal Length Measurement)

  • In order to achieve a successful endodontic treatment, root canals must be obturated three-dimensionally without causing any damage to apical tissues. Accurate length determination of the root canal is critical in this case. For this reason, I've used the conventional periapical radiography, Digora/sup (R)/(digital imaging system) and Root ZX/sup (R)/(the frequency dependent type apex locator) to measure the length of the canal and compare it with the true length obtained by cutting the tooth in half and measuring the length between the occlusal surface and the apical foramen. From the information obtained by these measurements, I was able to evaluate the accuracy and clinical usefulness of each systems. whether the thickness of files used in endodontic therapy has any effect on the measuring systems was also evaluated in an effort to simplify the treatment planning phase of endodontic treatment. 29 canals of 29 sound premolars were measured with #15, #20, #25 files by 3 different dentists each using the periapical radiography. Digora/sup (R)/ and Root ZX/sup (R)/. The measurements were then compared with the true length. The results were as follows: 1. In comparing mean discrepancies between measurements obtained by using periapical radiography(mean error: -0.449±0.444 mm), Digora/sup (R)/(mean error: -0.417±0.415 mm) and Root ZX/sup (R)/(mean error: 0.123±0.458 mm) with true length. periapical radiography and Digora/sup (R)/ system had statistically significant differences(p<0.05) in most cases while Root ZX/sup (R)/ showed none(p>0.05). 2. By subtracting values obtained by using periapical radiography, Digora/sup (R)/ and Root ZX/sup (R)/ from the true length and making a distribution table of their absolute values. the following analysis was possible. In the case of periapical film. 140 out of 261<53.6%) were clinically acceptable satisfying the margin of error of less than 0.5 mm. 151 out of 261 (53,6%) were acceptable in the Digora/sup (R)/ system while Root ZX/sup (R)/ had 197 out of 261(75.5%) within the limits of 0.5mm margin of error. 3. In determining whether the thickness of files has any effect on measuring methoths, no statistically significant differences were found(p>0.05). 4. In comparing data obtained from these methods in order to evaluate the difference among measuring methods, there was no statistically significant difference between periapical radiography and Digora/sup (R)/ system(p>0.05), but there was statistically significant difference between Root ZX/sup (R)/ and periapical radiography(p<0.05). Also there was statistically significant difference between Root ZX/sup (R)/ and Digora/sup (R)/ system(p<0.05). In conclusion, Root ZX/sup (R)/ was more accurate when compared with the Digora/sup (R)/ system and periapical radiography and seems to be more effective clinically in determining root canal length. But Root ZX/sup (R)/ has its limits in determining root morphology and number of roots and its accuracy becomes questionable when apical foramen is open due to unknown reasons. Therefore the combined use of Root ZX/sup (R)/ and the periapical radiography are mandatory. Digora/sup (R)/ system seems to be more effective when periapical radiographs are needed in a short period of time because of its short processing time and less exposure.

학생들이 사용한 세 종류 Ni-Ti file systems의 근관성형 효율 비교 (Relative efficacy of three Ni-Ti file systems used by undergraduates)

  • 이 연구의 목적은 Ni-Ti file을 사용한 경험이 없는 학생들이 세 종류 Ni-Ti file systems을 사용하였을 때 근관 형성 효율을 비교하는 것이다. Ni-Ti file의 사용 경험이 없는 부산대학교 치과대학 4학년 학생 50명이 세 종류의 Ni-Ti file system - $ProFile^{(R)}$ (PF : Dentsply Maillefer, Ballaigues, Switzerland), manual $ProTaper^{(R)}$ (MPT: Dentsply Maillefer, Ballaigues, Switzerland), rotary $ProTaper^{(R)}$ (RPT: Dentsply Maillefer, Ballaigues, Switzerland) -을 사용하여 각 system으로 하나의 근관씩, 모두 150개의 레진 블락 근관모형(Endo Training Bloc; Dentsply Maillefer, Ballaigues, Switzerland)을 형성하였다. 근관형성에 사용된 file의 파절이나 변형 및 근관형성 후 형태의 이상을 조사하고, 스캐너로 근관 형성 전후의 이미지를 채득하여 중첩 비교함으로써 근단부로부터 1mm, 3mm, 그리고 5mm 높이에서의 삭제된 근관의 폭경, 근관의 변위량, 그리고 중심변위율 등을 산출하고 통계학적 비교 분석을 하였다. 그리고 세 가지 systems에 관한 학생들의 주관적인 선호도를 조사하였다. 그 결과 졸업을 앞둔 치과대학 재학생들도 특별한 문제점 없이 세 가지 Ni-Ti file systems을 사용한 근관형성이 가능함을 확인하였다. 세 실험군을 비교하였을 때, 양적인 삭제 능력은 두 $ProTaper^{(R)}$ systems (manual and rotary)이 $ProFile^{(R)}$에 비해 우세하였지만, 근관의 변위는 두 $ProTaper^{(R)}$ systems에서 더 많이 유발되었다. 근관 중심 이동률 등의 질적 평가를 포함하여 학생들의 선호도를 종합적으로 고려할 때, 삭제 능력에 있어서는 rotary $ProTaper^{(R)}$가 더 효율적이지만 안전성을 고려하면 $ProFile^{(R)}$도 추천된다. 정규 교육과정에서 Ni-Ti file systems의 도입을 위해서 초심자와 관련한 더 많은 연구가 진행되어야 하겠다.

Fused roots of maxillary molars: characterization and prevalence in a Latin American sub-population: a cone beam computed tomography study

  • Objectives: The upper molars generally have three roots; therefore, different combinations of fusion can occur, increasing the possibility of finding more complex root canal systems. The purpose of this study was to evaluate the prevalence and characterization of fused roots in first and second maxillary molars using cone-beam computed tomography (CBCT) in a Colombian population. Materials and Methods: A total of 1274 teeth were evaluated, of which 534 were maxillary first molars and 740 were maxillary second molars. Axial sections were made at the cervical, middle, and apical levels to determine the prevalence of root fusion and the types of fusion. Results: Overall, 43% of the molars (n = 551) presented some type of fused root. Root fusion was present in 23.4% of the maxillary first molars. The most frequent type of fused root was type 3 (distobuccal-palatal; DB-P) (58.9%). Root fusion was observed in 57.6% of the maxillary second molars, and the most prevalent type of fused root was type 6 (cone-shaped) (45.2%). Of the maxillary molars, 12.5% were classified as C-shaped. Conclusion: Within the limitations of this study, there was a high prevalence of fused roots in maxillary molars in the Colombian population, mainly in the maxillary second molars. In first molars, the most common type of fused root was type 3 (DB-P) and in second molars, the most common type was type 6 (cone-shaped). Additionally, molars with root fusion presented variation at different levels of the radicular portion, with implications for treatment quality.
