Observation of an extracted premolar 2.5 years after mineral trioxide aggregate apexification using micro-computed tomography

  • Although numerous studies have been conducted on apexification using mineral trioxide aggregate (MTA), direct observation of extracted human teeth after the procedure has been rarely reported. This case report describes a mandibular premolar treated 2.5 years ago and extracted recently for orthodontic treatment. The tubercle of the right mandibular premolar of a 12-year-old boy with dens evaginatus was fractured and the pulp was exposed. The tooth was diagnosed with pulp necrosis and asymptomatic periapical abscess. During the first visit, copious irrigation was performed with 2.5% sodium hypochlorite. Calcium hydroxide paste was placed as an intracanal medicament. The sinus tract had disappeared at the second visit after 3 weeks. MTA was applied on to the bleeding point as a 4-mm-thick layer, followed by a 3-mm-thick gutta-percha filling and resin core build-up. After 2.5 years, the tooth and three other premolars were extracted for orthodontic treatment. The right and left mandibular premolars were scanned with micro-computed tomography to determine the root shape and canal anatomy. Irregular root growth was observed and the root outline of the right mandibular premolar differed from that of the contralateral tooth. Apexification with MTA leads to the formation of roots with irregular morphology, without any pulpal space.

Effect of Intracanal Medicaments on Push-out Bond Strength of Calcium Silicate-based Materials (근관내 약제가 규산칼슘 기반 재료의 압출 강도에 미치는 영향)

  • The purpose of this study was to evaluate the effect of the intracanal medicaments on the push-out bond strength of the calcium silicate-based materials. Forty extracted single-root human mandibular premolars were sectioned below cementoenamel junction. Standardized root canal dimension was obtained with a parallel post drill. The specimens were randomly divided into a control group (no medicament), and experimental groups received medicaments with either CH (calcium hydroxide), DAP (double antibiotic paste; a mixture of ciprofloxacin and metronidazole), or TAP (triple antibiotic paste; a mixture of minocycline, ciprofloxacin and metronidazole). Following removal of medicaments with irrigation, roots were cut into sections with 1-mm-thickness. Thereafter, calcium silicate-based materials are applied to the specimens : (i) ProRoot MTA$^{(R)}$ and (ii) Biodentine$^{(R)}$. A push-out bond strength was measured and each specimen was examined to evaluate failure mode. Intracanal medication using CH significantly increased the bond strength to the root dentin. But there are no significant differences on the bond strength of ProRoot MTA$^{(R)}$ or Biodentine$^{(R)}$ among TAP, DAP and control groups. The dislodgement resistance of Biodentine$^{(R)}$ from root dentin was significantly higher than that of ProRoot MTA$^{(R)}$ regardless of the type of intracanal medicaments.

Effect of intracanal cryotherapy on postoperative pain after endodontic treatment: systematic review with meta-analysis

  • Objectives: This study aimed to evaluate the effectiveness of final irrigation with cold saline solution after endodontic treatment compared with saline solution at room temperature against postoperative pain following endodontic treatment. Materials and Methods: A broad search was performed in the PubMed, Web of Science, Scopus, Cochrane Library, Virtual Health Library (LILACS), and Grey Literature databases. Two independent reviewers performed data extraction, risk of bias using the Cochrane methodology, and certainty of evidence using the Grading of Recommendations, Assessment, Development and Evaluations (GRADE) approach. Results: Eight studies were included in qualitative synthesis. Intracanal cryotherapy favored the reduction of postoperative pain in the systematic review. Four studies were included in meta-analyses. The meta-analysis showed that intracanal cryotherapy reduced postoperative pain in teeth with symptomatic apical periodontitis (SAP) at 24 hours. There was no association between intracanal cryotherapy and control (room temperature) groups in teeth with normal periapical tissue with respect to postoperative pain at 24 hours and 48 hours. Conclusions: Intracanal cryotherapy was effective in reducing postoperative pain after endodontic treatment in teeth with SAP.

TIME-DEPENDENT EFFECTS OF EDTA APPLICATION ON REMOVAL OF SMEAR LAYER IN THE ROOT CANAL SYSTEM (EDTA의 세정시간에 따른 근관 내 smear layer의 제거효과에 관한 연구)

  • This study was to verify that the combined application of NaOCl and EDTA was more effective in removal of smear layer than the application of NaOCl alone. furthermore it was aimed to find out the optimal time for the application of EDTA. Thirty five single rooted teeth were cleaned and shaped. NaOCl solution was used as an irrigant during instrumentation. After instrumentation, root canals of the control group were irrigated with 5 ml of NaOCl for 2 minutes. 30 sec, 1 min, and 2 min group were irrigated with 5 ml of 17% EDTA for 30 sec, 1 min, and 2 min respectively. Then the roots were examined with scanning electron microscopy for evaluating removal of smear layer and erosion of dentinal tubule. The results were as follows; 1. The control group: The smear layer was not removed at all. 2. The other groups: 1) $Middle\frac{1}{3}$: All groups showed almost no smear layer. And the erosion occurred more frequently as increasing irrigation time. 2) $Apical\frac{1}{3}$: The cleaning effect of 2 min group was better than the others. The results suggest that 2 min application of 17% EDTA should be adequate to remove smear layer on both $apical\frac{1}{3}\;and\;middle\frac{1}{3}$.

EFFECT OF IRRIGATION METHODS ON THE ADHESION OF RESILON/EPIPHANY SEALER AND GUTTA-PERCHA/AH 26 SEALER TO INTRACANAL DENTIN (근관 세척법이 Resilon/Epiphany sealer와 Gutta-percha/AH 26 sealer의 근관 상아질과의 접착에 미치는 영향)

  • The purpose of this study was to evaluate whether intracanal irrigation method could affect the adhesion between intracanal dentin and root canal filling materials (Gutta-percha/AH 26 sealer and Resilon/Epiphany sealer). Thirty extracted human incisor teeth were prepared. Canals were irrigated with three different irrigation methods as a final rinse and obturated with two different canal filling materials (G groups: Gutta-percha/AH 26 sealer, R groups: Resilon/Epiphany sealer) respectively. Group G1, R1-irrigated with 5.25% NaOCl Group G2, R2-irrigated with 5.25% NaOCl, sterile saline Group G3, R3-irrigated with 5.25% NaOCl, 17% EDTA, sterile saline Thirty obturated roots were horizontally sliced and push-out bond strength test was performed in the universal testing machine. After test, the failure patterns of the specimens were observed using Image-analyzing microscope. The results were as follows. 1. Gutta-percha/AH 26 sealer groups had significantly higher push-out bond strength compared with the Resilon/Epiphany sealer groups (p < 0.05). 2. Push-out bond strength was higher when using 17% EDTA followed by sterile saline than using NaOCl as a final irrigation solution in the Resilon/Epiphany sealer groups (p < 0.05). 3. In the failure pattern analysis, there was no cohesive failure in Group G1, G2, and R1. Gutta-percha/AH 26 sealer groups appeared to exhibit predominantly adhesive and mixed failure patterns, whereas Resilon/Epiphany sealer groups exhibited mixed failures with the cohesive failure occurred within the Resilon substrate.

  • This is the research to prepare a purification program for relatively less polluted stream by using phytoremediation. We calculated a treatment amount of nutrients followed by growth of Helianthus annuus (a kind of sunflower), setting up the plant reactor in the hothouse. Moreover, to investigate a field applicability, we could find increased contents of nitrogen, carbon and hydrogen in plants by setting up a H. annuus planted artificial floating island in an irrigation canal. When we changed the dissolved inorganic nitrogen(DIN) concentration of the influent from 28.5 to 199.2 mg/l and the dissolved inorganic phosphorus(DIP) concentration of the influent from 13.3 to 25.4 mg/l, growth disorder has not appeared though it is much higher than the criterion of water for irrigation. In this case, the removal rate of DIN was $81.7\sim98.6%$, and that of DIP was $81.9\sim98.4%$ in 3 days stay on average. It has appeared that the efficient hydraulic retention time(HRT) was 48 hours. The following contents of nitrogen, carbon and hydrogen of H. annuus appeared in the artificial floating island: nitrogen was $3.2\sim7.8%$ in the trunk and $3.0\sim6.3%$ in the root. Carbon was $40.1\sim57.7%$ in the trunk and $43.4\sim53.8%$ in the root.

Contralateral recurrence of necrotizing sialometaplasia of the hard palate after five months: a case report

  • Necrotizing sialometaplasia usually heals within 4 to 10 weeks with conservative treatment, and rarely recurs. When necrotizing sialometaplasia is present on the hard palate it may occur unilaterally or bilaterally. In this case, necrotizing ulceration occurred on the left hard palate of a 36-year-old woman after root canal treatment of the upper left first premolar under local anesthesia. After only saline irrigation the defect of the lesion completely healed and filled with soft tissue. After 5 months, however, a similar focal necrosis was found on the contralateral hard palate without any dental treatment having been performed on that side and progressed in similar fashion as the former lesion. We conducted an incisional biopsy and obtained a final pathological diagnosis for the palatal mass of necrotizing sialometaplasia. At the 3-year follow-up, the patient's oral mucosa of the hard palate was normal, without any signs and symptoms of the condition. We report a case of a second occurrence of necrotizing sialometaplasia on the contralateral side from the first, with a time lapse between the first and second occurrence.

Effect of two different calcium hydroxide paste removal techniques on apical leakage: an electrochemical study (수산화칼슘 제제의 제거 방법이 치근단 누출에 미치는 영향에 대한 전기화학적 연구)

  • This study evaluated the effect of two different calcium hydroxide ($Ca(OH)_2$) paste removal techniques on the apical leakage of canals obturated with gutta percha cones and sealer after removing a $Ca(OH)_2$ dressing using an electrochemical method. Seventy extracted single-rooted teeth were instrumented on with Profile rotary files under NaOCl irrigation. Fifty-eight canals were filled with calcium hydroxide paste, which was then removed using one of the following two techniques. In group A, calcium hydroxide was removed using only NaOCl irrigation, and in group B, the canals were re-prepared with a Profile rotary files-one size larger than the previous instrument and were irrigated with NaOCl. In both groups, the root surfaces were coated twice with nail varnish from CEJ to an area 4 mm away from the apex after canal obturation. Apical leakage was measured using an electrochemical method for 24 days. All the specimens showed leakage that increased markedly in the first three days. There was no significant difference between the two groups (p>0.05). The effect of two calcium hydroxide paste removal techniques on the apical leakage was not different during a short period.

THE ANTIMICROBIAL EFFECT OF HORSERADISH (ARMORACIA RUSTICANA) ROOT EXTRACTS AS AN ENDODONTIC IRRIGANT (근관세척제로써 서양산 고추냉이(Horseradish, Armoracia rusticana) 뿌리 추출물의 항균효과)

  • Microorganisms are the main causative factors of pulpal and periapical diseases, therefore successful endodontic treatment is depend on the effective elimination of intracanal bacterial populations. Many studies have been reported antimicrobial effect of Allyl isothiocyanate (AIT) which the principle ingredient of Horseradish (Armoracia rusticana) root extracts. The purposes of this study are to evaluate the antimicrobial effectiveness of Horseradish root extracts against Enterococcus faecalis in root canals of extracted human teeth and compare to sodium hypochlorite (NaOCl). Extracted human mandibular premolar root canals were infected with E. faecalis for 21 days, and then irrigated with Horseradish root extracts, NaOCl solution and saline. After canal irrigation, first samples (S1) were taken. After first sampling, the canals were additionally incubated 7 days, and then second samples (S2) were taken. The samples were inoculated on EHI agar plate to determine the colony forming units (CFU). 1. Mean values of CFU in S1 were $5.815{\times}10^3$ CFU/ml at Horseradish groups, and $3.465{\times}10^3$ CFU/ml at NaOCI groups. There was no statistically significant differences (p=0.086). 2. Mean values of CFU in S2 were $3.100{\times}10^3$ CFU/ml at Horseradish groups, and $5.252{\times}10^5$ CFU/ml at NaOCI groups. There was statistically significant difference (p<.05). 3. There was no statistically significant differences (p=0.076) between S1 and S2 at Horseradish groups in the mean values of CFU. However, there was statistically significant differences (p<.05) between S1 and S2 at NaOCI groups in the mean values of CFU.

Nonvital Pulp Therapy of Elongation of Roots of Mandibular Cheek Teeth in Pet Rabbits (애완토끼에서 과잉성장된 하악치아의치신경치료 연구)

  • Elongation of cheek teeth roots in pet rabbits is very common, and is associated with malocclusion followed by reduced appetite, salivation, periapical abscess, and epiphora. Conservative methods including medication, drainage, irrigation, tooth trimming, intraoral and extraoral extraction, surgical treatment of periapical abscessation, and diet control have been adapted as the only treatments to resolve elongation of teeth roots. However, remaining challenges include the long-term period of cure and recurrence. This study was performed to investigate the possibility of nonvital pulp therapy on elongation of the mandibular cheek teeth roots in pet rabbits. Thirty-one pet rabbits with dental problems due to root elongation were submitted. Ten pet rabbits among them were treated by nonvital pulp therapy procedures (group A), while the others were treated by conservative methods (group B). Appetite improved within 1-5 days after nonvital pulp therapy and the treatment was discontinued 1 month postoperatively in group A. Abscess occurred in another site not treated with nonvital pulp therapy in only two rabbits. Growth of the mandibular cheek teeth treated with nonvital pulp therapy stopped, resulting in malocclusion, intraoral inflammation from the enamel spur, and abscess of the teeth roots. In the group B rabbits treated with conservative therapy, partial drainage, long term medication, recurrent oral trimming and control of repeated oral inflammation occurred. Consequently, buccotomy or tooth extraction was performed in group B. Owners were satisfied with nonvital pulp therapy preventing dental root abscess and repeated troubles including inflammation and malocclusion and reduction of the treatment period. These results suggest that nonvital pulp therapy can be performed on pet rabbits with elongation of mandibular cheek teeth roots.
