회전식 nickel-titanium file을 이용한 유치의 근관치료 (ROOT CANAL TREATMENT ON PRIMARY TEETH USING NICKEL-TITANIUM NOTARY FILES)

  • 유치의 근관치료에서 치수절제술은 실활된 유치를 치열궁내에 유지하기 위해 치관과 근관부분의 괴사된 치수조직을 완전하게 제거하는 것을 말한다. 이 술식은 감염된 치수의 세균을감소시키는 것을 목적으로 하며, 다양하고 불규칙한 유치 근관의 형태가 주된 장애 요소로 작용한다. 더욱이 비협조적인 어린이의 경우에는 오랜 시간동안 정교한 시술을 하기가 곤란하다. 최근 들어 근관치료시에 회전식 기구를 이용하여 근관형성을 하는 장비들이 많이 개발되었으며 저속전기 모터에 회전식 NiTi file을 이용한 술식이 많이 사용되고 있다. 본 증례에서는 회전식 NiTi file과 전기모터를 이용하여 유구치의 근관치료를 시도하였다. 치수강을 개방하고 치관부 치수를 제거한 후 잠정적 작업장을 측정하였으며 적절한 NiTi file을 선택하여 잠정적 작업장까지 crown-down방법으로 점차적으로 형성하였다. 수용 file로 근단부 도달 및 성형을 하고 전체적인 마무리를 하여 Vitapex로 충전하여 양호한 결과를 얻었기에 보고하는 바이다.

CBCT-based assessment of root canal treatment using micro-CT reference images

  • Purpose: This study compared the root canal anatomy between cone-beam computed tomography (CBCT) and micro-computed tomography (micro-CT) images before and after biomechanical preparation and root canal filling. Materials and Methods: Isthmus-containing mesial roots of mandibular molars(n=14) were scanned by micro-CT and 3 CBCT devices: 3D Accuitomo 170 (ACC), NewTom 5G (N5G) and NewTom VGi evo (NEVO). Two calibrated observers evaluated the images for 2-dimensional quantitative parameters, the presence of debris or root perforation, and filling quality in the root canal and isthmus. The kappa coefficient, analysis of variance, and the Tukey test were used for statistical analyses(α=5%). Results: Substantial intra-observer agreement (κ=0.63) was found between micro-CT and ACC, N5G, and NEVO. Debris detection was difficult using ACC (42.9%), N5G (40.0%), and NEVO (40%), with no agreement between micro-CT and ACC, N5G, and NEVO (0.05<κ<0.12). After biomechanical preparation, 2.4%-4.8% of CBCT images showed root perforation that was absent on micro-CT. The 2D parameters showed satisfactory reproducibility between micro-CT and ACC, N5G, and NEVO (intraclass correlation coefficient: 0.60-0.73). Partially filled isthmuses were observed in 2.9% of the ACC images, 8.8% of the N5G and NEVO images, and 26.5% of the micro-CT images, with no agreement between micro-CT and ACC, and poor agreement between micro-CT and N5G and NEVO. Excellent agreement was found for area, perimeter, and the major and minor diameters, while the roundness measures were satisfactory. Conclusion: CBCT images aided in isthmus detection and classification, but did not allow their classification after biomechanical preparation and root canal filling.

Fused roots of maxillary molars: characterization and prevalence in a Latin American sub-population: a cone beam computed tomography study

  • Objectives: The upper molars generally have three roots; therefore, different combinations of fusion can occur, increasing the possibility of finding more complex root canal systems. The purpose of this study was to evaluate the prevalence and characterization of fused roots in first and second maxillary molars using cone-beam computed tomography (CBCT) in a Colombian population. Materials and Methods: A total of 1274 teeth were evaluated, of which 534 were maxillary first molars and 740 were maxillary second molars. Axial sections were made at the cervical, middle, and apical levels to determine the prevalence of root fusion and the types of fusion. Results: Overall, 43% of the molars (n = 551) presented some type of fused root. Root fusion was present in 23.4% of the maxillary first molars. The most frequent type of fused root was type 3 (distobuccal-palatal; DB-P) (58.9%). Root fusion was observed in 57.6% of the maxillary second molars, and the most prevalent type of fused root was type 6 (cone-shaped) (45.2%). Of the maxillary molars, 12.5% were classified as C-shaped. Conclusion: Within the limitations of this study, there was a high prevalence of fused roots in maxillary molars in the Colombian population, mainly in the maxillary second molars. In first molars, the most common type of fused root was type 3 (DB-P) and in second molars, the most common type was type 6 (cone-shaped). Additionally, molars with root fusion presented variation at different levels of the radicular portion, with implications for treatment quality.

치내치에 관한 치험례 (DENS INVAGINTUS : A CASE REPORT)

  • 치내치는 치아 발육 중에 석회화되기 전, 치아의 법랑상피가 일부 함입되어 설측에 깊은 소와를 형성하는 발생학적 변이이다. 주로, 상악 측절치에 호발한다. 치내치는 형태학적으로 다양하며, 종종 초기에 치수괴사에 이르기도 한다. 치내치는 복잡한 해부학적 구조로 인해 치수 치료시 많은 어려움이 따른다. 따라서, 치내치는 초기 진단이 중요하고, 예방적 치료가 추천된다. 이에 본 증례는 치내치를 가진 환아에서의 치료에 관해 보고하고자 한다.

하악 제1 대구치 근단부의 형태학적 연구 (MORPHOLOGY OF THE APICAL ROOT CANAL SYSTEM IN KOREAN MANDIBULAR FIRST MOLAR)

  • 본 연구는 한국인의 하악 제1 대구치 60개를 대상으로 생리적 근단공의 크기와 형태, 근심 치근의 치근단 3mm에서 isthmus의 발생 빈도와 형태, 그리고 원심 치근의 치근단 3mm의 근관 형태를 입체현미경하에서 60배로 조사한 결과 다음과 같은 결론을 얻었다. 1.근심 치근은 두 개의 근단공을 가지는 비율이 높았으며(51.67%), 원심 치근은 하나의 근단공을 가지는 비율이 높았다(71.66%). 2. 부근단공의 빈도는 근심 치근의 1개의 근단공을 가지는 경우가 가장 높았다(26.07%) 3. 생리적 근단공의 크기는 근심 치근에서 하나의 근단공을 가지는 경우 0.329mm, 근심 협측 근단공은 0.266mm, 근심 설측 근단공은 0.246mm였으며, 원심 치근에서 하나의 근단공을 가지는 경우 0.375mm, 원심 협측 근단공은 0.291mm, 원심 설측 근단공은 0.237mm로 나타났다. 4. 근단공의 형태는 oval 형태 (69.93%)가 가장 많았다. 5.근심 치근의 치근단 3mm에서 isthmus의 발생빈도는 55%였으며, partial isthmus는 31.66%, complete isth-mus는 23.34%로 나타났다. 6.원심 치근의 치근단 3mm의 근관 형태는 oval형태(50.64%)가 가장 많았다. 이상의 연구 결과 하악 제1대구치의 성공적인 근관 치료를 위해서는 생리적 근단공의 실제 크기와 형태 및 isthmus의 존재에 대한사전 지식이 필요할 것이다.

C-shaped root canals of mandibular second molars in a Korean population: a CBCT analysis

  • Objectives: The purpose of this study was to investigate the C-shaped root canal anatomy of mandibular second molars in a Korean population. Materials and Methods: A total of 542 teeth were evaluated using cone-beam computed tomography (CBCT). The canal shapes were classified according to a modified version of Melton's method at the level where the pulp chamber floor became discernible. Results: Of the 542 mandibular second molars, 215 (39.8%) had C-shaped canals, 330 (53%) had 3 canals, 17 (3.3%) had 2 canals, 12 (2.2%) had 4 canals, and 8 (1.7%) had 1 canal. The prevalence of C-shaped canals was 47.8% in females and 28.4% in males. Seventy-seven percent of the C-shaped canals showed a bilateral appearance. The prevalence of C-shaped canals showed no difference according to age or tooth position. Most teeth with a C-shaped canal system presented Melton's type II (45.6%) and type III (32.1%) configurations. Conclusions: There was a high prevalence of C-shaped canals in the mandibular second molars of the Korean population studied. CBCT is expected to be useful for endodontic diagnosis and treatment planning of mandibular second molars.

  • 이 임상증례의 목적은 상악 제2대구치의 해부학적 형태를 computed tomography (CT)를 이용해 확인하여 근관치료에 활용한 증례이다. 이번 증례에서는 두개의 구개치근을 갖는 상악 제 2대구치에서 성공적인 근관치료를 위해서 CT를 활용하여 해부학적 형태를 확인하여 활용하였다. Cone beam computed tomography (CBCT)의 사용은 구개치근과 협측치근이 겹쳐보이는 치근단 방사선 사진의 한계를 극복할 수 있게 한다.

Comparison of the shaping ability of novel thermally treated reciprocating instruments

  • Objectives: The present study aimed to evaluate the shaping ability of 2 thermally treated nickel-titanium reciprocating systems in simulated curved canals. Materials and Methods: Forty simulated canals were prepared to apical size 25 using Reciproc Blue R25 (VDW) and WaveOne Gold Primary (Dentsply Sirona) instruments. Standard pre- and post-preparation images were taken and superimposed. The removal of resin material was measured at 5 standard points: the canal orifice, halfway between the canal orifice and the beginning of the curve, the beginning of the curve, the apex of the curve, and the end-point of the simulated canal. The data were analysed using the independent sample t-test with a 5% significance threshold. Results: The canals in which Reciproc Blue R25 was used showed a significantly greater widening than those in which WaveOne Gold was used at 4 of the 5 measurement points (p < 0.05). The Reciproc Blue R25 instrument removed significantly more resin from the inner aspect of the curve at 2 of the 5 points and similar amounts at the remaining 3 points. At the 2 apical points, there was no significant difference between the Reciproc Blue R25 and WaveOne Gold Primary instruments. Conclusion: Both instruments respected the original canal anatomy; however, WaveOne Gold resulted in a more conservative shape with less transportation.

Isthmuses, accessory canals, and the direction of root curvature in permanent mandibular first molars: an in vivo computed tomography study

  • Objectives: This study was performed to assess the anatomy of mandibular first molars. Materials and Methods: In this in vivo study, cone-beam computed tomography (CBCT) volumes of 312 bilateral intact first mandibular molars from 156 patients (79 men and 77 women; average age, 35.6 ± 11.2 years) were investigated in terms of the direction of each canal's curvature in the buccolingual and mesiodistal dimensions (direction of the position of the apex in relation to the longitudinal axis of the root), the presence of an isthmus (a narrow, ribbon-shaped communication between 2 root canals) in 3 segments (0-2, 2-4, and 4-6 mm) from the apex), and the presence and number of accessory canals (smaller canals besides the main root canals, connecting the pulp to the periodontium). Data were analyzed statistically (α = 0.05). Results: Mesiolingual canals were mostly buccally and distally inclined, while mesiobuccal and distolingual canals were mostly distally curved. Isthmuses were more common in younger patients (χ2 test, p < 0.05). The average numbers of accessory canals in the apical, middle, and coronal segments were 9.9 ± 4.2, 6.9 ± 2.9, and 9.3 ± 3.0 canals per segment, respectively (analysis of variance, p < 0.001). Age and sex were not associated with the number of accessory canals (p > 0.05). Conclusions: The complex anatomy of these teeth deserves attention during non-surgical or surgical endodontic treatment. Around the apex, isthmuses might be more prevalent in younger and female individuals.

Nonsurgical endodontic retreatment of fused teeth with transposition: a case report

  • Tooth transposition is a disorder in which a permanent tooth develops and erupts in the normal position of another permanent tooth. Fusion and gemination are developmental disturbances presenting as the union of teeth. This article reports the nonsurgical retreatment of a very rare case of fused teeth with transposition. A patient was referred for endodontic treatment of her maxillary left first molar in the position of the first premolar, which was adjacent to it on the distobuccal side. Orthopantomography and periapical radiography showed two crowns sharing the same root, with a root canal treatment and an associated periapical lesion. Tooth fusion with transposition of a maxillary molar and a premolar was diagnosed. Nonsurgical endodontic retreatment was performed. At four yr follow-up, the tooth was asymptomatic and the radiolucency around the apical region had decreased, showing the success of our intervention. The diagnosis and treatment of fused teeth require special attention. The canal system should be carefully explored to obtain a full understanding of the anatomy, allowing it to be fully cleaned and obturated. Thermoplastic techniques were useful in obtaining hermetic obturation. A correct anatomical evaluation improves the set of treatment options under consideration, leading to a higher likelihood of esthetically and functionally successful treatment.
