AN EVALUATION OF ROTATIONAL STABILITY IN ENDODONTIC ELECTRONIC MOTORS (근관치료용 전기모터의 회전 안정성 평가)

  • The purpose of this study was to evaluate a rotational stability of endodontic electronic motors by comparing the changes of rotational speed, depending on the number of usages and with/without static load. Twelve new endodontic electronic motors were used in this study. Non contact type digital tachometer was used for measuring the rotational speed of handpiece. True RMS Multimeter was used for measuring the voltages and the electric currents. All measurements were recorded every 10 seconds during 10 minutes and repeated 9 times. Five repetition was done per each electronic motor. To statistical analysis, student t-test, repeated measures and Scheffe's post-hoc tests were performed. In the same motor group, there was no significant difference in all measurements. In all groups, there was no significant difference in the amount of rotational speed changes depending on the number of usages and with/without static load. In the limitation of this study, the results showed that all kinds of endodontic electronic motors in this study had an established rotational stability. Therefore they could be safely used in root canal treatment with a reliable maintenance of rotational speed, regardless of the number of usages and with/without load.

RADICULAR CYST ENUCLEATION ON MANDIBULAR ANTERIOR REGION (하악 전치부에 발생한 치근낭종의 치험례)

  • The radicular cyst is the most common odontogenic cyst which is caused by pulpal inflammation, pulp death, and secondary to trauma or dental caries. Usually, the radicular cyst is asymptomatic, but a secondary inflammation can cause pain, swelling and redness. Getting larger, the radicular cyst can cause facial asymmetry and paresthesia by pressure on nerves. It requires conservative endodontic treatment or surgical approach. When the size of cyst is large or invasion of the adjacent tissue is not expected, cyst enucleation is carried out. And most of the case can be completely cured and shows low recurrence. In these radicular cysts cases, by cyst enucleation or apicoectomy after root canal treatment simultaneously, the infected teeth can be preserved successfully.

  • It is common for clinicians to encounter endodontically tl·treated teeth that contain posts within their roots. If endodontic treatment is failed, these posts must be removed to facilitate successful nonsurgical retreatment. There have been many techniques such as ultrasonic instrument, Ruddle post removal system, Eggler post remover and Masserann kit developed to facilitate removal of posts from the root canal space. But these methods may be disadvantageous because long length of time required for post removal and fracture of post or teeth. In now days new post removal technique using ATD automatic bridge remover was introduced. Advantages of this method are simple and short time consuming compare to others. This article served as a successful case report of post removal using ATD automatic bridge remover.

A CLINICAL STUDY ON THE PROGNOSIS AND SUCCESS IN CONVENTIONAL ENDODONTIC TREATMENT (재래식 근관치료의 예후와 성공에 관한 임상적 연구)

  • The purpose of this study was to determine the incidence and the degree of interappointment pain and post obturation pain associated with the pretreated clinical factors or conditions, and to examine the correlation between the success or failure and the pretreated clinical factors or conditions and postoperative pain. The author experienced the conventional root canal therapy in one hundred fifty-one teeth after recording the following clinical factors or conditions - sex, age, pulp vitality status, presence or absence of periapical rarefaction, single or multirooted teeth, pretreatment pain, and investigated the pain during and after treatment for seven days. After six months all the cases were re-evaluated through the clinical sign, symptom, and the recalled radiograph. The following results were obtained; 1. Of the 151 teeth (73.5%) had no interappointment pain, 31 teeth (20.5%) slight pain; and 9 teeth (6.0%) moderate to severe pain. 2. The presence of the pretreatment pain significantly increased the incidence and degree of interappointment, and there were no significant relationship between interappointment pain and other clinical factors or conditions. 3. Of the 151 teeth, 142 teeth (4.0%) experienced no post obturation pain, 3 teeth (2.0%) slight pain, 6 teeth (4.0%) moderate to severe pain. 4. There were no statistical correlation between postobturation pain and any of the clinical factor or conditions. 5. 141 teeth (3.4%) of 151 teeth were evaluated as success in this study, and success rate was decreased significantly in the cases of periapical rarefaction before treatment and postobturation pain. But there were no significant relationship between success or failure and other clinical factors or conditions.

IDENTIFICATION OF PUTATIVE PATHOGENS IN ACUTE ENDODONTIC INFECTIONS BY PCR BASED ON 16S rDNA (중합효소연쇄반응법을 이용한 급성 치수 및 치근단 질환의 병원성 세균의 동정)

  • The purpose of this study was to investigate the frequency of 7 putative pathogens in endodontic infections. The specimens were collected from infected pulpal tissue of patients who were referred for root canal treatment to the department of conservative dentistry, Chosun University Samples were collected aseptically using a barbed broach and a paper point. The cut barbed broaches and paper points were transferred to an eppendorf tube containing 500 ml of 1 X PBS. DNAs were extracted from the samples by direct DNA extraction method using lysis buffer (0.5% EDTA, 1% Triton X-100). Identification of 7 putative pathogens was performed by PCR based on 16S rDNA. The target species were as follows : Porphyromonas endodontalis, Porphyromonas gingivalis, Prevotella intermedia, Prevotella nigrescens, Bacteroides forsythus, Actinobacillus actinomycetemcomitans, and Treponema denticola. Our data revealed that the prevalence of P. endodontalis was found in 88.6% (39/54), P. ginivalis 52.3% (23/44), P. nigrescens 18.2% (8/44), P intermedia 15.9% (7/44) B. forsythus 18.2% (8/44), A. actinomycetemcomitans 3.3% (1/44), T. denticola 25% (l1/44) of the samples. The high prevalence of P. endodontalis and P. ginivalis suggests that they may play an important role in the etiology of endodontic infections.

FINITE ELEMENT ANALYSIS OF STRESS AND TEMPERATURE DISTRIBUTION AFFECTED BY VARIOUS RESTORATIVE AND BASE MATERIAL (수복재와 이장재에 따른 응력과 온도 분포의 유한 요소 분석)

  • Dental caries, one of the most frequent dental disease, become larger because it can be thought as a simple disease. Further more, it can progress to unexpected root canal therapy with fabrication of crown that needs reduction of tooth structure. Base is required in a large caries and ZOE, ZPC, glass ionomer are used frequently as base material. They, with restorative material, can affect the longevity of the restoration. In this study, we assume that the mandibular 1st molar has deep class I cavity. So, installing the 3 base material, 3 kinds of fillings were restored over the base as follows; 1) amalgam only, 2) amalgam with ZPC, 3) amalgam with ZOE, 4) amalgam with GI cement, 5) gold inlay with ZPC, 6) gold inlay with GI cement, 7) composite resin only, 8) composite resin with GI cement. After develop the 3-dimensional model for finite element analysis, we observe the distribution of stress and temperature with force of 500N to apical direction at 3 point on occlusal surface and temperature of 55 degree, 15 degree on entire surface. The analyzed results were as follow : 1. Principal stress produced at the interface of base, dentin, cavity wall was smallest in case of using GI cement as base material under the amalgam. 2. Principal stress produced at the interface of base, dentin, cavity wall was smaller in case of using GI cement as a base material than ZPC under gold inlay. 3. Composite resin-filled tooth showed stress distributed over entire tooth structure. In other words, there was little concentration of stress. 4. ZOE was the most effective base material against hot stimuli under the amalgam and GI cement was the next. In case of gold inlay, GI cement was more effective than ZPC. 5. Composite resin has the small coefficient of thermal conductivity. So, composite resin filling is the most effective insulating material.

  • The purposes of this study were to compare the bleaching efficacy of sodium perborate when mixed either with Superoxol or distilled water and to evaluate the efficacy of different location of intracoronal base on the presence of apical leakage of tested bleaching agents. Forty eight extracted human permanent incisors were stained via whole blood and canal fillings with conventional gutta percha were performed after routine biomechanical perparations. The experimental intracoronal base was placed either at the cementoenamel junction(group 3, 4) or 2mm below cementoenamel junction(group 1, 2). Walking bleaching was performed by two different combinations of bleaching agents : sodium perborate with distilled water in group 1, 3 and sodium perbrate with Superoxol in group 2, 4. The roots of the teeth were evaluated for the presence of color change to assess the leakage of bleaching agents and the cervical one-thirds of the crown were evaluated for bleaching effect from the whiteness Indea calculated by spectrophotometer. The results were as follows : 1. At the end of 12 days, all the sample teeth demonstrated the increase of Whiteness Index at cervical 1/3 of crown although there were some minor differences among groups. 2. Regardless of location of the base, sodium perborate with superoxol(group 2, 4) showed better results in bleaching than the sodium perborate with distilled water(group 1, 3). 3. Bleaching agent leaked into the root area when the base was placed 2mm below cementoenamel junction but no leakage was found when the base was placed at the cementoenamel junction.

A RADIOGRAPHIC STUDY OF DIFFERENTIAL DIAGNOSIS BETWEEN DENTIGEROUS CYSTS AND UNICYSTIC AMELOBLASTOMAS (함치성낭종과 단방성 법랑모세포종의 감별에 관한 방사선학적 연구)

  • The purpose of this study was to obtain some informations for the radiographic differential diagnosis between dentigerous cysts and unicystic ameloblastomas in the mandible. The authors observed and compared the clinico-radiographic features of 38 cases of dentigerous cyst and 32 cases of unicystic ameloblastoma associated with impacted mandibular molar. The obtained results were as follows: Dentigerous cysts occurred the most frequently in the 3rd decade, but unicystic ameloblastomas in the 2nd decade, and both lesions occurred with slight predilection in males. Average of lesional size of unicystic ameloblastomas was larger than that of dentigerous cysts, and lesions of over 25㎠ were only in unicystic ameloblastomas. Cortical thinning and expansion were more frequently observed in unicystic ameloblastomas at 72.9% than in dentigerous cysts at 15.8%. Dentigerous cysts showed smooth border at 89.5%, but unicystic ameloblastomas showed smooth border at 53.1% and scalloped border at 46.9%. Dentigerous cysts showed well-defined outline at 81.6%, but unicystic ameloblastomas showed well-defined outline at 53.1% and moderate-defined outline at 46.9%. In both lesions, the mandibular 3rd molar was the most frequent causative tooth. Average of distance between the cemento-enamel junction and lesional wall attachment of the causative tooth was longer in unicystic ameloblastomas than in dentigerous cysts. Severe displacement of causative tooth was more frequent in unicystic ameloblastomas at 62.5% than in dentigerous cysts at 23.7%. Dentigerous cysts showed homogeneous lesional radiolucency at 89.5%, but unicystic ameloblastomas showed inhomogeneous lesional radiolucency at 53.1%. Root resorption of adjacent tooth and displacement of mandibular canal were more frequent in unicystic ameloblastomas at 65.2% and 61.5% than in dentigerous cysts at 15.8% and 38.1% respectively.

Full-mouth rehabilitation of a patient with reduced occlusal vertical dimension: a clinical report (교합 고경이 감소된 보철물을 가진 환자의 전악 수복 증례)

  • It is a great challenge for dentists to do prosthetic restorations of a patient with reduced occlusal vertical dimension (OVD). Proper determination of the OVD is significant. An 85-year-old male patient came in with missing right maxillary posterior teeth. After diagnostic wax-up, reversible provisional prostheses were used to evaluate the new OVD in the maxilla and mandible. After that, the patient was evaluated clinically based on the criteria of esthetic, phonetic, and OVD. After 8 weeks, teeth with excessive wear were treated with root canal therapy and fiber post and core and irreversible provisional prostheses were done. After 8 weeks of observation period, final restorations with fixed partial dentures and removable partial dentures were made. Throughout the follow-up period of 6 months, no symptoms related with increased OVD and mechanical complications were observed.

TREATMENT OF MINOCYCLINE STAINED NON-VITAL TOOTH (Minocycline에 의해 변색된 실활치의 치료)

  • A mixture of metronidazole, ciprofloxacin and minocycline(3Mix) in distilled water or in a root canal sealer were used to disinfect the infected endodontic and periapical lesions. It has been well acknowledged in recent literature that minocycline, a semisynthetic tetracycline derivative, causes discolorations in adult teeth and various other collagenous tissues. In these cases, pulp treatments were finished successfully using a combination of antibiotics but severe discolorations of the teeth were observed. Discolored non-vital posterior tooth was treated using the prosthodontic procedure. And anterior tooth was successfully treated using the walking bleach technique. Bleaching was performed by temporarily placing a mixture of sodium perborate and distilled water into the pulp chamber. There were significant esthetic improvement.
