Socioeconomic Mortality Inequalities in Korea Labor & Income Panel Study (사회경제적 사망률 불평등 : 한국노동패널 조사의 추적 결과)

  • This study is to examine relationships of several socioeconomic position indicators with mortality risk in a nationwide longitudinal study of South Korea. The Korea Labor & Income Panel Study, conducted on a probability sample of urban South Korean households by Korea Labor Institute, contains date of death information for the decedents which were used to estimate relative risks of mortality and their $95\%$ confidence intervals (CI) with Cox regression analysis. A total of 125 men and women among 8,415 subjects died between 1998 and 2002. Socioeconomic differentials in mortality were observed after adjustment for sex and age. Those with less than 12-year education had 1.90 times $(95\%\;CI=1.25-2.91)$ greater mortality risk than those with 12-year education or more. Greater mortality risks were also found among those with low occupational class and manual occupation. The magnitude of differentials in mortality risks between occupational class were similar in two different approaches to measuring women's occupational class: (1) approach 1 where women, married or not, retain their own occupational class, and (2) approach 2 where married women are assigned their husbands' occupational class. Relative risks of dying among those with low household Income were 1.62 $(95\%\; CI=1.08-2.42)$ compared with the counterparts. Those who reported economic hardship at the time of survey in 1998 had greater risk of mortality $(RR=1.83,\;95\%\;CI=1.21-2.78)$ than those who did not. In conclusion, increased social discourse and policy discussions about these health inequalities are needed in Korean society. Future studies should explore the causes and mechanisms of socioeconomic mortality inequalities.

  • Organizations and customers lose if business activities we discontinued by an incident of information systems under the current business environment because they pursue real time enterprise and on demand enterprise. The loss includes the intangible decline in brand image, customer separation, and the tangible loss such as decrease in business profits. Thus. it is necessary to have preparedness in advance and mitigation for minimization of a loss due to the business discontinuity and information system's operational risks. This paper suggests the mitigation model for minimizing the incidents of disk unit in information system's operational risks. The model will be represented by a network model which is composed of the three items as following: (1) causes, attributes, indicators of an operational risk, (2) a periodic time through an analysis of historical data, (3) an index or a regulation related to the examination of causes of an operational risk.

Field Application and Evaluation of Health Status Assessment Tool based on Dietary Patterns for Middle-Aged Women (중년 여성의 식생활 중심 건강상태 판정 도구의 현장 적용 및 평가)

  • Objectives: This study was performed to verify the validity and judgment criteria setting of a health status assessment tool based on dietary patterns for middle-aged women. Methods: A total of 474 middle-aged women who visited the Comprehensive Medical Examination Center at Hanmaeum Hospital in Changwon were enrolled (IRB 2013-0005). The validity was verified using clinical indicators for the diagnosis of metabolic syndrome (MS), and it was used to set the criteria for the tool. A logistic regression analysis was performed for validation. The area under-receiver operation (AUC), sensitivity, specificity, and Youden Index were calculated through ROC curve analysis. Statistical analysis was performed by SPSS 21, and p value <0.05 was considered to be statistically significant. Results: The mean score of the group with no MS (73.3 points) was significantly higher compared to the group with MS (65.7 points) (p<0.001). An analysis of the association between the tool scores and risk of MS showed a 0.15-fold reduction in the risk of MS every time the tool's score increased by one point. As the result of the ROC curve analysis, the assessment reference point was set to 71 points, indicating 77.0% sensitivity and 61.0% specificity. Risk of MS was significantly higher in the group with a score of less than 71.0 than a group with more than 71 points (OR=5.28, p<0.001). Conclusions: This study was the first attempt to develop a health status assessment tool based on the dietary patterns for middle-aged women, and this tool has proven its usefulness as an MS assessment tool through the application of middle-aged women in the field of health screening.

  • Background: Organizational changes that involve healthcare hospital departments and care services of health districts, and ongoing technological innovations and developments in society increasingly expose healthcare workers (HCWs) to work-related stress (WRS). Minimizing occupational exposure to stress requires effective risk stress assessment and management programs. Methods: The authors conducted an integrated analysis of stress sentinel indicators, an integrated analysis of objective stress factors of occupational context and content areas, and an integrated analysis between nurses and physicians of hospital departments and care services of health districts in accordance with a multidimensional validated tool developed in Italy by the National Network for the Prevention of Work-Related Psychosocial Disorders. The purpose of this retrospective observational study was to detect and analyze in different work settings the level of WRS resulting from organizational changes implemented by hospital healthcare departments and care services of health districts in a sample of their employees. Results: The findings of the study showed that hospital HCWs seemed to incur a medium level risk of WRS that was principally the result of work context factors. The implementation of improvement interventions focused on team development, safety training programs, and adopting an ethics code for HCWs, and it effectively and significantly reduced the level of WRS risk in the workplace. Conclusion: In this study HCW resulted to be exposed to occupational stress factors susceptible to reduction. Stress management programs aimed to improve work context factors associated with occupational stress are required to minimize the impact of WRS on workers.

Determinants of Accessibility to Fintech Lending: A Case Study of Micro and Small Enterprises (MSEs) in Indonesia

  • Several studies have revealed that information on borrower characteristics plays an important factor in approving their credit requests. Though the extent to which such characteritics are also applicable to the case of fintech lending remain uncertain. The aim of this study is, thus, to investigate the determinant factors that influence MSEs in obtaining credit through fintech lending. Here, we emphasize virtual trust in fintech lending encompasing the dimension of social network, economic attributes, and risk perception based on several indicators that are used as proxies. Primary data used in the study was gathered from an online survey to the respondents of MSEs in Java. The result of the study indicates that determinants of MSEs in obtaining credit from lender through fintech lending are statistically influenced by internet usage activities, borrowing history, loan utilization, annuity payment system, completeness of credit requirement documents and compatibility of loan size with the business need. These factors have a significant effect on credit approval because they can generate virtual trust of fintech lender to MSEs as potential borrowers. It concludes that the probability of obtaining fintech loans in accordance with their expectations are influenced by the dimensions of social network, economic attributes and risk perception.

  • Presenting the impact of meteorological disasters departs from the traditional weather forecasting approach for meteorological phenomena. It is important to provide impact forecasts so that precautions against disruption and damage can be taken. Countries such as the United States, the U.K., and France already conduct impact forecasting for heavy rain, heavy snow, and cold weather. This study improves and applies forecasts of the impact of heavy rain among various weather phenomena in accordance with domestic conditions. A total of 33 impact factors for heavy rain were constructed per 1 km grids, and four impact levels (minimal, minor, significant, and severe) were calculated using standard normal distribution. Estimated criteria were used as indicators to estimate heavy rain risk impacts for 6 categories (residential, commercial, utility, community, agriculture, and transport) centered on people, facilities, and traffic.

Determinants of Optimal Breastfeeding Practices in Indonesia: Findings From the 2017 Indonesia Demographic Health Survey

  • Objectives: Understanding the factors influencing mothers' decision to breastfeed their infants is essential to formulate effective breastfeeding interventions. This study explored the determinants of optimal breastfeeding indicators in Indonesia. Methods: We used the 2017 Indonesia Demographic and Health Survey to analyze factors associated with early initiation of breastfeeding, exclusive breastfeeding (EBF), and continued breastfeeding at 1 year (CBF-1) and 2 years (CBF-2). Multivariate logistic regression models were used to examine bio-demographic, socio-cultural, and behavioral characteristics associated with breastfeeding after considering the survey design effect. Results: The risk of delayed breastfeeding initiation was higher among infants who were born smaller, first-born children, were delivered via cesarean delivery, and did not have immediate skin-to-skin contact (p<0.01). Infant's age, birth pattern, household wealth index, and the mother's occupation and smoking status were predictors of EBF (p<0.05). CBF-1 was less common among first-time mothers and those working in the non-agricultural sector, mothers from wealthier families, and mothers who had cesarean deliveries (p<0.01). Infant's age was negatively associated with CBF-2 (adjusted odds ratio [aOR], 0.85; 95% confidence interval [CI], 0.74 to 0.99). Mothers attending college were less likely to practice CBF-2 than those with no education or primary education (aOR, 0.45; 95% CI, 0.26 to 0.77). The absence of postnatal visits was a risk factor for CBF-1 and CBF-2 (p<0.05). Conclusions: Breastfeeding interventions in Indonesia should pay particular attention to at-risk groups such as women from wealthier families, working outside the agricultural sector, and with a higher education level. Nutrition-sensitive programs (e.g., postnatal care and smoking cessation) should also be encouraged.

The prevalence of hypertension in Haiti : A retrospective study

  • The purpose of this study was to explore the prevalence of hypertension in Haiti and to describe regional characteristics, and thus to speculate Haitian specific regional risk factors. This is a descriptive and comparative study. Subjects were 145 male/female Haitians, who aged between 30 and 59 and have lived in rural areas, had their hypertension indicators being screened. Data were analyzed with descriptive statistics, t-test and ANOVA. More than four fifth of the subjects (82.8%) showed pre-hypertensive or moderate to severe hypertensive condition. 65.5% of the total had abnormal pulse pressure ($PP{\geq}50$) and 13.8% of the sample showed tachycardia (Heart $Rate{\geq}100$). Female Haitian tended to have higher diastolic blood pressure and faster heart rate than male Haitian. The residents who lived in Part 5-1 area with less accessibility to drinking water tended to have higher systolic and diastolic pressure. Hydration status of Haitians was highly considered as an affecting factor on the distribution of hypertension as evidenced by different level of accessibility to drinking water.

  • A maintenance system for an offshore plant uses an optimal maintenance method, process, and period based on operation information data and economic evaluation techniques. Maintenance is performed after one or more indicators show that equipment is going to fail or that equipment performance is deteriorating. A maintenance system is based on the use of real-time data to prioritize and optimize the LNG-FPSO topside equipment resources.

Comparative Efficacy of Four Imaging Instruments for Breast Cancer Screening

  • Sensitivity and specificity are the two most important indicators in selection of medical imaging devices for cancer screening. Breast images taken by conventional or digital mammography, ultrasound, MRI and optical mammography were collected from 2,143,852 patients. They were then studied and compared for sensitivity and specificity results. Optical mammography had the highest sensitivity (p<0.001 and p<0.006) except with MRI. Digital mammography had the highest specificity for breast cancer imaging. A comparison of specificity between digital mammography and optical mammography was significant (p<0.021). If two or more breast diagnostic imaging tests are requested the overall sensitivity and specificity will increase. In this literature review study patients at high-risk of breast cancer were studied beside normal or sensitive women. The image modality performance of each breast test was compared for each.
