A cyclotron is a kind of particle accelerator that produces a beam of charged particles for the production of medical, industrial, and research radioisotopes. More than 30 cyclotrons are operated in Korea to produce $^{18}F$, an FDG synthesis at hospitals. A 30-MeV cyclotron was installed at ARTI (Advanced Radiation Technology Institute, KAERI) mainly for research regarding isotope production. In this study, we analyze and estimate the items of risk such as the problems in the main components of the cyclotron, the loss of radioactive materials, the leakage of coolant, and the malfunction of utilities, fires and earthquakes. To estimate the occurrence frequency in an accident risk assessment, five levels, i.e., Almost certain, Likely, Possible, Unlikely, and Rare, are applied. The accident consequence level is classified under four grades based on the annual permissible dose for radiation workers and the public in the nuclear safety law. The analysis of the accident effect is focused on the radioactive contamination caused by radioisotope leakage and radioactive material leakage of a ventilation filter due to a fire. To analyze the risks, Occupation Safety and Health Acts is applied. In addition, action plans against an accident were prepared after a deep discussion among relevant researchers. In this acts, we will search for hazard and introduce the risk assessment for the research 30-MeV cyclotron facilities of ARTI.
Background: A comprehensive, traceable, and easy-to-understand radiation risk indicator is desired for radiological protection. The early-onset hypothesis could be used for this purpose. Materials and Methods: An indicator for early death (IED) was developed and calculated using the epidemiological dataset from the 14th Report of the Life Span Study (LSS) of Hiroshima and Nagasaki. By clarifying the calculation process, IED for all-cause mortality was estimated. In addition, the characteristics of IED for solid cancer mortality and cardiovascular mortality as well as those of men and women, and their dependence on age at exposure were investigated for detailed analysis. Results and Discussion: The IED for all-cause mortality was estimated to be approximately 4 years for an acute radiation exposure of 1 Gy regardless of the fitting dose range. The cumulative death rate for all solid cancers also indicated the early-death tendency (approximately 7-10 years at 1 Gy). Although, there is a slight difference in the characteristics of the risk obtained from the LSS study and this study, it is considered that the IED in a unit of years can also be used to show the overall picture of risk due to radiation exposure. Conclusion: We developed and calculated the indicator for early death, IED, for the cumulative mortality rate of all causes of death, all solid cancers, and circulatory diseases. The quantitative values of IED were estimated to be 4 years for all causes of death, 7-10 years for all solid cancers. IED has an advantage for intuitively understanding the meaning of radiation risk since it can be obtained by a simple and traceable method.
Recently, much attentions are paid to the risk associated with increased uses of medium size radiation sources in medical and industrial fields. In this study, radiation risks to the worker and to the general public due to $^{99m}Tc$ generator were assessed for both normal and accident conditions. Based on the event tree technique, exposure scenarios for various situations were derived. Uncertainty analysis based on the Monte-Carlo technique was applied to the risk assessment for workers and members of the public in the vicinity of the work place. In addition, sensitivity analysis was performed on each of the five independent input parameters to identify importance of the parameters with respect to the resulting risk. Because the frequencies of normal tasks are fat higher than those of accidents, the total risk associated with normal tasks were higher than the accident risk. The annual dose due to normal tasks were $0.6mSv\;y^{-1}$ for workers and $0.014mSv\;y^{-1}$ for public, while in accident conditions $3.96mSv\;y^{-1}\;and\;0.0016mSv\;y^{-1}$, respectively. Uncertainty range of accident risk was higher by 10 times than that of normal risk. Sensitivity analysis revealed that source strength, working distance and working time were crucial factors affecting risk. This risk analysis methodology and its results will contribute to establishment of risk-informed regulation for medium and large radioactive sources.

Objectives To investigate the accuracy and scientific validity of the current very low risk factor for hereditary diseases in humans following exposures to ionizing radiation adopted by the United Nations Scientific Committee on the Effects of Atomic Radiation and the International Commission on Radiological Protection. The value is based on experiments on mice due to reportedly absent effects in the Japanese atomic bomb (A-bomb) survivors. Methods To review the published evidence for heritable effects after ionising radiation exposures particularly, but not restricted to, populations exposed to contamination from the Chernobyl accident and from atmospheric nuclear test fallout. To make a compilation of findings about early deaths, congenital malformations, Down's syndrome, cancer and other genetic effects observed in humans after the exposure of the parents. To also examine more closely the evidence from the Japanese A-bomb epidemiology and discuss its scientific validity. Results Nearly all types of hereditary defects were found at doses as low as one to 10 mSv. We discuss the clash between the current risk model and these observations on the basis of biological mechanism and assumptions about linear relationships between dose and effect in neonatal and foetal epidemiology. The evidence supports a dose response relationship which is non-linear and is either biphasic or supralinear (hogs-back) and largely either saturates or falls above 10 mSv. Conclusions We conclude that the current risk model for heritable effects of radiation is unsafe. The dose response relationship is non-linear with the greatest effects at the lowest doses. Using Chernobyl data we derive an excess relative risk for all malformations of 1.0 per 10 mSv cumulative dose. The safety of the Japanese A-bomb epidemiology is argued to be both scientifically and philosophically questionable owing to errors in the choice of control groups, omission of internal exposure effects and assumptions about linear dose response.
Lung cancer is the most prevalent global cancer, ${\sim}90%$ of which is caused by cigarette smoking. The LNT hypothesis has been inappropriately applied to estimate lung cancer risk due to ionizing radiation. A threshold of ${\sim}1\;Gy$ for lung cancer has been observed in never smokers. Lung cancer risk among nuclear workers, radiologists and diagnostically exposed patients was typically reduced by ${\sim}40%$ following exposure to <100 mSv low LET radiation. The consistency and magnitude of reduced lung cancer in nuclear workers and occurrence of reduced lung cancer in exposed non-worker populations could not be explained by the HWE. Ecologic studies of indoor radon showed highly significant reductions in lung cancer risk. A similar reduction in lung cancer was seen in a recent well designed case-control study of indoor radon, indicating that exposure to radon at the EPA action level is associated with a decrease of ${\sim}60%$ in lung cancer. A cumulative whole-body dose of ${\sim}1\;Gy$ gamma rays is associated with a marked decrease in smoking-induced lung cancer in plutonium workers. Low dose, low LET radiation appears to increase apoptosis mediated removal of $\alpha$-particle and cigarette smoke transformed pulmonary cells before they can develop into lung cancer.
Background: To investigate differentiated thyroid cancer risk factors in natives of French Polynesia is of interest because of the very high incidence of this cancer in the archipelago. Materials and Methods: To assess the role of various potential risk factors of thyroid cancer in the natives of French Polynesia we performed a case-control study. The study included almost all the French Polynesians diagnosed with differentiated thyroid carcinoma between 1981 and 2003 (n=229) and 373 French Polynesian control individuals from the general population without cancer. Results: Thyroid radiation dose received from nuclear fallout before the age of 15, a personal history of neck or/and head medical irradiation, obesity, tallness, large number of children, an artificial menopause, a familial history of thyroid cancer, a low dietary iodine intake, and having a spring as the main source of drinking water were found to be significant risk factors. No roles of smoking habits, alcohol consumption, iodine containing drugs, and exposure to pesticides were evidenced. Conclusions: Except for smoking, differentiated thyroid carcinoma risk factors in natives of French Polynesia are similar to those in other populations. Our finding on the role of having a spring as a drinking water origin is coherent with some other studies and could be due to geological factors.

C-arm fluoroscopy is useful equipment in interventional pain management because it helps to guide correct needle targeting for the accurate injection and drug delivery. However, due to increased use of C-arm fluoroscopy in various pain procedures, the risk of radiation exposure is a significant concern for pain physicians. The harmful biological effects of ionizing radiation on the human body are well known. It is therefore necessary to strive to reduce radiation exposure. Lead aprons with thyroid shields are the most fundamental radiation protective devices for interventional procedures, and are very effective. However, the operator's radiation safety cannot be guaranteed because pain physicians seem to lack sufficient interest, knowledge, and awareness about radiation safety. Also, inappropriate care and use of radiation protective devices may result in a higher risk of radiation exposure. The purpose of this article was to review the literature on radiation safety with a focus on lead aprons and thyroid shields and present recommendations related to those devices during C-arm fluoroscopic-guided interventions by pain physicians.
The increased risk of cancer with exposure to low-dose radiation was estimated through longitudinal study for radiation workers at the nuclear power plants in Korea. The radiation dose data were collected from the Radiation Safety Management System(RSMS) of the Korea Hydro & Nuclear Power Co., Ltd(KHNP). The cancer risks with radiation exposure were evaluated in terms of relative mortality ratios(RMR) and relative incidence ratios(RIR) to the unexposed employees at the nuclear power plants, and of the standardized mortality ratios(SMR) and standardized incidence ratios(SIR). There were no significant increases of canters of all sites in the exposed group either in RIR[1.08, 95% confidence interval(CI) 0.74-1.58] or in RMR[1.21, CI 0.70-2.08]. Neither SIR[0.81, CI 0.28-0.96] nor SMR[0.86, CI 0.66-1.10] significantly deviated from 1.0 for cancers of all sites. The trend analysis did not identify evident dose-response relationship due to insufficient numbers of the cases. Consequently, it is concluded that increases in cancer risks in the radiation worker group exposed to low doses at the nuclear power plants in Korea are not identified at this time.
The purpose of this study was to evaluate the cancer incidence rate and provide basic data by measuring the photoneutron dose generated during intensity-modulated radiation therapy and volumetric modulated arc therapy used in radiation therapy for prostate cancer. The optically stimulated luminescence albedo neutron dosimeter for neutron measurement was placed on the Rando phantom in the abdomen and thyroid and photoneutron dose generated was measured. As a result of the study, intensity-modulated radiation therapy (7 portal) was measured to be higher than volumetric rotational radiation therapy in both abdominal and thyroid locations. When the cancer incidence rate was evaluated using the nominal risk coefficient of ICRP 103, the cancer incidence rate due to exposure to the colon and thyroid during intensity-modulated radiation therapy was 9.9 per 1,000 people, and volumetric rotational radiation therapy for 1,000 people. It was 3.5 per person. Based on the principle of ALARA (As low as reasonably archievable), it is considered to be a guideline for minimizing the exposure dose to normal organs in the establishment of a radiation treatment plan.
The measured mean activities of $^{226}Ra,\;^{232}Th,\;^{40}K\;and\;^{137}Cs$ in the soil of Bahawalpur, Bahawalnagar and Rahimyar Khan Bistricts were 32.9, 53.6, 647.4 and 1.8 Bq $kg^{-1}$. The average absorbed dose rate calculated from these activities was 74.3 nGy $h^{-1}$ and the mean annual effective dose rate was found to be 0.46 mSv $y^{-1}$. Absorbed doses to different body organs were derived from annual effective doses using tissue weighting factors. Radiation induced fatal cancer risks were assessed by using ICRP 60 Model. Estimations incurred 184deaths per year due to cancer.
