Evaluation of Healthcare Organization Based Management Program in Korea - Using Chronic Care Model - (국내 보건의료기관 기반 청소년 비만관리 프로그램 현황 - 만성질환 관리모형을 중심으로 -)

  • Obesity of adolescents causes mental and physical problems as well as social problems, which need prevention and management. Although a number of systematic reviews and meta-analyses on obesity programs for adolescents were conducted, there is no study evaluating the programs based on CCM(Chronic Care Model), an organizing framework for improving chronic illness care. This study was conducted to review the features of studies in obesity management programs and interventions of the selected studies were evaluated in terms of inclusion of components of the Chronic Care Model. 4 databases were searched for relevant studies in obesity management programs, which were published from 1994 to 2014 in Korea. Results were analyzed in a qualitative way. 14 studies were satisfied inclusion criteria. The interventions most frequently utilized the elements of self-management support(66.7%) and only 1 of the studies included more than three components of CCM. This study presents the direction of health policies about managements of metabolic syndrome, which means that we identified effective process of the obesity management programs for adolescents in Korea and also this study will be used as a basic information for the development of obesity management program.

Effectiveness of Obesity management programs: systematic review and meta-analysis (메타분석을 이용한 비만관리 프로그램의 효과분석)

  • As overweight and obese people have increased, obesity management programs have generated much concern in Korea. Many types of obesity management programs were operated, aimed at reducing weight, BMI, body fat percentage and so on. This study was conducted to review the features of studies in obesity management programs and systematically assess the effects which were published from 1995 to 2006 in Korea. Databases were systematically searched for published data in Korea. It were KISS (Korean studies information services system) and KERIS (Korean Education research information system), which were major literature search systems for all academic fields in Korea. Total 114 studies were initially identified, of which 21 satisfied our inclusion data. The effects of obesity management programs were assessed on the results in weight, BMI (body mass index), HDL (high density lipids), body fat percentage, self-efficacy. Effect size estimated on the equation of M1 - M2 / Sp, where M1 was mean of the experimental group, M2 was the mean of control group, and Sp was the pooled standard deviation. Magnitude of effect size was interpreted by using Cohen's definition. Cohen described small, medium, and large effect sizes as 0.2, 0.5, and 0.8 respectively. The studies about obesity management programs were the most published in 2005(26.1%). Obesity management programs were popularly operated as the type of 12 weeks intervention, exercise intervention(52.4%), quasi-experimental study(85.7%). Ten studies reported significant effects on weight, and nine studies reported significant effects on BMI and HDL. Only five and three studies reported significant effects on body fat percentage and self-efficacy respectively. The effects of each outcome were generally significant when the studies included these elements, intervention over 10 weeks, evaluation over 3 times, a comprehensive program (exercise, nutrition education, behavior modification) and reinforcing factors (self-monitoring, group discussion, one's goal setting, and etc.). Effective obesity management programs should contain these essential elements and objectives of obesity management programs must be set out evident at the beginning. The participants should be registered, educated and evaluated by continuous obesity management programs.

A Comparative Study on the Local Governments' Environmental Impact Assessment Regulations in Korea (지방자치단체 환경영향평가 조례 비교 연구)

  • This study aims to survey whether local governments have legislated laws and regulations on environment impact assessment system, to compare and analyze specific projects and their scale, assessment items, and procedures & discussion process, to identify issues and generate improvement plans, and to suggest a direction for future legislation to local governments that plan to legislate laws and regulations in the future. Major outcome of the study are as follows. First, terminologies used for environment impact assessment by local governments need to be unified. Also, laws and regulations need to be legislated soon. Second, in "urban development project" areas, a total of nine project areas including "quarrying of soil and stone, sand, gravel, and minerals" was essential common projects. A total of six project types were added or newly established compared to national systems. Among them, four project types were added within national-level project areas and two project types were not available under the national-level project areas and newly added due to the nature of local governments. Third, in terms of project scale, scale enhancement of "urban development project" was most common. Analysis showed that in case of clean natural environment such as Cheju Island, it is necessary to consider reinforcing project areas where development activities take place directly in forests or rivers such as "industrial base and complex development", "water resource development", and "development of tourism complex." Fourth, the discussion and review procedures of assessment reports were similar to those at government level. However, in case of Seoul city, it is required to write a "preparation plan" before drafting an assessment report. The city features partial introduction of scoping and screening, which allows to exempt discussion procedures if impact on environment is found to be minimal after drafting the assessment report. In case of national-level, it has a dual system that is split between Ministry of Construction and Transportation and Ministry of Environment. However, in case of environment impact assessment of local governments, it is a single system where city mayors and provincial governors are in charge of both project execution and environmental assessment. Therefore, the most important task is how to satisfy objectiveness and accountability.

  • In the case of Korea, both of modern medicine and oriental medicine are admitted as medical practices in the system. In other words, healthcare system is dualized. However, medical practice that corresponds to oriental medicine in Korea is substitution of medical practice in cases of foreign countries. For use of medical devices, it is provided only for doctors and medical technician relevant to use. Particularly, although oriental medicine is recognized as orthodox medicine in terms of the features of Korean medical system, superintendency of oriental doctors is not identical with that of doctors for use of medical devices and superintendency toward medical technicians. Recently, Cheongju District Court decided that superintendency of oriental doctor upon physical therapist is not acknowledged. It can be said that the judgement is opposed to the original verdict which judged that oriental doctors' employment and guidance of oriental doctors upon physical therapist is permissible. Hence this study aimed to review on domestic medical law system, which is dualized, roles of medical professionals, intent of the medical license system, provisions related to medical technician law and relevant precedents. Regulations on practices other than licensed practices by medical professionals are made because medical practices may affect on danger toward life and body of human and public health also. Therefore, the nation regulates medical professionals having licenses to perform medical practices within the range of the licenses. It is clearly prescribed that medical technicians may perform medical practices under instructions of doctors or dentists pursuant to the medical technician law. In addition, the court also judges that it is out of the license of oriental doctors if they use CT devices and limits the use of modern medical devices by oriental doctors. That is to say that it limits oriental doctors' employment of medical technicians and pursuant of oriental doctors on medical technicians as well.

Comparative Study on Korea and French Men's Bat - During 17th Century to 18th Century - (한국과 프랑스의 남성 모자 비교연구 - $17{\sim}18$세기를 중심으로 -)

  • A hat is a general term for what human beings wear on their heads in order to protect them from the cold or heat, to be used for ornament, or to symbolize social position. A hat represents the position of a man who wears it, attitude toward the society, and faith for a religion for males. Since men's hats can create their image and uniqueness in casual wear from current fashion items, they are proposed as a fashion accessory item of designers each season. The purpose of this study is to review hats, which are part of costume, in a more analytic method. The research range of this study is hats for men in the upper class in the 17th to 18th centuries from the history of costume when diverse periodic and characteristic costume changes occurred. In the 17th to 18th centuries in the middle of the Chosun Dynasty, changes in costume occurred as the feudal society of the dynasty had been dissolved since the Japanese invasion in 1592. Political power was established in France as the Baroque Age began in the 17th century. Since European costume, especially women's costume led fashion in France, the country represented the age better than any other countries. Aristocratic costume in the Rococo Age of the 18th century was changed to completely different shape of costume after the French Revolution. In this regard, this age was deemed to be the most appropriate for the comparison or costume. Another purpose of this study is to review the common and different features of periodic factors that affect changes in costume by researching the hats of the two countries in the same age in order to understand relation between periodic characteristics and costume throughout history of costume.

Clinical Experience of Pyoderma Gangrenosum with Extensive Soft Tissue Necrosis (광범위 연조직 괴사를 동반한 괴저농피증의 치험례)

  • Purpose: Pyoderma gangrenosum is a rare cutaneous ulcerative disease. First described in 1930, the condition is characterized by progressive ulceration with deeply undermined purple-red edge. The lower extremities are most commonly affected but other parts of the skin and mucous membranes may also be involved. Although medical treatments with topical wound therapy are commonly used, surgical intervention is still controversial. In this paper, we report an atypical case of pyoderma gangrenosum which was characterized by extensive soft tissue breakdown. Methods: A 27-year-old male patient was referred to our institution with a $7{\times}8cm$ sized deeply undermined ulceration with pus-like discharge and fever. Incision and drainage was performed at another clinic 3 days prior to admission to our institution. After a thorough physical examination and the MRI review, a diagnosis of necrotizing faciitis was made. Accordingly, fasciotomy and debridement was performed. However, the wound enlarged progressively and the patient remained highly febrile for 9 days after the treatment. Septic screening did not reveal any occult infection. After a secondary review of the case, the initial diagnosis of necrotizing fasciitis was rejected and changed to pyoderma gangrenosum. With the use of dexamethasone intravenously, the wound improved dramatically and the fever was eliminated. Steroid mediation was tapered with duration of 1 month. The wound was stabilized and subsequently covered with split-thickness skin graft. Results: Split-thickness skin grafting with 1 : 1.5 mesh was successfully taken. Conclusion: Initial clinical features of pyoderma gangrenosum are very similar to that of necrotizing fasciitis. High fever and progressive ulceration with severe pain could invite earlier surgical approach. The advancing wound margins (the well defined violaceous, undermined border and necrotic ulcer base) and lack of isolation of pathogenic organism was used to make the correct diagnosis of pyoderma gangrenosum. We achieved a good result with proper medication and split-thickness skin graft.

Cauldron Subsidence and Ore Mineralization in the Southeastern kyongsang basin: A review (경상분지 남동부 광상의 분포와 콘트론과의 관계 : 재검토)

  • Nine cauldrons have been recognized in the PVD (Pusan- Taegu Volcano-tectonic Depression) zone covering an area of nearly 7,000 $km^{2}$. They form characteristic landscape features with various mountains in the southeastern Kyongsang basin. Economically important ore deposits are also developed either in the ring fracture zone or the central pluton within the resurgent cauldrons or in the marginal area of the PVD, suggesting that these cauldrons played a major role in the distribution of ore deposits in the southeastern Kyongsang basin. Furthermore, the cauldron subsidences were more frequent with the more felsic volcano-plutonic complex, possibly indicating that the amounts of water and volatile components also acted as a controlling factor to cause the caldera subsidence and to concentrate the ore-forming elements in economic concentrations. The review of the relationship and variations of ore mineralization and cauldron subsidence is rather sketchy, but it provides a skeleton to carry out more detailed and quantitative studies related to temporal and spatial relationships between each cauldron subsidence accompanying its own ore mineralization. In the southeastern Kyongsang basin, additional calderas and associated ore deposits undoubtedly can be discovered through future detailed studies. The concept that cauldron subsidence are an important control for the formation of ore deposits will appear to be vindicated.

History of Research on Pharmacopuncture in Korea

  • Objectives: This study introduces the history and types of Korean pharmacopuncture and reports trends of research on Korean pharmacopuncture. Methods: Pharmacopuncture studies were searched from the first year of each search engine to 2014 by using seven domestic and foreign search databases. Selected studies were divided into the history of pharmacopuncture, kinds and features of pharmacopuncture, research types, and experimental and clinic studies and were then classified by year of publication, type of pharmacopuncture, disease, and topic. Results: Pharmacopuncture can be classified into four large groups: meridian field pharmacopuncture (MFP), eight-principles pharmacopuncture (EPP), animal-based pharmacopuncture (ABP) and mountain-ginseng pharmacopuncture, which is a single-compound pharmacopuncture (SCP). The largest numbers of studies were reported from 1997 to 2006, after which the numbers decreased until 2014. Of experimental studies, 51.9%, 18.7%. 14.3%, 9% and 3.4% were on SCP, ABP, MFP, formula pharmacopuncture (FP), and EPP, respectively. Of clinical studies, 54.7%, 15.3%. 14.9% 10.0% and 1.5% were on ABP, MFP, EPP, SCP, and FP (1.5%), respectively. Among clinical studies, case reports and case series accounted for 76.5%, followed by randomized controlled trials (RCTs, 16.4%) and non-RCT (13.9%). Musculoskeletal diseases, toxicity and safety tests, anti-cancer effects, and nervous system diseases were mainly treated in experimental studies while musculoskeletal diseases, nervous system diseases, toxicity and safety tests, and autonomic nerve function tests were addressed in clinical studies. Bee venom (BV) was the most frequently-used pharmacopuncture in mechanism studies. Pharmacopuncture was mainly used to treat musculoskeletal diseases. Conclusion: Pharmacopuncture and studies of it have made great progress in Korea. Studies on BV pharmacopuncture and musculoskeletal diseases accounted for most of the studies reported during the review period. Research on the types of pharmacopuncture and diseases has to be expanded. Especially, studies on the use of MFP and EPP for treating patients with various diseases are needed.

A Comparative Study on the Local Governments' Environmental Impact Assessment Systems between Korea and Japan I - forcusing on target projects and assessment items - (한국과 일본의 지방자치단체 환경영향평가 제도 비교 연구 I - 대상사업 및 평가항목을 중심으로 -)

  • This study aims to review the environmental impact assessment systems of Japanese local governments, to compare its outcome with the systems of Korean local governments, and to suggest the development direction for the environmental impact assessment systems of Korean local governments based on the result. The study results showed that, first, due to distinction in the political systems of Japan and Korea, while Japan has bottom-up environmental impact assessment systems, Korea has topdown environment impact assessment systems. Second, although introduction of national systems on planning was put on hold as a future initiative, local governments including Tokyo and Saitama are experimenting evaluation procedures tailored to local features by legislating them as regulations. On the other hand, while 'prior environmental review system' was in effect as a national system, nothing was practiced at all by local governments. Third, in a total of 47 Japanese local governments, about 45 projects and 44 assessment items were added to local government environmental impact assessment only and designated as target projects and assessment items. Fourth, in both national and local environment impact assessment systems in Japan, screening procedures to determine assessment by separating into Type 1 projects and Type 2 projects and scoping procedures to discuss assessment items in advance were introduced and in effect. This Japanese EIA system may serve as a good reference to the Korean national and local government EIA systems.
