A Review of Korean Medicine Treatment for Managing the Thoracolumbar Compression Fractures: A Retrospective Observational Study (흉요추 압박골절 치료에 대한 한의복합치료 고찰: 후향적 관찰 연구)

  • Objectives This study aims to find out effect of Korean medicine treatment on managing thoracolumbar compression fractures through retrospective observational study. Methods Among hospitalized patients at the Department of Korean Medicine Rehabilitation from January 1st, 2018 to February 28th, 2023, a total of 24 inpatients who were diagnosed with thoracolumbar compression fractures and received Korean medicine treatment were included in this study. Numeric rating scale (NRS) was used for pain assessment and clinical variables such as sex, symptoms, age, thoracolumbar injury classification and severity (TLICS) scores were collected. For subgroup analysis to analyze factors affecting treatment response, we divided patients into responders and non-responders according to NRS change. For statistical analysis, we compared before/after hospitalization and analyzed distinct features between two groups. Results Most of the patients were in their 70s and 83.33% were female. Average hospitalization period was 24.54±11.91 days. All patients had back pain as their chief complaint and only 2 patients received surgeries. In TLICS, only 1 patient got score 6, which represented surgery indication. After Korean medicine treatment, NRS of almost every patient got lower significantly at the time of discharge (3.02±1.93) than admission (5.52±1.95). Comparing two groups, responders had lower NRS at the time of discharge and TLICS score of them were lower than non-responders. Conclusions Our results show that Korean medicine treatment for thoracolumbar compression fractures was effective in reducing pain. There were distinct clinical features such as age, past history, surgeries between those with significant improvement in pain scores and those who did not.

  • Xanthogranulomatous (XG) inflammatory disease is a rare benign disease involving various organs, including the gallbladder, bile duct, pancreas, spleen, stomach, small bowel, colon, appendix, kidney, adrenal gland, urachus, urinary bladder, retroperitoneum, and female genital organs. The imaging features of XG inflammatory disease are nonspecific, usually presenting as a heterogeneous solid or cystic mass. The disease may also extend to adjacent structures. Due to its aggressive nature, it is occasionally misdiagnosed as a malignant neoplasm. Herein, we review the radiological features and clinical manifestations of XG inflammatory diseases in various organs of the abdomen and pelvis.

Outcomes and predictors of response to endotherapy in pancreatic ductal disruptions with refractory internal and high-output external fistulae

  • Backgrounds/Aims: Endoscopic retrograde cholangiopancreatography (ERCP) remains the primary treatment for a subset of patients with pancreatic fistulae. The objective of this study was reporting outcomes of ERCP and predictors of resolution in patients with pancreatic fistulae refractory to conservative therapy. Methods: Retrospective review of patients who underwent ERCP and pancreatic stent placement for pancreatic fistula not responding to medical therapy was performed. Clinical features, laboratory parameters, radiological features and pancreatogram findings were noted. Clinical resolution of fistula was the primary outcome measure. Results: Sixty-eight patients underwent ERCP for high-output pancreatic fistula (Mean age 34.1 years, 91.1% males, 35/68 chronic pancreatitis, 52.9% alcohol etiology). Internal fistulae (pancreatic ascites, pleural effusion, or pericardial effusion) were seen in 55 (80.9%) patients and external fistula in 13 (19.1%) patients. Technical success for ERCP was 92.6% (63/68). Leak was seen in 98.4% (62/63). The most common leak site was body (69.8%). Multiple leak sites were seen in 23.1%. Pancreatic stricture was found in 36.5%. In 44 (69.4%) patients, stent was placed beyond the site of the leak. Resolution at six weeks was achieved in 76.4% (52/68). On univariate and multivariate analyses, placement of stent beyond site of leak was significantly associated with resolution of high-output fistulae (3/41 [7.3%] vs. 5/19 [26.3%], p = 0.03; odds ratio: 6.5, 95% confidence interval: 1.211-34.94). Conclusions: In our experience, ERCP was successful in 76% of patients with pancreatic fistulae refractory to conservative therapy. Stent placement beyond the site of leak was associated with higher resolution of fistulae.

Clinical Characteristics of Pain in Patients with Parkinson's Disease Who Have Visited a Korean Medical Hospital : A Retrospective Chart Review (한방병원에 내원한 파킨슨병 환자의 통증의 임상적 특성에 관한 후향적 의무기록 분석 연구)

  • Objectives: To investigate the prevalence of pain, clinical characteristics of pain, association between clinical features and pain of patients with Parkinson's Disease(PD). Methods: We undertook a retrospective review of the medical records of patients diagnosed with PD between 2012 and 2019 at Kyung Hee University Korean Medicine Hospital at Gangdong in South Korea. Results: A total of 172 PD patients met entry criteria and 147 out of 172 patients(85.5%) reported pain. In comparison with general population, PD patients has high prevalence of pain. Female PD patients more frequently reported pain than male (P=0.03). 102 out of 147patients(69.3) complained of musculoskeletal pain, and musculoskeletal pain show significant difference depending on the PD motor subtypes (P=0.039). Pain was mainly locatedin the leg (57.8%) in all PD motor subtypes. Tremor-dominant PD more frequently felt pain in upper limb than postural instability-gait difficulty dominant(PIGD) PD, but it was not statistically significant. Conclusions: These findings showed high prevalence of pain in PD patients, the correlation between female and pain, and the relationship between PD motor subtype and pain type. Our study can contribute to the clinical approach based on a more in-depth understanding of PD patients with pain.

A retrospective analysis of risk factors of oromaxillofacial infection in patients presenting to a hospital emergency ward

  • Background: The purpose of this study was to review the clinical features of oromaxillofacial infections in patients presenting to a hospital emergency ward, to identify the key factors affecting the requirement for hospitalization, and the potential risk factors predisposing to a prolonged length of hospital stay. Methods: A retrospective medical record review of the 598 patients treated for oromaxillofacial infection from 2013 to 2017 at the oral and maxillofacial surgery department, Yangsan Pusan National University Hospital, was conducted. The following information was collected from each patient: sex, age, past medical history, site of infection, etiology, admission or outpatient care, level of C-reactive protein (mg/dL), fascial spaces involved, treatment method, and duration of hospitalization. Chi-squared tests were used to identify risk factors, which were further analyzed using multivariable logistic regression. Results: A total of 606 patients were eligible for inclusion in the study, of which eight were excluded due to having incomplete charts; thus, 598 patients were included: 55% were male, mean patient age was 47.1 ± 19.9 years, and 12.9% of patients were diabetic. Furthermore, 71.2% of patients had infection originating in the mandible; the most common tooth of origin was lower posterior, and 29.8% of patients were hospitalized. Risk factors for hospital admission were elderly patients with concurrent disease, elevated C-reactive protein level, and multiple-space infection in the oromaxillofacial area. The duration of hospitalization was correlated with both diabetes and age. Conclusions: The requirement for hospital admission is determined by the severity of the infection; even severe infections, once treated with appropriate surgery, have no relation to the length of hospital stay. The important risk factors for increased duration of hospitalization are diabetes mellitus and older age. The understanding of risk factors associated with a prolonged hospital stay during the treatment of oromaxillofacial infection will aid in treatment planning as well as highlight the importance of adequate diabetes control in patients at risk of such infection.

Prognosis of Eight Chinese Cases of Primary Vaginal Yolk Sac Tumor with a Review of the Literature

  • Background: Primary vaginal yolk sac tumor is a rare malignancy in the pediatric population, and a diagnostic challenge and appropriate initial treatment remains unsolved. The aim of this study was to investigate the clinicopathologic features, treatment and prognosis of this tumor. Materials and Methods: Eight cases of primary vaginal yolk sac tumor were reported with a literature review. Results: There were 4 pure yolk sac tumor cases and four mixed germ cell tumors containing yolk sac tumor element, including two cases with embryonal carcinoma and two cases with embryonal carcinoma and dysgerminoma. Partial vaginectomy was performed in four cases and all patients received chemotherapy. 85 cases in literatures were reviewed and 9 cases were misdiagnosed. Follow-up data was available in 77 cases and 5-year overall survival rate was 87.6%. 5-year survival rate of biopsy with chemotherapy, conservative surgery with chemotherapy and radical surgery with chemotherapy was 91.1%, 100% and 28.6%, respectively (p<0.001). Compared to cases without relapse or metastasis after initial treatment, patients with relapse or metastasis had a shorter overall survival (35.6% vs 96.6%, p<0.001). Conclusions: Mixed germ cell tumor containing yolk sac tumor element was not uncommon and partial vaginectomy may be a good choice for primary vaginal mixed yolk sac tumor type to eradicate local tumor cells and provide complete information for pathological diagnosis and postoperative adjuvant therapy.

A review of the taxonomic and ecological characteristics of Korean mistletoe types (Viscum, Korthalsella, Loranthus and Taxillus) (한국산 겨우살이류(Viscum, Korthalsella, Loranthus and Taxillus)의 분류 및 생태학적 특성 고찰)

  • In general, studies of aerial parasitic plants known collectively as mistletoe have been carried out to investigate their ecological and agricultural characteristics. However, with the recently increased level of interest in medicinal resources, research on different types of Korean mistletoe has also increased. This study was carried out to review the work on the taxonomy and ecology of Korean mistletoe in preparation for the industrial use of these plants in the future. Mistletoe types are flowering plants belonging to Santalales, which exist in the form of parasites on the branches of trees or shrubs. In Korea, five taxa of four genera in two families of mistletoe exist: Viscum coloratum (Komarov) Nakai f. coloratum, Viscum coloratum (Komarov) Nakai f. rubroaurantiacum (Makino) Kitagawa and Korthalsella japonica (Thunb.) Engl. in Santalaceae, along with Loranthus tanakae Franch. et Sav. and Taxillus yadoriki (Sieb. ex Maxim.) Danser in Loranthaceae. As taxonomic studies of these species remain insufficient and given that the distribution ranges of these species are very wide, further observations pertaining to the morphological variations in each species are necessary. The distribution of mistletoes is known to be determined by the host specificity, the interval between the hosts, the environmental condition, the habits of the host plant, the eating characteristics of mediators in the area, and their habitat selection features.

Reinforcement of Rubber Properties by Carbon Black and Silica Fillers: A Review

  • Enhancing the properties of rubber, such as the tensile strength, modulus, and wear abrasion, by the addition of carbon black and silica as fillers is very important for improving the performance of rubber products. In this review, we summarize the general features of 'the reinforcement of rubber by fillers' and the equations for representing the reinforcement phenomena. The rubber reinforcement was attributed to enhancement of the following: the rubber, bound rubber, formation of networks, and combination between rubber chains and silica followed by entanglement. The reinforcement capability of silica species with different surface and networked states demonstrated the importance of the connection between the silica particles and the rubber chains in achieving high reinforcement. The model involving combination followed by entanglement can provide a plausible explanation of the reinforcement of rubber by carbon black and silica because the combination facilitates the concentration of rubber chains near the filler particles, and entanglement of the rubber chains around the filler particles enforces the resistance against deformation and breakage of rubber compounds, resulting in high reinforcement.

  • As cloud computing has emerged as a hot trend in the IT industry, various types of cloud services have emerged. In addition, cloud service broker (CSB) technology has emerged to alleviate the complexity of the process of selecting the desired service that user wants among the various cloud services. One of the key features of the CSB is to recommend the best cloud services to users. In general, CSB can use a method to evaluate a service by receiving feedback about a service from users in order to recommend a cloud service. However, since each user has different criteria for giving a rating, there is a problem that reliability of service evaluation can be low when the rating is only used. In this paper, a method is proposed to supplement evaluation of rating based service by applying machine learning based sentiment analysis to cloud service user's review. In addition, the CSB prototype is implemented based on proposed method. Further, the results of comparing the performance of various learning algorithms is proposed that can be used for sentiment analysis through experiments using actual cloud service review as learning data. The proposed service evaluation method complements the disadvantages of the existing rating-based service evaluation and can reflect the service quality in terms of user experience.

Understanding STEM, STEAM Education, and Addressing the Issues Facing STEAM in the Korean Context (STEM, STEAM 교육과 우리나라 융합인재교육의 이해와 해결 과제)

  • Since 2011, after beginning of the systematic study on STEAM education, South Korea has developed a number of related programs. At this point we see that this is the time to clarify the challenges. The purpose of this study is to clarify the characteristics of their education through the review of many domestic and foreign papers in order to propose the challenges of STEAM education of Korea. The results are as follows. First, the course of integration cannot be separated by the difference in superiority but should be separated in accordance with the purpose of integration. Second, curriculum integration such as STEAM education is characterized by the emphasis on horizontal linking than vertical depth fusion. Accordingly, the content knowledge and vertical linkages are inevitably weakened. In order to overcome this problem, the key concepts and features that were emphasized in the previous curriculum need to also be emphasized in STEAM training, and the comparative study on the core concept and function of each subjects should be preceded. Third, after looking upon the current situation of our country's fusion research and talent training, the limits and the challenges that need to be overcome has been suggested. Fourth, with the basis on research results, we offered an example of the approach on STEAM education which is applicable to the current situation and proposed the challenges and implications that need to be addressed in the STEAM education of Korea in educational contexts such as curriculum, teaching and learning, and evaluation.
