A Case of Vocal Cord Paralysis Following Endotracheal General Anesthesia (삽관마취후의 성대마비 1례)

  • Endotracheal intubation is common practice for either general anesthesia or keeping artificial airway in various conditions. Despite of its great usefulness, however, laryngeal edema, ulceration, and granuloma are occasionally developed following endotracheal intubation as posttraumatic complications. Vocal cord paralysis is also rarely developed. Recently we experienced a case of left vocal cord paralysis with accompanying hoarseness following endotracheal general anesthesia for appendectomy. The symptoms of hoarseness and sore throat developed immediately after the general anesthesia. Periodic check revealed gradual return to normal function of the paralyzed cord with the complete function about 3 months after the anesthesia.

  • The purpose of this study is to analysis the concept of location efficiency, a base for the qualitative evaluation of TOD, and to propose the method of urban design for the improvement of location efficiency. Because of the environmental crisis by urbanization, sprawl and global warming, the importance of the solution through TOD has been increasing. For the future development of TOD, it becomes important to evaluate TOD qualitatively. From the study, it is grasped that location efficiency contains the concepts of value recapture, QoL and choice, financial return, and efficient regional land use and patterns. And the method of urban design for location efficiency is proposed as the followings: Form Based Zoning as land use plan, TOD and TOC model as node function, and street systems, mixed-use development, characteristic housing with various types, and networking of neighborhood facilities as place function.

  • Software Defined Networking (SDN) is a novel approach that have accelerated the development of numerous technologies such as policy-based access control, network virtualization, and others. It allows to boost network architectural flexibility and expedite the return on investment. However, this increases the system's complexity, necessitating the expenditure of dollars to assure the system's security. Network Function Virtualization (NFV) opens up new possibilities for network engineers, but it also raises security concerns. A number of Internet service providers and network equipment manufacturers are grappling with the difficulty of developing and characterizing NFVs and related technologies. Through Moodle's efforts to maintain security, this paper presents a detailed review of security-related challenges in software-defined networks and network virtualization services.

  • A comprehensive evaluation of cardiac function includes information in relation to cardiac output and systemic venous return. The heart is composed of four chambers: two atria and two ventricles, each with its own unique mechanical function. These four cardiac chambers, their valves, and the pulmonary circulation system are inter-related as they preload or afterload on each other. Cardiac dysfunction is a failure of global cardiac function, resulting in typical clinical manifestations. To investigate the underlying cause of cardiac dysfunction, a step-by-step evaluation of cardiac blood flow tracks is necessary. In this context, imaging markers showing details of the cardiac structures have an important role in assessing cardiac function. An image-based evaluation allows for investigation of function in terms of individual cardiac components. Evaluation of cardiac function using cardiac CT has recently been validated. This review aimed to discuss cardiac CT-based imaging markers for comprehensive and detailed cardiac function assessment.

Measured Return Loss and Predicted Interference Level of PCB Integrated Filtering Antenna at Millimeter-Wave

  • In this paper, an experimental investigation for return loss and a software-based prediction for interference level of single-packaged filtering antenna composed of dielectric waveguide filter and PCB(Printed Circuit Board) slot antenna in transceiver module have been carried out with several different feeding structures in millimeter-wave regime. The implementation and embedding method of the existing air-filled waveguide filters working at millimeter-wave frequency on general PCB substrate have been described. In a view of the implementation of each components, the dielectric waveguide embedded in PCB and LTCC(Low Temparature Co-fired Ceramic) substrates has employed the via fences as a replacement with side walls and common ground plane to prevent energy leakage. The characteristics of several prototypes of filtering antenna embedded in PCB substrate are considered by comparing the wideband and transmission characteristics as a function of bent angle of transmission line connecting two components. In addition, as an essential to the packaging of transceiver module working at millimeter-wave, miniaturization technology maintaining the performances of independent components and the important problems caused by integrating and connecting the different components in different layers are described in this paper.

  • A new approach has been proposed for the incremental analysis of the nonsteady state large deformation of planar anisotropic elastic-plastic sheet forming. A mathematical brief review of a constitutive law for the incremental deformation theory has been presented from flow theory using the minimum plastic work path for elastic-plastic material. Since the material embedded coordinate system(Lagrangian quantity) is used in the proposed theory the stress integration procedure is completely objective. A new return mapping algorithm has been also developed from the general midpoint rule so as to achieve numerically large strain increment by successive control of yield function residuals. Some numerical tests for the return mapping algorithm were performed using Barlat's six component anisotropic stress potential. Performance of the proposed algorithm was shown to be good and stable for a large strain increment, For planar anisotropic sheet forming updating algorithm of planar anisotropic axes has been newly proposed. In order to show the effectiveness and validity of the present formulation earing simulation for a cylindrical cup drawing and front fender stamping analysis are performed. From the results it has been shown that the present formulation can provide a good basis for analysis for analysis of elastic-plastic sheet metal forming processes.

  • This paper describes the optimal home positioning algorithm of Eclipse-II, a new conceptual parallel mechanism for motion simulator. Eclipse-II is capable of translation and 360 degrees continuous rotation in all directions. In unexpected situations such as emergency stop, riders have to be resituated as soon as possible through a shortest translational and rotational path because the return paths are not unique in view of inverse kinematic solution. Eclipse-II is man riding. Therefore, the home positioning is directly related to the safety of riders. To ensure a least elapsed time, ZYX Euler angle inverse kinematics is applied to find an optimal home orientation. In addition, the subsequent decrease of maximum acceleration and jerk values is achieved by combining the optimal return path function with cubic spline, which consequently reduces delivery force and vibration to riders.

  • There have been many studies to build a model that can help investors construct optimal portfolio. Most of the previous models, however, are based upon the path-breaking Markowitz model (1959) which is a quantitative model. One of the most important problems with that kind of quantitative model is that, in reality, most of the investors use not only quantitative, but also qualitative information when they select their optimal portfolio. Since collecting both types of information from the markets are time consuming and expensive, making a set of target assets smaller, without suffering heavy loss in the rate of return, would attract investors. To extract only desired assets among all available assets, we need knowledge that identifies investors' preference for the risk of the assets. This study suggests two-layer decision-making rules capable of identifying an investor's risk preference and an architecture applying them to a quantitative portfolio model based on risk and expected return. Our knowledge-based portfolio system is to build an investor's preference-oriented portfolio. The empirical tests using the data from Korean capital markets show the results that our model contributes significantly to the construction of a better portfolio in the perspective of an investor's benefit/cost ratio than that produced by the existing portfolio models.

  • The goal of rehabilitation after ACL reconstruction are return the patient to a reinjury level of activity with stable joint, removing pivot shift phenomenon, preservation of meniscus, restoration of range of motion, and minimize patello-femoral complication. The ACL reconstruction should avoid immediate surgery. The preoperative phase emphasizes two important factors. (1) The patient should have a resolution of knee swelling, a return of full ROM, and a normal gait. (2) The patient should be mentally prepared for the operation and subsequent rehabilitation. The postoperative rehabilitation program emphasizes extension, closed kinetic chain function exercises. The regular follow-up is important.

  • We can sec the case of hand injury patient in the industry field. Hand injury has more particularity than other injury cases. Because it is densely a lot of muscle, tendon, bone, vessels, and in progressing speedily to fibrosis, adhesion, stiffness than other joints. If it has baud injury, it is important to the physical therapy in early stage after operation. Otherwise, it is difficult to recover the function. Hand malfunction intimately has to do with the return to work and the grade of disability. There are many different hand injury cases but, we want to investigate several cases. : fracture. joint injury, tendon injury, and want to looking for treatment tine and method about these. So, we'll understand hand injury of industry disaster. and acknowledge of the importance of physical therapy in the early stage through these.
