  • Mouthguards were used to protect boxers from lip lacerations and other soft tissue injuries in the late 19th century. Now they are used various parts of dental treatment, which are sports protective aid, bleaching tray, orthodontic retainer, implant insertion guide tray, splint and so on. Repeated dislodgement of Class V restoration due to habitual clenching stress should be restored with stress control. Mouthguard can be used as stress relief device. This case describes methods that can relieve occlusal force to teeth by using mouthguard. Satisfactory results can be obtained by using mouthguard for retention of repeated dislodgement Class V restorations. If patients suffered from repeated restorations of Class V due to clenching, mouthguard can be used additional device to relieve the occlusal stress in conservative dentistry.

Marginal fit of anterior 3-unit fixed partial zirconia restorations using different CAD/CAM systems

  • PURPOSE. Few studies have investigated the marginal accuracy of 3-unit zirconia fixed partial dentures (FPDs) fabricated by computer-aided design/computer-aided manufacturing (CAD/CAM) system. The purpose of this study was to compare the marginal fit of zirconia FPDs made using two CAD/CAM systems with that of metal-ceramic FPDs. MATERIALS AND METHODS. Artificial resin maxillary central and lateral incisors were prepared for 3-unit FPDs and fixed in yellow stone. This model was duplicated to epoxy resin die. On the resin die, 15 three-unit FPDs were fabricated per group (45 in total): Group A, zirconia 3-unit FPDs made with the Everest system; Group B, zirconia 3-unit FPDs made with the Lava system; and Group C, metal-ceramic 3-unit FPDs. They were cemented to resin dies with resin cement. After removal of pontic, each retainer was separated and observed under a microscope (Presize 440C). Marginal gaps of experimental groups were analyzed using one-way ANOVA and Duncan test. RESULTS. Mean marginal gaps of 3-unit FPDs were $60.46{\mu}m$ for the Everest group, $78.71{\mu}m$ for the Lava group, and $81.32{\mu}m$ for the metal-ceramic group. The Everest group demonstrated significantly smaller marginal gap than the Lava and the metal-ceramic groups (P<.05). The marginal gap did not significantly differ between the Lava and the metal-ceramic groups (P>.05). CONCLUSION. The marginal gaps of anterior 3-unit zirconia FPD differed according to CAD/CAM systems, but still fell within clinically acceptable ranges compared with conventional metal-ceramic restoration.

A THREE DIMENSIONAL FINITE ELEMENT STRESS ANALYSIS ON THE FORCE DISTRIBUTION BY DISTAL EXTENSION PARTIAL DENTURES EMPLOYING ATTACHMENTS (Attachment를 이용한 후방연장 국소의치에 의한 압력분산에 관한 3차원 유한요소분석)

  • Although attachments have been utilized for a retainer of removable prostheses during several decades, there is little information on the force distribution by the attachments. This study was undertaken to evaluate the stress patterns developed by partial dentures employing rigid attachments on the supporting structures. Four types of the mandibular removable partial dentures were designed depending upon the position of the rigid attachment and the existency of lingual bracing. Under 100N of vertical and 25N of lateral loads on the 1st and 2nd lower molar of partial denture, stress distribution patterns and displacement were analysed with three dimensional finite element method by ANSYS version 5.3. 1. The highest stresses were concentrated on the distal alveolar crest of posterior abutment and the second stresses on the apical region of posterior abutment in the models. 2. The greatest displacement were shown on the distal alveolar crest of posterior abutment and the second displacement on the distal alveolar crest of anterior abutment in the models. 3. There was little difference between the models with intraoral attachment and those with extracoronal attachment. 4. There was little difference between the models with and without the lingual bracing.

The effect of bonded resin surface area on the detachment force of lingual bonded fixed retainers: An in vitro study

  • Objective: The aims of this study were to evaluate the relationship between the detachment force and bonding resin surface are and to determine the resin bonding surface area that would provide adequate bonding strength with minimum resin volume. Methods: One hundred and sixty human premolars were randomly divided into 4 groups of 40 teeth each. The diameter of the resin surface area in each group was as follows: group 1, 1.5 mm; group 2, 2.5 mm; group 3, 3.5 mm; and group 4, 4.5 mm. Respond Dead Soft straight (length 0.0175 inch) was used to fabricate the retainers, and $Transbond^{TM}$ XT was used to fix the retainers to the tooth surfaces. A pair of teeth was embedded in acrylic blocks for each specimen. Thus, each group comprised 20 samples. Fixed retainers were bonded to the teeth, and vertical force was applied at the middle of wire. The force was measured using a universal testing machine. Results: The mean value of detachment force was the highest for group 4 ($102.38{\pm}2.92N$), followed by group 3 ($63.54{\pm}2.21N$), group 2 ($51.95{\pm}1.61N$), and group 1 ($24.14{\pm}1.38N$). Conclusions: The detachment force of lingual fixed retainers was significantly affected as the area of the resin bonding surface increased. Considering the minimum bonding strength of brackets, a resin bonding surface area with a diameter of 3.5 mm would provide adequate bonding strength.

Comparison between fiber-reinforced polymers and stainless steel orthodontic retainers

  • Objective: The aim of this study was to examine the properties of fiber-reinforced composite and stainless steel twisted retainers for orthodontic retention. Methods: Three different span lengths (5.0, 8.0, and 14.0 mm) of fiber-reinforced composite were investigated. The three fiber-reinforced composite retainer groups were subdivided according to the storage condition (dry and wet), resulting in a total of six groups. Each stainless steel and fiber-reinforced composite group was comprised of six specimens. The three-point bending flexural test was conducted using a universal testing machine. ANOVA was used to assess differences in the maximum load and maximum stress according to the span length, material, and storage condition. Post-hoc comparisons were performed if necessary. Results: The maximum stress and maximum load were significantly (p < 0.001) associated with the span length, material, and storage condition. The significant interaction between the material and span length (p < 0.001) indicated the differential effects of the material for each span length on the maximum stress and maximum load, with the difference between materials being the highest for the maximum span length. Conclusions: Our findings suggest that fiber-reinforced composite retainers may be an effective alternative for orthodontic retention in patients with esthetic concerns or allergy to conventional stainless steel wires.

A Study on the Phased Cultural Product Design with Characteristics of Unlined Cheollik in Early 17th Century and Baby Typed Doll's Body (17세기 초 홑철릭 유물의 시대특성과 유아형 인형의 체형특성을 응용한 단계별 복식문화상품 디자인 연구)

  • This study will broaden the application of doll costume product with historical characteristics and the image of unlined cheollik in the $17^{th}$ century period of transition during the Joseon Dynasty. Historical design sources were extracted from old documents and precedent studies. Unlined cheollik of Shin Gyeong-yu, meritorious retainer, were selected as main reference-relic because of various fabric, preserved conditions and definite shape. 'Baby doll' was selected as main model because of consumer preferences and awareness. Design sources from unlined cheollik in the early $17^{th}$ century were about a 1:2 ratio of upper and under parts, removable separated doori-somae, knife-shaped collar (outside), projected square collar (inside), long pleats line, side slit, traditional flat fell seam sewing. Trapezoidal side line, round waistline, and thin fabric were applied in designs because of toddler-body of doll model. Three designs were produced in step 1: Cheollik A focusing on the historical remake (traditional type), Cheollik B with belt and side slit (crossover type), and Cheollik C with back- opening (modern type). In step 2, interview with fashion major student was conducted to increase the utility of designs. As a result, modern trend sources (frill, velcro, round armhole line, slope of sleeves, and floral pattern) were reflected in Cheollik B, C. Finally, three doll cheollik and cheollik-styled doll apron sample were produced. Various versions must be suggested in the study of doll costume products with traditional sources that balance historical characteristics and practicality to improve customer satisfaction.

Thermo-Elastic Analysis, 3-Dimensional Stress Analysis and Design of Carbon/Carbon Brake Disk (탄소/탄소 브레이크 디스크의 열탄성 해석과 3차원 응력해석 및 설계)

  • This paper presents the thermo-elastic analysis for searching the behavior of carbon/carbon brake system during the braking period and the 3-D stress analysis to find the shape of the brake disk which is safe to the failure. The mechanical properties of the carbon/carbon brake disk were measured for both in-plane and out of plane directions. The mechanical properties were used as the input of the thermo-elastic analysis and 3-dimensional stress analysis for the brake disk. The gap between rotor clip and clip retainer is an important parameter in the loading transfer mechanism of the rotor disk. The change of gap was considered both the mechanical deformation and thermal deformation. Because the rotor clip and clip retainers were not contacted, they were excluded from the analysis model. Rotor disk was modeled by using the cyclic symmetry condition. The contact problems between rotor clip and key drum as well as between rotor disk and rotor were considered. From the results of the 3-D stress analysis, the stress concentration at the key hole of the brake disk was confirmed. The stress distributions were studied thor the variation of the rotation angle of the contact surface and the radius of curvature at the key hole part.

A STUDY ON THE SHEAR BOND STRENGTH BETWEEN RESIN-BONDED RETAINERS AND ENAMEL ACCORDING TO THE ADHESIVE RESINS AND RETENTION TYPES (유지형태와 접착제 종류에 따른 수지 접착형 수복물과 법랑질간의 전단결합강도 및 파절양상에 관한 연구)

  • The purpose of this study was to evaluate the shear bond strength between various resin-bonded retainers and enamel according to the adhesive resins and retention types and observe the bond filure modes with scanning electron microscope(SEM). For this purpose, the followin eight sub-groups were tested in shear bond strength : 1) electrochemically etched group(Verabond) using Panavia EX and Superbond C&B 2) tin-plated group(PG-S) using Panavia EX and Superbond C&B 3) salt-treated group(Verabond) using Panavia EX and Superbond C&B 4) meshtreated group(Verabond) using Panavia EX and Superbond C&B. Thermocycling test was conducted on the condition of 15 second dwell time each in $5^{\circ}C$ and $55^{\circ}C$bath. Shear bond strength was measured by Instron Universal Testing Machine(medel 1125). The obtained results were as follows : 1. After thermocycling, the shear bond strengths of tin-plated group and electrochemically etched group were significantly greater than those of salt-treated group and mesh-treated group. And the shear bond strength of Panavia EX was greater than that of Superbond C&B with salt-treated group and tin-plated group(p<0.05). 2. Before thermocycling, electrochemically etched group using Superbond C&B produced the greatest shear bond strength(p<0.01). 3. The shear bond strength of electrochemically etched group using Superbond C&B was significantly decreased after thermocycling(p<0.01). 4. In observation of bond failure modes before thermocycling, Panavia EX highly exhibited enamel fracture. Tin-plated group using Superbond C&B adhesive failure between metal and resin and electrochemically etched group using Superbond C&B exhibited adhesive failure between enamel and rdsin. 5. In observation of failure modes after thermocycling, Panavia EX exhibited cohesive failure and Superbond C&B exhibited adhesive failure between resin and metal.

Treatment and retention of relapsed anterior open-bite with low tongue posture and tongue-tie: A 10-year follow-up

  • The purpose of the current report is to present 6-year long-term stability and 10-year follow-up data for an adult patient who was treated with a tongue elevator for relapsed anterior open-bite. The 19-year-old male patient presented with the chief complaint of difficulty in chewing his food. Collectively, clinical and radiographic examinations revealed an anterior open-bite, low tongue posture, and tongue-tie. The patient opted for orthodontic treatment alone, without any surgical procedure. A lingual frenectomy was recommended to avoid the risk of relapse, but the patient declined because he was not experiencing tongue discomfort. Initial treatment of the anterior open-bite with molar intrusion and tongue exercises was successful, but relapse occurred during the retention period. A tongue elevator was used for retreatment, because the approach was minimally invasive and suited the patient's requirements regarding discomfort, cost, and time. The appliance changed the tongue posture and generated an altered tongue force, which ultimately resulted in intrusive dentoalveolar effects, and a subsequent counterclockwise rotation of the mandible. The results showed long-term stability and were maintained for six years through continual use of the tongue elevator. The results of this case indicated that a tongue elevator could be used not only as an alternative treatment for open-bite, but also as an active retainer.

  • Thermoformed appliance, which has been recently introduced for dental usage, is an appliance made of thermoformed sheets and formed with positive or negative pressure under heat. Thermoplastic material is a kind of plastics and can be repeatedly softened by heat. It is classified into hard elastic foil, hard/soft compound foil and soft elastic foil, including BIOPLAST, BIOCRYL, IMPRELON, etc. It has been developed in 1969 and is available in various thickness, shape and color. There are two types of Vacuum former for thermoplastic materials; the pressure type and suction type. The former is much better than the latter for fabrication of various appliances due to its higher pressure. The authors have applied these appliances to some cases - chin cap, active retainer, individual Fluoride tray, mouth protector, bracket transfer mask, bruxism splint(night guard), Essix appliance - by pressure type Vacuum former($Biostar^{(R)}$). The thermoplastic appliances have numerous advantages such as simple procedure, short working time, clean and transparent product, less objectionable taste. But its outstanding advantage would be its excellent biocompatibility bacause it has no monomer and hence no tissue irritation. Although there is some limitations in its usage, it can be used widely for various purposes especaily for pediatric dentistry.
