자가 골연골 이식술을 이용한 Freiberg 병의 치료(1예 보고) (Autogenous Osteochondral Graft for Freiberg's Disease (A Case Report))

  • Freiberg disease is a osteochondrosis of the lesser metatarsal heads. Various surgical treatment have been recommanded including joint debridement and metatarsal head reshaping, metatarsal dorsal wedge osteotomy, metatarsal head excision and joint arthroplasty. Autogenous osteochondral graft for the treatment of Freiberg disease is an effective restorative procedure that provides early range of motion exercise, weight bearing, and reduces other morbidity. We report a case of late stage Freiberg disease treated with arthrotomy, removal of loose body and autogenous osteochondral graft.

How to improve the survival rate of implants after radiotherapy for head and neck cancer?

  • Implants have been widely used in restorative treatment for patients who have undergone head and neck cancer surgery. With the development of combination treatment of head and neck cancer, radiotherapy has been a common means of therapy. However, it could induce various changes in hard and soft tissues and reduce the success and survival rate of the implants. Some research, using either animal models or clinical studies, have shown that certain strategies could be used for improving the survival rate of implants. In this review, we discussed the changes in both hard and soft tissues, which may reduce the survival rate of the implants, and the proposed methods for improving the survival rate of patients after radiotherapy.

  • The purpose of this preliminary report was to describe the operating procedure of T-scan system and to identify the location, timing and force of occlusal contact in patient with normal occlusion using computerized T-scan system. From the preliminary observation , the author obtained the following results. 1. T-scan system displayed 2 dimensional and 3 dimensional description of occlusion: contact locations, timing (sequence) and forces of occlusal contacts. 2. The T-scan sensor was the most important part of the T-scan system. 3. The data of T-scan system cannot be stored in computer diskett. 4. The T-scan system is thought to be the most effective system to detect occlusal contacts and can be applied to the followings : occlusal diagnosis, occlusal equilibration, crown and bridge restorative procedures, denture adjustment, implant procedures, splint adjustment, laboratory procedures, periodontal treatment, orthodontics, TMJ treatment and patient education etc.

Endodontic treatment of a C-shaped mandibular second premolar with four root canals and three apical foramina: a case report

  • This case report describes a unique C-shaped mandibular second premolar with four canals and three apical foramina and its endodontic management with the aid of cone-beam computer tomography (CBCT). C-shaped root canal morphology with four canals was identified under a dental operating microscope. A CBCT scan was taken to evaluate the aberrant root canal anatomy and devise a better instrumentation strategy based on the anatomy. All canals were instrumented to have a 0.05 taper using 1.0 mm step-back filing with appropriate apical sizes determined from the CBCT scan images and filled using a warm vertical compaction technique. A C-shaped mandibular second premolar with multiple canals is an anatomically rare case for clinicians, yet its endodontic treatment may require a careful instrumentation strategy due to the difficulty in disinfecting the canals in the thin root area without compromising the root structure.

  • Patients with diabetes mellitus show delayed wound healing and increased susceptibility to infection. Therefore, the effects of diabetes on pulpal and periodontal healing should be taken into consideration when treating diabetic dental traumatized patients. This case presents the treatment for dental traumatized 20 yr old female with uncontrolled type II diabetes. The traumatized upper central incisors had showed pulpal healing in early days. However, 7 mon after the trauma, the teeth had been diagnosed with pulp necrosis with apical abscess. Eventually, non surgical root canal treatment on the teeth had been performed.

  • A knee joint's longevity depends on the proper integration of structural components in an axial alignment. If just one of the components is abnormally off-axis, the biomechanical system fails, resulting in arthritis. The complexity of various failures in the knee joint has led orthopedic surgeons to select total knee replacement as a primary treatment. In many cases, this means sacrificing much of an other-wise normal joint. Here, we review novel computational approaches to describe knee physiotherapy by introducing a new dimension of foot loading to the knee axis alignment producing an improved functional status of the patient. New physiotherapeutic applications are then possible by aligning foot loading with the functional axis of the knee joint during the treatment of patients with osteoarthritis.

Endodontic management of central incisor associated with large periapical lesion and fused supernumerary root: a conservative approach

  • Fusion and gemination are developmental anomalies of teeth that may require endodontic treatment. Fusion may cause various clinical problems related to esthetics, tooth spacing, and other periodontal complications. Additional diagnostic tools are required for the diagnosis and the treatment planning of fused tooth. The present case report describes a case of unilateral fusion of a supernumerary root to an upper permanent central incisor with large periapical lesion in which a conservative approach was used without extraction of supernumerary tooth and obturated with mineral trioxide aggregate to reach a favorable outcome.

  • Endodontic microsurgery is a predictable treatment option when orthograde treatment or retreatment is unsuccessful or unfeasible. However, when there is a gross compromise of periapical bone, achievement of bone regeneration after the surgical procedure may be hampered. In such cases, the application of guided tissue regeneration principles, with adjunctive use of leukocyte platelet-rich fibrin to fill the bone defect as a bone substitute and as a membrane to cover the site, provides a cost-effective solution with the benefits of accelerated physiological healing and reduced post-surgical pain and discomfort. This case report presents 2 cases of endodontic microsurgery of the upper lateral incisors with loss of buccal cortical plate, where platelet-rich fibrin was successfully applied.

손목형 심박수 측정 장치를 이용한 어린이의 치과진료 시 스트레스 평가 (The Assessment of Stress of Children under Dental Procedures Using Wrist Watch Type Heart Rate Monitoring Devices - Using New Device for Stress Assessment in Children During Restorative Dental Treatment)

  • 치과 의사는 어린이들이 치과에서 받는 스트레스로 인해 행동 관리에서 어려움을 주로 겪게 된다. 스트레스에 대한 평가는 생체적 표지자인 심박수를 이용하는 방법이 유용하게 사용되고 있다. 본 연구는 치과에서 침윤 마취를 행한 후 이루어지는 수복 진료에서 아이들이 받는 스트레스 정도를 손목형 심박수 측정 장치를 사용하여 알아보기 위해서 계획되었다. 첫 치과 치료를 받은 어린이에서 심박수 변화는 국소 마취 단계에서 가장 높게 나타났으며, 심박수는 치아 삭제에서 가장 높았으나, 단계별 유의한 차이는 관찰되지 않았다(p > 0.05). 이 중 3회 내원하여 치료를 마무리한 어린이에서는, 두 번째 내원의 심박수가 첫 번째 내원과 세 번째 내원에 비해 유의하게 높았으며(p < 0.05), 첫 번째와 세 번째 내원의 심박수는 유의한 차이가 관찰되지 않았다(p > 0.05). 기존 치과 진료 경험이 없는 환아는 국소 마취를 포함한 치과 진료 시 두 번째 내원에서 스트레스를 많이 받는 것으로 생각되며, 치과의 진료 환경에 익숙해지려면 적어도 3회 이상의 내원이 필요할 것으로 생각된다.

Healing after horizontal root fractures: 3 cases with 2-year follow-up

  • Among dental traumas, horizontal root fractures are relatively uncommon injuries. Proper initial management and periodical evaluation is essential for the successful treatment of a root-fractured tooth. If pulpal necrosis develops, endodontic treatment is indicated, exclusively for the coronal fragment. Fragment diastases exert a great influence on healing at the fracture line and on pulpal necrosis. An adequately treated root-fractured tooth has a good prognosis. This case report describes the treatment and 2-yr follow up of 3 maxillary central incisors, first with horizontal root fracture, second with horizontal root fracture and avulsion, and third with horizontal root fracture and lateral luxation. All three cases were treated with mineral trioxide aggregate (ProRoot, Dentsply). During 2 yr of follow-up evaluation, the root-fractured teeth of the present patients were well retained in the arch, showing periodontal healing, even after endodontic treatment.
