요추 만곡 지지가 경요도적 전립선 절제술 후 침상 안정기 환자의 요통과 안위 및 출혈성 합병증 발생에 미치는 효과 (The Effect of Support for Lumbar Curve on Back Pain, Comfort, and Bleeding Complications in Patients on Bed Rest after Transurethral Resection of Prostate)

  • Purpose: This study was done to identify effects of lumbar curve support on back pain and comfort during bed rest after transurethral resection of prostate (TURP). Method: A quasi-experimental design with a non-equivalent control group was used. All patients were diagnosed with benign prostatic hyperplasia and underwent TURP. Twenty participants were assigned to the experimental group and twenty to the control group. After TURP, the lumbar curve of patients in the experimental group was supported using gel pads for 6 hours while the control group received the usual care with the both leg straight. The intensity of back pain and comfort levels were assessed on immediate return to the unit and for six hours following TURP. Bleeding complications were detected from hemoglobin and hematocrit levels. Results: Support of lumbar curve was found to be significantly effective in reducing back pain. the need for analgesics was significantly less in the experimental group. Comfort levels were not significantly different between the two groups. There were no bleeding complications in either group. Conclusion: These results suggest that supporting of lumbar curve ameliorates back pain without causing an increased incidence of bleeding complications after TURP and this nursing intervention should help TURP patients to be more comfortable.

The Association Between Masticatory Function Assessment and Masseter Muscle Thickness in the Elderly

  • Purpose: This study investigated the association between the objective indicator of masticatory function assessment and the masseter muscle thickness (MMT) using ultrasound imaging. Methods: A total of 99 subjects (males: 24, females: 75, mean age: 76) were analyzed. The maximum bite force (MBF) was measured with a pressure-sensitive sheet and an image scanner. The mixing ability index (MAI) was calculated by image analysis after asking the subjects to chew a wax specimen. The MMT during rest and clenching were obtained with a diagnostic ultrasound system, and the difference in MMT during rest and MMT during clenching was defined as the difference in masseter muscle thickness (DMMT). Multiple regression analysis was performed to determine the independent variables affecting MBF and MAI. Results: The MBF showed correlation with the number of remaining teeth (β=0.346, p=0.002) and DMMT (β=0.251, p=0.011). The MAI correlated with only the number of remaining teeth (β=0.476, p<0.001). Conclusions: The DMMT reflects the state of masseter muscle contraction, and can be used as a predictor as well as the number of teeth when assessing masticatory function.

복부 수술 환자의 통증 정도 및 통증관리 만족도 영향 요인 - 통증 자가조절기 사용 환자를 중심으로 - (A Study for Assessing Pain Intensity and Investigating Factors of Satisfaction with Postoperative Pain Management)

  • Purpose: The purpose of this study was to identify pain intensity according to activities over postoperative days and to identify factors of patient satisfaction with pain management after abdominal surgery. Methods: The subjects were 123 patients who were admitted at a general surgical ward and had abdominal surgery from Mar. 2005 to June 2005. The collected data were analyzed using the SPSS 12.0 program. Results: The average postoperative pain scores were 6.13 at POD 1, 5.46 at POD 2, and 4.22 at POD 3. In stepwise multiple regression analysis a total of 40% of variance in satisfaction with pain management was accounted for by pain intensity on POD 3 at rest (29.4%) and attitude toward using pain medicine (6.6%), and side effects of pain medicine (4.0%). Conclusion: According to the findings of this study, patients had inadequate pain management after abdominal surgery. Therefore, nurses need to provide educational programs focused on changing attitudes toward using pain medicine, to reduce side effects of pain medicine, and to develop nursing intervention for relieving pain.

  • Cervical disc herniation is one of the most common causes of neck, shoulder and arm pain. There are many treatments for a cervical disc herniation, such as rest, physical therapy, medication, epidural steroid injection and surgery. However, conservative treatments sometimes have limited effectiveness, and a surgical discectomy is often associated with numerous complications. Nowadays, a percutaneous discectomy, using a $Dekompressor^{(R)}$, has been used in herniated disc patients, but a posterolateral extruded disc is not an indication. Herein, our experience using a 19 G $Dekompressor^{(R)}$, on a 52 year-old male patient with a left C6-⁣7 posterolateral extruded disc, is reported. Decompression was successfully performed, and the pain and range of motion was immediately improved.

일라이트 온수매트 사용이 고강도 운동으로 유발된 성인의 젖산, CRP, ACR에 미치는 영향 (Effects of Using Illite Warm Water Mats on Lactate, CRP and ACR Induced High Intensity Exercise in Adults)

  • Purpose : The purpose of this study was to investigate the effective recovery method of exercise-induced fatigue and muscle pain by comparing the effect of the use of illite hot mat product and general hot mat product on the recovery of muscle pain induced by high intensity exercise. Methods : To measure and analyze the changes in lactic acid, CRP, and ACR according to the high-intensity circuit training program, this study was conducted for the healthy adult men and women, who exercise st the K-region sports center. A total of 45 subjects were studied in 15 groups of 15 patients who received an illite hot-water mat recovery group (A group), 15 general hot-water mat recovery group (B group), and 15 control group (C group). The circuit training exercise program was conducted as a one-time exercise, and each exercise time consisted of 30 minutes of warm-up exercise, 5 minutes of main exercise, 20 minutes of clean-up exercise, and 5 minutes of strength exercise. The intensity setting was high intensity of subjective exercise intensity It carried out by setting to (14-16RPE). Results : Changes in Lactic Acid Concentration There was a significant difference in the lactic acid concentrations between the groups after the high intensity circuit training program (p <.05). The illite rest group (A) decreased 7.71 mmol / L and the control group decreased 4.03 mmol / L. Significantly decreased (p <.05). Changes in ACR Concentration. There was a significant difference in the ACR concentrations Significant differences were found in CRP and ACR during the recovery period after exercise. (p <.05), the elite rest group (A) decreased 2.47 mg / mmol, and the control group increased 1.63 mg / mmol. There was a significant difference (p <.05). Conclusion: The static rest on a heated mat after high-intensity exercise has an effect on changes in blood lactate and ACR levels.

하지불안증후군 환자의 휴식.활동 리듬, 수면 양상 및 삶의 질에 대한 연구 (Rest-Activity Rhythm, Sleep Pattern and Quality of Life in Patients with Restless Legs Syndrome)

  • Purpose: In this study rest-activity rhythm, sleep pattern and quality of life of patients with restless legs syndrome were compared with those of a normal group. Methods: The participants in this study were 36 patients with restless legs syndrome diagnosed by a neurologist and 36 participants in the normal group. An actigraph, sleep diary, Pittsburgh Sleep Quality Index and Insomnia Severity Index scale were used as measurement tools for the study. Chi-square test, Lamda test, t-test and Kendall's correlation with SPSS 12.0 program were used to analyze the data. Results: Patients with restless legs syndrome had a higher rest-activity rhythm curve of Least 5 hr's activity(L5) and Most 10 hr's activity(M10) than those of normal group and sleep problems included decreased sleep efficiency and increased sleep latency, wake time and number of awakenings. The scores for the subscales of quality of life in patients with restless legs syndrome were lower than the normal group for general health, physical functioning, role limitations due to emotional problems, role limitation due to physical problems, social functioning, bodily pain, vitality and mental health. Conclusion: The results suggest that further studies are needed to identify rest-activity rhythm according to symptom severity and to develop nursing interventions which consider rest-activity rhythm.

만성 요통 유무와 자세에 따른 복부근 두께변화 비교 (Comparison of Changes in the Thickness of the Abdominal Muscles in Different Standing Positions in Subjects With and Without Chronic Low Back Pain)

  • Purpose: This study aimed to compare changes in abdominal muscle thickness in different standing postures with a handheld load between subjects with and without chronic low back pain (CLBP). Methods: Twenty subjects with CLBP and 20 controls participated in this study. Ultrasound imaging was used to assess the changes in the thickness of the transverse abdominis (TrA), internal oblique (IO), and external oblique (EO) muscles. Muscle thickness in three different standing postures (standing at rest, standing with loads, standing with lifting loads) was compared with the muscle thickness at rest in the supine position and was expressed as a percentage of change in the thickness of the muscle. Results: While standing with loads, the change in IO muscle thickness in the CLBP patients increased more significantly than in the pain-free controls (p < 0.05). The standing with lifting loads posture showed a significant increase in the change in thickness of the TrA compared with the standing with loads posture (p < 0.05). In addition, the standing with lifting loads posture showed a significant decrease in the change in the thickness of the EO when compared with the standing with loads posture (p < 0.05). Conclusion: The automatic activity of the IO muscle in subjects with CLBP increased more than that of the pain-free controls in the standing with loads posture. These findings suggest that IO muscle function may be altered in those with CLBP while standing with loads. Additionally, TrA the activation level was found to be associated with increased postural demand caused by an elevated center of mass.

A Reproducible and Reliable Method for Measuring Masseter Muscle Thickness in Maximal Bite Force Using Ultrasonography

  • Purpose: The purpose of this study is to develop a reproducible and reliable method for evaluating the masseter's functional state by measuring the masseter muscle with ultrasonography (US). Methods: Nineteen healthy adults (9 males, 10 females) were the subjects of this study. During US scanning, the image was taken from the thickest part of the masseter muscle in the image. To evaluate changes in thickness during masseter function, US images were taken of the participant's masseter muscle at rest and during clenching. In this study, US scanning was conducted using two approaches to compare the difference in masseter muscle thickness determined when inducing maximum bite force (MBF). Results: All 19 subjects completed US scanning of the masseter muscle at rest and during clenching under the conventional method and the articulation paper method. There was no difference in masseter muscle thickness measured at rest. However, the thickness of the masseter muscles determined by the articulation paper during jaw clenching was greater than that measured by the conventional method. Conclusions: In conclusion, using the US for masseter muscle evaluation can offer objective and functional information on the masseter muscle. A standardized US scanning method needs to be developed to obtain reproducible and reliable information on the masseter muscle at rest and during clenching. In particular, generating MBF using an articulation paper can be a reproducible and reliable method of measuring the functional state of the masseter muscle.

  • The author performed electromyographic analysis on edentulous patients aging 52 to 77 who have upper and lower full dentures for 3 to 7 years of duration. And the graphs were recorded in physiologic rest position, incisal occlusion, molar occlusion, left lateral excursion, right lateral excursion and mandibular projection. The results obtained are as follows : 1. 80 per cent of the patients are in unilateral mastication. 2. The shorter history of edentulous mouth reveals the more electric potency. 3. Facial deviation owing to unilateral mastication is the cause of higher electric potency to the deviated site. 4. There were no difference in electric potency between mandibular projection and incisal occlusion.

하악안정위 유도에 관한 근전도학적 연구 (An Electromyographic Study on Mandibular Rest Position Induced by Several Methods)

  • Mandibular rest position is very essential position because in that position, masticatory muscularture can be reciprocally coordinative and unstrained. So obtaining a good ability to establish and maintain rest position in non-functioning state is mandatory for treatment of patient with craniomandibular disorders. In general, many types of mandibulr movement exercise start from rest position and use the sense of muscle relearning, that is, muscle smoothness and relaxation, throughout the exercise period. In this study, 44 normal subjects for control group and 37 patients with craniomandibular disorders for experimental group participated and they were classified into 3 subgroups, respectively. One method to guide mandibular rest position was used for one subgroup, so theree methods were used for this study, 1)self-guided rest position without any education, 2) guided by Rocabado's tongue rest position, 3) guided through swallowing after Dawson's centric relation. To record electromyographic activity, Bioelectric processor EM2 (Myotronics, U.S.A.) was used. The numbers of sessions from start to stable resting electromyographic level and muscle activities in stable state were recorded and two recordings which were first and second, 3 days after first recording, were done, The data were processed with SPSS/PC+package. The obtained results were as follows : 1. Mean number of sessions in second recording were fewer than those in first recording in both groups. In comparison among 3 subgroups, mean number of sessions of subgroup guided through Dawson's method were fewer than those of other groups though it is not statistically significant. 2. There was a difference of mean number of sessions between control and experimental group in first recording, but in second recording there was no difference in any cases. 3. Mean value of muscle activity were generally not different without regard to group and method if once come to stable resting position level. Mean value of muscle activity of guided subgroups showed a tendency of decreasing in second recording than in first recording. However, in self-guided subgroup there was a inconsistent pattern. 4. The amounts of change in session number from first to second recording in control group were fewer than those in experimental group.
