The Effectiveness of Meridian Acupressure Intervention Using Sticker Needles to Bowel Movement on Post Spinal Operative Patients (스티커 침을 이용한 경혈지압이 척추수술 후 배변에 미치는 영향)

  • This study was conducted to identify the effect of meridian acupressure on defecation of patients with post spinal operation. The nonequivalent control group posttest only design was used. The data were obtained from 77 post spinal operative patients, 34 in the experimental 43 in control group in Y Hospital, Seoul, Korea. The neurosurgical unit A and B ward, assigned by matched sample by the name of operation such as laminectomy and posterior lumbar interbody fusion are performed. Meridian acupressure meant the method that an examiner presses response points distributed in the pass of energy vessel. In this study, meridian acupressure program was performed on as points in order of Hegue (LI-4), Zhigou (TE-6), Zusanli (S-36), Shangjuxu (S-37), Xiajuxu (S-39), Tianshu (S-25), Taichong (L-3) which was known to be related to large intestine. Data were collected from 1, July 2003 to 1, September 2003. Meridian acupressure program was carried out for 20 minutes 4 hours after operation twice daily. In order to evaluate the effect of meridian acupressure intervention, they were asked time of bowel recovery, gas passing, and defecation though questionnaire method. Data were analyzed by the SPSS/ WIN 11.0 program. The results of this study were summarized as follows; 1. Homogeneity tests of general characteristics and operation related characteristics of the experimental group and the control group were performed. General characteristics included age, sex, defecation habit, eating pattern, fluid intake, life style, activity, usage of laxative and etc. 2. Hypotheses were verification as follows; 1) Recovery of bowel sound of the experimental group who received meridian acupressure intervention was faster than that of the control group after spinal operation (t=-6.770,P=.000). 2) Time of gas passing of the experimental group who received meridian acupressure program was faster than that of the control group after spinal operation (t=-8.003, P=.000). 3) Time to defecation of the experimental group who received meridian acupressure program was faster than that of the control group after spinal operation (t=-9.026, P=.000). 4) Abdominal discomfort due to defecation of the experimental group who received meridian acupressure program was lesser than that of the control group after spinal operation (t=-3.431, P=.001). From these results, meridian acupressure intervention was effective for recovery of bowel sound, reduce time to gas passing, time to defecation and lessen abdominal discomfort due to defecation on post spinal operative patients. And therefore this intervention can probably considered on clinical practice.

A Neural Network for Long-Term Forecast of Regional Precipitation (지역별 중장기 강수량 예측을 위한 신경망 기법)

  • In this paper, a neural network approach to forecast Korean regional precipitation is presented. We first analyze the characteristics of the conventional models for time series prediction, and then propose a new model and its learning method for the precipitation forecast. The proposed model is a layered network in which the outputs of a layer are buffered within a given period time and then fed fully connected to the upper layer. This study adopted the dual connections between two layers for the model. The network behavior and learning algorithm for the model are also described. The dual connection structure plays the role of the bias of the ordinary Multi-Layer Perceptron(MLP), and reflects the relationships among the features effectively. From these advantageous features, the model provides the learning efficiency in comparison with the FIR network, which is the most popular model for time series prediction. We have applied the model to the monthly and seasonal forecast of precipitation. The precipitation data and SST(Sea Surface Temperature) data for several decades are used as the learning pattern for the neural network predictor. The experimental results have shown the validity of the proposed model.

  • The IEEE standard 1149.1, which was proposed to increase the observability and the controllability in I/O pins, makes it possible the board level testing. In the boundary-scan environments, many shift operations are required due to their serial nature. This increases the test application time and the test application costs. To reduce the test application time, the method based on the parallel opereational multiple scan paths was proposed, but this requires the additional I/O pins and the internal wires. Moreover, it is difficult to make the designs in conformity to the IEEE standard 1149.1 since the standard does not support the parallel operation of data shifts on the scan paths. In this paper, the multiple scan path access algorithm which controls two scan paths simultaneously with one test bus is proposed. Based on the new algorithm, the new algorithm, the new board level BIST architecture which has a relatively small area overhead is developed. The new BIST architecture can reduce the test application time since it can shift the test patterns and the test responses of two scan paths at a time. In addition, it can reduce the costs for the test pattern generation and the test response analysis.

  • Prosthetic replacement is one of the most common methods of reconstruction after resection of malignant tumor around the knee. Gait analysis provides a relative objective data about the gait function of patients with prosthesis. The purpose of this study was to compare the gait pattern of the patients who underwent limb salvage surgery with prosthesis for distal femur and that of patients with prosthesis for proximal tibia. This study included ten patients (4 males, 6 females, mean age 22.7 years, range 14-36) who underwent a wide resection and Kotz hinged modular reconstruction prosthesis replacement and six normal adult(Control). The site of bone tumor was the distal femur (Group 1) in six patients and proximal tibia (Group 2) in 4 patients. The follow-up period ranged from 15 to 82 months (mean : 33 months). The evaluation consisted of clinical assessment, radiographic assessment, gait analysis using VICON 370 Motion Analysis System. The gait analysis included the linear parameters such as, walking velocity, cadence, step length, stride length, stance time, swing time, single support and double support time and the three-dimensional kinematics (joint rotation angle, velocity of joint rotation) of ankle, knee, hip and pelvis in sagittal, coronal and transverse plane. For the kinetic evaluation, the moment of force (unit: Nm/kg) and power (unit: Watt/kg) of ankle, knee and hip joint in sagittal, coronal and transverse plane. In the linear parameters, cadence, velocity, step time and single support were decreased in both group 1 and group 2 compared with control. Double support decreased in group 2 compared with control significantly(p<.05). In contrast to our hypothesis, there was no significant difference between group 1 and group 2.   • Pneumconiosis is a sort of pulmonary fibrosis consequent to the inhalation of the respirable dusts. Thus, the pathogenesis of silicosis have concentrated largely on the early response of alveolar macrophage and the later fibroblastic stimulation. But the role of the other cells and continuing cell injury in the pathogenesis has not been fully studied. And the chemical factors such as prostaglandin, fibroblast stimulating factor and inhibiting factor and chemotaxin are also participated in the mechanism of pulmonary fibrosis in silicosis. In order to clarify the role of alveolar cells and prostaglandin, we investigated the changes of the cellularities in bronchoalveolar lavage fluid and tissue pathology in the experimental silicosis with the time sequence. The experimental animals were divided into 3 groups; control group received only intratracheal injection of 0.5 ml saline, silica group received the intratracheal instillation of 40 mg silica with the same amount saline, and aspirin group received 450 mg/kg of aspirin after silica instillation. The results were as follows: 1) The total cells of bronchoalveolar lavage fluid in the silica group markedly increased in comparison with the control group, but there was no significant difference between the silica and aspirin groups. 2) The percentages of alveolar macrophages to the total number of cells in the silica group tended to be lower than those in the control group and also lower than those in the aspirin group at the 1st week after silica instillation. 3) The percentages of neutrophils to the total number of cells in the silica group were significantly higher than those in the control group during the entire period and also higher than those in the aspirin group at the 3rd day after silica instillation. 4) In the silica group, the percentages of lymphocytes to the total number of cells were increased 143 progressively with the time course and those were significantly higher than those in the control group from the 3rd week after silica administration. There were marked differences of lymphocyte percentages between the silica and aspirin groups at the 1st week after silica instillation. 5) The inflammatory change was observed in the rat lung at the 1st day after silica instillation. Also the silicotic nodule appeared in the silica group at the 1st week but we could not find out that nodule in the aspirin group at that time. The fibrotic changes in the rat lung tended to be increased progressively with the time course, therefore, the diffuse fibrotic pattern appeared in the whole field at the 20th week after silica instillation. 6) By the electron microscopy, there were gradual increases of phagosomes and vacuoles in the alveolar macrophage in the silica group as compared with the control group. These results suggest that the neutrophils and the lymphocytes have also participated in the pulmonary fibrosis even though the alveolar macrophage has a major role, and prostaglandin mediate the inflammation and pulmanary fibrosis in the experimental silicosis.

Sequential Pituitary Hormone Responses to Electroconvulsive Therapy (전기경련요법후 뇌하수체 호르몬의 순차적인 분비 반응)

  • Background : Most studies of the pituitary hormonal responses to electroconvulsive therapy(ECT) have used limited blood sampling schedules. Little is known about the precise sequence of neuroendocrine events immediately following en ECT application. or about the regulation of the hormonal responses. Methods : Blood was sampled at three minute intervals from eleven patients(two schizophrenics and nine affective disorder patients) undergoing ECT. Each sample was immunologically assayed for arginine vasopressin(AVP), adrenocorticotropic hormone(ACTH), prolactin(PRL), and cortisol. Baseline hormone concentrations and several measures of response were determined for each hormone. The temporal and quantitative relationships among the hormonal responses were determined. Correlations were calculated between seizure duration and secretory responses. Results : All four hormones demonstrated significant secretory responses to ECT, with AVP increasing from 1.2 to 33.3pg/ml(P<0.001), ACTH from 5.4 to 32.3fmol/ml(P<0001). PRL from 21.8 to 102.2ng/ml(P<0.005) and cortisol from 20.1 to 31.1ug/dl(P<0.001). The three pituitary hormones showed consistent time courses of secretion with onset of responses by three minutes but clearly differing peak times of 3, 6, and 12-15 minutes for AVP, ACTH, and PRL, respectively. Cortisol began to rise after 6minutes and pecked between 20-30minutes. There ware no significant correlations between seizure duration and any of the secretory response measures. Conclusions : 1) The pituitary hormone response to ECT is sequential rather than synchronous 2) The AVP response was extremely rapid and more massive than those of any other hormones. 3) The ACTH response of this study was more rapid and mare robust than thai revealed by the mast of past studies. 4) The results strongly suggest that the pituitary hormones are released as a result of the seizure rather tho, the electrical stimulus. 5) The sequential pattern of responses suggests that neuroendocine feedback-regulatory mechanisms determine the response profile.

Effective Tillering Pattern and Grain Yield on Different Seeding Dates in Barley (보리 파종기에 따른 유효분벽의 양상과 수량)

  • This study was aimed to provide understanding on the eco-physiological response of barley tillers as affected by difference in seeding date. Yield and yield contribution rates of tillers were investigated with the data of field experiments in the former Wheat and Barley Research Institute of Suwon, Korea from September 1982 to July 1984. When barley was sowed 15 days earlier than or at the locally recommended sowing date (i.e. October 5), it produced more mainstem leaves than the that sowed 15 days later than the local recommendation. The effective tillers (i.e. ear-bearing tillers) were observed in concurrently occurring leaves up to 10/0 (abbreviation of the 10th mainstem leaf) from early and optimum sowed crops, while 9/0 from late sowed crops. Tillering followed the production rule of Gatayama(l952) which has the linear leaf appearance pattern. Early sowing produced more effective tillers, but the highest production of effective ears (i.e. ears heavier than 0.505g) was noticed in sowing at the recommended date. The tillers in the axil of first leaf in mainstem (abbreviated as 1) produced more effective ears than other mainstem tillers (i.e. primary tillers such as C, 2, 3 and 4). The tillers from the axil of first leaf, whether they were primary or secondary, always performed better in the production of effective tillers, grains per ear, grain weight, and grain yield per ear than those from the axil of coleoptile or prophyll. Other tillers from subsequent leaves were also inferior in production with the order of their appearance, thus making first leaf tillers as the best performer(e.g. 1 against C, 2, 3, 4 of primary tillers, 11 and 21 against 1P, 12, 13, 2P, 22 of secondary tillers). Even though the first leaf tiller from the first mainstem leaf(i.e. 11) emerged at a same time with the fourth mainstem leaf tiller(i.e. 4), it was always a better producer of ears and grains. The above observations of hierarchy among tillers were persistent irrespective of cropping conditions and treatments. Sowing at the recommended date produced more effective tillers and grains per ear than early or late sowing. In early and late sowings, more grains per effective ear were observed by early sowing, whereas more grains per ear were produced by late sowing. The order of performance in production of effective tillers and ears per plant was as follows: optimum > early > late sowings. In optimum sowing the mainstems were the highest in grain weight per ear, while the first leaf tillers were the ones in early sowing probably due to winter damage on mainstem ears. Yield contribution by the tillers was greater with the following order irrespective of sowing dates: mainstem, 1, 2, 3, C, 11, 12 and Cl. The contribution of CP, 2P, 21 and 31 varied with sowings.

Shear Behavior and Performance of Deep Beams Made with Self-Compacting Concrete

  • An experimental study was carried out to evaluate fresh properties of a moderately high-strength (high-flowing) self-compacting concrete (SCC) and to investigate shear behavior and performance of deep beams made with SCC. Fresh and hardened properties of normal concrete (NC) and SCC were evaluated. The workability and compacting ability were observed based on casting time and number of surface cavities, respectively. Four-point loading tests on four deep beams (two made with SCC and two with NC) were then conducted to investigate their shear behavior and performance. Shear behavior and performance of beams having two different web reinforcements in shear were systematically investigated in terms of crack pattern, failure mode, and load-deflection response. It was found from the tests that the SCC specimen having a normal shear reinforcement condition exhibited a slightly higher load carrying capacity than the corresponding NC specimen, while the SCC specimen having congested shear reinforcement condition showed a similar load carrying capacity to the corresponding NC specimen. In addition, a comparative study between the present experimental results and theoretical results in accordance with ACI 318 (Building Code Requirements for Reinforced Concrete (ACI 318-89) and Commentary-ACI 318R-89, 1999), Hsu-Mau's explicit method (Hsu, Cem Concr Compos 20:419-435, 1998; Mau and Hsu, Struct J Am Concr Inst 86:516-523, 1989) and strut-and-tie model suggested by Uribe and Alcocer (2002) based on ACI 318 Appendix A (2008) was carried out to assess the applicability of the aforementioned methods to predict the shear strength of SCC specimens.

Finite Element Simulation of Elastic Waves for Detecting Defects and Deteriorations in Underwater Steel Plates (수중강판의 결함 및 열화 검출을 위한 탄성파 유한요소 시뮬레이션)

  • This paper presents the results of finite element simulations of elastic wave propagation in an underwater steel plate and the verification of a proposed method utilizing elastic wave-based damage detection. For the simulation and verification, we carried out the following procedures. First, three-dimensional finite element models were constructed using a general purpose finite element program. Second, two types of damages (mechanical defects and deteriorations) were applied to the underwater steel plate and three parameters (defect location, defect width, and depth) were considered to adjust the severity of the applied damages. Third, elastic waves were generated using the oblique incident method with a Gaussian tone burst, and the response signals were obtained at the receiving point for each defect or deterioration case. In addition, the received time domain signals were analyzed, particularly by measuring the magnitudes of the maximum amplitudes. Finally, the presence and severity of each type of damage were identified by the decreasing ratios of the maximum amplitudes. The results showed that the received signals for the models had the same global pattern with minor changes in the amplitudes and phases, and the decreasing ratio generally increased as the damage area increased. In addition, we found that the defect depth was more critical than the width in the decrease of the amplitude. This mainly occurred because the layout of the depth interfered with the elastic wave propagation in a more severe manner than the layout of the width. An inverse analysis showed that the proposed method is applicable for detecting mechanical defects and quantifying their severity.

Purification and characterization of a 1,3-β-D-glucan recognition protein from Antheraea pernyi larve that is regulated after a specific immune challenge

  • Pattern recognition receptors are known to participate in the activation of Prophenoloxidase system. In this study, a 1,3-${\beta}$-D-glucan recognition protein was detected for the first time in Antheraea pernyi larvae (Ap-${\beta}GRP$). Ap-${\beta}GRP$ was purified to 99.9% homogeneity from the hemolymph using traditional chromatographic methods. Ap-${\beta}GRP$ specifically bind 1,3-${\beta}$-D-glucan and yeast, but not E. coli or M. luteus. The 1,3-${\beta}$-D-glucan dependent phenoloxidase (PO) activity of the hemolymph inhibited by anti-Ap-${\beta}GRP$ antibody could be recovered by addition of purified Ap-${\beta}GRP$. These results demonstrate that Ap-${\beta}GRP$ acts as a biosensor of 1,3-${\beta}$-Dglucan to trigger the Prophenoloxidase system. A trace mount of 1,3-${\beta}$-D-glucan or Ap-${\beta}GRP$ alone was unable to trigger the proPO system, but they both did. Ap-${\beta}GRP$ was specifically degraded following the activation of proPO with 1,3-${\beta}$-Dglucan. These results indicate the variation in the amount of Ap-${\beta}GRP$ after specific immune challenge in A. pernyi hemolymph is an important regulation mechanism to immune response.
