요크셔테리어종에서의 울혈성 심부전을 동반한 판막하형 폐동맥판 협착증 (Subvalvular Pulmonic Stenosis with Congestive Heart Failure in a Yorkshire terrier)

  • A two-month-old female Yorkshire terrier was referred to the Veterinary Teaching Hospital, College of Veterinary Medicine, Kyungpook national University. The patient was presented with a history of dyspnea, cough, exercise intolerance and abdominal distension, but she was appetence. In physical examination the puppy was coughed on slight tracheal compression. Rectal temperature, pulse and respiratory rate were normal, and grade 3/6 systolic murmur heard at the left heart base. The murmur was crescendo-decrescendo. Electrocardiography showed sinus arrhythmia, right-ventricular hypertrophy pattern, and right axis deviation. Thoracic radiography revealed cardiomegaly, bulging of the main pulmonary artery, and enlarged left side heart. Abdominal radiography revealed abdkominal distention. Echocardiography showed hypertrophy of right ventricle and turbulence in the pulmonary artery in parasternal oblique view. Subvalvular pulmonic stenosis was diagnosis based upon the clinical signs, physical examination, electrocardiography, radiography and echocardiography. We treated the patient with furosemide, enalapril and $\beta$-blocker. After the clinical signs of cough, abdominal distension and dyspnea were disappeared, she was on just $\beta$-blocker for prevention of occurrence of congestive heart failure. Now she was recovered her health, and she is not on any medication.

촉각자극에 의한 자율신경계 및 뇌파 반응과 감성 (Emotional Preference Modulates Autonomic and Cortical Responses to Tactile Stimulation)

  • The purpose of the current study was comparative analysis of autonomic and electrocortical responses to passive and active touch of the tektites with different subjective emotional preference. Perspective goal of the project is development of a template for classification of tactile stimuli according to subjective comfort and associated physiological manifestations. The study was carried out on 36 female college students. Physiological signals were acquired by Grass and B10PAC 100 systems with AcqKnowledge III software. Frontal, parietal and occipital EEG (relative power spectrum /percents/ of EEG bands - delta, theta, slow and fast alpha, low and fast beta), and autonomic variables, namely heart rate (HR), respiratory sinus arrhythmia (RSA), pulse transit time (PTT), respiration rate (RSP) and skin conductance parameters (SCL, amplitude, rise time and number of SCRs) were analyzed for rest baseline and stimulation conditions. Analysis of the overall pattern of reaction indicated that autonomic response to tactile stimulation was manifested in a form of moderate HR acceleration, RSP increase, RSA decrease (lowered vagal tone), decreased n and increased electrodermal activity (increased SCL, several SCRs) that reflects general sympathetic activation. Parietal EEG effects (on contra-lateral side to stimulated hand) were featured by short-term alpha-blocking, slightly reduced theta and significantly increased delta and enhanced fast beta activity with few variations across stimuli. The main finding of the study was that most and least preferred textures exhibited significant differences in autonomic (HR, RSP, PTT, SCR, and at less extent in RSA and SCL) and electrocortical responses (delta, slow and fast alpha, fast beta relative power). These differences were recorded both in passive and active stimulation modes, thus demonstrating reproducibility of distinction between most and least emotionally preferred tactile stimuli, suggesting influence of psychological factors, such as emotional property of stimulus, on physiological outcome. Nevertheless, development of sufficiently sensitive .and reliable template for classification of emotional responses to tactile stimulation based on physiological response pattern may require more extensive empirical database.

  • The sense of touch has both objective and subjective characteristics. During hand evaluation of the fabrics. psycho physiological processes such as emotion and stimulation. On other site, the mode of touch (passive vs. active) is also capable to modulate somatosensory responses. I.e., suppress somatocensory perception during active electrocortical responses to passive and active touch of the textiles with different subjective emotional preference. The study was carried out on 36 female college students. 스마트폰을 이용한 심호흡 기반 자율신경계 테스트 (Testing of Autonomic Nervous System by Deep Breathing Using a Smartphone)

  • 심호흡을 통해서 심박수 변이를 측정하고, 이를 이용하여 자율신경계를 평가하는 것은 잘 알려진 방법으로 다양한 임상 분야에서 널리 이용되고 있다. 병원에서는 이를 위해서 고가의 장비가 설치되고 전문가에 의해서 운용되고 있다. 논문에서는 스마트폰을 이용하여 언제 어디서나 간편하게 심호흡을 통해서 자율신경계를 테스트할 수 있는 앱을 개발하고, 실험을 통해서 개발된 앱을 검증하였다. 실험은 지원자 8명에 대해서 순천향대학교 천안병원 신경과 임상실에서 진행되었다. 지원자에 대한 자율신경계 테스트를 다음 두 가지 방법으로 진행하였다. 먼저 병원에 설치된 근전도 검사기를 이용하였고, 다음에 동일한 조건에서 개발된 앱을 적용하였다. Pearson 방법을 이용하여 이 두 가지 방법의 실험 결과간에 상관 분석을 수행하였고, 분석 결과는 0.98의 매우 높은 상관관계를 보여준다.

폐쇄성 수면 무호흡에서 CPAP 치료가 전신성 혈압, 심조율 및 catecholamines 농도에 미치는 영향 (Effects of CP AP Therapy on Systemic Blood Pressure, Cardiac Rhythm and Catecholamines Concentration in Patients with Obstructive Sleep Apnea)

  • 연구배경 : 폐쇄성 수면 무호흡 증후군 환자는 수면중에 전신성 혈압의 상승과 심박동수의 현저한 변화를 보이며, 이러한 심혈관계 기능변화는 수면중 반복되는 무호흡에 의한 저산소증과 각성반응으로 인한 교감신경계 활성도 변화와 관련이 있을 것으로 생각되고 있다. 지속적 기도 양압 공급(CPAP) 치료는 폐쇄성 수면 무호흡을 가장 확실하게 치료할 수 있는 방법으로 알려져 있으나 이러한 치료법이 환자들의 심혈관계 교감신경계 활성도에 어떠한 영향을 미치는 지에 대해서는 명확히 알려져 있지 않다. 이에 저자들은 폐쇄성수면 무호흡 증후군 환지에서 CPAP 치료 적용시 나타나는 심혈관계 및 교감신경계 활성도 변화를 관찰하고자하였다. 방법 : 수면다원검사에서 폐쇄성 수면 무호흡 증후군으로 진단되어 CPAP 치료를 시행한 13명을 대상으로 치료 전과 후의 수면다원검사의 각 지표, 수면중 전신성 혈압, 수면중 심박동수와 부정맥, 그리고 혈장 catecholamines 농도 변화를 비교 분석하였다. 결과 : 수면다원검사의 각 지표중 CPAP 치료 후 무호흡-저호흡지수는 유의하게 감소하였고(p<0.01), 동맥혈 최저 산소 포화도는 유의하게 증가하였다(p<0.01). 수면중 수축기 혈압은 치료 후 감소하는 경향을 보였으나 통계적 유의성용 없었다. 수면중 심박동수는 치료 전에 비해 통계적으로 유의한 차이는 없었으나 심부정맥의 종류와 빈도는 치료 전에 부정맥이 관찰되었던 환자들에 있어서 감소하는 경향을 보였고 무호흡의 소실과 함께 동성 서맥성빈맥성부정맥은 사라졌다. 혈장 epinephrine 농도는 치료 전후에 유의한 차이가 없었으나 혈장 norepinephrine 농도는 치료 후 통계적으로 유의하게 감소하였다(p<0.05). 결론 : 폐쇄성 수면 무호흡 증후군 환자에서 CPAP 치료는 수면 무호흡과 저호흡 발생을 감소시켜 수면중 저산소증 발생을 예방할 수 있었고, 수면중 혈장 norepinephrine 농도 감소를 유도하였다. 이러한 변화를 통하여 CPAP 치료는 폐쇄성 수면 무호흡 증후군 환자에게 있어 수면중 전신성 수축기 혈압의 하강 및 수면중 심부정맥의 감소에 영향을 미칠 것으로 생각한다.

아스피린 중독에 의한 심전도 변화 2례 (Transient Change of Electrocardiogram in Two Young Women With Salicylate Intoxication - Two Cases Report -)

  • Symptoms of aspirin overdose may vary from acid-base disturbance, electrolyte abnormality, non-cardiogenic pulmonary edema, chemical hepatitis, seizure to cardiac toxicity. Cardiac adverse effects from aspirin are uncommon but there are reports of arrhythmia, cardiopulmonary arrest, and myocardial infarction. We report 2 cases of young women with aspirin overdose who exhibited ischemic changes on their ECGs a few hours after the ingestion with spontaneous recovery in a few days. First case, a 29 year old woman, presented to the emergency department 6 hours after ingesting 250 tablets of aspirin (325 mg/T). On examination, the temperature was $36.3^{\circ}C$: blood pressure, 105/72mmHg; Pulse, 111/min and respiratory rate, 24/min. Second case, a 27 year old woman, an hour after ingesting 60 tablets (325mg/T). On examination, the temperature was $36.0^{\circ}C$: blood pressure, 102/72 mmHg; pulse, 89/min and respiratory rate, 25/min. In both cases, ECG after 6 hours of ingestion had sinus tachycardia and developed T wave inversion on the anterior leads in the following ECGs. Their initial serum salicylate levels after 6 hours of ingestion were 71.2 mg/dL and 28.4 mg/dL respectively. These salicylate levels were resolving when these ECGs were observed. The ECG changes resolved in the following days and they were discharged without any further symptoms. Further studies are needed, but for the time being, when dealing with salicylate overdose, transient cardiac depression should be kept in mind to avoid adverse ischemic cardiac events.

놀람 자극에 대한 심혈관 반응 (Cardiovascular response to surprise stimulus)

  • 여러 연구자들이 정서특정적 자율신경계 활동을 밝히기 위하여 행복, 슬픔, 분노, 공포 및 혐오를 포함한 기본 정서들을 사용하여왔지만, Ekman 등(1983)의 기본정서들 중 놀람 정서에 대한 연구는 많지 않다. 본 연구의 목적은 ECG와 PPG를 이용하여 놀람 자극에 대한 심혈관 반응을 밝히는 것이다. 76명의 대학생에게 놀람자극을 제시하기 전과 후에 ECG와 PPG를 기록하였으며, ECG와 PPG 신호로부터 심박률(HR), R-R 간격의 표준편차(SD-RR), 연속된 R-R 간격 차이의 제곱평균제곱근(RMSSD-RR), 호흡성 부정맥(RSA), 손가락 혈류량 파형의 진폭(FBVPA), 손가락 맥파 전달시간(FPTT)을 산출하였다. HR과 SD-RR, RMSSD-RR은 놀람자극 제시 전에 비하여 놀람자극이 제시된 후에 유의하게 증가하였으며, FBVPA는 유의하게 감소하였고, FPTT는 유의하게 짧아졌다. 놀람 자극은 말초혈관을 수축시키고, 심박률을 증가시키는 교감신경계 반응특성을 가지는 것으로 볼 수 있으며, 심박률 변산성을 증가시키는 부교감신경계도 동시에 활성화될 가능성이 있는 것으로 나타났다. 이러한 연구 결과는 정서이론의 발전과, 인간정서를 탐지하는 컴퓨터 시스템의 기초를 확립하는데 기여하는 바가 있을 것이다.
