트레드밀 운동 시 호흡 저항 마스크가 호흡 기능에 미치는 영향 (Effect of Respiratory Resistance Mask on Respiratory Function during Treadmill Exercise )

  • PURPOSE: Recently, the proportion of respiratory diseases has been increasing worldwide, and deaths from respiratory diseases in Korea are increasing. Maintaining a healthy respiratory function is a crucial factor in preventing respiratory diseases. There are various ways to improve respiratory function, such as respiratory muscle and aerobic exercises. In other countries, respiratory muscle exercise is performed using a respiratory resistance mask, but such research is insufficient in Korea. Therefore, this paper proposes a respiratory exercise program using a respiratory resistance mask. METHODS: This study was conducted by dividing healthy adults in their 20s into a treadmill exercise + respiratory resistance mask group and a treadmill exercise group into an experimental group and a control group. The changes in the subject's physical function before and after exercise were confirmed by cardiopulmonary exercise and pulmonary function tests. RESULTS: As a result of the study, the experimental and control groups increased their physical function significantly (p < .05). On the other hand, when the increase rate according to the group was confirmed, the increase rate of the experimental group was higher. CONCLUSION: Based on this study, it is necessary to develop a respiratory exercise program using respiratory exercise tools such as a respiratory resistance mask and provide it easily to various subjects.

  • This study was designed to analyze normal nasal respiratory resistance in prepubertal children. The subjects consisted of 30 prepubertal children (male: 15, female: 15). The mean age was 11.4 years in male children and 11.5 years in female children. The results were as follows: I. The normal nasal respiratory patency was lower than the normal values from RION corp. 2. The normal nasal respiratory airflow rates showed no sexual differences. And there were no differences between inspiration and expiration. 3. Before and after use of nasal decongestants, there were no significant differences of normal nasal respiratory airflow rates and after the administration of nasal decongestants, nasal respiratory patency manifested lower variability. 4. The normal nasal respiratory resistance without nasal decongestants at 150 Pascal in inspiration was $0.30Pa/cm^3/sec({\pm}0.07)$ and peak nasal inspiratory airflow rate was $1016.83cm^3/sec({\pm}223.89)$. 5. The normal nasal respiratory resistance with nasal decongestant at 150 Pascal in inspiration was $0.25Pa/cm^3/sec({\pm}0.05)$ and peak nasal inspiratory airflow rate was $1148.33cm^3/sec({\pm}234.29)$.

진동 자극 유무에 따른 호흡 저항 훈련 시 폐 기능과 호흡근의 즉각적인 차이가 있을까? (Is There Any Immediate Difference between Pulmonary Function and Respiratory Muscle, with or without Vibration Stimulation in Respiratory Resistance Training?)

  • Purpose: The purpose of this study was to investigate the effect of whole body vibration combined breathing resistance on lung capacity and respiratory muscle and to suggest a mediation method for improvement of respiratory function and lung function in the future. Methods: This study was a preliminary study design of two groups of 54 healthy young adults who were randomly assigned to an experimental group (n=27) with core exercise combined with respiratory resistance and whole body vibration and a control group with respiratory resistance and core exercise (n=27). All interventions consisted of 6 core exercises every 40 seconds and rest for 20 seconds. To compare the effects of intervention, we measured spirometry and respiratory muscle strength. Results: Both the experimental group and the control group showed a significant increase in Forced vital capacity (FVC) and Maximum voluntary ventilation (MVV) (p<.05). However, FEV1 and FEV1% were significantly increase only in the experimental group (p<.05). FVC, FEV1%, Maximum Inspiratory Pressure (MIP), Maximum Expiratory Pressure (MEP) showed more significant increase in the experimental group than the control group. Conclusion: These findings indicate that whole-body vibration combined breathing resistance is an effective intervention for people, with FVC, FEV1%, MIP, MEP increase.

경피신경전기자극과 호흡근 저항운동 동시 적용이 만성 뇌졸중 환자의 호흡근 근긴장도와 폐 기능에 미치는 영향 (The Effect of the Resistance Respiratory Muscle Exercise with Transcutaneous Electrical Nerve Stimulation on Respiratory Muscle Tone and Pulmonary Function of Chronic Stroke Patients)

  • PURPOSE: This study was conducted to investigate the effect of the resistance respiratory muscle exercise with transcutaneous electrical nerve stimulation (TENS) on the respiratory muscle tone and pulmonary function of stroke patients. METHODS: Twenty stroke patients were divided into the TENS group (n = 7), placebo TENS group (n = 7), and control group (n = 6), and each intervention was performed on the three groups 5 times a week for 4 weeks. The assessment was carried out by measuring changes in the muscle tone of the latissimus dorsi and abdominal external obliques, and pulmonary function. RESULTS: In this study, the TENS group and the placebo TENS group had significant increases in the paretic side latissimus dorsi muscle tone, forced vital capacity (FVC), forced expiratory volume in 1 second (FEV1), and peak expiratory flow (PEF). There was no significant difference in changes in respiratory muscle tone between the three groups. The pulmonary function was significantly different between the three groups, and it was observed from the results of the post-hoc test that FVC showed a significant increase in the TENS group and the placebo TENS group compared to the control group. CONCLUSION: Through this study, it was found that the respiratory muscle resistance exercise was more effective as a method to increase respiratory muscle tone and pulmonary function in stroke patients than combined transcutaneous electrical nerve stimulation.

급성호흡곤란증후군에서 기도확장제 투여 전후에 호기말양압 수준의 변화가 호흡역학에 미치는 영향 (The Changes of Respiratory Mechanics by a Bronchodilator Inhalation Under the Variable Level of PEEP in Patients with Acute Respiratory Distress Syndrome)

  • 연구배경 : 급성호흡곤란증후군(ARDS)에서 호흡역학의 특징적인 변동은 폐 탄성(compliance) 감소와 폐 저항의 증가이다. 폐 저항의 증가 기전은 잘 밝혀지지 않았으나 임상에서는 ARDS 환자들의 기계환기시 기관지확장제를 흔히 투여하고 있다. 또한 상기 환자들에게 적용하는 호기말양압이 호흡저항에 미치는 영향에 대해서도 논란이 있다. 본 연구는 기계호흡을 받는 ARDS 환자들에서 기관지확장제 투여 및 호기말양압의 적용수준이 기도저항에 미치는 영향에 대하여 조사 하였다. 연구방법 : 2000년 1월부터 12월까지 서울중앙병원 내과계중환자실에서 내원하였던 15명의 ARDS 환자들 (남 : 여 14:1, 평균나이 58세) 이 대상이었다. 기관지확장제 투여 전과 투여 후(정량식흡입기로 salbutamol 100ug 씩 6회) 무작위로 호기말양압을 8, 10, 12 cm $H_2O$로 변화시키면서 일정 흡기류에서 흡기말폐쇄 방법을 이용하여 폐 탄성, 최대흡기저항, 최소흡기저항 및 부가적저항을 측정하였다. 각종 호흡역학치는 CP-100 pulmonary monitor(Bicore, Irvine, CA, USA)를 이용하여 측정하였다. 연구결과 : 1) 대상 환자들의 최대흡기저항은 $8.27{\pm}3.70cmH_2O$/L/sec로 정상치보다 증가되었으며, 최소흡기저항 및 부가적저항 모두 증가되었기 때문이다. 2) 호기말양압 10 cm $H_2O$에 비교시 호기말양압 12 cm $H_2O$에서 최대흡가저항과 최고기도압이 증가되었다. 3) 흡입제 투여시 호기말양압 8 cm $H_2O$과 10 cm $H_2O$에서는 최대흡기저항, 최소흡기저항 및 부가적저항 모두 변화가 없었으나 호기말양압 12 cm $H_2O$에서 최대흡기저항이 감소하였으며 주로 최소흡기저항이 감소되었고 부가적저항은 큰 변동이 없었다(각각 $15.66{\pm}1.99$에서 $13.54{\pm}2.41$으로, $10.24{\pm}2.98$에서 $8.04{\pm}2.34$으로, 그리고 $5.42{\pm}3.41$에서 $5.50{\pm}3.58cmH_2O$/L/sec 으로 변동). 4) 폐 탄성은 호기말양압 변화와 흡입제 투여에 변동이 없었다. 결 론 : ARDS 환자들에서 기도 저항이 증가되어 있으며 기도확장제에 의한 기도 저항의 감소 효과는 적용된 호기말 양압의 수준에 따라 다르게 나타났다.

  • The purpose of this study cephalometrically evaluated changes in the posterior airway space for patients with obstructive sleep apnea syndrome(OSAS) before and after surgical advancement of the maxilla and mandible. The change in calculated airway resistance was correlated with the respiratory disturbance index(RDI). Twenty cephalometric radiographs were traced before and after surgery to determine the posterior airway area and calculate resistance. Polysomnograms of each patient were obtained before and after surgery. All patients had a decrease in calculated airflow resistance in the airway. The mean amount of resistance was 865.15 before surgery, decreasing to 192.65 after surgery (p<0.01). Eighty-five percent of the patients experienced improvement in their RDI. Reduction in the RDI appears to be due to an increase in the posterior airway space and decrease in flow resistance.

Effect of High-Intensity Complex Exercise Program Using Whole-Body Vibration and Respiratory Resistance for Low Back Pain Patients with High Obesity

  • Objective: The purpose of this study was to investigate the effect of high-intensity complex exercise program using whole-body vibration (WBV) and respiratory resistance on pain and dysfunction, psychosocial level, balance ability, and pulmonary function in low back pain (LBP) patients with high obesity. Design: A randomized controlled trial Methods: A total of 44 LBP patients withhigh obesity (body mass index, BMI≥30kg/m2) were randomly assigned to an experimental group (n=22) and a control group (n=22). Both groups underwent a lumbar stabilization exercise program. In addition, the experimental group implemented the high-intensity complex exercise program combined with WBV and respiratory resistance. In order to compare the effects depending on the intervention methods, numeric pain rating scale (NRPS), Roland-Morris disability questionnaire (RMDQ), fear-avoidance beliefs questionnaire (FABQ), balance ability, and pulmonary function were used for measurement. Results: Both groups showed significant differences in NRPS, RMDQ, FABQ, balance ability before and after intervention (p<0.05). In addition, the experimental groupshowed significant difference in the amount of change in RMDQ, balance ability and pulmonary function values than the control group (p<0.05). Conclusions: High-intensity complex exercise program using WBV and respiratory resistance has been proven to be an effective and clinically useful method to decrease dysfunction, increase balance ablilty, and pulmonary function for LBP patients with high obesity.

Different Responses to Clarithromycin in Patients with Cryptogenic Organizing Pneumonia

  • Cryptogenic organizing pneumonia (COP) is an idiopathic interstitial pneumonia characterized by a subacute course and favorable prognosis with corticosteroids. However, some patients show resistance to steroids. Macrolides have been used with success in those patients showing resistance to steroids. A few reports showed treatment failure with macrolides in patients with COP who were resistant to steroids. In this report, we described two cases of COP who showed different responses to clarithromycin. One recovered completely, but the other gradually showed lung fibrosis with clarithromycin.

저항성 들숨근 훈련이 뇌졸중 환자의 호흡기능에 미치는 영향 (The effect of resistance inspiratory muscle training on respiratory function in stroke)

  • Background: The purpose of this study was to investigated the effects of threshold resistance inspiratory muscle training on respiratory function in chronic stroke patients. Design: Randomized Controlled Trial Methods: Eighteen patient with stroke were randomly assigned to the experimental group (n=9) and control group (n=9) all testing and training. The experimental group underwent threshold resistance inspiratory muscle training with resistance adjusted of maximal inspiratory pressure, 60 breathing a day and general physical therapy 30 minutes a day, 5 times a week for 4 weeks. The control group was taken general respiratory muscle training and general physical therapy for 4 weeks in the same way. Respiratory function, walking ability were evaluated before and after the intervention. Statistical significance of the results were evaluated by ANCOVA between control group and experimental group after intervention. Results: There was a significant increase in FVC and FEV1 in the experimental group in pulmonary function tests (p<0.05). There was a significant difference with the maximum inspiratory pressure and the maximum inspiratory flow rate between experimental and control group (p<0.05). There was no significant difference with the maximum inspiratory capacity between experimental and control group (p>0.05) but the maximum inspiratory capacity of experimental group some increased than that of control group. Conclusion: These finding gave some indications that the threshold resistance inspiratory training may benefit on pulmonary function in people with stroke, and it is feasible to be included in rehabilitation interventions with this population.

  • 본 연구는 호흡저항이 병행된 전신진동자극 훈련이 뇌졸중환자의 호흡기능 및 균형기능에 미치는 영향을 알아보기 위하여 실시하였다. 만성 뇌졸중환자 17명을 호흡저항이 병행된 전신진동자극 훈련을 적용한 실험군(n=8)과 일반 진동운동프로그램을 적용한 대조군(n=9)으로 무작위 배정하였다. 중재는 30분간 1일 1회 주 3회, 4주간 실시하였다. 중재 전후 폐활량과 호흡근력 그리고 동적/정적 균형능력을 측정하여 비교하였다. 실험군에서 정 동적 균형능력, 폐활량과 호흡근력의 전 후 비교 결과 유의한 향상을 보였다(p<.05). 그리고 실험군과 대조군을 비교하였을 때 동적 균형능력, 폐활량과 호흡근력의 결과값에서 유의한 차이를 보였다(p<.05).실험군의 폐활량과 호흡근력 또한 실험전후 유의한 향상을 보였으며, 대조군에 비해 유의한 변화량의 차이를 나타내었다(p<.05). 본 연구결과를 통하여 호흡저항이 병행된 전신진동자극훈련은 뇌졸중환자의 호흡기능 및 균형능력 향상 프로그램으로 유익할 것으로 사료된다.
