LABIAL APPROACH OF PULP TREATMENT AND RESIN RESTORATION ON DISCOLORED NECROTIC PRIMARY ANTERIOR TOOTH (변색된 유전치의 순측접근에 의한 치수치료 및 레진수복)

  • Traditional method of pulpectomy for a necrotic primary anterior tooth was done on lingual side. But it could not recover the discoloration of crown effectively. For the purpose of treating the discoloration of crown after lingual pulpectomy, additional methods of crown restoration were needed like : celluloid crown, open-faced crown, rasin-faced crown. Neverthless, these kinds of complete coverage methods had some disadvantages such as possibility of tooth fracture by increased tooth preparation. In order to overcome the shortcomings of lingual pulpectomy, labial treatment could be considered as an alternative. It is a method that treats necrotic pulp through the labial access opening. After finishing the pulp treatment, discolored labial tooth structure was removed extending from access opening. Discoloration of deep area could be masked effectively using opaque sealant. Cavity on labial side was restored with composite resin. This labial approach method has several advantages. First, it gives a direct vision for effective pulp treatment which is also very useful for children with poor behavior. Second, most of lingual tooth structure could be saved and occlusal contact of lingual surface remains undisrupted. Only nonfunctional discolored labial surface may removed. Third, complete removal of discolored part of a labial tooth and immediate resin restoration could be done effectively after pulp treatment. Moreover, it also could be used for pulp treatment having serious dental caries on labial surface with sound lingual tooth structure. This report presents cases with discolored upper anteior primary tooth, approaching labial side with successful restoration.

Adsorption Characteristic of Rare Earth Metal Ions on 1-Aza-15-Crown-5-Styrene (Hazardous Materials)-DVB Resin (1-Aza-15-Crown-5-Styrene (위험물)-DVB 수지에 의한 희토류 금속 이온들의 흡착 특성)

  • Resins were synthesized by mixing 1-aza-15-crown-5 macrocyclic ligand attached to styrene (a hazardous material) divinylbenzene (DVB) copolymer with crosslink of 1%, 2%, 5% and 20% by a substitution reaction. The characteristic of these resins was confirmed by the content of chlorine, element analysis, thermogravimetric analysis (TGA), surface area (BET), and IR-spectroscopy. The effects of pH, time, dielectric constant of solvents and crosslink on adsorption of metal ions by the synthetic resin adsorbent were investigated. The metal ion was showed a fast adsorption on the resins above pH 3. The optimum equilibrium time for adsorption of metallic ions was about two hours. The adsorption selectivity determined in ethanol was in increasing order of uranium ($UO_2^{2+}$) > manganese ($Mn^{2+}$) > praseodymium ($Pr^{3+}$). The adsorption was in the order of 1%, 2%, 5%, and 20% crosslink resin and adsorption of resin decreased in proportion to the order of dielectric constant of solvents.

Assessment of Internal Fitness on Resin Crown Fabricated by Digital Light Processing 3D Printer

  • Background: Recently, three-dimensional (3D) printing has been hailed as a disruptive technology in dentistry. Among 3D printers, a digital light processing (DLP) 3D printer has certain advantages, such as high precision and relatively low cost. Therefore, the latest trend in resin crown manufacturing is the use of DLP 3D printers. However, studies on the internal fitness of such resin crowns are insufficient. The recently introduced 3D evaluation method makes it possible to visually evaluate the error of the desired area. The purpose of this study is to evaluate the internal fitness of resin crowns fabricated a by DLP 3D printer using the 3D evaluation method. Methods: The working model was chosen as the maxillary molar implant model. A total of 20 resin crowns were manufactured by dividing these into two groups. One group was manufactured by subtractive manufacturing system (PMMA), while the other group was manufactured by additive manufacturing system, which uses a DLP 3D printer. Resin crowns data were measured using a 3D evaluation program. Internal fitness was calculated by root mean square (RMS). The RMS was calculated using the Geomagic Verify software, and the mean and standard deviation (SD) were measured. For statistical analysis, IBM SPSS Statistics for Windows ver. 22.0 (IBM Corp., USA) was used. Then, independent t-test was performed between the two groups. Results: The mean±SD of the RMS were 41.51±1.51 and 43.09±2.32 for PMMA and DLP, respectively. There was no statistically significant difference between PMMA and DLP. Conclusion: Evaluation of internal fitness of the resin crown made using a DLP 3D printer and subtractive manufacturing system showed no statistically significant differences, and clinically acceptable results were obtained.

A Surface Treatment Technique for Interim Crown Fabricated by Three-Dimensional Printing with Digital Light-Processing Technology

  • Purpose: The technique introduced in this study describes a technique for surface treatment that applies a photocuring resin to the surface of an interim crown fabricated by three-dimensional (3D) printing without a conventional polishing method. The purpose of this study was to evaluate marginal and internal fit and the intaglio surface trueness of interim crowns after surface treatment of 3D-printed crowns for clinical application. Materials and Methods: An interim crown was fabricated using a 3D printer with digital light-processing technology, and the surface support was removed. After the posttreatment process, the resin was thinly applied to the surface of the interim crown and polymerized to solve the esthetic problem of the surface without the conventional polishing process. In addition, the marginal and internal fits were measured to verify the clinical use of this technique, and the trueness was evaluated to confirm the deformation of the inner surface according to the technical application of the outer surface of the interim crown. The difference before and after the evaluation by a statistical method was verified using an independent t-test (α=0.05). Result: There was no significant difference in the marginal and internal fit before and after the application of this technique (P>0.05). There was no significant difference in intaglio surface trueness before and after the application of this technique (P=0.963). Conclusion: There was no change in the marginal and internal fit or in intaglio surface trueness of the interim crowns to which this technology was applied. This surface treatment technique is a more convenient method for interim crowns fabricated using 3D-printing technology without the conventional polishing process.

A comparison of marginal fitness by dental alloys (치관 보철물 제작에 사용되는 치과용 금속의 변연 적합도 비교)

  • This study was aimed to verify a comparison by dental alloys upon the marginal fitness of dental prosthesis. Firstly, we waxed up on 45 epoxy resin dies, pressed the margin with hands, and identified the marginal fitness with microscope. And we made a single direct spruing system type of sprue by 3 dental alloys - metal crown alloys, PFM crown alloys, and gold crown alloys - each 15, total 45 waxing up, adjusting W/P ratio and invested the cast ring. Then, we made the dental prosthesis using the electric casting machine. In these processes we followed the manufacturer's instructions, in order to maintain the other conditions from the inner and outer, which included investment and burning out. After we tried on the dental prosthesis on epoxy resin dies, we have got the means of marginal gap at 9 points with same distances, around the cervical line which was checked already, using microscope($\times$300). The results of this study were as follows : 1. Metal crown alloys showed 2.9% better in marginal fitness than PFM crown alloys, and the difference was not statistically significant(P=0.497). 2. Gold crown alloys showed 31.3% better in marginal fitness than Metal crown alloys, and the difference was statistically significant(P=0.004). 3. Gold crown alloys showed 32.4% better in marginal fitness than PFM crown alloys, and the difference was statistically significant(P=0.002). 4. We saw that Gold crown alloys was the best dental alloys in the marginal fitness among the three.(P=0.049).

A CASE REPORT ON PRIMARY INCISOR TREATMENT USING OPEN-FACE STAINLESS STEEL CROWN (Open-face Stainless Steel Crown을 이용한 유전치의 치료에 대한 증례보고)

  • In spite of the improvements of the techniques in the field of preventive dentistry, many children still present with extensive destruction of primary anterior teeth. Not only the practioner must consider the pulp state of the primary incisor, but also restore the form, function and esthetics of the tooth. Restorative treatment of primary incisor tooth requires durability, retention and esthetics. Stainless steel crowns used in restoring primary anterior teeth is retentive and durable in comparison with the composite resin, celluloid crown. But they are not esthetic. To enhance the esthetics of the anterior stainless steel crown without reducing its superior retention, an open-face stainless steel crown has been suggested. Several authors have suggested cutting away the labial portion of the stainless steel crown and placing the composite resin in that area. By following this technique, the practioner can prepare a retentive, durable, and esthetic restoration for primary teeth which have suffered from extensive loss of teeth structure. In addition, the single missing primary anterior teeth can be successfully restored by soldering the stainless steel crown together. Open-face stainless steel crown is indicated in the areas of large interproximal lesions involving incisal edge, crown fracture with pulp exposure and congenital malformation of the teeth. By this technique, the practioner can restore primary anterior teeth successfully regardless of the amount of remaining tooth structure, bruxism habit and presence of attrition. In this case, rampant caries with extensive loss of tooth structure and single missing of primary anterior tooth hart been successfully treated with open-face stainless steel crown.

Comparison of Crown Shape and Amount of Tooth Reduction for Primary Anterior Prefabricated Crowns (유전치 기성 크라운의 형태 및 치질 삭제량 비교)

  • The purpose of this study was to obtain instructions for size selection of prefabricated crown and tooth reduction by 3-dimensional analysis of the size and shape of the maxillary primary central and lateral incisors and prefabricated crowns (celluloid strip, resin veneered stainless steel, and zirconia crowns). The maxillary primary central and lateral incisors of 300 Korean children was scanned with three types of prefabricated crown to create standard three-dimensional tooth models and prefabricated crowns. The shapes of the prefabricated crowns and natural teeth were compared according to four parameters (mesio-distal width, height, labio-palatal width, and labial surface curvature coefficient) and calculated the amount of tooth reduction required for each prefabricated crown. The size 2 resin veneered stainless steel crown, size 1 zirconia crown, and size 2 celluloid strip crown were most similar in shape to the primary central incisor. The size 3 rein veneered stainless steel crown, size 2 zirconia crown, and size 3 celluloid strip crown were most similar to the primary lateral incisor. The amount of tooth reduction was similar in both maxillary primary central and lateral incisors. The incisal reduction was greatest for the zirconia crown. At the proximal surface, the zirconia and celluloid strip crowns required a similar amount of tooth reduction, but more than the resin veneered stainless steel crown. The labial surface reduction was greatest for the zirconia crown. The degree of lingual surface reduction was not significant among the three prefabricated crowns. Among the assessment parameters, mesio-distal crown width was the most important for choosing a prefabricated crown closest to the actual size of the natural crown.

  • Amelogenesis imperfecta represents a group of hereditary conditions that manifest enamel defects without evidence of generalized or systemic disorders. These enamel disorders are apparently heterogeneous in the basic chemical structure, resulting in a diverse presentation of clinical characteristics. The reported prevalence of amelogenesis imperfecta varies from 1 in 14,000-16,000 to 1.4 in 1,000 depending on specific population studied with the autosomal dominant hypocalcification type of amelogenesis imperfecta believed to be the least prevalent. The most widely accepted current classification system for delineating the amelogenesis imperfecta types considers the mode of inheritance and clinical manifestations. Three major groups are recognized; hypoplastic, hypocalcified, and hypomaturation types. Delineating specific types of amelogenesis imperfecta can be confusing due to the phenotypical similarity of many forms and that the most recent classification lists 14 different types. A 12 year-old female patient came to our pediatric dentistry clinic complaining of the ugly shape and color of her teeth, especially the upper front area. Although the goal of the treatment was mainly focused on the improvement of patient's esthetics, longevity of the restorations was also considered in selecting the appropriate restorative system, resin jacket crown, which can satisfy the both aspects.

The synthesis of these resins was confirmed by content of chlorine, element analysis, electron micrograph, and IR-spectrum. The effects of pH, time, dielectric constant of solvent and crosslink on adsorption of uranium (${UO_2}^{2+}$) ion were investigated. The uranium ion showed a fast adsorption on the resins above pH 3. The optimum equilibrium time for adsorption of metallic ions was about two hours. The adsorption selectivity determined in ethanol was in increasing order uranium (${UO_2}^{2+}$) > nickel ($Ni^{2+}$) > gadolinium ($Gd^{3+}$) ion. The adsorption was in order of 1%, 2%, 4%, and 8% crosslinked resin and adsorption of resin decreased in proportion to order of dielectric constant of solvents.

Resin Synthesis of Adsorbent Metal Ions using 1-Aza-12-Crown-4 (1-Aza-12-Crown-4를 이용한 금속 이온 흡착제 수지 합성)

  • Content of chlorine in s쇼rene-DVB copolymer was decreased as crosslink increased and it is because as crosslink increased $1\%,\;2\%,\;5\%\;and\;10\%$ DVB content increased and crosslink density increased and cavity was reduced. Functional group of resin almost disappeared as C-C1 peak around $700cm^{-1}$ was substituted with 1-aza-12-C-4 macrocyclic ligand and new peak of C-N around $1020cm^{-1}$ appeared, so it was confirmed that styrene-DVB copolymer and ligand were compounded. As crosslink increased in the analysis of element contents, it resulted in the reduction of nitrogen content and it is because as crosslink increased, it led to the reduction of chlorine content in the process of substitution reaction and it affected macrocyclic ligand substituted. Form of functional synthetic resin showed distortion of its particles as macrocyclic ligand was introduced to styrene-DVB copolymer and hydrogen of ligand caused substitution with chlorine element of styrene molecule.
