Effect of xylitol on dental caries prevention : a literature review

  • Xylitol is a sweetening agent used to prevent dental caries. It is formulated and consumed in various forms, and its use is recommended in particular for oral health. However, dental specialists are not familiar with xylitol or well-acquainted with the appropriate method to use it. Furthermore, the caries-prevention effect of xylitol has been controversial. Thus, in this study, we aimed to analyze previously published studies on the dental caries prevention effect of xylitol to verify its true effects and establish an appropriate method to use it. We analyzed 94 clinical research manuscripts obtained from domestic and international online databases and found that although 11 manuscripts reported no significant dental caries prevention effect of xylitol, most manuscripts reported dental caries prevention with xylitol. Compared to the manuscripts that reported dental caries prevention with xylitol, those are assessed lower amounts of xylitol or shorted consumption periods, and had dosage form that could not retain xylitol sufficiently long in the mouth, similar to milk. Thus, consumption of 5-11 g of xylitol in the form of gums, candies, or tablets over a long time period could effectively reduce the risk of dental caries. It has the advantage of ease of consumption, regardless of the gender, age, or disability of the person.

Factors Affecting Patient Satisfaction of Dental Services Organizations (치과의료서비스에 대한 환자만족 영향원인 -경남 일부 지역 치과의원을 중심으로-)

  • The main objective of this study is to investigate the factors affecting patient satisfaction, repeat-use intention, and inducement intention for providing the suggestion for patient-oriented management of dental services organizations. For this study, the patient satisfaction model including 14 facets of satisfaction factors such as distance(time/location), human services(dentist-hygienist), treatment results, and facilities(convenient facilities/parking lot) was examined. Data were collected from 196 patients who visiting the same dental services organization more than two times of ten dental clinics at Kyeongsangnam-do area using self-administered questionnaire. Major results of the empirical analysis are as follows: First, patient satisfaction, repeat-use intention, and inducement intention were significantly correlated with gender, age, economic conditions among patients of dental clinics. Second, kindness of hygienist, treatment results, kindness of receptionist, waiting time, emergency medical services, and right-fee for services were found to have significant influence on dental services organizations. These research findings suggest that improvement of service quality in dental care and advancement in treatment ability of dentist and hygienist are very important to improve patient satisfaction and patient-oriented service system in dental services organizations.

A Study on Curriculum of Dental Hygienists (치과위생사 교육과정 연구)

  • The aims of the study are as following : First, define and clarify the role and function of the dental hygienist. Second, based on thai definition, present an improved dental hygienist curriculum, improving their special competence. This study seeks to present a way for promoting role of dental hygienists as independent specialists, rather than as auxiliaries, contributing to better dental service in the process. The research was pursued by surveying by questionnaire those directly related to dental hygiene. They are 58 professors of dental hygiene, 164 dentists, 253 dental hygienists and 496 dental hygiene students, selected by the method of simple random sampling. Due to the small size of the sample, professors' group was analysed only through frequency and probabilities. For groups with larger samples(dentists, dental hygienists, dental hygiene students), methods of chi square(${\chi}^2$) tests, frequency analysis and one-way ANOVA were used in their analysis. Conclusions of the study are as following : First, role and function of the dental hygienist was defined and clarified. According to the analysis of the survey, main role of the dental hygienists as perceived by 62.9% of dentists was supporting treatments and managing patients and facility. This perception by dentists differs significantly to perception of other groups, which saw main role of dental hygienists as preventive dental treatment and dental hygiene education. Also, only 20.2% of dental hygienists perceived patients and facility management as their main role. This divergence of perception between groups weakens speciality competence of dental hygienists. To strengthen dental hygienists' speciality competence, preventive dental treatment and dental hygiene education must be defined as main role of dental hygienists. Furthermore, role of dental hygienists can be categorized into cooperative and auxiliary role. Second, in order to strengthen speciality competence of dental hygienists, their education must be augmented with curriculum emphasizing cooperative role. Specifically, courses on preventive dental treatment, dental hygiene education and dental X-ray photography must become compulsory, as well as becoming more comprehensive and organized. Also, training in schools and hospitals must be focused on those subjects. In addition, courses concerning auxiliary role must be incorporated into optional courses and more pedagogical courses are needed to widen students' career choices. Third, practical relevance of national dental hygienist certification examination must be heightened. In the survey, all groups gave negative response to questions concerning practical relevance of national dental hygienist certification test. Specifically, contents of the tests must be changed to test participants' competence in the cooperative area. Also, the test itself must be strengthened by adding practical examination to the certification scheme, as well as having more difficult examinations.

Bacteria reduction ratio by cleansing methods of latex gloves (라텍스 장갑의 세척 방법에 따른 세균 감소율 측정)

  • Objectives : Latex gloves hygiene is the most effective method to prevent infection of microorganisms and to reduce the incidence of cross infections. The aim of this study was to compare the bacteria reduction ratio of cleansing with water, liquid soap and alcohol gauze. Methods : The left side glove was the control group and the right side was the experimental group. The experimental group washed hand with water, soap, and alcohol gauze. The hand plate was inoculated by the hand and inoculated for 24 hours in $35^{\circ}C$. Results : Washing with water showed that CFU of control group was 1116.9 and that of experimental group was 302.8. Hand washing by water reduced 74.3% of bacteria. Liquid soap revealed that CFU of control group was 619.9 and that of experimental group was 8.3. Hand washing by liquid soap reduced 97.5% of bacteria. Alcohol gauze included 875.2 CFU in control group and 5.8 CFU in experimental group. Washing by alcohol gauze reduced 99.5% of bacteria. Conclusions : Based on the results, the most effective latex gloves cleansing method was recommended as the standardized hand washing with the liquid hand soap and alcohol gauze. The results can be used to improve training strategies for enhancing glove hygiene practice in dental clinic.

Effectiveness and Learning Experience of Dental Communication Course Training in Dental Hygiene Schools (치위생학과 교육과정에서의 치과의료커뮤니케이션 교과목 운영 사례연구)

  • The purpose of this study was to develop a dental communication course in dental hygiene schools that included theory, practice, and skill for effective dental communication. Thirty-six senior dental hygiene students in a dental hygiene school took a dental communication course and responded to a questionnaire. The instrument used in the study was a modified form of Kim's communication skill self-assessment sheet. The self-assessment questionnaire about communication competencies was administered before and after the class, and the difference between scores at each time point was analyzed using the Wilcoxon signed rank sum test. Among seven elements of dental communication competencies, the competency of "information gathering" was significantly improved (p=0.008). Students' scores on perception of the importance (p=0.019) and necessity (0.016) of a communication course significantly increased after the course. Competencies in communication are essential requirement for dental hygienists. This study showed the possibility of communication skill training. An objective evaluation tool regarding students' communication competencies should be developed. Further studies with larger samples size are needed to develop a standardized comprehensive communication course in dental hygiene schools.

A Study on dental hygiene and nursing students' perception and attitudes about medical market opening (치위생과, 간호과 학생의 의료시장개방에 대한 인식 및 태도에 관한 연구)

  • Objectives : This study was conducted during the period from August 30 to September 9, 2011 in order to survey difference in the general perception of medical market opening and factors related to the choice of foreign hospitals among dental hygiene and nursing students at universities in Seoul. Methods : For this purpose, 438 students were surveyed using a questionnaire and collected data were analyzed using SPSSWIN 18.0. Conclusions drawn from this study are as follows. Results : 1. With regard to dental hygiene and nursing students' perception of medical market opening according to general characteristics, significant difference was not observed according to gender, experience in working at a hospital, medical institution used, and the frequency of using medical institutions, but significant difference was observed according to department, and interest in healthcare-related news. 2. There was significant difference in dental hygiene and nursing students'pro/con attitude toward medical market opening, but not in gender, experience in working at a hospital, medical institution used, and the frequency of using medical institutions. 3. With regard to intention to visit and revisit foreign hospitals, there was significant difference between dental hygiene and nursing students in intention to visit but not in intention to revisit. Conclusions : The results of this study suggest that more research on the medical market opening portion dental hygiene and nursing students' perception and attitude did not differ significantly, so the more accurate and open markets for a variety of medical education and school education and a variety of materials through hands-on experience be grasped should allow. Furthermore, students' acquisition of accurate prior knowledge about medical market opening is expected to be helpful to activate their employment in overseas.

A study on anxiety about dental radiography among adults (성인의 치과 방사선 촬영에 대한 불안감에 관한 연구)

  • Objectives: This study was conducted to help reduce the anxiety about dental radiation exposures of people and to provide a way to instill proper awareness of dental radiation. Methods: To fulfill the research aim, this study conducted a survey to 330 subjects who are 20 years old or over living in Seoul and Gyeonggi-do during the month of December 2016. The collected data were analyzed using frequency analysis, t-test, one-way ANOVA, Pearson's correlation coefficient and multiple regression analysis among others. Results: As a result of examining the correlation between the anxiety of dental radiography and the correlations between the variables showed that the knowledge of radiation knowledge and the need for radiography (r=0.186, p<0.01), thinking about shooting (r=0.137, p<0.05), and the effectiveness of wearing protective equipment (r=0.120, p<0.01), showing statistically significant differences. As a result of examining the factors influencing anxiety of dental radiography among adults, high awareness of the need for dental radiography (p<0.01), the more the idea of dental anxiety in dental radiography is radiography showed higher (p<0.01). Conclusions: It seems proper that adequate dental radiography requires adequate explanation about it to patients, wearing of protective equipment, and appropriate dental radiation safety education.

Research study on the grade of subjective symptom and recognition of oral malodor of women's college students or co-eds (여대생의 구취자각정도와 인지에 관한 조사 연구)

  • I investigated the situation of self-realization for oral malodor and real occurrence of it and researched the situation of coincidence by self-administrated questionnaire and real oral malodor of dental hygiene students in Kyeonggi province and Kyeongbuk province to use as a reference data on prevention and treatment of oral malodor. The obtained results were as follows: 1. Concerning the grade of the subjective symptom of oral odor, a little bit oral malodor was the highest by reaching 77.6%, and no oral malodor was 20%. 2. The time when one feels the oral malodor highest was revealed immediately after awakening from the sleep by running up to 88.2%. 3. Concerning the extent of aversion during the occurrence of oral malodor from other people, 57.6% expressed as unpleasant, and 3.5% showed no aversion. 4. Concerning the intention to participate in the prevention program against the oral malodor, 51.8% had intention of it. 5. Hydrogen sulfide 7.61V19.30, methyl mercaptan 9.53V67.90, dimethyl sulfide 58.31V121.37(pF0.05) marked as causing factors in the 132 respondents who answered that they had a little bit oral malodor in comparison with the grade of subjective symptom and the measurement of actual oral malodor. As the above-mentioned results were obtained by limited subjects, the more diversified and precise comparative study is considered to be needed through the classification of various levels of research subjects.

A Study on the Impact of Relationship on the Oral Health Behaviors among University Students (일부지역 대학생들의 이성교제 여부가 구강건강행태에 미치는 영향)

  • Purpose : The purpose of this study was to investigate whether or not the effects of the relationship among college students on the oral health were affected by the increased interest in the appearance. Methods : After explanation of this study, we conducted the questionnaire to those who accepted questionnaire from April 26 to May 28, 2017. In order to collect the data, Naver-form (mobile) questionnaire was distributed to the research subjects and the collected data. The analysis technique used the frequency and percentage to examine the general characteristics of the subjects and the status of the relationship. Chi-square test was used to examine the number of toothbrushes, toothbrushing time, oral health concern, regular oral examination and scaling, use of oral care products, smoking status, and bad breath. Result : Among the total of 190 research subjects, 79 (41.6 %) were male and 111 (58.4 %) were female. The presence of brushing just before going out and carrying a toothbrush were more frequent for those subjects who are in relationship (p<.001). The subjects who considered oral hygiene to be important were higher in those who were in relationship (p<.001). Conclusion : Some oral health care behaviors such as toothbrushing just before going out or carrying a toothbrush were well formed, and oral hygiene was important in those who were in relationship.

Comparison of dental information websites in some nations (일부 국가의 치과 웹사이트 비교)

  • Objectives : The purpose of this study was to compare the dental information websites in some nations and to provide the basic data for clinical application. Methods : Two hundred twenty-four dental websites were chosen by using Yahoo search engine. The websites included 59 from the United States, 50 from the United Kingdom, 54 from Canada, and 61 from Korea. The survey was conducted from August 15 to September 30, 2011. Results : The guidance of medical treatment expenses was the most sustainable in the United Kingdom and followed by the United States, Canada, and Korea(p<0.001). Korean dental information website provided the visitor's message. The United Kingdom provided the best type of FormMail and the United States, Canada and Korea also provided(p<0.001). Korean dental information website showed the best types of messages and was followed by the United States, Cadana and the United Kingdom. Korean dental information had the best type of FormMail(p<0.001). Conclusions : In order to make the accurate access to the dental health information website, it is necessary to provide the easy access and continuous research efforts to the information system.
