성인초기 여성의 e헬스 문해력, 생식건강지식, 자아존중감이 건강증진행위에 미치는 영향: 설문조사연구 (The influence of eHealth literacy, reproductive health knowledge, and self-esteem on health-promoting behaviors in early adult women: a cross-sectional survey)

  • Purpose: The purpose of this study was to investigate the influence of eHealth literacy, reproductive health knowledge, and self-esteem on early adult women's health-promoting behaviors (HPB). This study was based on Pender's health promotion model as a theoretical underpinning. Methods: Early adult women aged 18 to 35 years (n=165) were recruited by posting advertisements on social network sites for a student club and a faith-based community in Ansan, Korea. Willing individuals were invited to participate in the online survey from June 1 to June 30, 2022. Standardized instruments were used to measure HPB, eHealth literacy, reproductive health knowledge, and self-esteem. General characteristics included income level, perceived subjective health, and internet usage time. The collected data were analyzed using the independent t-test, one-way analysis of variance, Pearson correlation coefficients, and multiple regression. Results: The mean age of the respondents was 21.97±3.87 years. The total HPB score was 120.69, corresponding to a moderate level; and the total scores for eHealth literacy (30.24), knowledge of reproductive health (23.04), and self-esteem (35.62) were higher than the midpoint. The model explained 53.3% of variance in HPB, and self-esteem (β=.48, p<.001) was the most influential factor. Other influential factors were, in descending order, higher economic level, higher subjective health status, greater eHealth literacy, and less internet use time (<2 hours/day). Conclusion: In order to promote the health of early adult women, counseling or programs that positively improve self-esteem appear promising, and eHealth literacy should be considered as a way to promote HPB using information technology.

Medicinal plants traditionally used for the management of female reproductive health dysfunction in Tana River County, Kenya

  • Reproductive dysfunction is a major health concern amongst the inhabitants of Tana River County. An ethno botanical study was conducted in Garsen, Itsowe and Ngao sub divisions of Tana River County to document the utilization of medicinal plants for the management of female reproductive ailments. The target population was practicing herbalists from Pokomo, Ormo and Giryama communities in the study area. Structured questionnaires and focussed group discussions were used to collect data. Forty eight plant species distributed in 40 genera and 29 families were documented as being important for the management of pregnancy related complications, menstrual disorders, infertility, fibroids and as contraceptives. The species most frequently cited by the herbalists were fourteen. Fifty two percent of the plant species were probably being mentioned for the first time as being useful in reproductive health management. In conclusion, Tana River has a pool of TMPs with a wealth of indigenous knowledge that needs to be exploited. The plants used to treat dysmenorrhea for example may be important analgesic agents that need further investigation while those with anti-fertility properties may contain steroidal phyto chemical compounds. Such species therefore need further investigation to establish their efficacy and mechanism of action.

  • This review summarizes the epidemiology of cancer of the female reproductive system and associated lifestyle factors. It also assesses the available evidence for occupational factors associated with these cancers. Cervical, endometrial, and ovarian cancers are relatively common, and cause significant cancer morbidity and mortality worldwide, whereas vulvar, vaginal, fallopian tube cancers, and choriocarcinomas are very rare. As several lifestyle factors are known to play a major role in the etiology of these cancers, very few published studies have investigated possible relationships with occupational factors. Some occupational exposures have been associated with increased risks of these cancers, but apart from the available evidence on the relationships between asbestos fibers and ovarian cancer, and tetrachloroethylene and cervical cancer, the data is rather scarce. Given the multifactorial nature of cancers of the female reproductive system, it is of the utmost importance to conduct occupational studies that will gather detailed data on potential individual confounding factors, in particular reproductive history and other factors that influence the body's hormonal environment, together with information on socio-economic status and lifestyle factors, including physical activity from multiple sources. Studies on the mechanisms of carcinogenesis in the female reproductive organs are also needed in order to elucidate the possible role of chemical exposures in the development of these cancers.

노출기준 설정 화학물질의 CMR물질 정보 제공에 관한 연구 (A study on the provide of CMR substances information for Threshold Limit Values (TLVs) chemicals in KMoEL)

  • Objectives: This study was performed to provide workplaces with political guidelines that apply international CMRs (Carcinogens, Mutagens, Reproductive toxins) information to Public Notice of TLVs (Threshold Limit Values). We analyzed information supply status about CMRs of international agencies and compared substances for which TLVs are set in KMoEL (Ministry of Employment and Labor in Korea). Methods: We referred to the reliable literature about classification criteria of CMRs corresponding to UN GHS (Globally Harmonized System of classification and Labeling of chemicals) and Public Notice No. 2009-68 'Standard for Classification, Labeling of Chemical Substance and Material Safety Data Sheet' in KMoEL. The classification system of CMRs in professional organizations (IARC, NTP, ACGIH, EU ECHA, KMoEL, etc.) was investigated through the internet and literature. Conclusions: 191 chemical substances among total 650 substances with TLVs are classified as carcinogens. Also, 43 substances classified as mutagens, and 44 as reproductive toxicants. These results suggest that the information of CMRs in Public Notice of TLV will be reorganized to 191 carcinogens, 43 mutagens, and 44 reproductive toxicants.

Awareness of Reproductive Health Risks, Sex Hormone Levels and Sperm Indices among Farmers Exposed to Pesticides in Akungba Akoko, Nigeria

  • Background: The indiscriminate use of pesticides in Nigeria may have harmful effects on reproductive health of farmers. Objectives: This study assessed the awareness of reproductive health, serum follicle stimulating hormone (FSH), luteinizing hormone (LH), testosterone, estradiol, progesterone and sperm characteristics of male farmers occupationally exposed to pesticides. Methods: Eighty four male farmers were recruited for the study. Structured questionnaire was used to obtain the socio-demographic data. Blood and semen samples were collected from the subjects in the morning for hormonal assays and semen analysis using enzyme linked immunosorbent assay (ELISA) method and SQAV sperm quality analyzer. Data were analyzed using chi square, Student's-t-test, and Regression analysis. Results: Serum FSH (p<0.01), LH (p<0.005) and Estradiol (p<0.001) were significantly higher while prolactin (p<0.02) and testosterone (p<0.001) were significantly lower among pesticides exposed farmers than nonexposed subjects. Some 34/84 (40.5%) of the pesticides exposed farmers had serum testosterone levels below the lower limit of the reference range. Those with low testosterone levels (p<0.001), also had FSH (p<0.05), LH (p<0.001) and Estradiol (p<0.002) significantly lower than those with normal testosterone levels. The sperm count among pesticides exposed farmers; total motility and percentage morphology were significantly lower than non-pesticides exposed subjects. Some 14/84 (16.7%) of the pesticides exposed farmers had sperm count below 15 million/mL (oligozoospermia). More than 70% of the farmers were not aware of the reproductive health risks associated with pesticides and only 23.8% of the farmers were using protective devices. Conclusions: Deliberate efforts to improve awareness, knowledge, personal hygiene, and interventions necessary to lessen both pesticides exposure and health risks by adopting safe practices are suggested.

Initiating Smokeless Tobacco Use across Reproductive Stages

  • Background: The use of smokeless tobacco (SLT) among women is increasing in India, especially among those with limited education and resources. Preventing the initiation of SLT among women is critical since it has known negative consequences for oral and reproductive health. Most research on tobacco initiation in India focuses on adolescents. This paper addresses the unrecognized issues of post marital initiation among women of reproductive age, highlighting the importance of reproductive stages in women's tobacco initiation. The objective is to examine the correlates of SLT initiation among low income women in Mumbai from pre-marriage through early marriage, first pregnancy and beyond, using case examples to illustrate initiation during each of these stages. Materials and Methods: In 2011-2012, cross-sectional community level survey data were collected from a representative sample of 409 daily SLT-using married women aged 18-40 years in a low income community in Mumbai. Information on socio-demographics, initiation by reproductive stage, types of tobacco use, childhood exposure to tobacco, learning to use, and initiation influences and reasons were collected through a researcher-administered survey. Univariate and bivariate analysis assessed factors influencing initiation of SLT use by reproductive stage. In addition 42 narratives of tobacco use were collected from a purposive sample of pregnant and non-pregnant married women addressing the same questions in detail. Narratives were transcribed, translated, and coded for key concepts including initiation of tobacco use. Results: Thirty-two percent of women initiated SLT use before marriage, 44% initiated after marriage but before pregnancy, 18.1% initiated during their first pregnancy and the remainder started after their first pregnancy. Mean age of marriage among women in this study was 16 years. Younger women (i.e. age at time of the interview of less than 30 years) were 0.47 [95% CI (0.32, 0.87)] percent less likely to initiate after marriage than women aged more than 30 years. Women who got married before 18 years of age were 2.34 [95% CI (1.40, 3.93)] times more likely to initiate after marriage than their counterparts. Childhood exposure was a predictor for initiating SLT use prior to marriage but not after. Women reporting tooth and gum pain were 1.85 times more likely to initiate after marriage than their counterparts. Husband and neighbours were the most significant influences on post-marital initiation. Narratives highlighted differences in processes of initiation pre and post marriage and during pregnancy. Conclusions: Most tobacco prevention interventions are directed to adolescents in school. This study suggests that especially for low literate or illiterate women, school based interventions are ineffective. To be effective strategies to prevent SLT initiation must reach women in urban areas at or immediately after marriage and during their first pregnancy. Messages must negate culturally rooted beliefs about the health benefits of SLT in order to prevent initiation and onset of daily use.

북한이탈여성의 성지식, 성태도, 생식건강 증진행위: 소외된 여성의 성 (Sexual Knowledge, Sexual Attitude and Reproductive Health Promotion Behavior among North Korean Women Defectors: alienated women's sex)

  • 본 연구는 북한이탈여성의 재북당시 생식건강 관련 경험을 탐색하고 성지식, 성태도, 생식건강 증진행위 수준을 확인하기 위해 수행된 수렴병렬 혼합설계 연구이다. 2020년 4월에서 7월까지 수도권에 거주하는 8명의 북한이탈여성을 눈덩이 방식으로 표집 후 심층면담하여 질적자료를 수집하였고 수도권, 충청권, 경상권에 거주하는 175명의 북한이탈여성을 편의표집하여 성지식, 성태도, 생식건강 증진행위에 대한 설문조사를 실시하였다. 심층면담 결과는 내용분석을 실시하였고, 설문 결과는 SPSS 23.0 프로그램을 이용하여 independent t-test, ANOVA, Pearson's Correlation Coefficients로 분석하였다. 질적자료 분석결과 북한이탈여성의 북한에서 경험한 생식건강 관련 경험은 '잘 알지 못하여 두려운 성', '의무만 강요하는 부끄럽고 귀찮은 성', '모든 것이 제한된 속에서 내가 스스로 알아서 관리해야하는 성'으로 나타났다. 양적자료 분석결과 대상자의 성지식은 연령이 높은 군과 기혼 군에서 높았고 성태도는 연령이 낮은 군에서 더 개방적인 것으로 나타났다. 생식건강 증진행위는 경제수준이 높은 군에서 더 활발하였고 성지식과 성태도는 양의 상관관계를 보였다. 국내에 정착한 대상자들이 재북기간에 가졌던 성에 대한 소극적인 태도를 극복하고 생식건강 증진행위를 수행할 수 있도록 지역사회의 지지체계 및 자원의 제공이 필요하며, 북한여성의 생식건강에 대하여 인도주의적 지원의 우선순위가 요구된다.

한국 농촌지역의 임신효율 (Reproductive Efficiency)에 관한 연구 : 충남 서산지역을 중심으로

  • Recently in Korea, integration of F.P. & MCH programs for effective and efficient implementation of the health programs has been discussed actively. In fact, categorical health workers in fields have been trained and changed as an integrated health workers by government. But one of the most important problems that had to be solved for successful integration of F.P. and MCH programs, is that there must be a common indicator for the evaluation of the two health services (integrated indicator). We regarded reproductive efficiency (=R.E.) that had been proposed by Charlotter M$\ddot{u}$ller et al, as the good integrated indicator. The object of this brief article is to introduce the meaning of reproductive efficiency and to illustrate the usefulnesses of R.E. as the integrated health indicator by applying this indicator to the data from preliminary survey of Seosan demonstration project for integration of F.P. & MCH service supported by WHO. The results and conclusions are as follows 1) Definition of R.E. is the percentage of pregnancies that succeed in production normal, surving children after taking into account the frequency of all measurable types of adverse outcomes (End point for evaluation of survival is one year of age). 2) On the basis of the past pregnant history, reproductive efficiency of the 2,484 eligible women (15-44 years) was roughly 75% (But, in the concept of good births, it is not regarded whether the survived infant is normal health or not). 3) Compared with the results of the other two surveys of the rural area in Korea, reproductive efficiency has been slightly decreased than before, in spite of family planning and MCH services for past 20 years. Because the quantity of increased abortion rate overwhelmed that of the decreased infant mortality rate. 4) Reproductive efficiency has the object for measure many events during the period from the conception (Wanted pregnancy) to an normal surviving children as an 1 year of age. So these heterogenous adversities, ie, induced abortion, still births, spontaneous abortion, neonate & infant death, are aggregated as R.E. However, if the information of these important events and reproductive efficiency were given, R.E. is used as the comprehensive evaluation indicator for F.P. and M.C.H. after meticulous analysis the various components of R.E. 5) Economic loss for adverse outcomes of preg were pregnancy were calculated applying the medical cost at the relatively small sized hospital of small city. Economic loss for 100 cases of adverse outcome is 10,420,000 won, and economic loss for infant death is 46.1% of the total loss. So, it is rational to invest much more effort and than before to MCH programs.

여대생의 인터넷 생식건강정보 탐색에 영향을 미치는 요인 연구 (Factors Affecting Female College Students' Reproductive Health Information Seeking Behaviors on the Internet)

  • 본 연구의 목적은 여대생들의 생식건강정보 탐색행위에 영향을 미치는 요인을 살펴보고 그 관계성을 살펴보는 것이다. 건강신념모델(HBM)과 계획된행동이론(TPB)을 기반으로 지각된 민감성, 지각된 심각성, 지각된 이익, 지각된 장애, 주관적 규범, 지각된 행동통제, 감정적 평가를 주요 요인으로 정의하고 연구를 설계하였다. 대학생 온라인 커뮤니티인 '에브리타임'을 통해 서울 소재 4년제 대학교의 여대생을 대상으로 온라인 설문을 실시하여 데이터를 수집하였다. 연구결과, 여대생들은 지각된 민감성, 지각된 이익, 주관적 규범이 높을수록, 반면에 지각된 장애는 낮을수록, 인터넷을 통해 생식건강정보를 탐색할 의도가 높은 것으로 나타났다. 또한 여대생들의 인터넷 생식건강 탐색에 영향을 미치는 요인들은 여대생들의 성경험 유무, 생식기계 질환 경험 유무, 건강관심도 등에 따른 그룹 간의 차이를 보이기도 했다. 본 연구결과는 여대생들을 대상으로 하는 대학도서관이나 보건기관 등이 온라인 건강정보 문해교육이나 관련 서비스 프로그램을 개발하는데 있어 여대생들의 생식건강 인식 정도를 파악하는데 기여할 수 있을 것으로 기대한다.
