Conservative Treatment of Thoracic Radiculopathy due to Ossification of the Yellow Ligament in a Young Patient -A case report- (황색인대 골화증으로 인해 발생한 흉추부 신경근병증 환자의 보존적 치료 -증례보고-)

  • Ossification of the yellow ligament (OYL) is a pathologic condition that causes spinal stenosis, which is a form of ectopic ossification. OYL causes compressive myelopathy and radiculopathy. Although the pathogenesis of OYL is still unclear, diffuse mechanical stresses and degenerative changes caused by extreme ranges of motion may be related to the development of OYL in young sportsmen. Here we report an interesting case of thoracic radiculopathy due to OYL in a 35-year-old male amateur judo player who was successfully treated with continuous thoracic patient controlled epidural analgesia and epidural adhesiolysis.

  • Industrial control system was designed mainly in the form of analog in early days. However, necessity of digital system engineering is increasing recently because systems become complicated. Consequently, stability of digital systems is improved so most industrial control systems are designed with digital. Because Using digital design of Industrial control system is expanded, various threatening possibilities such as penetration or destruction of systems are increasing enormously. Domestic and overseas researchers accordingly make a multilateral effort into risk analysis and preparing countermeasures. In this paper, this report chooses common security requirement in industrial control system and nuclear control system through relevant guidelines analysis. In addition, this report suggests the development plan of nuclear cyber security system which will be an essential ingredient of planning approvals.

Autonomic Self Healing-Based Load Assessment for Load Division in OKKAM Backbone Cluster

  • Self healing systems are considered as cognation-enabled sub form of fault tolerance system. But our experiments that we report in this paper show that self healing systems can be used for performance optimization, configuration management, access control management and bunch of other functions. The exponential complexity that results from interaction between autonomic systems and users (software and human users) has hindered the deployment and user of intelligent systems for a while now. We show that if that exceptional complexity is converted into self-growing knowledge (policies in our case), can make up for initial development cost of building an intelligent system. In this paper, we report the application of AHSEN (Autonomic Healing-based Self management Engine) to in OKKAM Project infrastructure backbone cluster that mimics the web service based architecture of u-Zone gateway infrastructure. The 'blind' load division on per-request bases is not optimal for distributed and performance hungry infrastructure such as OKKAM. The approach adopted assesses the active threads on the virtual machine and does resource estimates for active processes. The availability of a certain server is represented through worker modules at load server. Our simulation results on the OKKAM infrastructure show that the self healing significantly improves the performance and clearly demarcates the logical ambiguities in contemporary designs of self healing infrastructures proposed for large scale computing infrastructures.

  • Objectives We report 1 treatment case with Euphorbiae Kansui Radix which is based on Sasang constitutional Medicine for Chronic Dizziness in Soyangin. Methos We treated Soyangin patient suffering form chronic dizziness. We prescribed Euphorbiae Kansui Radix for her physical symptoms. The improvement of her dizziness was evaluated by Visual Analogue Scale(VAS). Results After the Euphorbiae Kansui Radix was given, the patient's Dizziness was improved. And there was not any adverse effect. Conclusions This study shows that using Euphorbiae Kansui Radix can be effective treatment for Soyangin chronic dizziness patient.

Operative Treatment of the Bilateral Multiple Rays Brachymetatarsia using Callotasis (A Case Report) (가골 신연술을 이용한 양측성 다발열 단중족증의 치료 (1예 보고))

  • Among congenital brachymetatarsias fourth metatarsal bone shortening is most common form and unusually involves multiple metatarsal bones. We report a case of the patient with bilateral 1st and 4th multiple rays brachymetatarsia treated with callostasis. Twenty eight months after procedure, right side showed 1st metatarsal lengthening about 11.5 mm (29.23%) and 4th metatarsal 17.86 mm (35.87%). Twenty one months later, left side showed 1st metatarsal lengthening about 14.58 mm (36.8%) and 4th metatarsal 20.52 mm (43.01%). In healing index right side showed 1st metatarsal 3.4 month/cm, 4th metatarsal 2.5 month/cm and left side showed 1st metatarsal 4.2 month/cm, 4th metatarsal 2.3 month/cm. Final follow-up results presented excellent outcome in cosmetic problem, easy wear of shoes, and measurement of patient's self satisfaction.

  • Purpose: The goals of cranioplasty are to protect the brain from trauma and restore normal cranial contour with as few complication as possible. In patient who suffered from cranial defect, the most satisfactory form of cranioplasty may be archived with autogenous bone grafts. We report on the treatment of large frontal bone defect using pericranial flap and split-rib graft. Methods: A 29-year-old male was referred to our department. He was involved in an automobile accident resulting in large frontal bone and sinus defect and skull basal defect. The reconstruction was undertaken using galeal frontalis myofascial flap and split-rib cranioplasty using bicoronal incision. Results: The postoperative course was successful. For 9 months follow up period, there was no complication about hematoma, infection, CSF leakage and washboard deformity. He was pleased with the results even when absolute symmetry was not achieved. Conclusion: We experienced large frontal bone and sinus defect and skull basal defect patient and successfully treated with split-rib graft and galeal frontalis myofascial flap.

  • Epidermoid cyst is a cystic form of teratoma and believed to be derived from trapped embryonic cells along the lines of embryonic closure. A 28-year-old woman presented with a painless swelling over the left mandibular area. On panoramic view, the mandible revealed a $5.5{\times}2.0\;cm^2$ multilocular radiolucent lesion of the left mandibular body and a computed tomography scan showed expansion of both the buccal and lingual plates in the same area. Microscopy found stratified squamous epithelium of the cystic wall and cystic contents of keratinized material. The histological diagnosis wasan epidermoid cyst. The most common location of epidermoid cyst at the head and neck is in the orbit (47%), followed by the mouth floor (23%) and the cervical area (9~24%), but in the jaw bone, it is considered very infrequent. We report the uncommon epidermoid cyst in the mandibular body that had a good healing outcome after treatment with a conservative marsupialization during the 40 months follow-up.

A Case Report of the Korean Medical Treatment for Patulous Eustachian Tube Patient with Headache (두통을 동반한 개방성 이관의 한의학적 치료 증례보고)

  • This study was performed to report the case of Korean medical treatment for the Patulous Eustachian tube patient with headache. The patient was treated by acupuncture, herbal medicine, bee venom injection, needle-embedding therapy, chuna therapy, release of cranial base and sacro-occipital therapy. Korean medical treatment was administered during 3 weeks. The improvement of clinical symptoms was evaluated by VAS (visual analogue scale) and SF-36 (36-Item Short Form Healthy Survey Instrument). After treatment, most symptoms decreased, headache VAS score changed 10 to 6 and tinnitus VAS score changed 10 to 5. Also, most SF-36 scores increased. Our study suggested that Korean medical treatments are effective in the patient with Patulous Eustachian tube. And further studies are required to identify underlying mechanism of treatment.

THE MANAGEMENT OF A COMPLEX IMPLANT CASE USING CAD-CAM TECHNOLOGY: A CLINICAL REPORT

  • PURPOSE: The application of computer-aided technology to implant dentistry has created new opportunities for treatment planning, surgery and prosthodontic treatment, but the correct selection and combination of available methods may be challenging in times. Hence, the purpose of this case report is to present a combination of several computer-aided tools as approaches to manage complicated implant case. MATERIAL AND METHODS: A 47 year-old female patient with severe dental anxiety, high expectations, financial restrictions and poor compliance presented for a fixed rehabilitation. A CT scan with a radiographic template obtained with software (SimPlant, Materialize, Leuven, Belgium) was used for treatment planning. The surgical plan was created and converted into a stereolithographic model of the maxilla with bone-supported surgical templates (SurgiGuide, Materialise, Leuven, Belgium), that allowed for the precise placement of 7 implants in a severely resorbed edentulous maxilla. After successful osseointegration, an accurate scan model served as the basis for the fabrication of a one-piece milled titanium framework using the Procera (Nobel Biocare, Gothenburg, Sweden) technology. The final rehabilitation of the edentulous maxilla was rendered in the form of a screw-retained maxillary metal-reinforced resin-based complete prosthesis. RESULTS: Despite challenging circumstances, 7 implants could be placed without bone augmentation in a severely resorbed maxilla using the SimPlant software for pre-implant analysis and the SurgiGuide-system as the surgical template. The patient was successfully restored with a fixed full arch restoration, utilizing the Procera system for the fabrication of a milled titanium framework.

A Case on Risperidone-Induced Tardive Dyskinesia (Risperidone 유발 지연성 운동장애 1례(一例))

  • Objective : Base on clinical practice, the authors report a case of tardive dyskinesia arising during the course of treatment with resperidal. Methods : This article was review and analysis of a case on risperidone-induced tardive dyskinea. Results : Mrs K, a 51-year-old woman with a 1-year history of schizophrenic disorder, gradually developed tardive dyskinetic movement of the mouth, lip, and tongue over a 4 month period(From July, 1996 to June, 1997) while taking risperidone. Initially she was treated with haloperidol and alprazolam. However, the haloperidol was subsequently discontinued because of EPS developed. From 11th March, 1997, she was observed to have a severe form of tardive dyskinesia involving her tongue, lip, and mouth. After risperidone was withdrawn at 9th May 1997, her tardive dyskinetic movement was disappeared. Conclusions : This is, to our knowledge, the first report of the onset of tardive dyskinesia in a patient taking risperidone. However, additional controlled studies of specific questions are needed ; e.g., the dose-response curves for produce tardive dyskinesia and the mechanism of producing risperidone-induced tardive dyskinea and so on.
