Effects of DCM Column Properties in Softground on Stabilities of Underground Roadways (연약지반내 DCM 개량체의 특성이 지하차도의 안정성에 미치는 영향)

  • In planning underground roadway foundation on soft ground, deep cement mixing method (DCM) is employed. The proper mixing ratio using batch test and replacement rates that meet strength criteria are used for deep cement mixing column. Stiffness ratio and distance between deep cement mixing columns (C.T.C) are varied to find out influences on stress, displacement, and differential settlement. The replacement ratios that meet settlement criteria are 10~35%. As stiffness varies, stress reaches at 769.kPa that exceed criteria due to stress concentration when stiffness ratio difference is over 30. Also, when C.T.C is 5 m, stress spreads to soils, so C.T.C need to be considered carefully. The vertical displacement is 0.6~1.56 cm, and angular distortion is 1/909~1/510.

A Literature Study about Clinical Outcome Parameters for Total Knee Replacement to Develop Core Outcome Set for Osteoarthritis by Korean Medicine Treatment (슬관절 전치환술 후 한의 핵심 결과 지표를 개발하기 위한 임상 평가지표에 대한 문헌 연구)

  • Objectives Osteoarthritis is hard to manage with both conventional and Korean medicine treatment. The core outcome set (COS) to demonstrate the effectiveness of Korean medicine treatment has not been established yet. We aimed to present preliminary data of COS by performing a literature review on the evaluation indices used in existing clinical research. Methods We examined the literature from 2000 to 2017 in two Korean electronic databases (Korea citation index and oriental medicine advanced searching integrated system) by searching for the following 3 terms 'total knee replacement (Korean)', 'total knee replacement,' and 'knee surgery.' We found 333 articles; among them, 50 duplicates were removed. Finally, we selected 160 articles after complete screening. We then extracted measured indices and clinical outcomes from the selected articles and categorized the relevant criteria. Results According to this study, the hospital for special surgery and knee society, range of movement angle, cross leg, Berg balance scale and balance ability, muscle strength, 6 minutes walking test, visual analogue scale, self-efficacy, the 12-item and 36-item short form survey and self-rated health status are the most commonly used outcomes of knee. Conclusions This study found that the several categories after total knee replacement (TKR) are being evaluated in the literature, and we were able to verify the most frequently used evaluation indices in these categories. The results of this study will be used to establish evaluation indices for the treatment of TKR in the future using Korean medicine.

Development of Simple Evaluation Method for Determining the Priority of Medical Device Replacement in Hospitals (병원내 의료기기 교체의 우선순위 결정을 위한 단순평가법 개발)

  • The importance of quality control has been emphasized to maintain safety of patients and satisfaction of medical service with medical devices used in clinic. Accordingly, objective evaluation method and standards consistently has been brought up for demand to replace outdated medical devices in appropriate time and to maintain the quality of medical service. Though many studies made suggestions methods to determine what medical devices to be replaced first, these methods were difficult in practice for its complex evaluation criteria and long time to evaluate. Therefore, in this study, a simple evaluation method is developed to identify and prioritize medical devices that are ought to be replaced. For the development of this simple evaluation method, four major characteristics of technology, safety, finance, and user satisfaction, which are considered in clinics to evaluate medical device replacement, and 14 minor attributes are distinctively selected. Each characteristic is assessed in binary form of "YES" or "NO" to minimize its subjective nature. Using this method to evaluate sampled medical devices belonging to four different characteristics, devices are suggested to be replaced in the current financial year or in the following financial year, or re-evaluated by the end of this financial year. Such results of evaluation can amend the subjective nature of existing evaluation method and give objective standards more promptly.

Application of sand compaction pile method of row replacement ratio as foundation of the dyke (호안기초로서 저치환율 모래다짐말뚝 공법의 적용)

  • In this study, sand compaction pile method was adopted to improve the soft ground under the permanent dyke, namely west sea dyke of Incheon New Port. The row replacement ratio 30% was applied to consider the ground condition, environmental side and the construction cost of the site. The stability and displacement analysis was carried out by respectively SLOPE/W and PLAXIS 2D program. Based on this analysis, it is found that the safety factor and displacement is within an allowable criteria. The model experiment was carried out using the acryl soil box with $400(H){\times}1200(L){\times}250(W)mm$ to show the displacement of the dyke and behavior of soft ground. Based on this experiment results, it is found that the settlement does not occur from 1 and 2 loading phases and horizontal displacement of 0.0075% occurs from 2 phases. It is also found that the differential settlement occurs 0.05mm corresponding respectively 0.02% and 0.03% of the dyke height(15cm).

Two Dimensional Finite Element Analysis on the Composite Ground Improved by Sand Compaction Piles with Low Area Replacement Ratio (저치환율 SCP 복합지반의 2차원 유한요소 해석기법 개발과 적용)

  • This study developed two dimensional finite element program(FE-SCP) for the analysis of a composite ground reinforced by sand compaction piles with a low area replacement ratio based on the Mohr-Coulomb elastic perfectly plastic constitutive model. Program FE-SCP give some conveniences to users such as automatic mesh generation according to the replacement ratio and the effective sand pile diameter in the post processor. Also, it contains optimum processor in calculation of In-situ stress equilibrium considering different coefficient of earth pressure between sand pile and surrounding clay. Estimated stress-strain behavior using FE-SCP and the measured one from a centrifuge test showed good agreement comparing to the result from a general finite element program.

Effects of Squat Exercise according to Weight Support on Balance and Gait in Patients after Total Hip Replacement: a Pilot Study

  • Purpose: The purpose of this pilot study is to identify the problems and stability of a study to investigate "Effects of Squat Exercise according to Weight Support on Balance and Gait in Patients after Total Hip Replacement." before proceeding with the study. Methods: Twenty-two rehabilitation patients after THR surgery who met the selection criteria participated. The study subjects were randomly assigned to a squat group using a slider or a squat group using a reformer. The interventions were applied for two weeks. The patients were assessed using Berg balance scale (BBS), Timed up and go test (TUG), and 10-meter walking test (10MW). Results: Although twenty-two study subjects participated in this study, eight study subjects participated dropouts occurred during the study period. There was a significant difference within the group in BBS and TUG in two groups (p<0.05). The difference between the two groups was not significant in all outcome measures (p>0.05). The largest effect size was 1.21 and the smallest effect size was 0.39, all from the BBS. Conclusion: This pilot study suggest that it is feasible with minor adjustment to conduct a larger scale, powered RCT to examine the efficacy of squat exercise according to weight support with patients after THR.

Diagnosis and Management of Patients with Mucopolysaccharidoses in Malaysia

  • In Malaysia, diagnosis and treatment of patients with mucopolysaccharidoses (MPS) is mainly localized at Hospital Kuala Lumpur, which is the national referral center for rare diseases. To date there are 83 patients diagnosed with MPS in our center, with MPS II being the commonest. The Malaysian National Medicines Policy second edition has a specific section on the orphan drugs which includes recombinant human enzyme for enzyme replacement therapy (ERT) in MPS. So far, National Pharmaceutical Regulatory Agency Malaysia has approved recombinant human enzyme for MPS types I (Loranidase), II (idursulfase), IVA (elosulfase alfa), and VI (Galsufase). Access to Idursulfase beta (another recombinant human enzyme for MPS II) and vestronidase alfa-vjbk (MPS VII) required special authorization on named patient basic. Currently there are 25 patients receiving ERT, 70% of the funding are from Ministry of Health (MOH), the remaining 30% are from various charitable funds and humanitarian programs. Thirteen newly diagnosed patients have to queue for an additional fund. Four patients have been treated with Hematopoietic stem cell transplant. MOH has also published guidelines regarding the patient selection criteria for ERT and treatment monitoring schedule.

  • Three cases of multiple cardiac valve replacement were done in March 1983 at the department of thoracic and cardiovascular surgery, Chosun University College of Medicine. The patients were moderately symptomatic in all cases, and belonged to the class II and III of the NYHA functional criteria. The diseased valved were replaced with Bjork--Shiley, Ionescu-shiley valve prosthesis under cardiopulmonary bypass using hemodilution technique. The kind of cardioplegic solution used in our institute were Young and GIKs solution with core surface cooling. The average cardiopulmonary bypass time was 251.6 minutes and the average aortic cross clamping time was 223 minutes for aortic and mitral valve replacement. There was no operative morality.

Management system for ensuring safety of HMR (Home Meal Replacement) products (가정간편식(HMR)의 안전성 관리체계)

  • Due to the nature of HMR food that is susceptible to contamination, its safety management is becoming more important. The relevant food types in food code corresponding to HMR foods are addressed, and the criteria for hygiene indicator bacteria and food poisoning bacteria, and storage and distribution standards according to the product type were presented. The government's safety management for HMR foods is basically carried out through the Food Sanitation Act. Those who intend to do HMR business must complete business registration or declaration, hygiene education, health examination of employees, and comply with legal obligations such as HACCP application. The government confirms compliance with legal requirements through hygiene inspection and monitoring inspection of products. However, the safety of HMR foods is not realized by the safety management system alone. A food safety culture should be established in which industry workers and consumers carry out actions to ensure food safety.

Analysis of Tensile Strength Characteristics of ACSR due to White Rust (백화현상에 따른 ACSR 송선선로의 인장강도 특성 분석)

  • In this study, a tensile test, one of the mechanical tests, was performed with the collected natural aging ACSR. In order to be used as basic data for predicting the replacement cycle of ACSR, the tensile strength with the normal cables was compared for cables which was caused white rust due to exposure of the hard-drawn aluminum wire surface. Among the ACSR wires collected from various regions, white rust was found on the surface of the small wire, and by checking the tensile strength of them, we would like to suggest the criteria for the ACSR replacement cycle, focusing on changes in mechanical properties.
