척추마취 전립선절제술환자의 음악요법효과에 대한 융합연구 (Convergence study on Effects of Music Therapy in Patients Undergoing Prostatectomy with Spinal Anesthesia)

  • 본 융합연구의 목적은 척추마취하에서 전립선절제수술을 받는 환자의 불안, 피로 및 활력징후에 미치는 선호 음악요법의 효과를 확인하기 위함이다. 본 연구는 비동등성 대조군 전후시차설계에 따라 환자 45명을 대상으로 하였으며 실험군에게는 수술 중 선호하는 음악을 들려주었다. 자료는 수술 30분전, 수술 20분 경과, 수술 40분 경과, 및 마취회복실 입실직후에 구조화된 질문지와 직접 계측을 통해 수집하였으며 기술통계, ${\chi}^2-test$, Fisher's exact test, t-test 및 repeated measures ANOVA로 분석하였다. 연구결과 실험군의 불안과 피로수준은 대조군보다 유의하게 낮았다(p=.001; p=.020). 그러나 수축기혈압, 이완기혈압 및 맥박에서는 두 집단 간 유의한 차이가 없었다(p=.821; p=.473; p=.782). 이상의 연구결과를 통해 척추마취 전립선절제술을 시행하는 환자에게 수술 중 환자의 선호 음악요법을 제공하는 것은 수술 관련 불안과 피로를 완화하는 유용한 간호중재가 될 수 있음을 확인하였다.

상황리더십 프로그램이 간호리더의 비판적 사고, 코칭 및 임파워먼트에 미치는 효과 (The Effect of Situational Leadership Program on Critical Thinking, Coaching and Empowerment of Nurse Leader)

  • 본 연구는 상황리더십 프로그램이 간호리더의 비판적 사고, 코칭, 임파워먼트를 높이는데 도움이 되는지를 알아보기 위한 실험연구이다. 표본 수는 G*Power프로그램을 이용해 추출하였고, 대상자는 대도시에 소재한 대학병원의 간호 리더 54명과 간호사 342명이었다. 자료 분석은 SPSS WIN 18.0을 사용하여 빈도분석, t-test, ${\chi}^2$-test, repeated measures ANOVA로 하였으며, 연구의 결과는 다음과 같다. 1. 상황리더십 프로그램으로 인해 비판적 사고는 실험군이 유의하게 증가하였고 대조군은 유의하게 감소하였다. 2. 상황리더십 프로그램으로 인해 코칭은 실험군이 유의하게 증가하였고 대조군은 유의하게 감소하였다. 3. 상황리더십 프로그램으로 인해 임파워먼트는 실험군이 유의하게 증가하였고 대조군은 유의하게 감소하였다. 따라서 상황리더십 프로그램은 간호리더의 비판적 사고, 코칭, 임파워먼트에 효과가 있다고 할 수 있다. 앞으로 상황리더십 프로그램을 간호사에게 적용시키면 임상 중에 발생한 상황의 대처나 조직생활에 있어서의 비판적 사고, 코칭, 임파워먼트 증진에 도움이 될 것 이다.

내·외측 Wedge와 넙다리네갈래근 각의 차이가 안쪽빗넓은근/가쪽넓은근 비에 미치는 영향 (Effects of Medial, Lateral Wedge and Difference of Quadriceps Angle on Vastus Medialis Oblique/Vastus Lateralis Muscle Activity Ratios)

  • Patellofemoral pain syndrome (PFPS) is often attributed to malalignment and maltracking of patella within the patellofemoral joint. Most exercise for PFPS has focused on selectively strengthening the vastus medialis oblique muscle (VMO). This study was designed to identify the effect of medial, lateral wedge and difference of Quadriceps angle (Q-angle) on vastus medialis oblique/vastus lateralis muscle (VL) activity ratios. The subjects were twenty young adult males who had not experienced any knee injury. They were asked to perform isometric contraction exercises in three postures using medial and lateral wedge. The EMG activity of the VL and VMO were recorded in three postures by surface electrodes and normalized by %MVC values derived from seated, isometric knee extensions. The normalized EMG activity levels (%MVC) of the VL and VMO for the three postures of the lower extremities were compared using 2-way repeated measures ANOVA with 1 between-subject factor (group), and 1 within-subject factor (wedge). Results of repeated measures of ANOVA's revealed that the medial wedge isometric contraction exercise produced significantly greater EMG activity of VMO/VL ratios in Group I (Q-angle $18^{\circ}$ or less) (p<.05). But, the medial wedge isometric contraction exercise was no significant difference of VMO/VL ratios in Group II (Q-angle $19^{\circ}$ or more) (p>.05). These results have important implications for selective VMO muscle strengthening exercises in PFPS patients.

정적인 스쿼트 운동시 복합적인 하지의 자세가 가쪽넓은근과 안쪽빗넓은근의 근활성도에 미치는 영향 (Effects of a Combined Posture of the Lower Extremity on Activity of the Vastus Medialis Oblique Muscle and Vastus Lateralis Muscle During Static Squat Exercise)

  • Most exercise for Patellofemoral pain syndrome (PFPS) has focused on selectively strengthening the vastus medialis oblique muscle (VMO). Although open chain knee extension exercises are effective for increasing overall quadriceps strength, they are not always indicated for PFPS rehabilitation. This study was designed to identify the effect of combined posture of lower extremity on Electromyographic (EMG) activity of the vastus lateralis muscle (VL) and VMO during static squat exercises. The subjects were twenty young adult males who had not experienced any knee injury and their Q-angle was within a normal range. They were asked to perform static squat exercises in five various postures using their lower extremities. The EMG activity of the VL and VMO were recorded in five exercises by surface electrodes and normalized by %MVC values derived from seated, isometric knee extensions. The normalized EMG activity levels (%MVC) of the VL and VMO for the five postures of the lower extremities were compared using one way ANOVA with repeated measures. Results of repeated measures of ANOVA's revealed that exercise 3 and exercise 5 produced significantly greater EMG activity of VMO/VL ratios than exercise 1 (p<.05). When the static squat exercise was combined with hip adduction and toes pointed outwardly, the EMG activity of VMO/VL rates was increased. The EMG activity of VMO/VL ratio was highest during static squat exercises performed on a decline squat. These results haveimportant implications for progressive and selective VMO muscle strengthening exercises in PFPS patients.

Brief, Intense TENS 자극이 신경전도, 통증역치의 변화에 미치는 효과 (The effects of Breif, Intense Transecutaneous Electrical Nerve Stimulation on Nerve conduction, Pain Threshold in Healthy subjects)

  • Purphose. This present study examines the effect of brief, intense transcutaneous electrical nerve stimulation(BTENS) on sensory nerve conduction, electrical pain threshold, and two-point discrimination measured at the superficial radial nevre distribution in 20 healthy subjects. Subjects. Twenty volunteercs, (10 females and 10 males(age range : 20-38 years : $mean{\pm}SD\;:\;27.00{\pm}5.12$), only subjects without prior traumatological and pathological were eligible to participated in this study. Methods. Nerve conduction were determined for the right superficial radial nerve. Electrical pain threshold were determined for the right wrist ipsilateral to the site of BTENS. Small disc electrodes were attached to the surface of the skin stradding the end of the radius. Square wave electrical pulses were delivered from an isolated stimulator through a constant current device at a frequency of 2 Hz(5 ms pulse width). Two-point discrimination, measured on the sensory distribution of superficial radial nerve. BTENS was delivered using a Max-SD( Medical design co.) portable battery powered stimulator. A cicular Ag/AgCl electrode in contact with hypertonic saline gel was attached to the lateral(radial side) surface of the forearm. Results. No significant effects were observed between stimulation methods in the prestimulation cycle(multi-way ANOVA repeated measures : distal latency ; F1.14=0.332. amplitude ; F 0.80=0.445, pain threshold ; F0.06=0.940.2 point discrimination ; F1.50=0.236). Highly significant effects were observed time with the pretreatment and 6 posttreatment cycles(p<0.01). Mighty significants differences in nerve conduction and pain threshold were found using un multi-way ANOVA repeated measures among stimulation methods for each cycles(p<0.01). Conclusion and Discussion The authors concludes that both nerve conduction and pain threshold changes are associated with therapy (stimulation) level of BTENS.

담요제공이 Brain MRI 검사를 받는 환자의 불안에 미치는 간호중재적 접근 (Nursing Approach of an Offered Blanket on the Anxiety of Patients Undergoing Brain MRI)

  • 본 연구의 목적은 담요 제공이 Brain MRI 검사를 받는 환자들에게 미치는 효과를 알아보기 위한 것이다. 연구대상은 G시에 소재한 일 대학병원에서 Brain MRI 검사를 받는 52명의 환자였다. 실험군(n=26)은 검사 전에 담요를 제공받았다. 자료는 2015년 3월부터 12월까지 수집되었으며, chi-squared test, the independent t-test, repeated measures ANOVA를 이용하여 분석되었다. STAI로 측정된 불안은 두 집단 간에 유의한 차이가 없었으나 시각적상사척도로 측정된 불안 정도는 실험군에서 유의하게 감소되었다(t=-2.75, p=.008). 혈압과 맥박에서는 실험군과 대조군 간에 유의한 차이가 없었다. 따라서 추후에는 Brain MRI 검사를 받는 환자의 불안 정도를 경감시켜줄 수 있는 연구가 필요하다.

Effects of Targeted Knee Flexion Angle on the Biomechanical Factors of Upward and Downward Phases during Forward Lunge

  • Objective: The aim of this study was to investigate the effect of targeted knee flexion angle on biomechanical factors of knee joint between upward and downward phases during the forward lunge. Method: Eight elderly subjects (age: $22.23{\pm}1.51years$, weight: $69{\pm}6.63kg$, height: $174.88{\pm}6.85cm$) participated in this study. All reflective marker data and ground reaction force during a forward lunge were collected. The knee joint movement and reaction force and joint moment at maximum knee flexion angle were compared by repeated measures one-way analysis of variance (ANOVA) (p<.05). The peak knee joint reaction force and joint moment between upward and downward phases were compared by repeated measures two-way ANOVA (p<.05). Results: The anterior and vertical knee joint movements, reaction force, and extensor moment of $80^{\circ}$ targeted knee flexion condition at maximum knee flexion angle was greater than both $90^{\circ}$ and $100^{\circ}$ conditions (p<.05). The $80^{\circ}$ knee flexed angle condition had greater peak joint reaction force and extensor moment compared with both $90^{\circ}$ and $100^{\circ}$ conditions between upward and downward phases during the forward lunge. Conclusion: As the targeted knee joint flexion angle increases, knee joint movement and kinetic variables become greater during the forward lunge exercise.

The Difference in Bone metabolism markers and Adipocytokine according to the applying Modern Dance with Osteopenia elderly women

  • 본 연구는 골감소증 노인 여성들을 대상으로 모던댄스 적용에 따른 골대사 지표 및 아디포사이토카인의 변화를 분석하여 골다공증 예방을 위한 효과적인 운동 방법을 제안하는데 있다. 연구의 대상자는 T-score -1.0 ~ -2.5사이 여성노인을 선정하였고, 모던댄스 그룹(n=10), 컨트롤 그룹(n=10)으로 무선 배정했다. 모던댄스는 주3회, 60분씩 12주간 실시하였고 자료 분석은 SPSS를 이용해 이원반복분산분석(two-way repeated measures ANOVA)으로 분석했다. 본 연구 결과 모던댄스 그룹에서 osteocalcin(p<.01), T-score(p<.05)가 유의하게 증가했다. Adiponectin(p<.05)은 모던댄스 그룹에서 유의하게 증가했고, TNF-α(p<.05), IL-6(p<.05)는 모두 모던댄스 그룹에서 유의하게 감소하였다. 이상의 결과를 종합해 보면 모던댄스는 골감소증 노인여성의 골다공증 예방을 위해 효과적인 전략이 될 것으로 사료되며 더불어 신진대사 및 기능 향상에도 긍정적인 영향을 미칠 것으로 생각된다.

통증자가조절기 융합교육이 척추수술환자의 수술 후 통증관리에 미치는 효과 (Effects of Patient Controlled Analgesia Convergence Education on Postoperative Pain Management in Spinal Surgery Patients)

  • 본 연구목적은 통증자가조절기(patient controlled analgesia:PCA)융합교육이 척추수술 후 통증관리에 미치는 효과를 조사하기 위함이다. 60명의 척추수술환자를 대상으로, 실험군(30명)에게 수술 전 PCA동영상, 소책자, 및 PCA실습으로 구성한 PCA융합교육을 적용하였다. 구조화된 질문지로 자료수집한 후 기술통계, ${\chi}^2-test$, Fisher's exact test, t-test 및 repeated measures ANOVA로 분석하였다. 연구결과 척추수술 후 실험군의 PCA지식태도, PCA만족도, 통증관리만족도는 대조군보다 유의하게 높았다(p<.001; p=.001; p<.001). 실험군의 수술 후 통증과 추가 진통제사용은 대조군보다 유의하게 낮았다((p<.001; p=.001). 따라서 시청각매체와 PCA실습을 접목한 PCA융합교육은 척추수술환자의 통증관리에 효과적인 실무중재임을 확인하였으며 다양한 간호실무에서 융합중재개발에 응용 가능할 것이다.

오십견 환자의 관절 가동 범위 회복과 통증 완화에 미치는 정적 신장 기법과 유지-이완 기법의 효과 (Effects of static stretching and hold-relax on recovering range of motion and reducing pain of patients with frozen shoulder)

  • Background : Patients with frozen shoulder have pain and limited range of motion (ROM) at the affected shoulder. Shoulder pain of these patients usually decreases with recovery of range of motion. The aim of this study is to identify effects of static stretching and hold-relax on recovering ROM and reducing pain of patients with frozen shoulder. Methods : In this study, 15 patients with frozen shoulder who were only treated with physical therapy were recruited. Patients who received any surgical procedure or hydrotherapy were excluded. The subjects were divided randomly into 3 groups; control(n=5), static stretching(n=5) and hold-relax(n=5) groups. Patients were treated with stretching and the total number of treatments was 12 times. The ROM was measured at baseline, after fourth, eighth and final treatments. Shoulder pain and disability index(SPADI) scores were assessed before and after doing all treatments. Repeated measures ANOVA was used to analyze ROM change within each group. Changes of ROM between groups was analyzed by one way ANOVA followed by post hot analysis(Tukey's HSD). SPADI score within each group analyzed using Wilcoxon signed ranks test. A p-value<0.05 was considered statistically significant. Study Design : Repeated measures design. Result : There were significant differences of ROM in both treatment groups except internal rotation of static stretching group. ROM was significantly increased in treatment groups compared with control group, but the difference of ROM change between two treatment groups did not differ significantly. SPADI scores for treatment groups significantly decreased compared with pre-SPADI score. Conclusion : The result suggests that hold-relax can be more effective than static stretching to recover range of motion and alleviate shoulder pain on patients with frozen shoulder.
