Bursoscopic Evaluation for Degree of the Rotator Cuff Tear with Air Infusion Method (공기 주입과 동시에 시행한 견봉하 관절경 술식을 이용한 회전근개 파열의 정도 평가)

  • Purpose : To evaluate the diagnostic efficiency of the subacromial bursoscopy with glenohumeral air infusion technique for detection of the full thickness tear which we used to think as a partial thickness tear, because it was hidden by bursal tissue. Materials and methods : We chose 65 cases and divided them into 2 groups. The group I was 18 cases with partial thickness rotator cuff tears on glenohumeral arthroscopic evaluation, and the group 2 was 37 cases with full thickness rotator cuff tears which were repaired. We inflated the glenohumeral Joint with 50-100ml of air and observed air bubble leakage simultaneously on bursoscopy. Results : In group I, we could detect air leakage in 3 cases and found that it was full thickness tear. In group II, 2 cases revealed too much air leakage proved that it was incomplete repair and was in need of additional suture. Conclusion : Air infusion technique is though to be a valuable method in differentiation between the partial and full thickness tear and for evaluation of the security of the repair.

  • In recent years, digital technology has been developed in dentistry, which denture frameworks can be manufactured using DMLS (Direct Metal Laser Sintering) technique. A traditional impression method can be replaced by oral scanning and wax pattern production process can be achieved by the use of CAD/CAM techniques. The designed STL files can be sent to DMLS devices to fabricate final components of removable partial dentures (RPD). The advantages of digital dentistry are concision and precision. In this case study, a fracture of occlusal rests providing support and indirect retention was repaired by DMLS and laser welding techniques. It shows satisfactory results in adaptation accuracy and functional properties of the repaired denture.

  • The repair technique utilizing surface impregnant is widely used due to its simple construction procedures and economical benefit. Surface protection through this reparing technique cannot increase the bearing capacity of concrete members much but increase the durability performance and service life efficiently. In this study, fundamental tests such as water suction and permeation are performed for concrete specimens with several organic/inorgarnic impregnant and suitable impregnant is selected on the basis of the results. Finally, durability tests such as carbonation, freezing and thawing, and chloride attack are carried out for the concrete specimens with selected impregnant and it is experimentally verified that they have good resistance to deteriorations.

Arthroscopic Meniscal Repair with Bioabsorbable Devices (생체 흡수성 기구를 이용한 반월상 연골 봉합술)

  • Purpose : The purpose of this study was to evaluate the clinical results of meniscal repair using the bioabsorbable devices. Materials and Methods : From 1998, 21 meniscal repairs were underwent using the bioabsorbable devices and followed more than 1 year. The Meniscus $Arrow^{\circledR}$(Bionx) and the Meniscal $Fastener^{\circledR}$(Mitek) had been applied. The indication for meniscus refixation was an unstable longitudinal tear in the posterior horn of the medial or lateral meniscus. Concurrent ACL reconstruction with meniscal repair was performed on 9 knees. The clinical result was evaluated using Lysholm knee score. Result : The average Lysholm knee scores for all patients improved from 56 preoperatively, to 89 postoperatively. The type of the device were not significantly related to clinical outcome or Lysholm knee score. But the concurrent ACL reconstruction group and the patients treated within 8 weeks had better clinical result than the others. Conclusion : The all-inside meniscal repair technique using bioabsorbable devices can be considered to be an useful method in the well selected patients and concurrent ACL reconstruction surgery.

Clinical & Radiologic Result of Arthroscopic Bankart Repair Using Knotless Suture Anchor (비매듭 금속 봉합 나사못을 이용한 관절경적 방카트 복원술: 임상적 및 방사선학적 결과의 비교)

  • Purpose: To study the clinical and radiologic results with arthroscopic Bankart repair using knotless metal suture anchor. Materials and Methods: From February 2001 to January 2005, 68 patients, who underwent arthroscopic Bankart repair using knotless suture anchor and were followed up more than 12 months, were evaluated. A mean follow-up period was 34 months. All shoulders were evaluated by Rowe scoring system, range of motion of the shoulder, pain degree of VAS, and This was compared by radiologic findings after surgery. Results: The Rowe scoring system was 43.30 preoperatively, which improved to 95.55 postoperatively. At last follow-up, there was no significance difference between operated shoulder and non-operated shoulder in range of motion. The degree of VAS was measured from 3.3 preoperatively to 0.5 postoperatively. The radiolucent line was shown around suture anchor in 15 shoulders. 2 shoulders of 15 shoulders were reoperated due to redislocation and anchor arthropathy. In Odds ratio, this group (15 patients) had more 2.6 times the subjective instability than other group (53 patients). Conclusion: Arthroscopic Bankart repair using knotless anchor suture is very effective operative technique. But we have to be careful because the radiolucent line around anchor showed up during a follow -up period may indicate poor prognosis.

Arthroscopic Posterior Cruciate Ligament Repair (관절경적 후방 십자 인대 봉합술)

  • Purpose: The purpose of this study is to evaluate the results of arthroscopic primary repair of a posterior cruciate ligament(PCL) avulsion injury without bony fragment at the femoral attach site. Materials and Methods: From Jan. 1993 to Dec. 2002, we performed 10 cases of PCL repair by arthroscopic suture technique. The mean follow-up period was $38.7{\pm}11$ months and the mean age of the patients was $28.2{\pm}6$ years old. 8 cases were men. 2 cases were women. At last follow-up, all cases were evaluated with the Lysholm and Gillquist knee rating system and International Knee Documentation Committee criteria. Posterior tibial displacement on stress lateral radiograph and posterior instability with posterior drawer test were measured also in all cases. Results: The mean Lysholm and Gillquist score was $94.5{\pm}2.6$. 4 cases were IKDC A(normal) and the other 6 cases were IKDC B(nearly normal). Posterior instabilities by posterior drawer test were grade I in 5 cases and grade II in 5 cases. A mean posterior translation of tibia was $3{\pm}2.3$ mm on stress lateral radiographs. Conclusion: Arthroscopic primary repair of PCL avulsion without bony fragment at the femoral attach site is one of the useful methods that reduce the posterior instability and improve the functional outcomes.

Evaluation of DNA Damage and Repair Kinetics in the Earthworm (Eisenia fetida) Exposed to Radiation and Mercury (방사선과 수은에 의해 유도된 Eisenia fetida 체강세포의 DNA 손상 및 수복 평가)

  • The single cell gel electrophoresis (SCGE) assay is a microelectrophoretic technique for assessments of DNA damage at the level of the individual eukaryotic cell. The SCGE assay, due to its simplicity, sensitivity and need of a few cells, has advantages compared to other genomic damage assays such as sister chromatid exchange, chromosomal aberration and micronucleus test. In this study, investigated were the levels of DNA damage and the repair kinetics in the coelomocytes of Eisenia fetida treated with HgCl2 and ionizing radiation by means of the SCGE assay. For detecting DNA damage and repair in coelomocytes, earthworms (E. fetida) were irradiated with six doses of ${\gamma}$-rays (0, 2.5, 5, 10, 20 and 50 Gy) and in vivo exposed to mercuric chloride at 0, 80 and 160 mg $kg^{-1}$ for 48 hours. Then the Olive tail moments were measured during 0~12 hours after irradiation and 0~72 hours after Hg treatment. The results showed that the more the oxidative stress was induced by mercury and radiation, the longer the repair time was required. Also, the results suggest that the SCGE assay may be used as an important tool for comparison of the sensitivity of different species to oxidative stresses.

Association of XRCC1 Gene Polymorphisms with Breast Cancer Susceptibility in Saudi Patients

  • Background: X-ray repair cross-complementing group 1 (XRCC1) plays a key role in the base excision repair pathway, as a scaffold protein that brings together proteins of the DNA repair complex. XRCC1 is reported to be a candidate influence on cancer risk. The aim of our present study was to assess the association of rs1799782 (Arg194Trp) and rs25487 (Arg399Gln) XRCC1 gene polymorphisms with breast cancer in the Saudi population. Materials and Methods: The two SNP's were analyzed in breast cancer patients and healthy control subjects. Genotypes were determined by TaqMan SNP genotype analysis technique and data were analyzed using Chi-square or t test and logistic regression analysis by SPSS16.0 software. Results and Conclusions: Results showed that rs1799782 significantly increased susceptibility to breast cancer with Arg/Trp, Arg/Trp+Trp/Trp genotypes and at Trp allele overall study. It also increased risk of breast cancer in older age patients (above 48) and with the ER positive category. XRCC1rs25487 (Arg399Gln) did not showed any significant association. In conclusion the XRCC1rs1799782 polymorphism may be involved in the etiology of breast cancer in the Saudi population. Confirmation of our findings in larger populations of different ethnicities is warranted.

The Usefulness of Leukosan SkinLink for Simple Facial Laceration Repair in the Emergency Department

  • Background Repair of facial laceration in the emergency department can pose a number of difficulties. Children can be uncooperative, but adults can also be if they have sustained head trauma or are intoxicated. Leukosan SkinLink consists of topical adhesive and adhesive tape that can be applied easily to long or tense wounds. In this study, the authors compared conventional suturing with Leukosan SkinLink for facial laceration patients in the emergency department. Methods The prospective study was carried out from March 2013 to September 2013 with linear facial laceration patients visiting the emergency department. Exclusion criteria were open fractures, joint injuries, skin defects, hairy skin, and mucosa. The author used Leukosan SkinLink for skin closure in the experimental group and used conventional suturing in the control group. The scar evaluation using the Patient and Observer Scar Assessment Scale (POSAS) along with satisfaction scores, procedure times, and complications were compared. Results A total of 77 patients (30 in the control group and 47 in the experimental group) participated and underwent follow-up for 6 months postoperatively. The scar assessment using the POSAS and the satisfaction score in both groups were similar. The average procedure time in the experimental group was shorter. In the control group, there were four cases of wound dehiscence, two of infection, and one of skin necrosis, whereas four cases of wound dehiscence and one allergic reaction occurred in the experimental group. Conclusions With a simple application technique, Leukosan SkinLink is a new effective method for facial laceration repair especially useful for children and uncooperative adults.

Use of a Temporary Shunt as a Salvage Technique for Distal Extremity Amputations Requiring Repair by Vessel Grafting during Critical Ischemia

  • Background Although the use of temporary shunts in proximal extremity amputations has been reported, no study has described the use of temporary shunts in distal extremity amputations that require vein grafting. Moreover, the total volume of blood loss when temporary shunts are used has not been reported. The aim of this study was to investigate the applicability of a temporary shunt for distal extremity amputations requiring repair by vessel grafting with an ischemia time of >6 hours. This study also aimed to determine the total volume of blood loss when temporary shunts were used. Methods Patients who underwent distal major extremity replantation and/or revascularization with a vessel graft and who experienced ischemia for 6-8 hours between 2013 and 2014 were included in the study. A 6-Fr suction catheter was cut to 5 cm in length after the infusion of heparin, and secured with a 5-0 silk suture between the distal and the proximal ends of the artery. While bleeding continued, the bones were shortened and fixed. After the complete restoration of circulation, the arterial shunt created using the catheter was also repaired with a vein graft. Results Six patients were included in this study. The mean duration of ischemia was 7.25 hours. The mean duration of suction catheter use during limb revascularization was 7 minutes. The mean transfusion volume was 7.5 units. No losses of the extremity were observed. Conclusions This procedure should be considered in distal extremity amputations requiring repair by vessel grafting during critical ischemia.
