The Clinical Analysis of Lateral Discoid Meniscus in Adults Over 40 years old (40세 이상에서 발견된 외측 원판형 연골의 임상분석)

  • Purpose: The purpose of this study was to analyze the clinical results of symptomatic lateral discoid meniscus found in adults over 40 years old. Materials & Methods: From March 1994 to March 2004, 53 cases treated for lateral discoid meniscus in 48 patients aged over 40 years with the mean age $48.4(40{\sim}62)$ who were followed up more than 1 year, were analyzed by physical findings, simple X-ray, MRI and arthroscopic exam. Results: Most of the cases (42 cases, 79.2%) had symptoms for less than 1 year duration. 26 cases(49.1 %) out of 53 had trauma history. Simple radiologic evaluation showed that lateral condylar abnormality such as the flattening of lateral femoral condyle, marginal osteophytes, and subchondral sclerosis was seen in 37cases(69.8%). Also medial condyle showed degenerative changes in 16 cases (30.2%). Tear of discoid meniscus in 45 cases(84.9%) were examined by arthroscope and tear was not detected in 8 cases(15.1%). Type of tear was complex(18 cases), longitudinal(12 cases), horizontal(11 cases) and transverse(4 cases). Concomitant medial meniscal rupture was found in 7 cases(13.2%). The simple procedure done was reshaping(46 cases), subtotal resection(5 cases), total resection(1 case) and meniscal repair after reshaping in 6 cases, and arthroplasty was performed after arthroscopic examination in 1 case. Conclusion: Onset of symptom in adult lateral discoid meniscus is usually traumatic in origin. Most cases showed radiologic abnormality such as degenerative change in lateral condyles as well as medial condyles and the results of preservative surgical treatment was the most preferred option in most patients.

Clinical and Histopathological Study in Repaired Cartilage after Microfracture Surgery in Degenerative Arthritis of the Knee (퇴행성 슬관절염에서 미세 천공술후 재생된 연골의 임상 및 병리조직학적 연구)

  • Purpose: The purpose of this study is to evaluate the clinical, radiological and histopathological results after microfracture surgery for degenerative arthritis of the knee. Materials and Methods: From Oct. 1997 to Dec. 1998, 48 knees in 46 patients were treated by microfracture technique. Their mean age at the time of operation was 56 years(range, 40-75 years) and mean period of follow-up study was one year(range, 7-20 months). For 24 knees in 22 patients, 'second-look' arthroscopies and biopsies were performed at 6 months following microfracture. At the last follow up clinical results were evaluated with Baumgaertner's scale. The specimens of 24 cases were stained with H-E, Safranin-O, and Masson's trichrome. Eighteen of 24 cases were stained immunohistochemically and the Western blotting test was performed on 12 cases for type II collagen. We analyzed the relationship of the Western blotting for type II collagen with clinical score, preoperative varus deformity, joint space widening in radiological result, extent of repaired articular cartilage in '2nd-look' arthroscopic findings, patient's age and weight. Results: Clinical results were excellent in 90% and good in 10%. Among the 24 knees, more than 80% of areas of chondral defect were covered with regenerated cartilage in 21 knees Histologically, the repaired tissue appears to be a hybrid of hyaline cartilage and fibrocartilage. Repaired cartilage contains variable amounts of type II collagen with immunohistochemical staining. The results of the Western blotting test were similar. The amounts of type II collagen formation had positive correlation with the extent of repaired cartilage and preoperative varus deformity. Conclusion: 'Second-look' showed that the chondral defect areas were covered with newly grown grayish white tissue. Articular cartilage repair was confirmed with histological and immunohisto-chemical study qualitatively, and the amount of type II collagen was calculated with the Western blotting test quantitatively. The exact nature and fate of repaired cartilagenous tissues need further long term follow-up study. The results of this study provide the rationale to select osteoarthritic patients indicated for microfracture surgery.

A Study on the 3D Measurement Data Application: The Detailed Restoration Modeling of Mireuksajiseoktap (미륵사지석탑 정밀복원모형 제작을 중심으로 한 3차원 실측데이터의 활용 연구)

  • After dismantled, Mireuksajiseoktap(Stone pagoda of Mireuksa Templesite) is being in the stage of restoration design. Now, different ways - producing restoration model, a 3 dimension simulation - have been requested to make more detailed and clearer restoration design prior to confirmation of its restoration design and actual restoration carry-out. This thesis proposes the way to build the detailed model for better restoration plan using extensively-used Reverse Engineering technique and Rapid Prototyping. It also introduces each stage such as a 3-dimension actual measurement, building database, a 3-dimension simulation etc., to build a desirable model. On the top of that, this thesis reveals that after dismantled, MIruksaji stone pagoda's interior and exterior were not constructed into pieces but wholeness, so that its looks can be grasped in more virtually and clearly. Secondly, this thesis makes a 3-dimension study on the 2-dimension design possible by acquiring basic materials about a 3-dimension design. Thirdly, the individual feature of each member like the change of member location can be comprehended, considering comparing analysis and joint condition of member. Lastly, in the structural perspective this thesis can be used as reference materials for structure reinforcement design by grasping destructed aspects of stone pagoda and weak points of the structure. In dismantlement-repair and restoration work of cultural properties that require delicate attention and exactness, there may be evitable errors on time and space in building reinforcement and restoration design based on a 2-dimension plan. Especially, the more complicate and bigger the subject is, the more difficult an analysis about the status quo and its delicate design are. A series of pre-review, based on the 3-dimension data according to actual measurement, can be one of the effective way to minimize the possibility that errors about time - space happen by building more delicate plan and resolving difficulties.
