종합병원 설비집약적인 부문의 리모델링 방안에 관한 연구 - 중앙진료부와 공급부를 중심으로 - (A Study on the Remodeling Strategy for the Equipment Intensive Areas of General Hospitals - Focused on the Diagnosis/Treatment Areas, the Supply Areas -)

  • If remodeling is defined as activities that restore the overall function of a building up to the standards currently demanded rather than as a simple replacement or repair of old facilities and equipment, the main task of remodeling should be the reduction in differences between areas, establishment of a functional linkage between departments and setup of a smooth circulation system. Therefore, this study is an analysis of remodeling strategies and construction processes to resolve major tasks of remodeling. Through this research we acquire concluding remarks. 1) The remodeling strategies of equipment-intensive areas can be divided into two measures: utilizing the existing buildings (the main buildings) the most, and of concentrated relocation in the new buildings. The former method is advantageous in that the main building serves as a center in hospital layouts because concentrated placement of the diagnosis/ treatment area and the supply area benefit the overall operation in terms of functions. However, the measure has limits in that it is difficult to install the facilities and equipment of the new demands due to the low ceilings, which serve as a potential challenge in future growth and changes. 2) The latter measure is the one in which equipment-intensive areas and the areas that are absolutely short of space are first placed in the new buildings while other areas (the outpatient area, the administration area and the miscellaneous areas) are in the existing building (main building). Given the possibility of development In the future, concentrated placement in the new buildings can be said to be more effective order to actively address future growth and changes.

  • 리모델링은 기존 건물을 완전 철거하지 않고 구조체의 일부만을 철거하여 자원의 재활용 이라는 측면에서 매우 중요한 이슈가 되고 있다. 그러나 리모델링의 경우에 신축과 달리 철거와 보수보강 공사가 추가되고, 다수의 발주자 조직으로 인한 의사결정에 어려움이 발생하고 있다. 이러한 문제를 해결하기 위한 방안으로 건물의 라이프사이클 동안에 발생하는 정보를 생산하고 관리하는 통합 도구로서 BIM이 해결 방안으로서 가능성을 가지고 있다. 그러나 현재 BIM의 경우 신축 공사시 3D 모델을 통한 간섭체크나 디자인 대안 결정에 주로 활용하고 있으며, 공사비 산출 등을 위한 BIM 데이터베이스의 경우 신축사의 BIM 적용을 위한 프레임웍과 BIM 데이터베이스를 제시하고 사례적용을 통하여 리모델링 공사를 위한 BIM 데이터베이스의 적용 가능성을 검증하고자 하였다.

Repair of Recurrent Pectus Excavatum with a Huge Chest Wall Defect in a Patient with a Previous Ravitch and Pectus Bar Repair: A Case Report

  • Recurrent pectus excavatum (PE) after a Ravitch operation is not uncommon. Extensive costal cartilage resection from the previous Ravitch procedure can lead to an irregular, unstable chest wall depressions with a varying degree of deformity. The optimal approach to cover the chest wall defect and remodel the deformity, remains unknown. We report the case of a 27-year-old woman seeking surgery for the third time for recurrent PE. The patient presented with 2-time recurrent pectus excavatum following a failed Ravitch procedure and subsequent pectus bar repair. The entire chest wall reconstruction and remodeling entailed covering the chest wall defect with 2 titanium plates across both sides of the rib cage, and lifting and fixing the depressed chest wall with 2 parallel pectus bars.

  • To obtain basic data used for establishing the criteria of deterioration in renovation of educational facilities, current problems of material and method and quality improvements were examined. In this study, the elementary schools built 20 and 70 years ago were unintentionally chosen and studied. With the data of the repair of old educational facilities, the results are as follows. The First problem is that the construction priority which was based on non-experts decision. The Second problem is that maintenance method which changes the materials after their life was ended. So, refer to remodeling of educational facilities, the selections of proper materials and methods to use of each function were main object in this study.

수로교 경관선호도 및 이미지 특성 분석 (The Study on the Landscape Preference and Characteristics of the Agricultural Aqueduct Bridges)

  • The purpose of this study is to analyze the landscape preference and landscape images of the Agricultural Aqueduct Bridges(AAB) by several different criteria. Semantic Differential Scale(SD scale) with landscape adjectives and landscape preference are used to estimate the alternatives of the AAB. The statistic methods such as descriptive analysis, t-test, factor analysis and regression, cluster analysis, are operated. The landscape preference of the alternatives is generally positive, 3.977 out of 7.000. The gate type is the most preferred, but the road-along type is the worst, by the location types. The simple repair type is the worst preferred, but total remodeling is the most, by the repair types. The characteristics of the AAB are analyzed and 4 factors of visual landscape are contracted; interest, orderliness, naturality and spatiality. Cumulative factor loading of these factors is about 65%, which is quite high. The higher and bigger AAB's are preferred, and the advanced finishing materials are also preferred, such as aluminum or wood panels. The long span is also preferred and the high repair cost would be preferred. But in this study, the cost-benefit analysis is not included, so it is recommended to research further, considering the cost variable with the visual factors.

  • The purpose of this study is to present an in-site evaluation method for building insulation performance using Infra-red camera. The research contents of this study are to evaluate validity and suitability of building insulation defect survey using Infra-red camera for apartment housing with temperature & heat flow pattern analyze method. By establishing the in-site evaluation method building insulation performance using infra-red camera, it is expected that 1) building envelope repair market will be activated by using developed method in ESCO and remodeling companies, 2) the method developed in this research will contribute to reduce national energy consumption by promoting old building insulation remodeling.

Loss of ARID1A Expression in Gastric Cancer: Correlation with Mismatch Repair Deficiency and Clinicopathologic Features

  • Purpose: The AT-rich interactive domain 1A (ARID1A ) gene encodes BRG1-associated factor 250a, a component of the SWItch/Sucrose NonFermentable chromatin remodeling complex, which is considered a tumor suppressor in many tumors. We aimed to investigate the prognostic significance of ARID1A expression in gastric cancers and explore its relationship with clinicopathologic parameters such as mismatch repair protein expression. Materials and Methods: Four tissue microarrays were constructed from 191 resected specimens obtained at Soonchunhyang University Cheonan Hospital from 2006 to 2008. Nuclear expression of ARID1A was semiquantitatively assessed and binarized into retained and lost expression. Results: Loss of ARID1A expression was observed in 62 cases (32.5%). This was associated with more frequent vascular invasion (P=0.019) and location in the upper third of the stomach (P=0.001), and trended toward more poorly differentiated subtypes (P=0.054). ARID1A loss was significantly associated with the mismatch repair-deficient phenotype (P=0.003). ARID1A loss showed a statistically significant correlation with loss of MLH1 (P=0.001) but not MSH2 expression (P=1.000). Kaplan-Meier survival analysis showed no statistically significant difference in overall survival; however, patients with retained ARID1A expression tended to have better overall survival than those with loss of ARID1A expression (P=0.053). In both mismatch repair-deficient and mismatch repair-proficient groups, survival analysis showed no differences related to ARID1A expression status. Conclusions: Our results demonstrated that loss of ARID1A expression is closely associated with the mismatch repair-deficient phenotype, especially in sporadic microsatellite instability-high gastric cancers.

  • Osteopontin (OPN) is a glycosylated phosphoprotein which mediates cell adhesion and migration, and is produced by bone, macrophages, endothelial cells, and epithelial cells. The many regulatory functions of OPN include bone remodeling, tumor invasion, wound repair, and promotion of cell survival. It is produced by renal tubular epithelial cells, and expression is upregulated in glomerulonephritis, hypertension, ischemic acute renal failure, renal ablation, and UUO. In this review, we discuss about osteopontin in general aspect, expression, role on the development and pathologic condition of neonatal kidney.

건축물 수선교체주기 산정현황과 개선방안에 관한 연구 (A study on estimation status and improvement plan of the repair and replacement cycle of a building)

  • 연구의 수행결과 얻어진 결과는 다음과 같다. 먼저 문제점으로는 국내의 경우 수선주기와 교체주기에 대한 명확한 구분기준이 없으며 건축공법과 재료성능의 발달 및 건물의 경과연수에 따른 변화대응력을 감안한 수선교체주기를 적용하지 못하고 있다. 즉 25~35년 사이에 리모델링 수준의 대규모 개수가 이루어 질 경우를 고려한 경제적인 수선교체주기의 산정이 필요하다. 수선교체주기의 산정에 있어 외국의 경우, 건물의 용도나 성능수준에 따라 이를 규정하고 있으나 국내의 경우에는 그렇지 못한 현실이다. 이를 개선하기 위해서는 개별건물의 특성을 반영하고, 새로운 건축재료나 공법에 대해 수선교체주기 산정기준을 마련하여야 하며 더불어 수선교체 대상항목의 분류 방식이 건축물 초기공사항목의 기준을 적용할 수 있도록 재편성하는 것이 필요하다. 또한 수선교체 이력데이터의 관리를 통해 기존의 확정적 수선교체주기설정에서 벗어나 건축물의 상태에 기반하여 수선교체주기를 재설정할 할 수 있는 업무기준이 필요할 것으로 판단된다.

  • The purpose of this study is to provide a basis for calculation of school operation fees for normal functioning of the school facilities, and facility maintenance and repair cost through their life spans, considering remodeling of the facilities and changes of their functions. This study analyzes maintenance and repair records during the three years from 2003 to 2005 in primary, middle and high schools in Incheon Metropolitan city and also the facility management and operation expenditure of the same schools in the city during the year of 2005, and presents quantified unit cost of each item according to the area, the age of the facilities, and the number of classroom, student, and lessons given. The calculation can be a basis for both BTL school facility managers and the general school facility mangers to budget school facility maintenance cost appropriately and the quantity of energy, water, electricity, and telephone fees calculated here can be a criteria for the managers to check the normal maintenance of their facility.
