Tc-99m DMSA SPECT for Follow-Up of Non-Operative Treatments in Renal Injuries: A Prospective Single-Center Study

  • Objective: The assessment of cortical integrity following renal injuries with planar Tc-99m dimercaptosuccinic acid (DMSA) scintigraphy depends on measuring relatively decreased cortical uptake (i.e., split renal function [SRF]). We analyzed the additive values of the volumetric and quantitative analyses of the residual cortical integrity using single-photon emission computed tomography (SPECT) compared to the planar scintigraphy. Materials and Methods: This prospective study included 47 patients (male:female, 32:15; age, 47 ± 22 years) who had non-operatively managed renal injuries and underwent DMSA planar and SPECT imaging 3-6 months after the index injury. In addition to planar SRF, SPECT SRF, cortical volume, and absolute cortical uptake were measured for the injured kidney and both kidneys together. The correlations of planar SRF with SPECT SRF and those of SRF with volumetric/quantitative parameters obtained with SPECT were analyzed. The association of SPECT parameters with renal function, grades of renal injuries, and the risk of renal failure was also analyzed. Results: SPECT SRF was significantly lower than planar SRF, with particularly higher biases in severe renal injuries. Planar and SPECT SRF (dichotomized with a cutoff of 45%) showed 19%-36% of discrepancies with volumetric and quantitative DMSA indices (when dichotomized as either high or low). Absolute cortical uptake of the injured kidney best correlated with glomerular filtration rate (GFR) at follow-up (ρ = 0.687, P < 0.001) with significant stepwise decreases by GFR strata (90 and 60 mL/min/1.73 m2). Total renal cortical uptake was significantly lower in patients with moderate-to-high risk of renal failure than those with low risk. However, SRF did not reflect GFR decrease below 60 mL/min/1.73 m2 or the risk of renal failure, regardless of planar or SPECT (count- or volume-based SRF) imaging. Conclusion: Quantitative measurements of renal cortical integrity assessed with DMSA SPECT can provide more clinically relevant and comprehensive information than planar imaging or SRF alone.

만성신부전(慢性腎不全)의 고혈압(高血壓)에서 Furosemide 정주(靜注)에 대(對)한 Renin 반응(反應) (Renin Response to Intravenous Furosemide in Hypertension of Chronic Renal Failure)

  • It has been suggested that plasma renin activity (PRA) and its response to volume depletion may be abnormal in that it shows little or exaggerated change in patients with chronic renal failure and hypertension. Intravenous furosemide stimulation test was performed in 46 control subjects and 51 patients with chronic renal failure and/or malignant hypertension in order to evaluate PRA response. In contrast to the consistent increase in PRA in control subjects (from $2.5{\pm}1.95\;to\;4.5{\pm}2.51ng/ml/hr$), no consistent increase was observed in patients with chronic renal failure, especially in those who showed favorable response to antihypertensive therapy (from $2.5{\pm}2.21\;to\;2.9{\pm}2.46ng/ml/hr$). But poor responder to antihypertensive treatment showed considerably higher PRA before and after furosemide stimulation (from $4.9{\pm}1.96\;to\;6.4{\pm}1.71ng/ml/hr$) than the responder group did. Moreover, this group seemed to retain the ability to increase PRA in response to intravenous furosemide stimulation. Thus it became apparent that responder group was unable to increase PRA normally in response to furosemide as well as volume depletion, while poor responder seemed to retain that ability. Thus intravenous furosemode may serve as a convenient way to differenfiate those who might be benefited by conservative antihypertensive measures from those who would require more drastic measures such as bilateral nephrectomy for their optimal blood pressure control.

무마취가토 신동맥내 투여에 의한 Trifluoperazine의 신장기능에 미치는 영향 (Effects of Renal Arterial Trifluoperazine on the Renal Function in Conscious Rabbit)

  • Renal arterial infusion of renotropic agents has been a very useful technique in the renal function studies. This type of experiments have usually been conducted in the large animals such as dogs and sheep. In these animals a catheter can be placed in the site without much disturbances of renal blood flow. Rabbits as an experimental model, however, caused a disturbances of renal blood flow by a catheterization of renal artery by its properties. Therefore we have developed a new technique that allows a simple and selective access to one side of renal arteries and the other as a control, without any disturbances of renal function. The distance between the both bifurcations of renal arteries on abdominal aorta is about 7 mm. To locate the tip of catheter on one side renal artery, ascending cannulation performed via femoral artery was done. We did an experiment with the technique to clarify the effect of calmodulin inhibitor on the renal function. One of the phenothiazine derivatives, trifluoperazine known as a powerful calmodulin inhibitor. Trifluoperazine, actual dose ranges of $2.76-5.20\;ug\;{\cdot}\;kg^{-1}\;{\cdot}\;min^{-1}$, increased urine volume and glomerular filtration rate significantly. Significant increases in urinary excretion of sodium, chloride and potassium were found. Fractional excretion of sodium and free water clearance increased significantly. These data suggest that this new technique is very useful in field of renal physiology and that striking effect of trifluoperazine on the renal function may be caused by increasing the renal hemodynamics, and by the inhibition of sodium, chloride and potassium reabsorption in the renal tubules.

황기가 흰쥐의 만성 신손상에 미치는 영향 (Effects of Astragali Radix (AR) on Chronic Renal Injury in Rats)

  • Objective : This study was designed to investigate the possibility of AR for chronic renal injury. Methods : The author first investigated the expression levels of DNA by inducing of chronic renal injury. Then, the author investigated the effects of AR on chronic renal injury induced by combination treatment with Adriamycin and cisplatin in terms of changes in body weights and renal tissues, urine volume, BUN and creatinine levels, creatinine clearance and histopathological changes in renal tissues. Total expression levels of 546 genes were elevated or lowered by induction of chronic renal injury. Genes of which whose expression levels were elevated by induction of chronic renal injury were related to the PPAR signaling pathway and fatty acid mechanism, etc. Genes of which expression levels were lowered by induction of chronic renal injury were related to the neuroactive ligand-receptor signaling pathway. Results : Oral administration of AR restored renal mass which was reduced by induction of chronic renal injury. AR also restored creatinine clearance and lowered serum BUN level. In histopathological observation, the AR group has a tendency to prevent tissue damages as shown in the chronic renal injury group. Conclusions : AR can be used to treat patients with chronic renal injury although further study will be needed to elucidate the exact mechanisms in the efficacy of AR on chronic renal injury.

$K^+$ Channel 개방제인 BRL 34915의 신장작용 (Renal Action of BRL 34915, a $K^+$ Channel Opener, in Dog)

  • The effect of BRL 34915, a $K^{+}$ channe$Na^{+}$l opener, on renal function was investigated in anesthetized dog. BRL 34915, when given into the vein, elicited the decrease of urine volume accompanied with the reduction of renal plasma flow (RPF), osmolar clearance ($C_{osm}$) and amounts of sodium excreted into urine ($E_{na}$), whereas reabsorption rate of sodium in renal tubules ($R_{na}$), ratio of $K^{+}$ against $Na^{+}$ in urine ($K^{+}$ /$Na^{+}$) were elevated significantly with a partial fall of mean arterial pressure (MAP). BRL 34915 injected into a renal artery produced the diuretic action along with the increase in RPF $C_{osm}$, $E_{na}$ and amounts of potassium excreted in urine ($E_{k}$), and the decrease in $R_{na}$, reabsorption rate of potassium in renal tubules ($R_{k}$), free water clearance ($C_{H20}$) and $K^{+}/Na^{+}$ ratio in only ipsilateral kidney, however changes of the renal function were not observed in control kidney. BRL 34915 given into carotid artery exhibited the same aspect as changes of renal function induced by intravenous BRL 34915. These results suggest that BRL 34915 has dual effects, renally acting diuretic and centrally acting antidiuretic action.n.

Pharmacokinetics of Diltiazem and its Major Metabolite, Deacetyldiltiazem after Oral Administration of Diltiazem in Mild and Medium Folate-Induced Renal Failure Rabbits

  • The pharmacokinetic changes of diltiazem (DTZ) and its main metabolite, deacetyldiltiazem (DAD) were studied after oral administration of DTZ to normal rabbits and mild and medium folate-induced renal failure rabbits. DTZ 10 mg/kg was given to the rabbits either orally (n=6). Plasma concentrations of DTZ and DAD were determined by a high performance liquid chromatography assay. The area under the plasma concentration-time curves (AUC) and maximum plasma concentration ($C_{max}$) of DTZ were significantly increased in mild and medium folate-induced renal failure rabbits. The metabolite ratio of the DTZ to DAD were significantly decreased in mild and medium folate-induced renal failure rabbits. The volume of distribution ($V_{d}$) and total body clearance ($CL_{t}$) of DTZ were significantly decreased in mild and medium folate-induced renal failure rabbits. The elimination rate constant ($\beta$) of DTZ was significantly decreased in folate-induced renal failure rabbits, but that of DAD was significantly increased. These findings suggest that the hepatic metabolism of DTZ was inhibited and the $V_{d}$, $CL_{t}$ and $\beta$ of DTZ were significantly decreased in mild and medium folate-induced renal failure rabbits.

  • This study was conducted to investigate the effects of vitamin I supplementation on renal function in high fat diet and adriamycin (ADR) induced experimental nephrotic syndrome in model rats. The effects of vitamin E supplementation on renal function in high fat diet and ADR treated rats were as follows. Kidney weight was decreased by vitamin E supplementation. Serum total protein was increased by the excess supplementation. Blood urea nitrogen(BUN) was decreased by the high supplementation. However, serum albumin and creatinine showed no significant differences between groups. Urinary volume tended to increase by vitamin I supplementation. Urinary urea-N tended by vitamin I supplementation. Particularly glomerular filtration rate(GFR) was significantly decreased by vitamin E supplementation. These results suggested that vitamin E supplementation could alleviate the adverse effects caused in renal function by highfatdiet and ADR treatments.

복령의 이뇨작용 (Diuretic action of hoelen in the dog)

  • Intravenously administered total water extract of Hoelen, (Pachymae fungus) in a dose of 10mg/kg and its methanol lextract, in a smaller dose than total water extract, produced significantly increase on urinary volume, sodium nad potassium excretion, and osmolar nad free water clearances. Increasing the doses produced more pronounced renal responses. Glomerular filtration rate and renal plasma flow changed littl with both extract. On the contrary, water extract from residue obtained on extraction with organic solvents exhibited no significant changes on the parameters of the renal function. In experiments, in which the total water extract was infused directly into a renal artery and urines from both ureters were collected separetely, a small dose of 0.3mg/kg/min showed no diuresis, but a large dose of 1.0mg/kg/min elicited diuretic action even on the contralateral kindneys. It is, therfore, concluded that Hoelen induces diuresis, mainly by inhibiting reabsorption of electrolytes in the renal tubules, and that the renotropic action may be mediated by some endogenous humoral substances.

  • It has long been suggested that the cardiac atrium is a low pressure volume receptor controlling body fluid volume and blood pressure. Recently, the cardiac atrium has been found to contain a family of powerful peptides. To clarify the relationship between high blood pressure and the biologically active atrial peptides, experiments were done to define the characteristics of atrial natriuretic peptide secretion in the isolated perfused atria of renal hypertensive rats. Higher concentrations of plasma atrial natriuretic peptide and renin activity were observed in the two-kidney, one clip hypertensive rat compared to the normotensive rat. Atrial volume changes in response to pressure elevations were attenuated in hypertensive rats compared to normotensive rats. Incremental response to atrial volume changes in ANP secretion was accentuated in hypertensive rats. These date suggest that the accentuated atrial natriuretic peptide response to volume changes of hypertensive rats may be a physiological or pathphysiological adaptation to the high blood pressure and may be, at least in part, responsible for the elevated levels of plasma atrial natriuretic peptide observed in hypertensive rats.

  • In this study, Schizandriae fructus which has been used in oriental medicine and folks remedy, was studied to apply to functional foods and oriental medicinal cuisine. The aim of this experiments was to investigate the effects of Schizandriae fructus water extract(SFE) on the renal function, plasma renin activity, plasma levels of aldosterone and arterial natriuretic peptide(ANP) in rats. Spargue-Dawley rats weigh 200g, were randomly assigned to 3 groups such as basal diet only(BDG), basal diet with $0.5{\mu}L/g$ SFE(LAG) and basal diet with $1.0{\mu}L/g$ SFE(HAG). The results were as follows. Water balance decreased significantly after administration for 2 weeks compared with the control period in HAG. Urine volume increased significantly after administration for 1 week compared with the control period in LAG and HAG. Urinary excretion of sodium increased significantly after administration for 1 week and for 2 weeks compared with the control period in LAG and HAG. Urinary excretion of creatinine increased significantly after administration for 2 weeks compared with the control period in HAG. Plama levels of ANL decreased significantly after administration of $SFE(0.5{\mu}L/g)$. Plasma levels of aldosterone decreased significantly after administration of $SFE(1.0{\mu}L/g)$. There results indicated that Schizandriae fructus can improve the renal function through increased urine volume and sodium excretion. These results imply that SFE could be used as a potent food resource for diet therapy or clinical nutrition.
