Background: Cardiogenic pulmonary edema increases nonspecific airway responsiveness in humans and animals. Increased extravascular lung water from overt pulmonary edema to subclinical interstitial edema is a common finding in patients with chronic renal failure. Several studies carried out to assess pulmonary function disturbances in this condition have documented a reduction in forced expiratory volume that usually reverses after hemodialysis, suggesting airway edema as the underlying mechanism. This interstitial edema may also lead to nonspecific bronchial hyperresponsiveness. We hypothesized that patients with chronic renal failure may present nonspecific bronchial hyperresponsiveness due to subclinical interstitial pulmonary edema. Methods: We studied 18 chronic renal failure undergoing regular hemodialysis 3 times a week(New York Heart Association Class II) without concomittent disease. These patients were checked pulmonary function test and metacholine provocation test before hemodialysis and same procedure was repeated if responsive, after hemodialysis. Results: 1) 12 out of 18 patients before hemodialysis were reactive in metacholine provocation test(66.7%) before hemodialysis. This airway hyperresponsiveness were decreased after hemodialysis. 2) Pulmonary function was improved after hemodialysis and change in $FEV_1$ was correlated with change in weight(r=-0.62, p<0.01). 3) There was a close correlation between log $PD_{20}$ and $FEF_{25}$, which is one of the variables of the peripheral airways(r=0.58, p<0.05). Conclusion: We speculated interstitial pulmonary edema may play a significant role in bronchial hyperresponsiveness and lung function impaired in patients with chronic renal failure.
The effects of Soohwabunchungum on rats with nephrosis induced by a single tail-intravenous injection of PAN(puromycin aminonucleoside), 2.5mg/100g of body weight was evaluated in the present study. The effects of Soohwabunchungum on PAN nephrosis was evaluated by measuring the concentrations of albumin, total protein, total lipid, cholesterol, triglyceride, creatinine, BUN(blood urea nitrogen) and uric acid in the serum the amount of protein, creatinine, glucose, occult blood and volume of the 24 hours urine and the volume of intake water, To conclude, it can be inferred that Soohwabunchungum has the effects of improving proteinuria, hypoproteinemia, hyperlipidemia in nephrotic syndrome, and relieving azotemia when nephrotic syndrome is accompanied by the acute renal failure.
The relative efficacy on the renal function of rabbits by oral administration of furosemide and 1 : 2 furosemide-PVP coprecipitate was compared by measuring the urine volume in response to maximal response and the amounts of electrolytes excreted in urine. The furosemide produced a rapid onset, short duration of diuresis, in contrast, the 1: 2 furosemide-PVP coprecipitate, a rapid onset, significantly larger magnitude, and longer duration of diuresis and therefore the bioavailability of furosemide from the coprecipitate were increased significantly. The average urine volume and the amount of sodium and potassium excreted in urine were increased about 2.9-, 14.8-, and 1.8-fold from furosemide, and about 6.2-, 24.2-, and 3.6-fold from 1 : 2 furosemide- PVP 40,000 coprecipitate, rerpectively, comparing by their control values.
The present study was undertaken to investigate the role of endogenous nitric oxide in renin release under different physiological conditions. In the first series of experiments, renin release was either inhibited by acute volume-expansion (VE) or stimulated by clipping one renal artery in the rat. VE was induced by intravenous infusion of saline (0.9% NaCl) up to 5% of the body weight over 45 min under thiopental (50 mg/kg, IP) anesthesia. VE caused a decrease of plasma renin concentration (PRC). With $N^G-nitro-L-arginine$ methyl ester $(L-NAME,\;5\;{\mu}g/kg\;per\;min)$ superadded to VE, PRC decreased further. The magnitude of increase in plasma atrial natriuretic peptide levels following VE was not affected by the L-NAME. In two-kidney, one clip rats, L-NAME-supplementation resulted in a decrease, and L-arginine-supplementation an increase of PRC. Plasma atrial natriuretic peptide levels were significantly lower in the L-arginine group than in the control. Blood pressure did not differ among the L-NAME, L-arginine, and control groups. In another series of experiments, the renin response to a blockade of NO synthesis was examined using in vitro preparations from isolated renal cortex. L-NAME significantly increased basal renin release, although it was without effect on the isoproterenol-stimulated release. These findings suggest that endogenous nitric oxide significantly contributes to the renin release. Since many factors may affect the renin release in vivo, an interaction between NO and renin under various pathophysiological states is to be further defined.
Objective : We performed this study in order to investigate the effects of Paljeongsan-gamibang(八正散加味方) on the renal failure. Methods : We injected 8ml/kg of 50% glycerol on the rats. And then administered Paljeongsan-gamibang extract ; 209mg/200g/day to sample group during 3 days and observed the body temperature, urine volume, stool volume and the levels of creatinine, glucose, inorganic phosphorus, Na+, K+, Cl-, threonine, tyrosine in blood and creatinine, glucose, threonine, tyrosine in urine. Results : 1, The sample group administered Paljeongsan-gamibang showed a suppressive effect of body temperature, an improving effect in capability of concentrating urine at convoluted tubule and showed an increasing effect of stool volume. 2. The sample group administered Paljeongsan-gamibang showed a lowering effect in creatinine level in blood and showed an increasing effect in the levels of inorganic phosphorus, Na+, K+, Cl-, threonine and tyrosine in blood. 3. The sample group administered Paljeongsan-gamibang showed an increasing effect in creatinine level in urine and showed a lowering effect in the levels of glucose, inorganic phosphorus, threonine and tyrosine in urine. Conclusions : Conclusively, Paljeongsan-gamibang was recognized to have a curative effect against the damage of rat kidney induced by 50% glycerol, especially to improve the capability of reabsorption in proximal convoluted tubule.
This study was undertaken to find out the effect of persimmon leaves on histopathological changes of cadmium toxicity in mice. Seventy two BALB/c mice of male were divided into a control group(A) and five experimental groups (B, C, D, E, F) : group A received tap water and basal diet, group B received tap water and diet supplemented with 3% persimmon leaves alone, group C received basal diet and 300 ppm cadmium, group D, E and F received basal diet supplemented with 1, 3% and 7% persimmon leaves and 300 ppm cadmium respectively. Cadmium dissolved in tap water was used, and the persimmon leaves were mixed with feed. All mice were dissected on the 84th day. Pathological changes in liver, kidney, cortical osseous tissue of femoral shaft, bone trabecular of femur, and epiphyseal cartilage plate of femur were observed. Group B showed no significant changes as the control group. But group C showed the unclearness of specific cells in liver, the loss of architecture and necrosis of hepatocyte, degeneration and necrosis of renal convoluted tubules, desquamation and vacuolization of the greater part of the renal tubular epithelium, marked thinning of the cortical osseous tissue in femoral shaft, reduction of cancellous bone volume and decreaswe of trabecular number, and marked thinning of epiphyseal cartilage plate in femur. On the other hand, persimmon leaves-treated group showed a little convalescent changes an maintained their normal architectures in liver, kidney, cortical osseous tissue of femoral shaft, bone trabecular of femur, and epiphyseal cartilage plate of femur.
Pharmacokinetic parameters of aminoglycosides are dependent on renal function, sex, age, hematocrit, fever, lean body weight (LBW) and disease states, etc. Therefore, the individual pharmacokinetic parameters such as half life $(t^{1/2})$ and volume of distribution(Vd) are needed to achieve optimal therapy. However these parameters had not been determined in Koreans. The purpose of this study was to evaluate the Vd and $t^{1/2}$ of amikacin in Korean patients who had normal renal function, to compare the mean values of study group with that reported in the literature and to compare the measured $t^{1/2}$ with the expected $t^{1/2}$ based on actual body weight (ABW), LBW and ideal body weight (IBW), respectively. Based on data, the Vd was greater than the literature and $t^{1/2}$ was similar to the literature. The predicted $t^{1/2}$ based on IBW was the closest to actual $t^{1/2}$. And postpartum patients had greater Vd than other group and had lower correlation between actual elimination rate constant and calculated creatinine clearance but higher correlation between actual elimination rate constant and Vd than other group.
This research developed an intravenous (IV) vancomycin dosing nomogram based on the clinical pharmacokinetic data of Korean adult patients. Total 99 pairs of steady-state peak and trough serum concentrations of vancomycin were obtained from 73 adult patients in a tertiary general hospital. Serum vancomycin concentrations were determined to assess the appropriateness of initial vancomycin dosing. Only 47.2% of the cases were within therapeutic range. To characterize the clinical pharmacokinetics (PK) of vancomycin, PK parameters including elimination rate constant ( $K_{e}$) half-life( $T_{1}$2/), clearance (C $l_{van}$), volume of distribution ( $V_{d}$) were calculated by using one-compartment, first order pharmacokinetic equations. PK parameters were evaluated based on the differences of patients'renal function and age. Regression analysis showed a significant correlation between C $l_{van}$ and $C_{cr}$ (C $l_{van}$ = -1.89+0.914 $C_{cr}$ , r=0.763) and between $K_{e}$ and $C_{cr}$ , ( $K_{e}$=-0.0037+0.00139 $C_{cr}$ =0.724). The relationship between $K_{e}$ and $C_{cr}$ , and the mean $V_{d}$ were utilized for developing the nomogram to individualize the initial dosing regimen of vancomycin for the patients with various degrees of renal functions. Voiding cystourethrography (VCUG) is a commonly performed diagnostic procedure for the evaluation of vesicoureteral reflux with urinary tract infection or congenital renal diseases in children. The procedure is relatively simple and cost-effective, and complications are very rare. The iatrogenic complication of VCUG range from discomfort, urinary tract infection to bacteremia, as well as bladder rupture. Bladder rupture is a rare complication of VCUG, and only a few cases were reported. Bladder rupture among healthy children during VCUG is an especially uncommon event. Bladder rupture associated with VCUG is usually more common in chronically unused bladders like chronic renal failure. Presented is a case of bladder rupture that occurred during a VCUG in a healthy 9-month-old infant, due to instilled action of dye by high pressure. This injury completely healed after 7 days of operation, and it was confirmed with a postoperative cystography. The patient's bladder volume, underlying disease, velocity of the contrast media instilled, catheter size, and styles of instillation are important factors to prevent bladder rupture during VCUG. Management of bladder rupture should be individualized, but the majority of infants are treated with the operation. In conclusion, bladder rupture is a rare complication, however, delicate attention is needed in order to prevent more dire situations.
The extraction of important features in cancer cell image analysis is a key process in grading renal cell carcinoma. In this study, we applied three-dimensional (3D) texture feature extraction methods to cell nuclei images and evaluated the validity of them for computerized cell nuclei grading. Individual images of 2,423 cell nuclei were extracted from 80 renal cell carcinomas (RCCs) using confocal laser scanning microscopy (CLSM). First, we applied the 3D texture mapping method to render the volume of entire tissue sections. Then, we determined the chromatin texture quantitatively by calculating 3D gray-level co-occurrence matrices (3D GLCM) and 3D run length matrices (3D GLRLM). Finally, to demonstrate the suitability of 3D texture features for grading, we performed a discriminant analysis. In addition, we conducted a principal component analysis to obtain optimized texture features. Automatic grading of cell nuclei using 3D texture features had an accuracy of 78.30%. Combining 3D textural and 3D morphological features improved the accuracy to 82.19%. As a comparative study, we also performed a stepwise feature selection. Using the 4 optimized features, we could obtain more improved accuracy of 84.32%. Three dimensional texture features have potential for use as fundamental elements in developing a new nuclear grading system with accurate diagnosis and predicting prognosis.
