Purpose: This study was designed to compare physical symptoms and fatigue of hemodialysis patients on dialysis day and non-dialysis day. Methods: Data were collected from 63 outpatients undergoing hemodialysis at a hospital in A university hospital in August and September, 2008 using structured questionnaires. The data were analyzed with SPSS WIN 12.0 program with Paired t-test and Pearson's correlation. Results: Physical symptoms in patients between dialysis day and non-dialysis day were not different statistically (t=1.76, p=.08). Fatigue on dialysis day was higher than on non-dialysis day (t=3.27, p=.00). Physical symptoms and fatigue positively correlated with dialysis day (r=.42, p=.00) and non-dialysis day (r=.52, p=.00). Conclusion: Intervention to decrease physical symptoms and fatigue in dialysis day and non-dialysis day should be developed.
Objectives : We wanted to evaluate the economic value of a pharmaceutical product, Kremezin, for treating patients with chronic renal failure (CRF) by estimating the amount of cost savings due to its effect for delaying the initiation of dialysis treatments. Methods : We defined a conventional treatment for CRF accompanied by Kremezin therapy as 'the treatment group' and only conventional treatment as 'the alternative group.' The types of costs included were direct medical and nonmedical costs and costs of productivity loss. The information on the effect of Kremezin was obtained from the results of earlier clinical studies. Cost information was derived from the administrative data for 20 hemodialysis and 20 peritoneal dialysis patients from one tertiary care hospital, and also from the administrative data of 10 hemodialysis patients from one free-standing dialysis center. Per-capita cost savings resulting from Kremezin therapy were separately estimated for the cases with delay for the onset of hemodialysis and the cases with immediate performance of peritoneal dialysis. By computing the weighted average for the cases of hemodialysis and peritoneal dialysis, the expected per-capita cost savings of a patient with CRF was obtained. Using a discount rate of 5%, future cost savings were converted to the present value. Results : The present value of cumulative cost savings per patient with CRF from the societal perspective would be $18,555,000{\sim}29,410,000$ Won or $72,104,000{\sim}112,523,000$ Won if Kremezin delays the initiation of dialysis by 1 or 4 years. Conclusions : The estimated amount of cost savings resulting from treating CRF patients with Kremezin confirms that its effect for delaying the onset of dialysis treatments has a considerable economic value.
Background: The converging epidemics of tuberculosis (TB) and end-stage renal disease (ESRD) have generated a significant public health burden, however, previous studies have been limited to a small number of patients. This nationwide cohort study aimed to assess the rate of developing active TB among patients receiving dialysis for ESRD. Methods: The Korean national health insurance database was used to identify patients receiving dialysis for new-onset ESRD during 2004-2013, who were propensity score matched to an equivalent number of non-dialysis subjects from the general population. The incidences of active TB in the ESRD and control cohorts were calculated for 2004-2013, and multivariable Cox proportional hazards model was used to evaluate the ESRD-related risk of active TB. Results: During 2004-2013, 59,584 patients received dialysis for newly diagnosed ESRD. In the dialysis and control cohorts, 457 (0.8%) and 125 (0.2%) cases of active TB were detected, respectively. Patients with ESRD were associated with a significantly higher risk of active TB compared to the controls (incidence rate ratio, 4.80). The ESRD cohort had an independently elevated risk of active TB (adjusted hazard ratio, 4.39; 95% confidence interval, 3.60-5.37). Conclusion: We found that patients receiving dialysis for ESRD had an elevated risk of active TB. These results highlight the need for detailed and well-organised guidelines for active TB screening among patients with ESRD.

Peritoneal dialysis has been widely considered to be the dialytic treatment of choice for acute renal failure in infants and young children, because the technique is simple, safe and easily adapted for these patients. Also peritoneal dialysis in infants might have more effective ultrafiltration and clearance than in adults. In certain circumstances associated with hemodynamic instability, ordinary volume peritoneal dialysis(30-50 ml/kg body weight per exchange) or hemodialysis may not be suitable unfortunately. But frequent cycled, low volume, high concentration peritoneal dialysis may be more available to manage the hemodynamically untable acute renal failure of newborns and infants. Seven infants underwent peritoneal dialysis for hemodynamically unstable acute renal failure with low exchange volume($14.2{\pm}4.2ml/kg$), short exchange time(30 to 45 minutes) and hypertonic glucose solution(4.25% dextrose). Age was $1.9{\pm}1.3$ months and body weight was $4.6{\pm}1.6kg $. Etiology of acute renal failure was secondary to sepsis with or without shock(5 cases) and postcardiac operation(2 cases). Catheter was inserted percutaneously with pigtail catheter or Tenkhoff catheter by Seldinger method. Dialysate was commercially obtained Peritosol which contained sodium, chloride, potassium, magnesium, lactate and calcium. Net ultrafiltration(ml/min) showed no difference between low volume dialysis and control($0.27{\pm}0.09$ versus $0.29{\pm}0.09$) Blood BUN decreased from $95.7{\pm}37.5$ to $75.7{\pm}25.9mg/dl$ and blood pH increased from $7.122{\pm}0.048$ to $7.326{\pm}0.063$ after 24 hours of peritoneal dialysis. We experienced hyperglycemia which were controlled by insulin(2 episodes), leakage at the exit site(2), mild hyponatremia(1) and Escherichia coli peritonitis(1). Two children of low volume dialysis died despite the treatment. In our experience, low volume and high concentration peritoneal dialysis with frequent exchange may have sufficient ultrafiltration and clearance without significant complications in the certain risked acute renal failure of infants.
Calcification in patients with endstage renal disease on renal replacement therapy can occur in extraskeletal area such as conjunctiva and cornea. Conjunctival and corneal calcification(CCC) has mostly has been reported in adults with endstage renal disease on hemodialysis. CCC seems to be associated with the duration of renal replacement therapy, and high Ca$\times$P value. We report a 10-year-old girl who was on peritoneal dialysis for 31 months and presented with CCC on both eyes. Her corneal calcification was resolved after the epithelial debridement and ethylenediaminetetraacetic acid(EDTA) soaking therapy.
Removal of urea and vitamin B12 was simulated using two pool models with closed loop dialysis, open loop dialysis, and hemodiafiltrations with predilution and postdilulion. It was found that urea removal was limited by dialyzer clearance, however B12 removal was limited both by the resistance between ECF and ICF and by dialyzer clearance. Open loop dialysis was better than closed loop dialysis in terms of dialysate requirement and removal efficiency. Residual renal function plays an important role in removing vitamin B12. Dialysis frequency more than twice/wide does not have great effect on removal efficiency, but has the effect of reducing the difference between maximum and minimum cancentrations during dialysis period.

The purpose of the present study was to compare pulsatile type(Twin Pulse Life Support; TPLS) with rotary type hemodialyzer(AK95) in order to reduce the dialysis time and to improve dialysis effect. Three healthy dogs(about 30 kg BW) were used. Experimental renal failure was induced by the ligation of bilateral renal artery. A pair of catheters were implanted in jugular vein for dialysis. Daily investigated parameters included clinical signs such as vomiting, fecal appearance and activity, and also laboratory data such as PCV, TP, BUN and creatinine. Hemodialysis was started above 90 mg/100 ml BUN level and, laboratory data were measured every an hour for 4 hours. Heparin was administered 300 IU/Kg before dialysis and 150 IU/Kg via IV route every 90 minutes during dialysis. Clinical signs after induction renal failure were shown severe vomiting, anorexia, diarrhea, mucous feces, ataxia, dilated pupil and episcleral hyperemia. The average of BUN value decreased hourly $99{\pm}12.1,\;84{\pm}12.2,\;72{\pm}8.0,\;58{\pm}7.1,\;48{\pm}5.2,\;and\;39{\pm}3.2mg/100ml$ by hemodialysis. The average of creatinine value decreased $7.8{\pm}0.61,\;6.4{\pm}0.40,\;5.3{\pm}0.42,\;4.5{\pm}0.23,\;4.0{\pm}0.41,\;and\;3.4{\pm}0.42mg/100ml$ according to hemodialysis an hour. There are not significantly differences BUN, creatinine, PCV and TP values between pulsatile and rotary type hemodialysis. These results suggested that effects of hemodialysis with Pulsatile type(TPLS) are not significantly difference as compared with hemodialysis of rotary type(AK95). Further research is needed in order to estimate the influence of cardiovascular and pulmonary system in hemodialysis of pulsatile type.
Purpose : Dialysis in children with chronic renal failure presents with many difficulies. The purpose of this study is to find an improved method in chronic dialysis in infants and children less than 2 years of age by analyzing the experience with 10 cases. Methods : A retrospective review of the medical records of 10 patients(6 boys and 4 girls) was conducted. The patients had chronic renal failure and underwent chronic dialysis at Samsung medical center from March 1999 to February 2007. Results : At Initiation of dialysis, the median age was 3 months old(22 days-20 months), the median body weight was 3.75 kg(2.2-10.3 kg), and the median serum creatinine level was 4.3 mg/dL(2.0-11.4 mg/dL). The median duration of dialysis was 29.5 months(3-62 months). Dysplastic kidney disease was the most common underlying renal disease. Two patients were treated with hemodialysis, 4 patients with peritoneal dialysis, and 4 patients eventually switched dialysis modality. Nine of the 10 patients took erythropoietin and anti-hypertensive drugs. At the end of the follow up period, 1 patient received kidney transplantation, 2 patients died due to sepsis, and 5 patients were treated with peritoneal dialysis. Two patients were lost to follow up. The most common complication of dialysis was infection. Achieving vascular access and maintaining proper catheter function were the most important factors in treating patients with hemodialysis. The growth status of patients was aggravated after 6 month of dialysis but improved after 1 year of dialysis. Patients showed better growth on peritoneal dialysis than hemodialysis. Conclusion : Chronic dialysis can be performed successfully in infants and children under 2 years of age. Vascular access was the main limitation of hemodialysis, and infection was the common problem in both hemodialysis and peritoneal dialysis. To improve the patients survival rate and quality of life, major efforts should be directed toward the prevention of infection and preservation of catheter function. Purpose: The purpose of the study is to identify the effects of cool dialysis on pruritus of chronic renal failure patients. Methods: The degree of pruritus in 30 patients treated at Hemodialysis Unit in D Medical Center was examined by the itching NRS tool. The data were collected from February 1 to February 28, 2009 and analyzed by the student t-test. The 30 patients were divided into two groups, 15 of experimental group and the same number of control group. Results: The first Hypothesis, that after two hours of hemodialysis the degree of pruritus for the experimental group would be lower than that of the control group is rejected (t=0.726, p=.474). The second Hypothesis, that right after receiving hemodialysis the degree of pruritus would be lower for the experimental group than the control group, is also rejected (t=1.321, p=.199). Conclusion: The research on searching for the effective method of nursing intervention to relieve pruitus for the chronic renal failure patients who receive hemodialysis should be continued.

To assess the economic value of pharmaceutical therapy with Kremezin, we investigated the maximum amount of willingness-to-pay (WTP) of patients with chronic renal failure (CRF) for a hypothetical effect of Kremezin in delaying the initiation of dialysis treatments. A face-to-face survey was carried out in a sample of 141 CRF patients from 2 dialysis centers, composed of 82 hemodialysis patients, 38 peritoneal dialysis patients, and 21 non-dialysis CRF patients. Using a bidding game method with a starting point of 320,000 Won, which is the average monthly out-of-pocket payment for dialysis treatment, we asked the study subjects how much they would pay per month to receive Kremezin therapy. The mean out-of-pocket monthly WTP for Kremezin was 310,000, 430,000, and 520,000 Won (p<0.05, repeated one-way ANOVA)) when Kremezin delays the initiation of dialysis treatments by 1, 2, and 4 years. Significant correlation between the respondent's WTP and income $(r=0.266{\sim}0.368,\;p<0.05)$ confirmed the construct validity of the WTP instrument. Regression results showed that patients with a higher education, with diabetes as a major causes of CRF, and undergoing hemodialysis treatments tended to express higher WTP for Kremezin. The economic value of WTP from the perspective of patients varied from 310,000 to 520,000 Won depending on the effect size of Kremezin. The mean WTP was higher than 32,000 Won, only when the hypothetical effect of Kremezin in delaying the initiation of dialysis is for 2 years. This implies that Kremezin might be the preferred choice of therapy by CRF patients if it delays the initiation of dialysis treatment for at least 2 years.
