신장(腎臟)mass 감소(減少)가 혈액요소질소(血液尿素窒素)와 혈청(血淸) 크레아티닌 농도(濃度)에 미치는 영향 (Effects of Reductions in Renal Mass on Blood Urea Nitrogen and Serum Creatinine Concentrations in dogs)

  • Renal dysfunction was experimentally induced with reductions in renal mass in dogs and then the remaining kidney function was estimated by blood urea nitrogen and serum creatinine concentrations. During experimental period, blood urea nitrogen and serum creatinine concentrations showed no variance in dogs with 50% reductions in renal mass, but these values were remarkably increased in dogs with 75% reductions in renal mass. It was considered that 75% reductions in renal mass was applicable as experimental models for renal dysfunction.

Differential Diagnosis and Management of a Pituitary Mass with Renal Cell Carcinoma

  • The small pituitary mass was incidentally found in 40-years-old women with renal cell carcinoma. The endocrinological and ophthalmological evaluation revealed no deficit and the short-term follow-up was recommended. In 6 months later, the visual disturbance was reported and the size of mass was increased. The tumor was removed totally via the trans-sphenoid approach. The post-operative endocrinological insufficiency was not noticed. During one year of follow-up period, there was no evidence of recurrence without adjuvant radiotherapy. The clinical features of pituitary metastasis from renal cell carcinoma were similar to those of pituitary adenoma. The possibility of pituitary metastasis should be kept in mind in patients with sellar mass and renal cell carcinoma.

  • Primary renal echinococcosis, a rare disease involving the kidney, accounts for 2-3% of human echinococcosis. A 64-year-old female patient from Uzbekistan presented with complaints of left flank pain. A CT scan revealed a cystic mass in the upper to midpole of the left kidney. We regarded this lesion as a renal malignancy and hand-assisted laparoscopic radical nephrectomy was performed to remove the renal mass. The mass consisted of a large unilocular cyst and multiple smaller cysts without any grossly visible renal tissue. The final pathologic diagnosis was a renal hydatid cyst. For patients from endemic areas, hydatid cyst should be included in the differential diagnosis. Here, we present a case of renal hydatid cyst in a female patient who relocated from Uzbekistan to Korea.

신장 호산성과립세포종의 영상의학적 소견 (Radiologic Findings of Renal Oncocytomas)

  • RO는 경계가 좋은 고형종괴로써, 석회화를 포함할 수 있다. 조영전 CT 영상에서 신실질과 유사한 밀도의 균일한 종괴로 보인다. 조영 후 영상에서는 불균일하게 조영증강되는데, 역동적 조영증강 검사의 동맥강조기에서 주변부는 강하게 조영증강되었다가 시간에 따라 점차 조영이 약해지고, 중앙부는 시간에 따라 점차 조영이 증강되어 시간에 따른 조영증강의 부분적 역전현상이 관찰되었다. 이러한 영상의학적 특징이 관찰되는 경우에는 RO의 가능성을 시사할 수 있으며 이를 통해 환자의 예후를 예측하고 수술 방법을 술전에 결정하는데 도움을 줄 수 있으리라 생각한다.

개에서 발생한 신장원성 혈관육종의 면역조직화학적 진단 (Immunohistochemical Diagnosis of Primary Renal Hemangiosarcoma in a Dog)

  • 7년령의 중성화된 수컷 Yorkshire Terrier가 복강에 종괴가 촉진되어 이를 주증으로 내원하였다. 방사선 검사 상 복강의 등쪽면에 신장 종괴와 작은 복강 종괴가 관찰되었다. 수술적으로 적출된 우측 신장에서는, 육안적으로 붉은 갈색조를 띄는 종괴와 낭상 구조가 함께 관찰되었다. 병리조직검사 상에서 다각형 또는 타원형의 내피세포를 가진 다수의 불규칙한 모양의 신생혈관이 관찰되었으며, 이 혈관들과 종양세포들은 주변조직으로의 강한 침습성을 보였다. 복강의 또 다른 작은 종괴에서는 신장 종괴와 동일한 병리조직학적 소견을 관찰할 수 있었다. 면역조직화학염색을 실시한 결과 종양세포들은 vimentin, CD31 및 von Willbrand factor에 대해 양성 반응을 보였으나, cytokeratin에 대해서는 음성 반응을 보였다. 육안적인 특징, 병리조직학적 소견 및 면역조직화학적 검사를 바탕으로 본 증례는 신장원성 혈관육종으로 진단하였다.

개에서의 편측성 신장 림프육종 1예 (A Case of Unilateral Renal Lymphosarcoma in a Dog)

  • A 10-year-old, female spayed mixed-breed dog with a history of vomiting and anorexia was examined. Abnormal findings of comlete blood count and serum chemistry included polycythemia, thrombocytopenia, hyper-globulinemia and hypoalbuminemia. Abdominal radiographs revealed severe unilateral renomegaly, and ultrasonography showed a left-sided renal mass. During the operation, left kidney was resected. Cytologial and histopathological examinations revealed neoplastic lymphoid proliferation with high mitotic figures in renal mass. Immunohistochemistry revealed tumor cells were CD3-positive and CD79${\alpha}$-negative, consistent with T-cell lineage. The renal mass of this case was diagnosed as renal T cell lymphosarcoma.

Use of intraoperative impression smear cytology to guide successful treatment of a large renal cyst in a dog: a case report

  • A 13-yr-old Shih tzu was referred for surgical management of right-sided cranial abdominal mass, which corresponded to large, cavitated renal mass on ultrasonography, and was suspected to represent neoplasia. Intraoperative impression smear cytology (ISC) of the renal mass wall was consistent with benign renal cyst (RC), without evidence of neoplasia or infection. Deroofing and omentalisation were performed and histopathology was consistent with benign RC. Chronic kidney disease was diagnosed 4 mon postoperatively, however, the dog was asymptomatic, without cyst reoccurrence. Intraoperative ISC is an expedient and inexpensive diagnostic technique that can guide most appropriate treatment in dogs with large RCs.

신세포암에서 $^{18}F-FDG-PET/CT$ ($^{18}F-FDG-PET/CT$ in Renal Cell Carcinoma)

  • Renal cell carcinoma is the most common histological type of renal malignancy, predominant in men and the primary treatment modality of this tumor is surgery. The role of diagnostic imaging in the management of this tumor is the evaluation of extent of disease as well as the detection and characterization of renal mass. US has long been a routine screening tool for kidney but tomographic imaging modalities such as CT and MRI begin to be also commonly used these days. On the other hand, the sensitivity of $^{18}F-FDG-PET$ in detection of renal mass is relatively low because of inherent limitation caused by FDG excretion pathway despite avid uptake of FDG to tumor cell per se. Many studies revealed FDG PET scan could play an important role in detection of metastatic lesions although the sensitivity for the detection of primary lesion is not so high. Furthermore, development of PET/CT scanner will make it possible to expand the indication of FDG PET scan in this malignancy.

  • Renal inflammatory pseudotumor is a very rare benign condition of unknown etiology characterized by proliferative myofibroblasts, fibroblasts, histiocytes, and plasma cells. In the case we report, the lesion appeared on contrast-enhanced power Doppler US images as a well-defined hypoechoic mass with intratumoral vascularity, and on CT as a low-attenuated mass. Differentiation from malignant renal neoplasms was not possible.

Seeding Metastasis of Chromophobe Renal Cell Carcinoma after Robot-Assisted Laparoscopic Partial Nephrectomy

  • Chromophobe renal cell carcinoma (RCC) is an uncommon subtype of RCC having a better prognosis than clear cell RCC. Although there are several reports of seeding metastasis of RCC after biopsy, seeding metastasis of chromophobe RCC after surgical resection has seldom been reported. Here, we describe a case of multiple seeding metastases in the abdomen and pelvis 78 months after robot-assisted laparoscopic partial nephrectomy, without prior history of biopsy for chromophobe RCC in the right kidney. As magnetic resonance imaging (MRI) of the pelvic mass showed a similar appearance to the primary renal mass and displayed separate margins with the rectum and prostate gland, we were able to make a diagnosis before pathologic confirmation.
