Work Conditions and Practices in Norwegian Fire Departments From 1950 Until Today: A Survey on Factors Potentially Influencing Carcinogen Exposure

  • Background: Meta-analyses have shown firefighters to be at an increased risk of several cancer types. Occupational carcinogen exposure may explain these increased risks. This study aims to describe Norwegian fire departments' work conditions from 1950 until today, focusing on factors relevant for potential occupational carcinogen exposure. Methods: With the help of a reference group, we developed a questionnaire on topics related to occupational exposure to carcinogens for the period 1950-2018. Selected Norwegian fire departments provided department-specific responses. Results: Sixteen departments, providing fire services for 48% of the Norwegian population as of 2019 and mainly consisting of professional firefighters, responded to our questionnaire. The introduction of synthetic firefighting foams, more regular live fire training, the introduction of chemical diving, and a higher number of diesel-driven fire service vehicles were identified as changes thought to increase exposure to occupational carcinogens. Changes thought to decrease exposure included the switch from negative to positive pressure self-contained breathing apparatuses, the use of self-contained breathing apparatuses during all phases of firefighting, the use of ventilating fans during firefighting, increased attention to flammable materials used during live fire training, increased attention to handling and cleaning of turnout gear and other equipment, and installment of exhaust removal systems in apparatus bays. Conclusion: Norwegian fire departments' work conditions have seen several changes since 1950, and this could influence firefighters' occupational carcinogen exposure. A peak of carcinogen exposure may have occurred in the 1970s and 1980s before recent changes have reduced exposure.

Application of Subsurface Flow Wetland using the Phragmites australis for Water Quality Improvement of the Agricultural Reservoi (농업용 저수지 수질개선을 위한 지하흐름 갈대 인공습지의 적용)

  • Constructed wetlands are regarded as an important water treatment system for agricultural water quality improvement and management. The purpose of this study is to evaluate the application of subsurface flow wetland(SFW), using the Pharagmites australis as macrophytes, and to clarify the basic and essential factors to be considered in the construction and management of constructed wetlands. This study was carried out relatively short hydraulic residence time(HRT), 6hr ~ 72hr (3days), using eutrophic reservoir water with relatively low concentrations of influent and large quantity to be treated. The effluent satisfied the criteria of agricultural water quality. Removal efficiencies of Biochemical oxygen demand(BOD), Chemical oxygen demand(COD), Suspended solids(SS) and Chlorophyll a(Chl-a) were high in HRT 24hr, not any more significant increasement of removal efficiencies in HRT 48hr and 72hr. However, removal efficiencies of nitrogen and phosphorus increased as HRT increased, showing the highest efficiency at the 72hr of HRT in nitrogen, and 48hr in phosphorous. The SFW was very effective system for reservoir water quality improvement, and had the advantages of the reduction of purchasing cost to land required, lack of odors, and harmful insects, especially mosquito, the improvement of the scenic beauty and minimal risk of public exposure. Therefore, it was evaluated that the SFW was very available water treatment system for the water quality improvement of agricultural reservoir. However, it was need to consider with application of the SFW in high cost of construction and troublesome of management.

Efficacy of Ilizarov External Fixation and Autologous Bone Grafting for Performing Arthrodesis on the Ankle Arthropathy with Risk Factors (불유합의 위험 인자를 가진 족관절 병변에 시행한 족관절 유합술로서 Ilizarov 외고정 기구 장착술 및 자가골 이식술의 유용성)

  • Purpose: The aim of this study is to evaluate if the Ilizarov external fixation procedure with axial compression can help to obtain fusion across the ankle joint in patients with a high risk of nonunion. Materials and Methods: From January 2006 to December 2015, the study reviewed 17 patients who underwent ankle arthropathy with a high risk of nonunion and who underwent ankle fusion using the Ilizarov external fixator with axial compression and auto bone grafting. After the lateral surface of the ankle joint was exposed through a lateral trans-fibular approach, massive removal of the articular cartilage and excision of any loose or avascular bone were done. With the cortical bone harvested from the pelvis as corticocancellous bone blocks, we inserted the two cortical blocks longitudinally into the anterior and posterior part of the free ankle space from lateral to medial to make the rectangular chamber to fill the cancellous bones. After the Ilizarov external fixator was equipped, we tightened the frame by 5 mm to compress the bone graft space. We accessed the American Orthopaedic Foot and Ankle Society (AOFAS) AnkleHindfoot score both preoperatively and postoperatively. Results: The average age at the time of operation was 63.4 years (range, 47~78 years). The mean frame time was 17.4 weeks (range, 15~23 weeks). The average follow-up period was 3.7 years (range, 2~6 years). Osseous fusion was obtained in 15 patients (88.2%). There were two stable pseudarthroses among the rheumatoid arthritis patients, and we continued their follow-up. The mean AOFAS AnkleHindfoot score improved from 48.5 to 73.7 points. Conclusion: Ankle arthrodesis using the Ilizarov external fixation with axial compression and auto bone grafting on the ankle arthropathy that had difficult conditions to achieve union is considered one of the useful methods with a correspondingly low incidence of complications.

  • Sebaceous carcinoma is a malignant neoplasm that usually arises in the sebaceous glands of the eyelids. Its pathogenesis is unknown; however, irradiation history, immunosuppression, and use of diuretics are known risk factors. The mainstay of treatment for sebaceous carcinoma of the eyelid is wide surgical resection with a safety margin of 5 to 6 mm, which often results in full-thickness defects. The reconstruction of a full-thickness defect of the eyelid should be approached using a three-lamella method: a mucosal component replacing the conjunctiva, a cartilage component for the tarsal plate, and a flap or skin graft for the skin of the eyelid. In this case, a full-thickness defect of the upper eyelid was reconstructed after tumor removal using a combination of a nasal septum chondromucosal composite graft and a forehead transposition flap, also known as a "Fricke flap." The flap was designed to include a line of the eyebrow on the lower margin of the flap to replace the eyelash removed during tumor excision. The wound healed completely, without any early or late complications, and the outcome was satisfactory.

  • The high fatality of COVID-19 (Corona Virus Disease 2019) is closely related to acute pneumonia and severe blood clot formation in brain, heart, kidney and lung. The use of low-molecular weight heparins has been shown to reduce the risk of thrombosis and reduce fatality rates among COVID-19 patients. In this study, we investigated the antithrombotic activity of different parts of C. sativa extracts to determine its potential in preventing blood clots in patients with thromboprophylaxis. The extracts of leaf, stem, root, immature flower and seed of C. sativa showed strong inhibitory activities on blood clot formation. In particular, the flower extract showed the strongest inhibitions against blood coagulation factors and thrombin. Strong hemolysis activities were observed in flower extract and seed extract, suggested that removal of the hemolysis active compounds in th extracts is necessary. This is a first report of anti-thrombosis activities of different parts of C. sativa extracts, and our results suggest that C. sativa extract has potential has a valuable bioresource for high-value products.

  • Aspiration of tooth can occur not only during dental treatment, but also due to factors like trauma or physiologic exfoliation of primary tooth. If this occurs, complications such as fever, cough, dyspnea, pain, and bronchitis can be appeared. 9 years-old girl with Lennox-Gastau syndrome visited the dental clinic for regular checkups. Calcified tooth-like material was observed in the chest PA x-ray, and maxillary left primary first molar was not observed in the patient's oral cavity. She had a history of hospitalization for pneumonia two months ago. Because it was considered that the tooth was likely to be aspirated and caused pneumonia, the extraction of remaining primary teeth with mobility was performed. It is necessary to reduce the risk of aspiration in patient with disabilities by performing active treatment such as removal of primary tooth with mobility or ill-fitting restorations.

  • Cholesterol gallstones are a digestive disease of high prevalence that has many risk factors; for this reason, research has focused mainly on how to prevent it rather than how to treat it. Many molecules of the hepatic, bile and intestinal systems are involved in the pathophysiology of this disease, making it very difficult to find a therapeutic target. The pharmacological treatment is limited, so when gallstones generate symptoms, medical treatment indicates gallbladder removal. Ursodeoxycholic acid is used to dissolve cholesterol stones, and ezetimibe and statins are other drugs with possible applications in the treatment of this disease. Given the small number of drugs that have been developed for treating this disease, the research of natural products becomes of paramount importance. Resources such as black radish, glucosinolates, fenugreek, capsaicin, curcumin, garlic, and onion, have all shown significant effects in the prevention and treatment of cholesterol gallstones. In this review, we made a synthesis of the scientific reports that deal with these natural products and that can serve as antecedents for finding a way to treat the most common disease of the gallbladder.

Clinical characteristics of patients with the hardware failure after surgical stabilization of rib fractures in Korea: a case series

  • Purpose: Surgical stabilization of rib fractures (SSRF) is widely used in patients with flail chests, and several studies have reported the efficacy of SSRF even in multiple rib fractures. However, few reports have discussed the hardware failure (HF) of implanted plates. We aimed to evaluate the clinical characteristics of patients with HF after SSRF and further investigate the related factors. Methods: We retrospectively reviewed the electronic medical records of patients who underwent SSRF for multiple rib fractures at a level I trauma center in Korea between January 2014 and January 2021. We defined HF as the unintentional loosening of screws, dislocation, or breakage of the implanted plates. The baseline characteristics, surgical outcomes, and types of HF were assessed. Results: During the study period, 728 patients underwent SSRF, of whom 80 (10.9%) were diagnosed with HF. The mean age of HF patients was 56.5±13.6 years, and 66 (82.5%) were men. There were 59 cases (73.8%) of screw loosening, 21 (26.3%) of plate breakage, 17 (21.3%) of screw migration, and seven (8.8%) of plate dislocation. Nine patients (11.3%) experienced wound infection, and 35 patients (43.8%) experienced chronic pain. A total of 21 patients (26.3%) underwent reoperation for plate removal. The patients in the reoperation group were significantly younger, had fewer fractures and plates, underwent costal fixation, and had a longer follow-up. There were no significant differences in subjective chest symptoms or lung capacity. Conclusions: HF after SSRF occurred in 10.9% of the cases, and screw loosening was the most common. Further longitudinal studies are needed to identify risk factors for SSRF failure.

Assessment Module Formulation for the Trapped-Oil Recovery Operations from Sunken Vessels (침몰선 잔존유 회수작업 평가모듈 개발에 관한 연구)

  • When oil tankers, large passenger ships and cargo ships sink under the sea owing to various maritime accidents, the residual cargo oil or fuel oil in the such vessels could pose direct risk to factor for the marine environment and it require safe and rapid removal. Although domestic and foreign salvage companies are adopting various recovery methods and technologies with customizations according to each site during recovery operations studies on reasonable assessment modules for the operation process are relatively insufficient. In this study, the data from trapped-oil recovery operations performed at different site conditions were collected and analyzed in order to designed an operation assessment module, define the operational process steps in terms of preparation, implementation and completion, and derive key factors for each detailed process. Subsequently, the module was designed in such a way as to construct performance indicators to assess these key factors. In order to exclude subjective opinions from the assessment as much as possible, the assessment each item was constructed with indicators based on data that could be evaluated quantitatively and its usefulness was verified by applying the module to the trapped-oil recovery operation cases. We expect this the method and the technology assessment module for the trapped-oil recovery operation on sunken vessels will help to verify the adequacy of the trapped-oil recovery such operation before or after. Furthermore, it is expected that the continuous accumulation of assessment data and feedback from past or future operation cases will contribute toward enhancing the overall safety, efficiency and field applicability of trapped-oil recovery operation.

Current Trends in the Treatment of Syndesmotic Injury: Analysis of the Korean Foot and Ankle Society (KFAS) Member Survey (원위경비골인대 손상의 치료 동향: 대한족부족관절학회 회원 설문조사 분석)

  • Purpose: This study was based on the Korean Foot and Ankle Society (KFAS) member survey and aimed to report the current trends in the management of syndesmotic injuries over the last few decades. Materials and Methods: A web-based questionnaire containing 36 questions was sent to all KFAS members in September 2021. The questions were mainly related to the preferred techniques and clinical experiences in the treatment of patients with syndesmotic injuries. Answers with a prevalence ≥50% of respondents were considered a tendency. Results: Seventy-six (13.8%) of the 550 members responded to the survey. The results showed that the most preferred method to diagnose a syndesmotic injury was magnetic resonance imaging (MRI). Intraoperatively, the external rotation stress test and the Cotton test were most frequently used to confirm syndesmotic diastasis. The reduction was usually done by a reduction clamp. One 3.5-mm screw was used most frequently over three cortices at 2~4 cm above the ankle joint. The preferred ankle position during fixation was 0° dorsiflexion. Removal of the syndesmotic screw was routinely done by most surgeons, mainly because of the limitation of movement and risk of screw breakage. Factors that affect suture button selection included non-rigid fixation which enables adequate fixation, early weight-bearing, and an infrequent need to remove the hardware. Inadequate reduction was considered the main factor that affects poor prognosis. Conclusion: This study proposes updated information about the current trends in the management of syndesmotic injuries in Korea. Consensuses in both the diagnostic and therapeutic approach to patients with syndesmotic injury were identified in this survey study. This study may raise the awareness of the various possible approaches toward the injury and should be used to further establish a standard protocol for the management of syndesmotic injuries.
