Multifactorial analysis of the surgical outcomes of giant congenital melanocytic nevi: Single versus serial tissue expansion

  • Background Giant congenital melanocytic nevus (GCMN) is a rare disease, for which complete surgical resection is recommended. However, the size of the lesions presents problems for the management of the condition. The most popular approach is to use a tissue expander; however, single-stage expansion in reconstructive surgery for GCMN cannot always address the entire defect. Few reports have compared tissue expansion techniques. The present study compared single and serial expansion to analyze the risk factors for complications and the surgical outcomes of the two techniques. Methods We retrospectively reviewed the medical charts of patients who underwent tissue expander reconstruction between March 2011 and July 2019. Serial expansion was indicated in cases of anatomically obvious defects after the first expansion, limited skin expansion with two more expander insertions, or capsular contracture after removal of the first expander. Results Fifty-five patients (88 cases) were analyzed, of whom 31 underwent serial expansion. The number of expanders inserted was higher in the serial-expansion group (P<0.001). The back and lower extremities were the most common locations for single and serial expansion, respectively (P =0.043). Multivariate analysis showed that sex (odds ratio [OR], 0.257; P=0.015), expander size (OR, 1.016; P=0.015), and inflation volume (OR, 0.987; P=0.015) were risk factors for complications. Conclusions Serial expansion is a good option for GCMN management. We demonstrated that large-sized expanders and large inflation volumes can lead to complications, and therefore require risk-reducing strategies. Nonetheless, serial expansion with proper management is appropriate for certain patients and can provide aesthetically satisfactory outcomes.

The Status of Endangered Plants Distributed in the Middle Eastern Area of Korea and Evaluation of the Risk Factors (우리나라 중동부지역에 분포하는 멸종위기야생식물 현황과 위험요인 평가)

  • Evaluation of the extinction risk of endangered plants at international, national as well as at regional levels is essential to the implementation of plans for direct conservation activities. Reports indicate that 34 endangered plants are distributed in the middle eastern area of Korea. For each endangered plant, we investigated the sites, area of extents, population size, and factors that affect population extinction. We assessed risk factors based on 10 evaluation criteria including the results from the investigation and the life traits each endangered plant has. As a result of evaluating the risk factors, these 34 endangered plants are classified into 3 groups: the first category comprises 12 endangered plants that require active and urgent conservation of habitats due to multiple risk factors; the second group has 16 endangered plants that should be able to persist with the removal of a few direct risk factors; the third category has 6 endangered plants that can persist with minimal management due to comparatively large distributed area and numerous individuals. It was found that most major risk factors in the population of endangered plants are caused by disruption of habitats and population extinction due to the increase of human habitation in the concerned areas, development and illegal harvesting. Futhermore, ecological collapse from decreasing habitats and malfunctioning mechanism of extinction and regeneration due to the changes of vegetational environment can be the other causes. From the area of the present investigations, we selected 5 regions according to the number of species and the frequency of appearance and importance of conservation measures. Also, we suggested a conservation strategy according to the regional characteristics. We suggest that the method for evaluating extinction risk of endangered plants includes distributional data and life traits of species. In addition, we underscore the necessity for understanding population dynamics and ecological niche of the each target species.

  • Today's size of production operations in the packaging paper sector I sconstantly increasing , featuring integral processing systems, high-speed paper machines, and the increased marketing of lower basis weight products. The risk factors involved in these large investments lie more than ever in the available furnish potential and ongoing requirements for further closure of water loops and lower operating costs. A comparison is drawn between recovered paper furnish mixtures used in European mills with regard to fibre properties, strength potential and contaminant content, Both the furnish situation and development trends in modern fluting and testliner machines are instigating concept changes in stock preparation, the approach flow as well as in process water and rejects technology. Developments are currently focussed for example on more efficient debris separation using fine slotted screening, optimized refining , more systematic process water management with loop separation and integral water clarification as well as more efficient removal of microstickies, fillers and fines through appropriate combination of rejects handling and water clarification processes. Product quality differentiation depends decisively on strength characteristics as a function of refining , fines removal , paper machine technology, and sizing . Maximum availability of today's high speed paper machines and , as a result, cost effective production can be ensured by optimum balancing of all subsystems with in the overall concept and by reliable control of problems with contaminants and deposits.

Medical adhesive related skin injury after dental surgery

  • An 87-year-old woman was referred for the extraction of residual teeth and removal of tori prior to prosthetic treatment. After surgery under general anesthesia, the surgical tape was removed to detach the bispectral index sensor and the hair cover. After the surgical tape was removed, skin injury occurred on the left side of her face. After epidermis repositioning and ointment application, a dressing was placed over the injury. Her wound was found to have healed completely on follow-up examination. Medical adhesive related skin injury (MARSI) is a complication that can occur after surgery and subjects at the extremes of age with fragile skin are at a higher risk for such injuries. Careful assessment of the risk factors associated with MARSI is an absolute necessity.

Risk Factors for the Treatment Failure of Antibiotic-Loaded Cement Spacer Insertion in Diabetic Foot Infection (당뇨병성 족부 감염에서 항생제 혼합 시멘트 충전물 사용의 치료 실패 위험 인자 분석)

  • Purpose: To evaluate the efficacy of antibiotic-loaded cement spacers (ALCSs) for the treatment of diabetic foot infections with osteomyelitis as a salvage procedure and to analyze the risk factors of treatment failure. Materials and Methods: This study reviewed retrospectively 39 cases of diabetic foot infections with osteomyelitis who underwent surgical treatment from 2009 to 2017. The mean age and follow-up period were $62{\pm}13years$ and $19.2{\pm}23.3months$, respectively. Wounds were graded using the Wagner and Strauss classification. X-ray, magnetic resonance imaging (or bone scan) and deep tissue cultures were taken preoperatively to diagnose osteomyelitis. The ankle-brachial index, toe-brachial index (TBI), and current perception threshold were checked. Lower extremity angiography was performed and if necessary, percutaneous transluminal angioplasty was conducted preoperatively. As a surgical treatment, meticulous debridement, bone curettage, and ALCS placement were employed in all cases. Between six and eight weeks after surgery, ALCS removal and autogenous iliac bone graft were performed. The treatment was considered successful if the wounds had healed completely within three months without signs of infection and no additional amputation within six months. Results: The treatment success rate was 82.1% (n=32); 12.8% (n=5) required additional amputation and 5.1% (n=2) showed delayed wound healing. Bacterial growth was confirmed in 82.1% (n=32) with methicillin-resistant Staphylococcus aureus being the most commonly identified strain (23.1%, n=9). The lesions were divided anatomically into four groups; the largest number was the toes: (1) toes (41.0%, n=16), (2) metatarsals (35.9%, n=14), (3) midfoot (5.1%, n=2), and (4) hindfoot (17.9%, n=7). A significant difference in the Strauss wound score and TBI was observed between the treatment success group and failure group. Conclusion: The insertion of ALCSs can be a useful treatment option in diabetic foot infections with osteomyelitis. Low scores in the Strauss classification and low TBI are risk factors of treatment failure.

  • Foreign body indigestion during dental procedures is uncommon but sometimes associated with severe and life threatening morbidity. The dentist should decide whether multidisciplinary intervention is necessary and how urgently it has to be done. The active removal of foreign bodies depends on the size, shape, type and duration of impaction, along with the location in the gastrointestinal tract. In this report, we presented how to manage gastric foreign body related swallowing during dental procedure and investigated the predictive risk factors, precaution, and prompt management in foreign body ingestion.

  • Desquamative gingivitis (DG) is a gingival manifestation of systemic mucocutaneous disorders such as mucous membrane pemphigoid, oral lichen planus, and pemphigus vulgaris. The lesion is very painful, so affects the patient's ability to do proper oral hygiene practices. This may be a potential risk factor for long-term periodontal health. However, there is some controversy about the relationship between the existence of DG and periodontal status. Although the correlation between DG-associated diseases and periodontal status is not to be certain, early diagnosis and appropriate treatment including adequate plaque control and removal of local factors is very important for preventing the progression of diseases and destruction of periodontal tissues.

  • Microblogging services (such as Twitter) are the representative information communication networks during the Web 2.0 era, which have gained remarkable popularity. Weibo has become a popular platform for information dissemination in online social networks due to its large number of users. In this study, a microblog information dissemination model is presented. Related concepts are introduced and analyzed based on the dynamic model of infectious disease, and new influencing factors are proposed to improve the susceptible-infective-removal (SIR) information dissemination model. Correlation analysis is conducted on the existing information dissemination risk and the rumor dissemination model of microblog. In this study, web hyper is used to model rumor dissemination. Finally, the experimental results illustrate the effectiveness of the method in reducing the rumor dissemination of microblogs.

  • The causes, risk factors and sequelae of mastectomy were studied, and the physical therapy approaches on post-mastectomy was discussed in this study. It was found that the patients taken mastectomy have experienced pins and needles in muscle, weakening of muscle, pains, deterioration of motion in joint region and activities of daily living, psychiatric sequelae, and etc even after the conservertive therapies like the chemical therapy, radiotherapy, immunotherapy, and hormone therapy. However, few study on the physical therapy approaches for patients with breast cancer has been carried in Korea at present. The followings were proposed as the physical therapy approaches. 1. Shoulder joint motion approach to relax the limit of range of motion 2. Control of breathing exercise for dealing with removal of the pectorailis muscle 3. Method to reduce the edema of arms for tackling the cut of lymph node 4. Method to reduce pains, pins and needles 5. Support home exercise program after discharging from hospital, sexual life and pregnancy, and activities of daily living training method.

A Study on the Actual Situation and Improvement Method of Safety Accidents of Elementary School's Facilities - Focused on Elementary Schools on Chungcheongnamdo - (초등학교 시설의 안전사고 실태 및 개선방안 - 충남지역 초등학교를 중심으로-)

  • The school is the educational and living space for students and teachers. And, the safety of the school facilities is very important element on the school activities. For this safety of school facilities, there is a need to prevent of safety-accident by the reinforcement of the systematic and the removal of risk factors. In order to minimize the damages from school facilities this study tried to suggest the solution for the prevention of safety accidents according to research about actual conditions of safety accidents from school facilities.
