Remote Medical Equipment Training for Public Health Doctors in Vulnerable Medical Areas Using Smart Glasses (스마트 글래스를 활용한 공중보건의 대상 의료장비 원격교육)

  • In medically vulnerable areas in Korea, public health doctors play a significant role in providing not only general medical care but also emergency medical services to the local residents. However, it has been observed that public health doctors generally lack field experience, resulting in insufficient ability to handle emergency patients and to effectively use medical equipment. This study confirmed the effectiveness of education after conducting remote education using smart glasses on how to use medical equipment necessary for public health doctors. Specifically, real wear was used for smart glasses for medical equipment utilization education, and 10 public health officials in 10 islands in Shinan-gun were targeted. After the training, both the effect of using the equipment and the level of satisfaction were 3 or higher. Therefore, it was confirmed that remote education using smart glasses can be usefully used for public health doctors in medically vulnerable areas.

  • The goal of this research is to develop a Remote Medical Examination System on Oriental Medicine on the very high sped information communication network. We developed a remote medical examination system based on the sample examination data of 10 patients and develop the information of search database based on contents. We analyzed the sample data and the requirements of patients, doctors, and nurses. By the analyzed result, we designed and constructed a prototype are as follow: the multimedia ORDBMS server system, network interface technology, internal/external database schema design, oriental medicine expert knowledge base design, inter-data search algorithm design and thesaurus.

  • A multimedia remote ultrasound image transfer system for tole-medical services was implemented. The image data of the ultrasound scanner system of the slave is compressed using image compression system. It is transferred to the master PC via ISDN UNI(User-Network Interface) and decompressd by image decompression system. The total system is composed of three parts; first, the image capture card which transfers bidirection- al data between ultrasound scanner system and PC, second, image compression and decompression card, finally, ISDN TA(Terminal Adaptor) card for transferring the image. This system has a easy user interface because it is executed on the basis of MS-WINDOWS. So it is capable of serving medical services at a remote place.

Telephone Survey for Developing Clinical Trial Protocol on Individualized Acupuncture Treatment for Knee Osteoarthritis (퇴행성 슬관절염의 맞춤형 침구 임상시험 프로토콜 개발을 위한 전화조사)

  • Objectives : This study was accomplished in order to find out how Korean medical doctors take acupuncture prescription for knee osteoarthritis in clinical practice Methods : The survey questions were developed by the consensus from 4 professors and 10 residents who major in acupuncture & moxibustion for developing clinical trial protocol on individualized acupuncture treatment for knee osteoarthritis. 206 persons having more than 10 years experience were randomly selected from a list of Korean medical doctors. Telephone interviews with them were conducted by the well-trained interviewers between May 21 and May 27 in 2005. Results : 139 of 206 Korean medical doctors made an acupuncture prescription prior to treatment of knee osteoarthritis. 103 of 139 Korean medical doctors used both local and remote points, and 26 of them used remote points only, and 10 of them used local points only. In case of doctors who use remote points, the five element (45 of 129) or Saam (28 of 129) acupoints were used dominantly, and its was based on the pattern identification of the Jangbu organs related with the pain (56 of 129) or of the pain location (35 of 129). Conclusion : This study showed that Koran medical doctors prefer to use remote points such as five element or Saam acupoints based on the pattern identification of the Jangbu organs related with the pain or the pain location. These result may be useful for developing protocols for clinical trial on acupuncture on knee osteoarthritis.

  • This paper presents the content regarding electronic medical examination chart and data treatment for efficient medical examination and prompt treatment by realizing mutual conversation type remote medical examination system among 3 parties(patient, doctor, pharmacist) on internet base. This is an intelligence type remote medical examination system for both on-line and off-line mode to transcend time and space on the web being participated by anybody, which is cheap type to solve problems in existing remote medical examination system such as high price based on hardware, incompatibility, and so on. By interconnecting ASP and SQL on IIS 4.0 web server, database enables system integration for efficient data processing, on-line consultation between patient and doctor, medical examination on off-line, transmission of medical prescription to pharmacist designated by patient and preparation of medicine, semi-eternal storage of medical examination data owing to storage and search of medical examination data, exact medical examination and prescription using this medical examination data by patient and doctor, and so on.

  • A wireless 3 channel ECG monitoring system was developed so that it could monitor the health and movement state during subject's daily life. The developed system consists of a wireless biomedical signal acquisition device, a personal healthcare server, and a remote medical server. Three experiments were performed to evaluate the accuracy, reliability and operability, applicability during daily life of the developed device. First, ECG signals were measured using the developed device and commercial reference device during sitting and marking time and compared to verify the accuracy of R-R intervals. Second, the reliable data transmission to remote server was verified on two types of simulated emergency event using patient simulator. Third, during five types of motion in daily life, the accuracy of data transmission to remote server using CDMA network was verified on two types of event occurring. By acquiring and comparing subject's biomedical signal and motion signal, the accuracy, reliability and operability, applicability during daily life of the developed device were verified. In addition, PDA-phone based wireless system enabled subject to be monitored without any constraints. Therefore, the developed system is expected to be applicable for monitoring the aged and chronic diseased people and giving first-aid in emergency.

Remote Epidural Hematoma Following the Removal of Brain Tumors : Report of Three Cases (뇌종양 제거 후 원격부위에 발생한 뇌경막외혈종 - 증례보고 -)

  • Objective : The authors present three cases of brain tumors in which epidural hematomas(EDHs) were developed postoperatively in the remote areas from craniotomy sites. The preventive tactics as well as possible mechanisms of development of remote EDH are discussed. Material and Methods : The magnetic resonance imagings of three patients revealed a left lateral ventricular mass located just aside of foramen Monro in a 27-year-old male, a large cystic mass in the temporal lobe in a 35-year-old male, and a partially calcified pineal mass in a 27-year-old male patient. The surgical removals of these tumors were performed without any noticeable events during surgery via left frontal transcortical transventricular approach for lateral ventricular tumor, left temporal craniotomy for cystic temporal tumor, and right occipital transtentorial approach for pineal tumor. Results : Postoperative EDHs remote from the sites of craniotomy were detected by the immediate postoperative computerized tomographic scans. We obtained good outcomes without any morbidity in all three patients with emergent evacuation of the hematoma. The pathologic diagnoses were lateral ventricular ependymoastrocytoma, temporal craniopharyngioma and mixed germinoma of the pineal region. Conclusion : It is postulated that a sudden reduction of intracranial pressure(ICP) at the time of tumor removal may strip the dura from the inner table of the skull to cause EDH from the remote site of craniotomy. Gradual reduction of ICP with slow drainage of cerebrospinal fluid before tumor removal as well as lowering the head position of patient during surgery might be helpful for preventing this unusual complication.

Remote digital monitoring during the retention phase of orthodontic treatment: A prospective feasibility study

  • Objective: To evaluate if a remote digital monitoring system added at the end of orthodontic treatment could positively influence the retention phase by reducing the occurrence of misfit of removable appliances, number of emergency appointments (EA), and orthodontic relapse. Methods: Twenty-seven patients who completed active orthodontic treatment were divided into the study and control groups. In addition to the standard chairside follow-up appointments at month 1 (T1), month 3 (T2), month 6 (T3), the study group patients were monitored using Dental Monitoring® with monthly intra-oral scans. Occurrence of misfit of removable retainers, number of EAs, and intercanine width change were recorded for both groups. Differences in EAs and retainer fit were assessed using the chi-square test. Intra-group and inter-group differences in the intercanine width were assessed with Friedman test and Mann-Whitney U test, respectively (α = 0.05). Results: The study group showed a significantly lower occurrence of misfit of removable retainers (p = 0.027) compared to the control group. No significant inter- and intra-group difference was found in the EAs and intercanine width change at each time-point. Conclusions: Integrating remote monitoring systems, such as Dental Monitoring®, to the retention phase of the orthodontic treatment may lower the occurrence of misfit of removable retainers. However, a small sample size and a short observation period limit the strength of this evidence. These preliminary results tentatively suggest that remote monitoring technologies may be beneficial, especially during the COVID-19 pandemic, when the regularity of in-office visits might be disrupted.

Korean Medicine Doctors' Perception of Telemedicine (원격의료 도입에 대한 한의사의 인식)

  • Objectives : This study aimed to investigate the perception level of Telemedicine among Korean medical doctors. Methods : The on-line survey was administered for Korean Medicine doctors. The survey consisted of 46 questions, addressing issues on concepts and adoption model of Telemedicine. Data were collected from 528 Korean medical doctors, and were analyzed using frequency analysis, t-test, ANOVA, and Kruskalwallis test. Results : The results showed that more than half of respondents had positive attitude towards implementation of Telemedicine in Korean Medicine and conventional medicine areas. Respondents were most positive about the aspect that Telemedicine could reduce 'hospital use by people with disabilities', while they were most concerned about the possibility that patients could be concentrated in large-scale hospitals. As prerequisites for the introduction of Telemedicine, accurate information delivery during remote communication between doctors and patients, clarification of responsibility for Telemedicine at the policy level, and development of equipment for accurate information delivery at the technical level received high responses. Conclusions : Korean Medicine doctors were positive about the implementation of Telemedicine, and they preferred to remote monitoring between doctors and patients. The development of medical equipment for accurate patient information delivery and the establishment of an institutional basis for clarifying responsibilities in case of medical accidents are required.
