Can single buccal infiltration with 4% articaine induce sufficient analgesia for the extraction of primary molars in children: a systematic literature review

  • This systematic review aims to determine if a single buccal infiltration (without palatal infiltration in the maxilla and Inferior Alveolar Nerve Block in the mandible) with 4% articaine can induce adequate analgesia for the extraction of primary molars (Maxillary and Mandibular) in children. PubMed, Ovid SP, and Embase were searched for studies published between January 1990 and March 2020 with the relevant MeSH terms. Titles and abstracts were screened preliminarily, followed by the full-texts of the included studies. Five articles were included for this systematic review. The outcome investigated was "Procedural pain during the extraction of primary molars after injection with single buccal infiltration of 4% articaine in comparison to single buccal infiltration, double infiltration (buccal and palatal/lingual), and inferior alveolar nerve block with 2% lignocaine." Of the five studies that evaluated subjective pain during extraction, two reported no significant difference between the articaine and lignocaine groups, and the remaining three reported lower subjective pain during extraction in the articaine group. Only two studies evaluated objective pain scores during extraction, and both studies reported lower pain scores in the articaine group. There is insufficient evidence to justify the statement that a single buccal infiltration of 4% articaine alone is sufficient for the extraction of primary molars. Further evidence is required to justify the claim that palatal infiltrations and IANB can be replaced with the use of 4% articaine single buccal infiltration for the extraction of primary molars in children.

Effects of fresh mineralized dentin and cementum on socket healing: a preliminary study in dogs

  • Objectives: Dentin is composed of many minerals and growth factors. Based on this composition, we studied its effect as a possible regenerative material for alveolar healing. Materials and Methods: This study was conducted using four 2.5-year-old mongrel dogs (male; weight, 25 to 30 kg). The third mandibular premolars were carefully mobilized with a dental elevator and then removed using forceps. The crown portions of the extracted teeth were removed with cutters, and the root portions of the remaining teeth were collectively trimmed as closely as possible to 350 to $500{\mu}m$. Dentin and cementum (DC) chips harvested from the extracted teeth were soaked in blood and packed into the fresh sockets (autograft). Biopsies were performed at the ends of day 14 and day 56 following implantation. Data were expressed as $mean{\pm}standard$ deviation and compared with t-test results. Results: The ratio of $S_A$(bone) to total area of each probe was determined and was $170{\pm}16{\mu}m^2$ for the control group and $71{\pm}14{\mu}m^2$ for the DC group, a significant difference (P <0.05). Conclusion: DC particulate grafts offered no improvement in bone regeneration in alveolar extraction sockets.

  • Among the effects of tooth bleaching on composite resins, degree of color change and dye deposition onto composite resins after office bleaching were investigated in vitro. Seventy two disc-shaped resin samples were fabricated with hybrid type composite resin, Z-100 under 3 different environments(24 samples for each environment) characterized by 3 kinds of light-curing intensity and duration($250mW/cm^2$-20sec., $250mW/cm^2$-40sec., $550mW/cm^2$-20sec.). As control, one-third samples of each group were not treated with bleaching agent. The remaining two-thirds samples of each group were treated with bleaching agent(bleaching group). Then, before thermocycling procedure in coffee bath, the half of the samples treated with bleaching agent were polished(polishing group) with polishing system. SofLex, but the other half(not-polishing group) and control group were not polished. Another 72 samples were also made with microfilled type composite resin. Sillux Plus and treated according to the experimental procedures mentioned above. The color of each resin sample was measured before bleaching, after bleaching, and after thermocycling preceded by bleaching. And color difference was evaluated. It was concluded as follows: 1. The amount of color change of resin samples after office bleaching was not statistically significant(p>0.05). But the samples which were treated with bleaching agent showed more color change than that of control group. 2. After thermocycling in coffee bath, the amount of color change of resin samples between control and bleaching group was not statistically significant(p>0.05). 3. After thermocycling in coffee bath, the polishing procedure of resin samples showed no statistically significant difference(p>0.05) between polishing and not-polishing group in the aspect of color change.

치아우식증환자와 치아정상인(齒牙正常人)의 치태(齒苔)에서 분리(分離)한 Streptococcus mutans의 혈청형(血淸型) 분포(分布)에 관(關)한 조사(調査) 연구(硏究) (A Survey of the Serotype Distribution of Streptococcus mutans Isolated from Dental Plaques of Caries-active and Caries-free Subjects)

  • An epidemiological survey was carried out to identify the serotypes of Streplococcus mutans isolated from carious lesions of 65 caries-active subjects(CAS) and sound tooth surfaces of 40 caries-free subjects(CFS). The autoclaved antigen extract was performed on each isolate, and then, serotypes of the isolates were determined in agar-gel immunodiffusion test. The results were as follows: 1. S. mutans was found in 78% of the samples of CAS, and of CFS. The difference of isolation frequency between CAS and CFS was not observed. 2. Only one serotype per single subject was detected in 61% of total samples, in remaining 39% of samples two or more serotypes were detected. 3. In 41.2% of CAS samples plural serotypes of S. mutans were found, whereas 35.5% of CFS samples showed plural serotypes distribution. 4. The most frequently identified serotype in each subject was serotype c; 69.5% of subjects harbored serotype c S. mutans. Serotype d was next most frequently isolated from subjects, comprising 23.2%. 5. Serotype c strain was found in 64.7% of CAS, 77.4% of CFS. 6. Of the isolates from CAS and CFS, serotype c was most commonly found, comprising 48.8%, serotype d was found in 16.3%, serotypes f. e, and g comprising 13.2%, 9.3%, and 7.8% respectively. Serotypes a and b were also found but in far lower frequencies(2.3%, 0.8%). 7. Serotype c strains were more found in CFS than in CAS, but serotypes d and e were more found in CAS.

Xylitol이 구강세균의 부착에 미치는 영향에 관한 연구 (EFFECT OF XYLITOL ON BINDING OF ORAL BACTERIA TO SALIVA-COATED SURFACES)

  • Cariogenicity of the bacteria is attributed to their binding capacity to the teeth. Bacterial attachment to oral surfaces is an essential step for colonization and subsequently infection. Therefore, it is conceivable that caries prevention can be achieved fundamentally by inhibition of bacterial attachment. The rationale for caries prevention through the use of sugar substitutes or limited use of sugar has been revealed. Among many sugar substitutes, xylitol has been shown to exhibit the most profound cariostatic effect, inhibiting glucose metabolism and possibly binding of mutans streptococci. The purpose of this study was to examine the effect of xylitol on binding of different species of oral bacteria. The effect of xylitol on binding of [$^3H$]-labeled oral bacteria to hydroxyapatite coated with human saliva(SHA) as a model for the pellicle-coated tooth surfaces was investigated. The strains of oral bacteria used in this study were A. viscosus T14V, A. viscosus WVU627, P. gingivaiis 2561, P. gingivalis A7Al-28, S. gordonii G9B, S. gordonii Challis, S. sobrinus 6715, S. mutans UA101, S. mutans KPSK -2, S. mutans T8, and S. mutans UA130. The obtained results were as follows: 1. P. gingivalis A7 Al-28, S. mutans UA130, S. mutans T8 grown with xylitol showed greater binding to SHA than the organism grown without xylitol. Among these, S. mutans T8 showed the greatest rate of increase in its binding to SHA ; 8-fold increase in its binding with xylitol. 2. S. mutans KPSK -2 grown with xylitol showed 2 times lesser binding to SHA than the organism grown without xylitol. 3. Binding ability of the remaining strains grown with xylitol to SHA was almost same as that of the organisms grown without xylitol. The overall results suggest that use of xylitol in the oral cavity may affect the complex oral bacterial ecosystem.

Effect of 38% carbamide peroxide on the microleakage of silorane-based versus methacrylate-based composite restorations

  • Objectives: This study aimed to assess the effect of 38% carbamide peroxide on the microleakage of class V cavities restored with either a silorane-based composite or two methacrylate-based composites. Materials and Methods: A total of 96 class V cavities were prepared on the buccal surface of extracted human teeth with both enamel and dentin margins and were randomly assigned into three groups of Filtek P90 (3M-ESPE) + P90 system adhesive (3M-ESPE)(group A), Filtek Z250 (3M-ESPE) + Adper Prompt L-Pop (3M-ESPE)(group B) and Filtek Z350XT (3M-ESPE) + Adper Prompt L-Pop (group C). Half of the teeth were randomly underwent bleaching (38% carbamide peroxide, Day White, Discus Dental, applying for 15 min, twice a day for 14 day) while the remaining half (control) were not bleached. Dye penetration was measured following immersion in basic fuchsine. Data were statistically analyzed using Kruskal-Wallis and Mann-Whitney U tests at a level of 0.05. Results: No significant differences were found between composites in the control groups in enamel (p = 0.171) or dentin (p = 0.094) margins. After bleaching, microleakage of Z250 (in enamel [p = 0.867] or dentin [p = 0.590] margins) and Z350 (in enamel [p = 0.445] or dentin [p = 0.591]margins) did not change significantly, but the microleakage of P90 significantly increased in both enamel (p = 0.042) and dentin (p = 0.002) margins. Conclusions: No significant differences were noted between the bleached and control subgroups of two methacrylate-based composites in enamel or dentin margins. Microleakage of silorane-based composite significantly increased after bleaching.

Glass ionomer cement를 이장한 Composite resin의 변연 적합성에 관한 연구 (MARGINAL ADAPTATION OF COMPOSITE RESIN USING GLASS IONOMER CEMENT BASES)

  • The purpose of this study was to observe the microleakage of composite resin filling using several glass ionomer cements. The Class V cavities of eighty noncarious human molars were prepared at the cementoenamel juction on the facial and lingual surfaces of each tooth with a No.330 carbide bur in a high speed handpiece. The cavity dimensions were $3.0{\pm}0.5mm$ wide, $2.0{\pm}0.5mm$ high, and $1.5{\pm}0.5mm$ deep and all enamel cavosurface margins were beveled with a No.558 carbide bur in low speed handpiece. The bevel was approximately $45^{\circ}$ and 0.5-1.0mm in width. A total of the 160 cavities was divided into four groups, and then 144 cavities among them were three experimental groups and remaining sixteen cavities were control group. All of the prepared cavities were restored as follows: group 1 : Preparations were restored with there three glass ionomer cements. group 2 : Preparations were restored with a composite resin with three glass ionomer cement bases placed $0.2{\pm}0.1mm$ short of the cavosurface margin. group 3 : Preparations were restored with a composite resin with three glass ionomer cement bases extened to the cavosurface margin. group 4 : As control group, preparations were restored with a composite resin, PALFIQUE. The specimens were then thermocycled in a range of $6^{\circ}C-60^{\circ}C$ and immersed in a bath of 2.0% aqueous basic fuchsin solution for 24 hours. Dye penetration was read on a scale of 0 to 4 by Tani and Buonocore's method. The following conclusions were derived from the results obtained; 1. All groups showed significantly more leakage at the gingival margins than at the occlusal margins(p<0.0005). 2. At the gingival margins, group 1 showed less leakage than group 3(p<0.01) and group 4(p<0.0005), while group 3 exhibited less leakage than group 2(p<0.01) and group 4(p<0.0005). 3. At the occlusal margins, group 4 showed less leakage than group 3(p<0.1) and group 1(p<0.005), while group 3 exhibited less leakage than group 2(p>0.1) and group 1(p<0.025).

Prevalence of missing and impacted third molars in adults aged 25 years and above

  • Purpose: The purpose of this study was to determine the prevalence of missing and impacted third molars in people aged 25 years and above. Materials and Methods: The study sample of 3,799 patients was chosen randomly from patients who visited Pusan National University Dental Hospital and had panoramic radiographs taken. The data collected included presence and impaction state, angulation, and depth of impaction of third molars, and radiographically detected lesions of third molars and adjacent second molars. Results: A greater percentage of men than women retained at least one third molar. The incidence of third molars decreased with increasing age. The incidence of partially impacted third molars greatly declined after the age of 30. Vertically impacted maxillary third molars and horizontally impacted mandibular third molars were most frequent in all age groups. Among the maxillary third molars, those impacted below the cervical line of the second molar were most frequent in all age groups, and among the mandibular third molars, deeply impacted third molars were most frequent in those aged over 40. Dental caries was the most common radiographic lesion of the third molars. Mesioangularly impacted third molars showed radiographic lesions in 13 (9.5%) adjacent maxillary second molars and 117 (27.4%) mandibular second molars. Conclusion: The number of remaining third molars decreased and the percentage of Class C depth increased with age. Caries was the most frequent lesion in third molars. Partially impacted mesioangular third molars showed a high incidence of caries or periodontal bone loss of the adjacent second molar. Regular oral examination will be essential to keep asymptomatic third molars in good health.

수종의 역충전 재료의 치근단 밀폐력 비교 (COMPARISON OF THE SEALING ABILITY OF VARIOUS RETROGRADE FILLING MAIERIALS)

  • This study was performed to evaluate the sealing ability of various retrograde filling materials by using bacterial penetration and dye penetration test. One hundred and forty extracted human teeth with single, straight canals and mature apiece were collected and used for this study. All canals were instrumented using an engine driven Ni-Ti file (ProFile). After removing 3mm from the apex of tooth, a standardized 3mm root end cavity was prepared using an ultrasonic instrument. The 70 teeth were randomly divided into 7 groups : 6 groups for retrograde filling using Super-EBA, ZOE, Chelon-Silver, IRM, ZPC and amalgam. The 7th group was used as a negative control. Nail varnish was applied to all external root surfaces to the level of the reseated root ends to prevent lateral microleakages. The specimens were then sterilized in an ethylene oxide sterilizer for 24 hours. 2 mm of the reseated root was immersed in a culture chamber containing a Tripticase Soy Broth with a phenol red indicator. The coronal access of each specimen was inoculated every 72 hours with suspension of Proteus vulgaris. The culture media were observed every 24hours for color change indicating bacterial contamination. The specimens were observed for 4weeks. The remaining 70 teeth were submitted to a dye penetration test. The canals of all teeth were first sealed with AH26 and obturated using an Obtura II system. Root resection, root end preparation and retrograde filling was performed as above. All specimens were suspended in 2% methylene blue dye for 72 hours before being ion gitudinally split. The degree of dye penetration was then measured using a stereomicroscope at 10 magnification and evaluated. The results were as floows : 1. In the bacterial penetration, the degree of leakage was the lowest in the Super-EBA, followed by, in ascending order, ZOE, Chelon-Silver IRM and ZPC. The amalgam showed highest bacterial leakage of all(p<0.01). 2. In the dye penetration, the degree of microleakage was the lowest in the Chelon-Silver and Super-EBA, followed by, in ascending order, IRM, ZPC. The ZOE and amalgam showed the highest microleakage of all (p<0.05). These results suggested that the eugenol based cement, Super-EBA, have excellent sealing ability as a retrograde filling material.

구강건강상태와 EuroQol-5 Dimension을 활용한 건강 관련 삶의 질의 관계 (Correlation between Oral Health Condition and Life Quality Related to General Health Using EuroQol-5 Dimension)

  • 질병발생 양상이 변화되고 건강에 대한 관심이 증가하며, 건강에 대한 임상적이며 객관적인 평가와 동시에 환자의 주관적 인식을 포괄하는 건강 상태를 평가하기 위한 노력이 계속되고 있다. HRQoL을 측정하는 것을 이러한 노력 중 하나로 꼽을 수 있는데, 본 연구는 일반적인 HRQoL과 구강건강 상태와의 관련성을 알아보기 위해 수행되었다. 대표성 있는 국민건강영양조사 자료 중 제6기 2차년도(2014년) 자료를 활용하여 분석을 수행하였다. EQ-5D로 측정된 일반적인 HRQoL에 영향을 미치는 요인은 성별, 연령, 교육수준, 소득수준, 민간건강보험 가입 여부, 주관적으로 인식하는 전신건강상태, 최근 1년간 치통 경험, 현존 치아 수로 확인되었다(p<0.05). 구강건강은 전반적인 건강상태를 결정짓는데 중요한 역할을 하기 때문에 객관적인 구강건강 상태가 전신건강에 영향을 미치고, 이는 결국 일반적인 HRQoL에 관련성이 있는 것으로 볼 수 있다.
