Alveolar bone height according to the anatomical relationship between the maxillary molar and sinus

  • Purpose: The aim of this study was to investigate the available alveolar bone height between the maxillary molars and the sinus floor according to their anatomical relationship using cone-beam computed tomographic (CBCT) images. Methods: A total of 752 maxillary first (M1) and second molars (M2) on CBCT scans of 188 patients were selected. First, each maxillary molar was categorized as type 1, 2, 3, or 4 according to the relationship of the molar root with the maxillary sinus floor. The frequency distribution of each type was analyzed. Second, the shortest vertical distance (VD) of each molar was measured from the furcation midpoints of the roots to the lowest point of the sinus floor by 2 observers. Intraclass correlation coefficients and the t-test were calculated for the VD measurements. Results: For M1, type 3 was the most frequent, followed by type 2. For M2, type 3 was the most common, followed by type 1. The VD measurements of type 1 were 9.51±3.68 mm and 8.07±2.73 mm for M1 and M2, and those of type 3 were 3.70±1.52 mm and 4.03±1.53 mm for M1 and M2, respectively. The VD measurements of M2 were significantly higher in female patients than in male patients. Conclusions: Type 3 was the most frequent anatomical relationship in the maxillary molars, and showed the lowest alveolar bone height. This information will help clinicians to prevent complications related to the maxillary sinus during maxillary molar treatment and to predict the available bone height for immediate implant planning.

모집단 약물동태학 방법에 의한 겐타마이신 약물동태에 미치는 환경의 영향 (Enviromental Influences on Gentamicin Pharmacokinetics by Using Population Pharmacokinetic Methods)

  • Enviromental differences in gentamicin pharmacokinetics by using population pharmacokinetic methods were compared with 20 Korean patients and 24 Korean-American appendicitis patients. Two to six blood specimens were collected from all patients at the following times : just before a regularly scheduled infusion and at 0.5 hour after the end of a 0.5 hour infusion. Nonparametric expected maximum (NPEM) algorithm for population modeling was used. The estimated parameters were the elimination rate constant (K), the slope (KS) of the relationship between K versus creatinine clearance ($C_{cr}$), the apparent volume of distribution (V), the slope (VS) of the relationship between V versus weight, gentamicin clearance (CL) and the slope (CS) of the relationship between CL versus $C_{cr}$ and the V. The output includes two marginal probability density function (PDF), means, medians, modes, variance and CV%. The mean K (KS) were $0.402{\pm}0.129\;h^{-1}(0.00486{\pm}0.00197\;[h{\cdot}ml/min/1.73\;m^2]^{-1})$ and $0.411{\pm}0.135\;h^{-1}(0.00475{\pm}0.00180\;[h{\cdot}ml/min/1.73\;m^2]^{-1})$ for Korean and Korean-American populations, respectively. The mean V (VS) were not different at $14.3{\pm}3.6l(0.241{\pm}0.0511l/kg)$ and $15.1{\pm}3.84l(0.239{\pm}0.0492l/kg)$ for Korean and Korean-American populations, respectively (p>0.2). The mean CL (CS) were $5.68{\pm}1.69l/h(0.0714{\pm}0.0222l/kg[h{\cdot}ml/min/1.73\;m^2])$ and $5.70{\pm}1.77l/h(0.0701{\pm}0.0215l/kg[h{\cdot}ml/min/1.73\;m^2])$ for Korean and Korean-American populations, respectively. There were no enviromental differences in gentamicin pharmacokinetics between Korean and Korean-American appendicitis patients.

두개하악장애와 두부전방자세와의 관계 (Relationship between Forward Head Posture and Craniomandibular Disorders)

  • This study was performed to investigate the relationship between Forward Head Posture(FHP) and Craniomandlbular Disorders(CMDs). Many studies reported that there was some relationship between them, however, there is still controversy. So It Is necessary to observe and compare many more patients with CMDs wirh normal controls. For the study 85 patients with CMDs and 37 dental students were selected as experimentals and controls, respectively. And the experimentals were classified Into two groups, that is, TMJ internal derangement group and muscle disorders group according to clinical diagnosis. For measuring the FHP, CROM(Cervical-Range-of-Notion)was used. This goniometer is composed of three part. First, gravity goniometer for flexion and extension. Second, magnetic compass and yoke for rotational movement. And last, forward head arm and vertebra locator for forward head posture. Next T-Scan, electronic occlusal analyzer, was used for recording of occlusal contact state. Other items such as maximum opening, lateral excursion, Helkimo's anamnestic index, and muscle palpation point from Friction's craniomandibular index were checked clinically by one examiner. The result of this study were as follows : 1. In male, control group showed much more measurement in resting forward head posture than did experimental group. But there were not significant differences between groups in female subject. From this results, the author contended that CROM is new measuring system and differ from other goniometers in some aspect, so that results should be re-evaluated 2. Mean value of maximum mouth opening in nearly all groups were greater than 40mm. and mouth opening had a significant correlation with occlusal force and with anamnestic index both sex. 3. Mean value of palpation point had not any correlationship with forward head posture in both sex, but there was significant difference between upper and lower group by rounded shouldes. 4. In summary, there was no significant relationship between forward head posture and sign and symptom of Craniomandibular Disorders.

수근관 증후군 환자의 수술 후 증상 호전과 전기생리학적 호전시기의 관련성에 관한 예비연구 (Temporal Relationship between Symptomatic and Electrophysiological Improvement to Postoperative Carpal Tunnel Syndrome Patients: Preliminary study)

  • Background and Objectives: A nerve conduction study (NCS) has been known as a useful method to evaluate the therapeutic effect of operation in carpal tunnel syndrome (CTS). To evaluate the temporal relationship between symptomatic and electrophysiological improvement, we compared the preoperative symptoms and electrophysiological results with postoperative those. Methods: We analyzed the NCS changes before and after minimal release of carpal tunnel in 26 patients (34 hands) with CTS. The time of postoperative symptomatic changes, postoperative electrophysiological changes and temporal relationship between symptomatic and electrophysiological changes were evaluated. Results: The mean age was $49{\pm}13$ years. The proportion of males to female was 8 and 92 percent. The median interval days between date of operation and those of postoperative NCS was 28.5 days. Postoperative symptoms improved in 17 hands, slightly improved in 13 hands, and have not changed in 4 hands. Electrophysiological improvements after operation were observed in 26 hands, and mostly appeared within 2 months. Symptomatic relief accompanied with electrophysiological improvement reported in 13 hands (50%). Moreover, the four hands with symptom, not relieved by decompression, showed electrophysiological improvement. Conclusions: In this study, electrophysiological improvement was in consistency with symptomatic relief to some extent, but we got the result of disagreement between electrophysiological and symptomatic improvement.

간동맥 화학색전술을 받은 간세포암 환자의 삶의 질 영향요인 (Factors Influencing Quality of Life in Patients with Hepatocellular Carcinoma Receiving Transarterial Chemoembolization)

  • Purpose: The purpose of this study was to examine the relationship among family support, self-care and quality of life(QOL) in Patients with hepatocellular carcinoma receiving transarterial chemoembolization, including the effects of these variables on QOL. Methods: The research was a cross-sectional, descriptive design. Participants were 103 patients with hepatocellular carcinoma who were receiving transarterial chemoembolization. Data were analyzed using descriptive statistics, t-test, one way ANOVA, and hierachial multiple regression analysis with SPSS 18.0 program. Results: QOL had a significant correlation with family support(r=.60, p<.001) and self-care(r=.38, p<.001). Family support had a significant correlation with self-care(r=.41, p<.001). Hierarchial multiple regression analysis for QOL revealed that the most powerful predictor was family support followed by self-care. Family support, self-care, gender, and occupation explained 50% of the variance in quality of life. Conclusion: These results indicate that various factors are related to the quality of life of these patients. Further, nursing strategies to improve the family support and self-care of patients with hepatocellular carcinoma are needed.

당뇨병 환자의 자기효능, 자기조절, 상황적 장애, 자기간호행위간의 관계 (A Study of the Relationship among Self Efficacy, Self Regulation, Situational Barriers and Self Care Behavior in Patients with Diabetes Mellitus)

  • The purpose of this study was to identify the degree of self efficacy, self regulation, situational barriers and self care behavior in patients with diabetes and to identify the relationships among those variables. Ninety five non insulin dependent diabetic patients participated. Data were collected by a self report questionnaire. The results are as follows 1) Mean scores for self care behavior were 4.64 (diet) and 6.60(medication) on a 7 point scale. 2) Mean scores for self efficacy were 65.12(diet) and 88.46 (medication) on a 100 point scale. 3) Mean score for self regulation was 0.42 on a 0-1 point scale. 4) Mean score for situational barriers was 1.48 on a 4 point scale. 5) Self efficacy was significantly highly correlated with self care behavior (r=0.72, P<0.01). 6) Self regulation(r=0.28, P<0.01), situational barriers(r=-0.32, P<0.01) were significantly correlated with self care behavior. 7) Self efficacy was significantly correlated with self regulation(r=0.25, P<0.01), situational barriers(r=-0.22, P<0.05). These results suggest that for improvement in self care behavior nurses should increase the level of self efficacy and self regulation in patients with diabetes and help these patients to cope with situational barriers.

Knowledge of Human Papillomavirus and its Association with Head and Neck Benign and Malignant Lesions in a Group of Dental Patients in Pakistan

  • Human papillomaviruses (HPVs) remain a serious world health problem due to their association with cervical and head and neck cancers. While over 100 HPV types have been identified, only a few subtypes are associated with malignancies. HPV 16 and 18 are the most prevalent oncogenic types in head and neck cancers. Although it has been proven that some subsets of benign and malignant head and neck lesions are associated with HPV, the general population have very little awareness and knowledge of their association with HPV. Therefore, the purpose of this study was to determine the knowledge of HPV and its links with head and neck benign and malignant lesions in a group of Pakistani dental patients who attended the Dental Department of the Sandeman provincial hospital in Quetta, Pakistan. One hundred and ninety-two patients were recruited and requested to answer a questionnaire. It was revealed that there was a low level of knowledge about HPV and its association with head and neck benign and malignant lesions among the participants. This result suggested that more education regarding the relationship of HPV in inducing head and neck benign and malignant lesions is required in this group of patients.

Novel variants of IDS gene, c.1224_1225insC, and recombinant variant of IDS gene, c.418+495_1006+1304del, in Two Families with Mucopolysaccharidosis type II

  • In this report, the phenotypes of three patients from two families with mucopolysaccharidosis type II (MPS II) are compared: a novel variant and recombinant variant of IDS gene. The results of urine in patients showed a pronounced increase in glycosaminoglycan excretion with decreased iduronate-2-sulfatase enzyme activity in leukocyte, leading to a diagnosis of MPS II. A patient has a novel variant with 1 bp small insertion, c.1224_1225insC in exon 9, which caused frameshifts with a premature stop codon, and two patients have a recombination variant, c.418+495_1006+1304del, leading to the loss of exons 4, 5, 6, and 7 in genomic DNA, which is relatively common in Korean patients. They had different phenotypes even in the same mutation. The patients have now been enzyme replacement therapy with a significant decrease in glycosaminoglycan excretion. Further study on residual enzyme activity, as well as experience with more cases, may shed light on the relationship between phenotypes in MPS II and gene mutations.

심박변이도를 통한 침자극과 스트레스의 상관관계 연구 (A Study on the Relationship with Acupuncture Stimulation and Stress Using Heart Rate Variability)

  • Objectives : The purpose of this case-control research was to investigate the effects of acupuncture stimulation on autonomic nervous system for patients with HRV and to find out relationship with anti-stress effects. Methods : The study group consists of 24 patients with self-recognition of stress as the case group, and 20 normal person as the control group by similar age. We measured HRV of case and control groups before acupuncture stimulation, prick acupuncture in Hegu(LI4), Taichong(Liv3), Shenme(H7), Neiguan(P6), Zusanli(S36). After treating for 20 minutes, measurement values of HRV and PSV were compared for pre-acupuncture and post-acupuncture. Results : 1. LF norm, HF norm, LF/HF between the case and control groups were significant different in HRV before acupuncture stimulation in the 1st experiment. 2. HRT, SDNN, SDSD, LF norm, HF norm, and LF/HF of the case group were significant different in HRV after acupuncture stimulation in the 1st experiment. HRT of the case group was significantly different in HRV after acupuncture stimulation in the 1st experiment. 3. LF norm, HF norm, LF/HF of the case group were significant different between the 1st and 2nd experiment in HRV before acupuncture stimulation. 4. LF norm, HF norm, and LF/HF were significant different between the 1st and 2nd experiment in HRV of patients whose symptoms improved. But HRV of patients whose symptoms unimproved didn't show significant difference. Conclusion : The results suggest that acupuncture stimulation is associated with changed activity in the sympathetic and parasympathetic nervous system. Measurement values of HRV is suitable to estimate the activity of automatic nervous system.

루푸스 환자의 질병특성, 심리사회적요인 및 치료 행태 (A Study on Disease Characteristics, Psycho-social Factors and Treatment Behavior of Patients with Lupus)

  • This study was conducted to describe disease characteristics, psycho-social factors and treatment behavior of patients with lupus and to analyze relationships anions these variables. The subjects were consisted of 120 lupus patients at G university hospital in Seoul. Data were collected by means of structured interview with questionnaires, and analyzed by using frequencies, $x^2$test, t-test, ANOVA, and correlation. The results were as follows; 1. The average age of the subjects was 31.34 years and average duration of suffering lupus was about 7.3years. Severity of symptoms was moderate and the disturbance of living activities was at the very low level. 2. Self-efficacy score of lupus patients was high, and life satisfaction was at moderate level. 3. Dosage for most of the subjects was about twice a day. 16.7% of them took non-prescribed medicine and only 27.5% perform exercise. 4. Positive correlation was shown among symptom, discomfort of living activities, self-efficacy and life satisfaction, while there was negative correlation among symptom and self-efficacy, symptom and life satisfaction, discomfort of living activities and self-efficacy, and discomfort of living activities and life satisfaction. In addition, exercise showed significant relationship with discomfort of living activities, and non-prescribed medicine showed significant relationship with discomfort of living activities and self-efficacy. These results show that lupus is a chronic disease of long suffering duration among young people. Severity of the symptom was at modest level, while discomfort of living activities were relatively insignificant. However self-efficacy was relatively high. Furthermore self-efficacy has influences on symptom and discomfort of life activities, just as exercise affects discomfort of life activities. Therefore it is required to examine further measures for continuous exercise.
