In general, treatment of the patients with closed lock divides into a conservative and a surgical treatment. Surgical treatment has been often applied in case that occlusal splint therapy was not effective on the patient with closed lock. In recent, some clinicians reported good results with mandibular manipulation. Three patients complained limitation of month opening-(mean mouth opening was 22.3mm) and TMJ pain. Articular discs were displaced anteriorly on MRI. Two patients didn't improve the symptoms with long term occlusal splint therapy. We applied mandibular manipulation after injection with 2% lidocaine into the upper joint space of the affected TMJ and directly inserted occlusal splint to all patients. At the follow-up check, mean mouth opening was 41.7mm. TMJ pain decreased, condyle and disc relationship was improved functionally on MRI.
Objectives : The aim of this study was to assess the relationship between metabolic syndrome and erythrocyte deform ability in acute stroke patients. Methods : Among 88 of the recruited patients, 52 were diagnosed as metabolic syndrome. We assessed their general characteristics, risk factors. We compared the assessed variables between metabolic syndrome and control group. We analyzed the relationship between metabolic syndrome and erythrocyte deform ability. We analyzed relationship between cardiovascular risk factors and erythrocyte deformability. Results : The general characteristics waist and hip circumference, waist/hip ratio were higher in metabolic syndrome group. The metabolic syndrome group was also diagnosed with hypertension, DM, and hyperlipidemia more often than the control group. The blood test metabolic syndrome group showed higher triglycerides, total lipids, fasting blood sugar, and 2 hours postprandial plasma glucose level and lower HDL-cholesterol than the control group. There were more patients diagnosed with Dampness-Phlegm in the metabolic syndrome group. There were more patients showing lower erythrocyte deform ability in the metabolic syndrome group. The plasma homocysteine level was negatively correlated with erythrocyte deform ability. Conclusion : The results reconfirmed that the risk factors are more in metabolic syndrome group. The results indicated that metabolic syndrome lead to a lower erythrocyte deform ability in small vessel disease stroke patients. The Plasma homocysteine level was negatively correlated with erythrocyte deform ability.
This descriptive correlational study was undertaken in order to investigate the relationship of family support and personality with quality of life in patients receiving radiotherapy for cervix cancer and to provide basic data to help them improve a better quality of life. The subjects for this study Were 80 out-patients undergoing a radiation therapy at C university hospital in K-city, from April, 1992 to October, 1994. The data were obtained using a convenience sampling technique. The tools used for this study were Ro's quality of life scale, Kang's family support scale and Wallston & others health locus of control scale. The collected data were analyzed by the SAS program using percentage, mean, standard deviation, pearson's correlation coefficients. The result were as follows : 1. The total average score of the quality of life of the subjects was 138.95(minimum score 121-maximum score 164), item mean score(range 1-5) was 2.95. The total average score of the family support of the subjects was 32.55 (minimum score 16-maximum score 47), item mean score(range 1-5) was 2.95 The total average score of the health locus of control of the subjects was 37.00 (minimum score 24-maximum score 49), item mean score (range 1-6) was 3.36. 2 The results of the analysis of the relationship between the quality of life scale and the health locus of control were as follows : the total average score in the quality of life of internal locus of control scale was 136.97, the total average score in the quality of life of external locus of control scale was 144.90. 3. There was a significant positive correlation between the health locus of control and the quality of life(r = 0.2927, p<0.01). The result of the analysis of the relationship between the each factor in the quality of life and health locus of control were as follows : There were significant differences between the health locus of control and emotional state factor(r=0.1514, p<0.01), economic life factor(r=0.2560, p<0.05), self-esteem factor(r=0.2289, p<0.05), physical state and function factor(r=0.1455, p<0.05), relationship with neighbors factor(r=0.0754, p<0.05), relationship with family factor (r=0.3324, p<0.01). 4. There was a significant positive correlation between the family support and the quality of life(r=0.459, p<0.001). The result of the analysis of the relationship between the each factor in the quality of life and family support were as follows : there were significant differences between family support and emotional state factor (r=0.3891, p<0.01), self-esteem factor(r=0.2661, p<0.05), relationship with family factor (r=0.4353, p<0.001).
Purpose: To investigate the relationship between radiographic parameters of the forefoot and plantar pressure in patients with hallux valgus. Materials and Methods: Plantar pressures of 21 patients with hallux valgus were examined with EMED-ST system. The data were compared with the parameters on the simple weightbearing dorsoplantar radiographs of the feet. The radiographic parameters that were measured were hallux valgus angle, 1-2 intermetatarsal angle, relative lengths of the metatarsals which were measured with the methods described by Maestro et al. and Okuda et al. Results: Statistically significant correlation was found between peak pressures under 2, 3 metatarsal heads and the relative lengths of 2, 3 metatarsals which were measured with the method described by Maestro et al. However they could explain only 13% of the pressures under the 2, 3 metatarsal heads by multivariate analysis. Conclusion: These results suggest that we cannot predict the plantar pressures under the foot with the parameters on the simple weightbearing dorsoplantar radiographs of the feet.
The study analyzes relationship between Increasing tendency of urinary stone patients and changes of food intake through statistical data during recent 10 years(1982∼1991) in Korea. In four university hospitals, the number of urinary stone patients increased 2.5 times during the period. Study statistics showed sex ratio of male and female patient as 2.1 : 1.The types of the urinary stone of each patient were broad and mostly Ca and others(85.8%), and Ca oxalate stones were 65.2%. The incidence of patients with hypercalciuria and hyperuricosuria was observed in the Ca stone patients than in normal person. The same period showed increase of total monthly household expenditure as 3.2 times, food as 2.5 times and medical expenses as 2.4 times : The food expenditure decreased relatively In the expenditure composition. Daily hood consumption (g/person) increased as 3 times in meat, 4 times in milk, 2.4 times in seaweeds. Daily total Ca nutrition consumption was undulant and became 1.1 times during the same period. Daily food supply (g/person) was increased 1.9 times in meat, 1.4 times in spinach. However, anchovy, laver, brown seaweed, herring and makerel showed undulant tendency. Relationship (correlation coefficient) between the increase of urinary stone patients and of food intake were studied as follows. In the case of food intake, the relative index was 0.923 in seaweeds, 0.865 in meat and 0.대27 In milk And In food supply, the relative index was 0.855 in purine group which constitute meat, anchovy, herring and makerel. The relative index of Ca components which constitute anchovy, laver, brown seaweed and shrimp was 0.270. In the case of nutrition intake, the relative index was medium in protein(r: 0.443) and In animal protein (r: 0.488) but was negative in Ca(r: -0.028).
Object : The aim of this study was to assess the relationship between left ventricular hypertrophy and Dampness-Phlegm diagnosis in cerebral infarction patients. Methods : Among 227 of the total recruited patients, 59 patients were diagnosed as left ventricular hypertrophy. We assessed their general characteristics, risk factors, lab findings and Korean medical diagnosis. We compared the assessed variables between left ventricular hypertrophy group and non left ventricular group. We analyzed the relationship between left ventricular hypertrophy and risk factors. And we also analyzed the relationship between left ventricular hypertrophy and dampness-phlegm diagnosis. Results : 1. The rate of left ventricular hypertrophy in female patients was larger than the rate of male patients. 2. There were more patients finally diagnosed hypertension in left ventricular hypertrophy group. 3. According to the analysis about the rate of Dampness-phlegm related Index for Pattern Identification by left ventricular hypertrophy, Sallow complexion and obesity were significantly higher in the left ventricular hypertrophy than in the non left ventricular hypertrophy group. 4. In multivariate analysis, Dampness-phlegm group showed close relationship with left ventricular hypertrophy. Conclusions : According to the analysis, significance between dampness-phlegm diagnosis diagnosed group and left ventricular hypertrophy were clarified. These results can be utilized in the future as a basic material to be used for diagnosis and management of dampness-phlegm diagnosis on cardiovascular diseases.
Glucagon regulates glucose and fat metabolism as well as being involved in the production of ketone bodies. The new antidiabetic drug, a sodium-glucose co-transporter-2 inhibitor, increases glucagon, and reduces the risk of cardiovascular death and hospitalization due to heart failure. The presence of metabolic syndrome is an important risk factor for cardiovascular diseases(CVD) in type 2 diabetes(T2DM) patients. We, thus, investigated the association between glucagon levels and metabolic syndrome in T2DM patients. This cross-sectional study involved 317 T2DM patients. Fasting and postprandial (30 min after ingestion of a standard mixed meal) glucagon levels were measured. Metabolic syndrome was defined according to the criteria of the International Diabetes Federation. A multiple regression logistic analysis was employed for statistical evaluation. A total of 219 (69%) subjects had metabolic syndrome. The fasting and postprandial glucagon levels did not differ between the group with metabolic syndrome and the group without. Postprandial glucagon levels increased significantly with the increase in the number of metabolic syndrome components, but the fasting levels did not. However, a hierarchical logistic regression analysis revealed that the postprandial glucagon levels did not contribute significantly to metabolic syndrome even after adjusting for other covariates. Fasting and postprandial glucagon levels are not associated with metabolic syndrome in T2DM patients. However, further studies are needed to investigate the relationship between glucagon and cardiovascular risk in patients with T2DM.
Background: The incidence of abdominal trauma with intra-abdominal organ injury or bowel rupture is increasing. Articles on the diagnosis, symptoms and treatment of small bowel perforation due to blunt trauma have been reported, but reports on the relationship of mortality and morbidity to clinical factors for prognosis are minimal. The purposes of this study are to evaluate the morbidity and mortality of patients with small bowel perforation after blunt abdominal trauma on the basis of clinical examination and to analyze factors associated with the prognosis for blunt abdominal trauma with small bowel perforation. Methods: The clinical data on patients with small bowel perforation due to blunt trauma who underwent emergency surgery from January 1994 to December 2009 were retrospectively analyzed. The correlation of each prognostic factor to morbidity and mortality, and the relationship among prognostic factors were analyzed. Results: A total of 83 patients met the inclusion criteria: The male was 81.9%. The mean age was 45.6 years. The mean APACHE II score was 5.75. The mean time interval between injury and surgery was 395.9 minutes. The mean surgery time was 111.1 minutes. Forty seven patients had surgery for ileal perforations, and primary closure was done for 51patients. The mean admission period was 15.3 days, and the mean fasting time was 4.5 days. There were 6 deaths (7.2%), and 25 patients suffered from complications. Conclusion: The patient's age and the APACHE II score on admission were important prognostic factors that effected a patient's progress. Especially, this study shows that the APACHE II score had effect on the operation time, admission period, the treatment period, the fasting time, the mortality rate, and the complication rate.
This study examined burdens of primary family caregivers, and family functioning of patients with cancer. In addition, the relationship between two concepts was assessed to develop nursing intervention to reduce the burdens of caregiving, and to improve family functioning. Ninety-two primary family caregivers of patients with cancer at a general hospital in Seoul participated in this study. The patients with cancer aged from 19 to 84 years with a mean age of 51 years, and sixty-one percent were male. About 30 percent of the patients suffered liver and billiary tract cancer. Fifty-six percent of the primary family caregivers were spouses of the patients and 70.7 percent were women. Primary family caregivers' burdens were assessed by the Burden Scale originally developed by Zarit (1980) and Novak & Guest(1989) and modified by Jang (1995) for use in Korea. The instrument consists of six subscales: time-dependent burden, developmental burden, physical burden, emotional burden, social burden, and financial burden. Family functioning was assessed by the Family APGAR developed by Smilkstein(1978). The results were as follows: 1. The average burden score was 86.1, indicating a moderate level of burden. The time-dependent burden scored highest followed by developmental, physical, social, financial, and emotional burdens. The mean score of family APGAR was 9.71; among subjects 82.6% were included in dysfunctional families. 2. Of the characteristics of patients, age, gender, number of admissions, and job were found to be associated with the level of burden. There was no significant difference between patient characteristics and family functioning. Of the characteristics of primary family caregivers, caregiver's perception of patient prognosis was significantly related to the level of burden, and family functioning. Caregiver's sex and age were also related to family functioning. The quality of relationship between a patient and a caregiver was significant situational factors affecting the level of burden, and family functioning. In addition, the income of family, and help from other family members were related to the level of burden. Given the results, it is essential to develop nursing intervention to reduce burden and to improve family functioning, such as support groups.
Purpose: To explore the relationship between SER (time between the start of any treatment and the end of radiation therapy) and the survival of patients with limited-stage small cell lung cancer. Materials and Methods: Between 2008 and 2013, 135 cases of limited-stage small cell lung cancer (LS-SCLC) treated with consecutively curative chemoradiotherapy were included in this retrospective analysis. In terms of SER, patients were divided into early radiotherapy group (SER<30 days, n=76) and late radiotherapy group ($SER{\geq}30$ days, n=59) with a cut-off of SER 30 days. Outcomes of the two groups were compared for overall survival. Results: For all analyzable patients, median follow-up time was 23.8 months and median overall survival time was 16.8 months. Although there was no significant differences in distant metastasis free survival between the two groups, patients in early radiotherapy group had a significantly better PFS (p=0.003) and OS (p=0.000). Conclusions: A short SER may be a good prognostic factor for LD-SCLC patients treated with concurrent chemoradiotherapy.
