Comparison of three different exercise trainings in patients with chronic neck pain: a randomized controlled study

  • Background: Neck pain is a common problem in the general population and second only to low back pain in musculoskeletal problems. The aim of this study is to compare three different types of exercise training in patients with chronic neck pain. Methods: This study was conducted on 45 patients with neck pain. Patients were divided into 3 groups: Group 1 (conventional treatment), Group 2 (conventional treatment plus deep cervical flexor training), and Group 3 (conventional treatment plus stabilization of the neck and core region). The exercise programs were applied for four weeks, three days a week. The demographic data, pain intensity (verbal numeric pain scale), posture (Reedco's posture scale), cervical range of motion ([ROM] goniometer), and disability (Neck Disability Index [NDI]) were evaluated. Results: In all groups, a significant improvement was found in terms of pain, posture, ROM, and NDI values in all groups (P < 0.001). Between the groups, analyses showed that the pain and posture improved more in Group 3, while the ROM and NDI improved more in Group 2. Conclusions: In addition to conventional treatment, applying core stabilization exercises or deep cervical flexor muscle training to patients with neck pain may be more effective in reducing pain and disability and increasing ROM than conventional treatment alone.

보행 장애인을 위한 능동형 보행훈련 시스템 개발 및 평가 (The Development and Evaluation of the Active Gait Training System for the Patients with Gait Disorder)

  • Modem concepts of gait rehabilitation after stroke favor a task-specific repetitive approach. In practice, the required physical effort of the therapists limits the realization of this approach. Therefore, a mechanized gait trainer enabling nonambulatory patients to have the repetitive practice of a gait-like movement without overstraining therapists was constructed. In this study, we developed an active gait training system for patients with gait disorder. This system provides joint movements to patients who cannot carry out an independent gait. It provides a normal stance-swing ratio of 60:40 using an eccentric configuration of two gears. Joint motions of the knee and the ankle were evaluated with using the 3D motion analysis system and compared with the results from the multi-body dynamics simulation. In addition, clinical investigations were also performed for low stroke patients during the 6-week gait training. Results from the dynamics simulation showed that joint movements of the knee and the ankle were affected by the gear size, the step length and the length of the foot plate, except the radius of curvature of the foot guide plate. Also, the 6-week gait training revealed relevant improvements of the gait ability in all low subjects. Functional ambulation category levels of subjects after training were 2 in three patients and 1 in a patient. The developed active gait trainer seems feasible as an adjunctive tool in gait rehabilitation after stroke.

균형 재활 훈련을 위한 특정 회전 움직임에서 피검자 동작 분석을 위한 깊이 센서 기반 키넥트 시스템의 정확성 및 부정확성 평가 (Evaluation of Accuracy and Inaccuracy of Depth Sensor based Kinect System for Motion Analysis in Specific Rotational Movement for Balance Rehabilitation Training)

  • The balance ability significantly decreased in the elderly because of deterioration of the neural musculature regulatory mechanisms. Several studies have investigated methods of improving balance ability using real-time systems, but it is limited by the expensive test equipment and specialized resources. Recently, Kinect system based on depth data has been applied to address these limitations. Little information about accuracy/inaccuracy of Kinect system is, however, available, particular in motion analysis for evaluation of effectiveness in rehabilitation training. Therefore, the aim of the current study was to evaluate accuracy/inaccuracy of Kinect system in specific rotational movement for balance rehabilitation training. Six healthy male adults with no musculoskeletal disorder were selected to participate in the experiment. Movements of the participants were induced by controlling the base plane of the balance training equipment in directions of AP (anterior-posterior), ML (medial-lateral), right and left diagonal direction. The dynamic motions of the subjects were measured using two Kinect depth sensor systems and a three-dimensional motion capture system with eight infrared cameras for comparative evaluation. The results of the error rate for hip and knee joint alteration of Kinect system comparison with infrared camera based motion capture system occurred smaller values in the ML direction (Hip joint: 10.9~57.3%, Knee joint: 26.0~74.8%). Therefore, the accuracy of Kinect system for measuring balance rehabilitation traning could improve by using adapted algorithm which is based on hip joint movement in medial-lateral direction.

아급성기 뇌졸중 환자의 보행동안 청각적 피드백과 기능적 전기 자극 적용이 균형, 보행 및 하지 기능에 미치는 영향 (Study on Effects of Auditory Feedback and Application of Functional Electrical Stimulation During Gait on Balance, Gait and Lower Extremity Function in Patients with Subacute Stroke)

  • PURPOSE: Examine the effects of auditory feedback and functional electrical stimulation on balance, walking ability, and lower extremity function of subacute stroke patients. METHODS: Twenty-seven subjects diagnosed with subacute stroke within six months were randomly divided into three groups: test group 1, which performed walking exercises with auditory feedback and functional electrical stimulation; test group 2, which performed walking exercises only with functional electrical stimulation; control group applied only functional electrical stimulation, with nine subjects each. RESULTS: There were significant pre- to post-intervention differences in BBS in the gait training group with auditory feedback and functional electrical stimulation treatment, and significant pre- to post-intervention differences in BBS, sit-to-stand time, and average step speed in the gait training group with functional electrical stimulation, but no statistically significant differences in between-group comparisons. CONCLUSION: Gait training with auditory feedback and functional electrical stimulation can improve the balance and gait performance in stroke patients. Therefore, in the future, gait training with auditory feedback and functional electrical stimulation therapy may be suggested as a gait rehabilitation training tool for stroke patients.

Posture Symmetry based Motion Capture System for Analysis of Lower -limbs Rehabilitation Training

  • This paper presents a motion capture based rehabilitation training system for a lower-limb paretic patient. The system evaluates the rehabilitation status of the patient by using the bend posture of the knees and the weight balance of the body. The posture of both legs is captured with a single camera using the planar mirror. The weight distribution is obtained by the Wii Balance Board. Self-occlusion problem in the tracking of the legs is resolved by using k-nearest neighbor based clustering with body symmetry and local-linearity of the posture data. To do this, we present data normalization and its symmetric property in the normalized vector space.

  • We proposed a new system for the postural balance rehabilitation training. For the purpose, we used the virtual hiking system using virtual reality technology. We evaluated the system by measuring the parameters such as path deviation, path deviation velocity, cycling time, and head movement. From our results, we verified the usefulness of virtual reality technology in rehabilitation. Our results showed that this system was effective postural balance rehabilitation training device and might be useful as the clinical equipment.

고령자를 위한 실감객체기반 인지재활 시스템의 개발과 사용성 연구 (Development and Usability of a Cognitive Rehabilitation System Based on a Tangible Object for the Elderly)

  • 목적: 본 연구의 목적은 다양한 인지기능 수준을 가진 고령자를 위한 실감객체 기반 인지재활시스템을 개발하고, 사용성을 검증하는 것이다. 방법: 사용성 연구는 시스템의 강점과 약점을 확인하기 위해 15명의 환자 집단과 4명의 작업치료사 집단에 설문조사를 실시하였으며 응답에 근거한 수정을 통해 시스템을 보완 및 개선하였다. 응답자들은 세 종류의 훈련-실행기능 훈련, 기억력 훈련, 집중력 훈련을 20분~30분간 경험해본 후, 구조화된 설문에 응답하였으며 설문의 영역은 각 훈련 영역별 콘텐츠, 중앙처리장치이자 화면 제시 매체인 태블릿 PC 및 실감객체를 포함하는 하드웨어, 시스템 이용에 따른 만족도로 구성되었다. 결과: 설문 응답 결과 환자 집단과 작업치료사 집단 모두에게 가장 흥미로웠던 영역은 실행기능으로 나타났고 가장 흥미롭지 않은 영역은 집중력으로 나타났다. 하드웨어 사용성 설문에서 태블릿 PC의 화면의 크기가 부적절하다는 응답이 6건, 실감객체의 크기가 부적절하다는 응답이 5건으로 나타났다. 시스템 이용에 대한 만족도 설문에서 전체 응답자의 40%가 시스템에 만족하는 것으로 응답하였다. 결론: 본 시스템은 훈련과제를 수행하기 위해 필요한 실감객체 도구의 조작이 용이하다는 점과 사용자의 요구에 따라 인지훈련의 영역을 쉽게 선택하여 훈련할 수 있다는 점, 사용자의 수행도에 의해 난이도가 자동으로 조정된다는 점을 특징으로 갖는다. 본 시스템에서 입력장치로 사용되는 실감객체도구는 양손에 쥐고 사용함으로써 훈련환경을 자연스러운 일상에서의 도구 사용과 유사하도록 고안하였다. 본 시스템은 지역사회 인지 재활전문가 부족의 대안으로 사용가능 할 것으로 기대된다.

중년여성의 재활트레이닝을 통한 골프운동의 효과 (The Effect of Golf Exercise through Rehabilitation Training for Middle-aged Women)

  • 본 연구의 목적은 중년여성의 재활트레이닝을 통한 골프운동의 효과를 검증하여 올바른 골프 활동을 할 수 있도록 제시하는 데 주된 목적이 있다. 이러한 연구 목적을 달성하기 위하여 2020년 2월에 경남진주에 소재한 중년여성 40-50세 대상으로 10일간 스윙궤도에 문제점이 있고 교정이 필요로 한 여성희망자에 한하여 통제에 응한 여성 8명을 대상으로 선정하고 정확한 측정검사를 위하여 연구의 목적과 활용방안에 대한 설명을 한 뒤 프로그램을 시행하였다. 실험전 체력의 수준과 비거리를 검사하고, 실험 후 체력의 수준과 비거리를 최종적으로 검사하여 수집된 데이터를 최종 분석 사용하였다. 수집된 자료의 통계처리는 SPSS win18.0 프로그램을 이용하였으며, 통계기법은 빈도분석(Frequencies)으로 평균(M)과 표준편차(sd)를 산출하였고, t-test(independent two samples t-test)와 일원배치 분산분석(One-way ANOVA)과 다중회귀분석(Multiple Regression Analysis)을 실시하였다. 이와 같은 방법과 절차를 통하여 본 연구에서 얻어진 결과는 다음과 같다. 첫째, 일반적 특성의 재활트레이닝은 골프운동에 높은 차이가 나타났다. 둘째, 재활트레이닝과 체력수준은 스윙궤도와 비거리에 높은 차이가 나타났다. 셋째, 재활트레이닝과 체력수준은 스윙궤도와 비거리에 높은 영향이 미치는 것으로 나타났다.

뇌성마비 아동의 서기 균형 훈련시 간헐적 방법과 지속적 방법에 의한 시·청각 되먹임의 효과 (The Effect of Intermittent and Continuous Visual and Auditory Feedback at Standing Balance Training in Children With Cerebral Palsy)

  • The purpose of this study was to find a more effective balance training method. The subjects of this study were 14 children with cerebral palsy (7 males, 7 females) being treated at Seran Pediatric Developmental Research Center in Taegu. Two groups of children with cerebral palsy (everyday trained group, every-other-day trained group) were evaluated with visual & auditory feedback. Evaluation and training device was Balance Performance Monitor (BPM) Dataprint Software Version 5.3. There was statistically significant difference of the balance score between the pre-and the post-training in both group (p<.05), but there was no difference of the balance score between two groups (p<.05). In conclusion, it is likely that the visual and auditory feedback in children with cerebral palsy was effective in improving standing balance, but there was no difference between everyday trained group and every-other-day trained group.
