Effect of Early Ankle Exercise with Functional Electrical Stimulation on Strength and Range of Motion of Ankle in Patients with Stroke (기능적 전기자극을 이용한 조기 발목재활운동이 급성기 뇌졸중 환자의 발목 근력과 가동범위에 미치는 영향)

  • Purpose: The purpose of this study was to investigate the effect of early ankle exercise with functional electrical stimulation(FES) on spasticity, strength and active range of motion of ankle in patients with stroke. Methods: This study included 21 patients with stroke, who were performed early ankle exercise combined FES. The exercise program comprised 5 sessions of 30 minutes per week for 4 weeks. The spasticity, strength and active range of motion of ankle were evaluated before and after training. The spasticity was measured by modified ashworth scale(MAS), strength of ankle was measured by hand-held dynamometer and active range of motion of ankle dorsi-flexion was measured by goniometer. All data were analyzed using SPSS 18.0. Results: Significant differences were observed the subjects for strength of ankle and active range of motion. The results of the study were as follow: strength of ankle was significantly increased subjects(p<.001) and active range of motion was significantly increased subjects(p<.001). Conclusion: Ankle is very important part of body in stroke patients. early ankle exercise with FES is effective for improvement of strength of ankle and active range of motion in patients with stroke. ealry ankle exercise with FES about stroke patient is very useful and effective. It is effective in clinical practice.

Case Analysis of Visiting Nursing Center for Improving Efficiencies: Based on Business Management Consulting (방문간호센터 경영효율성 개선 사례 분석: 경영 컨설팅 적용을 중심으로)

  • Purpose: This study aimed to present the management strategies necessary to improve the operational efficiency of visiting nurse centers and evaluate their effectiveness. Methods: The subjects of this study were visiting nurse centers registered as long-term care centers. Based on value chain analysis, cost information analysis, and data envelope analysis, the study was carried out according to the Magerison's management consulting procedure, for six months. This procedure comprised eight sub-steps of approach and application. Results: The following management strategies were agreed upon: establishment of a cooperative network with other visiting care centers, creation of high satisfaction of external customers by providing practical training to care workers, and making rehabilitation and exercise services as the core nursing activities to be focused on. Conclusion: The management consulting process and analysis method applied in this study can referred to as a useful methodological framework for revitalizing visiting nursing centers in the future.

Effectiveness of the Infectious Disease (COVID-19) Simulation Module Program on Nursing Students: Disaster Nursing Scenarios

  • Purpose: This study aimed to develop an emerging infectious disease (COVID-19) simulation module for nursing students and verify its effectiveness. Methods: A one-group pretest-posttest quasi-experimental study was conducted with 78 under-graduate nursing students. A simulation module was developed based on the Jeffries simulation model. It consisted of pre-simulation lectures on disaster nursing including infectious disease pandemics, practice, and debriefings with serial tests. The scenarios contained pre-hospital settings, home visits, arrival to the emergency department, and follow-up home visits for rehabilitation. Results: Disaster preparedness showed a statistically significant improvement, as did competencies in disaster nursing. Confidence in disaster nursing increased, as did willingness to participate in disaster response. However, critical thinking did not show significant differences between time points, and neither did triage scores. Conclusion: The developed simulation program targeting an infectious disease disaster positively impacts disaster preparedness, disaster nursing competency, and confidence in disaster nursing, among nursing students. Further studies are required to develop a high-fidelity module for nursing students and medical personnel. Based on the current pandemic, we suggest developing more scenarios with virtual reality simulations, as disaster simulation nursing education is required now more than ever.

Comparison of Kinematic Data during Walking between Healthy People and Persons with Mild Intellectual Disability (건강한 성인과 경미한 지적장애를 가진 성인의 보행 중 운동학적 데이터 비교)

  • Purpose: The purpose of this study was to analyze the gait patterns of adults with intellectual disability and healthy adults based on collected kinematic data on the lower extremities and to investigate the gait patterns of intellectually disabled people by comparing the differences between the two groups. Methods: The participants were divided into in one group of healthy adults (n = 9) and one group with mild intellectual disabilities (n = 9). 3D motion analysis (Myomotion) was used to collect kinematic data from each group while the participants walked 3 times over 10 m. As a statistical method, each group's kinematic data during walking was analyzed and compared using an independent sample t-test. Results: Comparing the kinematic data of the lower extremities during walking between the group with mild intellectual disability and the healthy group, there were significant differences between the two groups in the hip and ankle joints in the stance and swing phases. Conclusion: The analysis suggests that people with intellectual disabilities have kinematic differences compared with healthy people. Based on the results of this study, it is necessary to conduct further research on rehabilitation programs for joint stabilization, exercise for increasing joint range of motion, muscle strengthening exercise, and proprioception training for people with intellectual disabilities with insufficient physical function.

  • Treatment of schizophrenia has as its ultimate goals, the functional recovery of the patients and improvement of their quality of life. While antipsychotic medication is the fundamental method for treating schizophrenia, it has certain limitations in terms of treating the illness beyond its positive symptoms. Therefore, psychosocial intervention should be used in tandem with pharmacological methods in treating schizophrenia. The efficacy of several modes of psychosocial intervention for improving outcomes in schizophrenia is well attested. Approximately 10 modes of psychosocial intervention have been recommended based on existing evidence, including family intervention, cognitive behavioral therapy, supported employment, early intervention services, lifestyle intervention for physical health enhancement, treatment of comorbid substance abuse, assertive community treatment, cognitive remediation, social skills training, and peer support. Ideally, these interventions are offered to patients in combination with one another. Over the last decade, increased emphasis has been placed on early detection and intervention, with particular focus on long-term recovery. Early intervention with comprehensive psychosocial interventions should be enacted promptly from the initial detection of schizophrenia.

Correlation of isokinetic strength and angular velocity of knee during Fente motion in elite fencer (엘리트 펜싱 선수들의 팡트 동작에 따른 무릎 신전근 근력과 각가속도의 상관)

  • The purpose of this study is to identify the relation between isokinetic strength of knee extensors and angular velocity of knee joint during fente motion in elite fencers by three disciplines and gender, and to provide the preliminary data for developing the training program and rehabilitation program to enhance sports-performance by three disciplines and gender. Total 31 elite fencers who trained at the national training center of the Korean Olympic Committee (KOC) participated in this study.; 10 fleuret (4 males), 10 epee (6 males) and 11 sabre (5 males). Subjects were measured eccentric and concentric isokinetic strength (PT/BW, WK/BW) of the knee extensors at angular velocity of $60^{\circ}/s$, $180^{\circ}/s$ and $300^{\circ}/s$, and flexion (P1) and extension (P2) angular velocity of knee joint during fente motion. To examine the relationship between isokinetic strength of knee extensor and angular velocity of knee motion, Pearson's correlation coefficient (r). ${\alpha}=.05$ was considered significant. In conclusion, the relationship of isokinetic strength of knee extensors and angular velocity of knee was different depending on three disciplines and gender.

The Effect of Balance and Function in Children with Spastic Cerebral Palsy using Motor Learning training with Treadmill (트레드밀 운동학습 훈련이 경직성 뇌성마비 아동의 기능과 균형에 미치는 영향)

  • The purpose of this study was to apply treadmill training through motor learning to cerebral palsy children and examine its effects on their motor Functions and balance. The subjects of this study were 16 spastic diplegia children who had difficulty in independent gait, and GMFCS level III, IV. The participant's were allocated randomy to 2 groups: a motor learning group(n=8) and the control group(n=8), Both groups received muscle strengthening exercise for 3 session, 30 minutes per week over 7 weeks period. Data collected from the 16 spastic diplegia children the results were as follows. The motor learning group showed significant increase in motor function(p<.05). The motor learning group showed significant increase in balance(p<.05). Between motor learning group and control group, motor functions and balance was a statistically significant difference(p<.05).

Effects of Hip Strengthening Exercise on Chronic Ankle Instability (만성 발목 불안정성 요인에 대한 엉덩이 강화 운동의 효과)

  • This study examined the effects of 8-weeks hip muscle training on the hip joint abductor muscle isokinetic strength and foot pressure distribution in elite players with chronic ankle instability (N=19). A total of 19 subjects had chronic ankle instability from at least 2 ankle sprains, and were given 8-weeks hip muscle training exercise. The hip muscle strength of the elite players with chronic ankle instability increased significantly, but the foot pressure distribution and ratio of the foot pressure distribution of the elite players with chronic ankle instability did not reach statistical significance and the ratio of the foot pressure distribution showed a similar trend. These results suggest that the strength and foot pressure distribution of the affected-side might increase to that of the unaffected-side. The 8-weeks hip muscle training helped improve the chronic ankle instability of the elite players. Therefore, the hip muscle strength and foot pressure distribution are the primary factors of a rehabilitation program on ankle sprains.

Characteristics of ROM and EMG to Balance Training in Unstable Plate System: Primary Study (균형 훈련 플레이트 시스템을 이용한 생체역학적 특성 연구)

  • The purpose of this study was to investigate the unstable plate system for the advanced balance ability. 7 male volunteers (age $33.7{\pm}1.2$ years, height $174.7{\pm}3.8cm$, weight $86.0{\pm}3.6kg$, BMI $28.2{\pm}2.0kg/m^2$) performed the partial squat motion on the shape of CAP type(${\cap}$) and BOWL type(${\cup}$) plate system. The range of motion (ROM) and muscle activation were acquired by the motion analysis system and the EMG system. Results of ROMs of the CAP type plate system were shown the widely range of the deviation in the ankle joint on the sagittal plane (sagittal plane - hip joint $10.7^{\circ}$ > $5.4^{\circ}$, knee joint $16.3^{\circ}$ > $6.4^{\circ}$, ankle joint $18.8^{\circ}$ > $6.3^{\circ}$ ; transverse plane - hip joint $3.5^{\circ}$ > $1.8^{\circ}$, knee joint $5.3^{\circ}$ > $3.4^{\circ}$, ankle joint $11.3^{\circ}$ > $5.3^{\circ}$ ; frontal plane - hip joint $0.9^{\circ}$ > $0.5^{\circ}$, knee joint $0.8^{\circ}$ > $0.6^{\circ}$, ankle joint $4.8^{\circ}$ > $3.7^{\circ}$). Muscle activation results of the CAP type plate system were indicated higher in major muscles for balance performance than the BOWL type plate system (vastus lateralis 0.90 > 0.62, peroneus longus 0.49 > 0.21, biceps femoris 0.38 > 0.14, gastrocnemius 0.11 > 0.05). These findings may indicate that the CAP type plate system would expect better effectiveness in perform the balance training. This paper is primary study for developing balance skills enhancement training device.

Efficacy of a Training Program for Long-Term Disease-Free Cancer Survivors as Health Partners: A Randomized Controlled Trial in Korea

  • Background: To determine whether the Health Partner Program is effective in training long-term cancer survivors to be health coaches. Materials and Methods: We randomly assigned cancer survivors who were selected through a rigorous screening process to either the Health Partner Program or the waiting-list control group. The program consisted of 8 weeks of training in health management, leadership, and coaching. At baseline, 8, and 16 weeks, we measured primary outcomes using the Seven Habit Profile (SHP), the Korean Leadership Coaching Competency Inventory (KCCI), Ed Diner's Satisfaction with Life Scale (SWLS), and the Posttraumatic Growth inventory (PTGI) and secondary outcomes using the Hospital Anxiety and Depression Scale (HADS), the Impact of Event Scale-Revised (IES-R), and the Medical Outcomes Study (MOS) short form 36-item questionnaire (SF-36). Results: We recruited 70 subjects and randomly assigned 34 to the intervention group. The Sharpen the Saw habit of the SHP increased significantly more in intervention group than in the control group (p=0.049), as did most PTGI factors. The intervention group also showed a significantly greater enhancement of vitality (p=0.015) and mental health (p=0.049) SF-36 scores but no improvement in KCCI, SWLS, HADS, or IES-R scores. The intervention group also showed a greater clinically meaningful improvement in the "Think Win-Win" of SHP (p=0.043) and in the personal strength score (p=0.025) and total score (p=0.015) of the PTGI. Conclusions: Long-term cancer survivors can benefit from the Health Partner Program to become health coaches.
