Temporal Epidemiological Assessment of Colorectal Cancer Incidence and Mortality in East Kazakhstan, 2004-2013

  • Colorectal cancer incidence and mortality in Kazakhstan are relatively high but exact statistics have hitherto been lacking and trends over time are unclear. The present study was therefore undertaken to retrospectively assess data for East Kazakhstan, accessed from the central registration office, for the period 2004-2013. Approximate age standardized data for incidence and mortality were generated and compared across age groups, gender and year. It was determined that during the studied period 3,417 new cases of colorectal cancer were registered and 2,259 died of this pathology. Average cancer cancer incidence and mortality over the ten years were $24.1/10^5$ and $15.9/10^5$ respectively, and the overall ratio of mortality/incidence (M/I) was 0.69:1 (range 0.58-0.73). Both incidence and mortality tended to remain constant in both males and females. The male to female ratios also did not significantly vary over time but a trend for improvement of the mortality to incidence ratio was observed, especially for rectum. Whether this might be related to screening remains unclear. These preliminary data indicate that whereas colorectal cancer continues to be important, change in environmental factors are not having a great impact on incidence in East Kazakhstan.

A Korean nationwide investigation of the national trend of complex regional pain syndrome vis-à-vis age-structural transformations

  • Background: The present study employed National Health Insurance Data to explore complex regional pain syndrome (CRPS) updated epidemiology in a Korean context. Methods: A CRPS cohort for the period 2009-2016 was created based on Korean Standard Classification of Diseases codes alongside the national registry. The general CRPS incidence rate and the yearly incidence rate trend for every CRPS type were respectively the primary and secondary outcomes. Among the analyzed risk factors were age, sex, region, and hospital level for the yearly trend of the incidence rate for every CRPS. Statistical analysis was performed via the chi-square test and the linear and logistic linear regression tests. Results: Over the research period, the number of registered patients was 122,210. The general CRPS incidence rate was 15.83 per 100,000, with 19.5 for type 1 and 12.1 for type 2. The condition exhibited a declining trend according to its overall occurrence, particularly in the case of type 2 (P < 0.001). On the other hand, registration was more pervasive among type 1 compared to type 2 patients (61.7% vs. 38.3%), while both types affected female individuals to a greater extent. Regarding age, individuals older than 60 years of age were associated with the highest prevalence in both types, regardless of sex (P < 0.001). Conclusions: CRPS displayed an overall incidence of 15.83 per 100,000 in Korea and a declining trend for every age group which showed a negative association with the aging shift phenomenon.

  • 무선 네트워크를 통해 인터넷에 접속하려는 이동노드 수가 늘어남에 따라 다수의 이동노드에게 무선 서비스를 제공해야하는 HA(Hone Agent)와 FA(Foreign Agent)에서의 부하도 함께 증가하고 있다. 특히 지역적 등록(Regional Registration)에서와 같이 투선 네트워크 도메인의 최상위 FA가 현재 도메인에 로밍한 모든 이동노드의 COA로 동작함으로 인해 발생하는 부하는 최상위 FA에서 혼잡 및 지연의 원인이 된다. 이에 본 논문에서는 최상위 FA에서 발생말 수 있는 부하를 하위 FA에게 분산하기 위하여 두 가지 접근방법인 Passive Approach와 Active Approach를 제안한다 Passive Approach는 부모 FA가 부하 분산 시점을 결정하여 자식 FA에게 분산해주는 기법이고 Active Approach는 임의의 FA가 스스로 분산 시점을 결정하여 상위 FA에서 발생할 수 있는 부하를 완화시켜주는 기법이다. 시뮬레이션을 통해 Active Approach의 한 형태를 띠고 있는 기존 연구인 LNSP 기법과 성능을 비교하여 가장 효율적인 부하 분산 방법을 제시한다.

  • 이동 컴퓨팅 환경에서 사용자의 이동 특성은 실시간 멀티미디어 서비스를 제공하기 위해서 요구되는 QoS(Quality of Service)에 심각한 영향을 미친다. RSVP(Resource reservation Protocol)는 유선을 기반으로 실시간 서비스를 제공하기 위해서 제안된 프로토콜로써 이동 환경에 적용하기에는 부적합하다. MRSVP(Mobile RSVP)는 RSVP를 기반으로 사용자의 이동이 QoS에 미치는 영향을 최소화하기 위해서 사용자가 이동할 것으로 예상되는 영역에 대한 자원을 미리 예약하는 프로토콜이다. 본 논문은 기존의 MRSVP를 확장하여 Mobile IP 지역등록(Regional Registration) 프로토콜과 이웃 라우터 발견 메커니즘(Neignbor Router Discovery Mechanism)을 결합하여 동적 앵커 에이전트(DAA: Dynamic Anchor Agent)를 계층적으로 구성하는 QoS지원 방안을 제안한다. 이는 기존의 MRSVP에서 미리 예약되는 자원예약 범위를 네트워치 위상을 고려하여 지역적으로 제한함으로써 자원예약을 최소화하는 반면에 기존의 MRSVP와 유사한 수준의 QoS 보장을 목적으로 한다.

Difference between Korean and Occidental Group-specific Label-based Probabilistic Brain Atlas

  • Probabilistic atlases for the human brain structure are more suitable than single brain atlases for representing population anatomy. In this study, we hypothesized the group-specific probabilistic atlas for accurate characteristic feature coding. Our proposed method for a new group comparison study, using a subpopulation specific probabilistic atlas, was based on this hypothesis. A knowledge-based automatic labeling technique using nonlinear registration was applied to encode group-specific regional probabilistic information. Direct atlas-based comparison using volume counting above the probability threshold, distance measurement and correlation analysis were performed based on the probabilistic atlas. Here, we applied this method for comparison between Korean and occidental groups. The results showed that this method could provide simple but intuitive regions of interest-based group analysis for the entire cortex area.

  • 최근의 무선 네트워크의 급속한 발전으로 이동 컴퓨팅 분야도 IMT-2000 기술과 더불어 전세계 어디에서든지 위치에 상관없이 Mobile IP 프로토콜과 같은 이동성을 지원하는 IP 프로토콜을 통해서 인터넷에 접속할 수 있는 환경을 제공하게 될 것이다. Mobile IP 환경에서 MH(Mobile Host)는 이동하는 도중에도 인터넷에 접속을 유지하려면 자신의 care-of address가 변경될 때마다 이동한 셀의 FA(Foreign Agent)를 걸쳐서 HA(Home Agent)에게 등록하는 과정을 계속해서 수행해야 한다. 만약 MH의 FA와 HA간의 거리가 먼 경우에는 등록하는 시간이 길어질 것이며, 그만큼 인터넷에 접속되는 시간이 지연되어 인터넷 서비스가 늦어질 것이다. 본 논문에서는 MH의 FA와 HA와의 거리가 먼 경우에 지역적인 등록을 통해서 등록 지연 시간을 줄일 수 있는 동적인 지역적 등록 기법을 제안한다. 이러한 동적인 지역적 등록 기법은 MH의 등록 지연 시간을 줄이며 MH의 매끄러운 핸드오프(smooth handoff) 기능도 지원하여 핸드오프 기간동안의 패킷 전송 오류를 줄일 수 있는 기법이다.

  • 모바일 환경에서 멀티캐스트를 구현하는 방법으로 대표적인 방법으로는 원격가입(MIP-RS)와 양방향 터널링(MIP-BT)이 있으며 이의 단점을 보완한 MoM이 제안되었다. 하지만 터널링 경로가 최적으로부터 멀어지는 현상을 처리하지 못함에 따라 이를 해결하기 위해 터널링 범위에 제한을 가하는 RB MoM이 제안되었다. RB MoM의 경우 터널링 경로의 조절은 가능할 수 있었으나 이동호스트의 이동에 따른 HA 등록 등과 같은 등록메시지의 양은 줄일수는 없었다. 따라서 본 논문에서는 RB MoM의 장점인 터널링 경로 조절기능을 살리면서 이동호스트의 이동에 따른 등록메시지를 줄이고 GFA의 터널링 부하를 분산시키기 위해 멀티캐스트 터널링 범위에 제한을 둔 지역등록 기법을 제안하였다.

Late-term effects of hypofractionated chest wall and regional nodal radiotherapy with two-dimensional technique in patients with breast cancer

  • Purpose: Hypofractionated radiotherapy (RT) is becoming a new standard in postoperative treatment of patients with early stage breast cancer after breast conservation surgery. However, data on hypofractionation in patients with advanced stage disease who undergo mastectomy followed by local and regional nodal irradiation (RNI) is lacking. In this retrospective study, we report late-term effects of 3 weeks post-mastectomy hypofractionated local and RNI with two-dimensional (2D) technique in patients with stage II and III breast cancer. Methods: Between January 1990 and December 2007, 1,770 women with breast cancer who were given radical treatment with mastectomy, systemic therapy and RT at least 10 years ago were included. RT dose was 35 Gy/15 fractions/3 weeks to chest wall by two tangential fields and 40 Gy in same fractions to supraclavicular fossa (SCF) and internal mammary nodes (IMNs). SCF and IMNs dose was prescribed at dmax and 3 cm depth, respectively. Chemotherapy and hormonal therapy was given in 64% and 74% patients, respectively. Late-term toxicities were assessed with the Radiation Therapy Oncology Group (RTOG) scores and LENT-SOMA scales (the Late Effects Normal Tissue Task Force-Subjective, Objective, Management, Analytic scales). Results: Mean age was 48 years (range, 19 to 75 years). Median follow-up was 12 years (range, 10 to 27 years). Moderate/marked arm/shoulder pain was reported by 254 (14.3%) patients. Moderate/marked shoulder stiffness was reported by 219 (12.3%) patients. Moderate/marked arm edema was seen in 131 (7.4%) patients. Brachial plexopathy was not seen in any patient. Rib fractures were noted in 6 (0.3%) patients. Late cardiac and lung toxicity was seen in 29 (1.6%) and 23 (1.3%) patients, respectively. Second malignancy developed in 105 (5.9%) patients. Conclusion: RNI with 40 Gy/15 fractions/3 weeks hypofractionation with 2D technique seems safe and comparable to historical data of conventional fractionation (ClinicalTrial.gov Registration No. NCT04175821).

Mortality and Morbidity Based on Secondary Data Analysis for Respiratory System Diseases among Residents around Ansim, Daegu, Korea (호흡기계 질환 관련 이차자료에 근거한 대구 안심 지역주민의 사망 및 이환 현황)

  • Objectives: This study was conducted to evaluate the changes and regional differences of mortality and morbidity particularly respiratory system diseases in the area of exposure to coal dust(Ansim area, Dong-gu, Daegu). Methods: The authors analyzed secondary data(cancer registration data, mortality data, and health insurance data) for respiratory system diseases. We calculated age standardized incidence ratio(SIR), mortality ratio(SMR), and health care utilization ratio(SHR) using those data. Results: There were no significant differences between Ansim area(or Dong-gu, Daegu) and the control area for cancer registration data and mortality data. In the results for the health insurance data, significant increased SHR in asthma was observed compared to the control area. Conclusions: Although confounders such as selection bias were not clearly ruled out, our findings reveal increased asthma SHR in the area of exposure to coal dust. Further prospective studies are required to clarify the increasing respiratory disease due to exposure to coal dust.

Cost Effective Location Management Scheme in MIPv6 (비용 효과적인 MIPv6의 위치 관리 방안)

  • Recently MIP becomes more important for the macro mobility support in the emergence of the global system such as IMT-2000. However this Protocol suffers from many weaknesses on the location management. MIPv4 regional registration is presented for the local registration of MNs. MIPv6 can benefit from the reduced mobility signaling with external networks by employing a local MAP, that is called HMIPv6. The total signaling cost of HMIPv6 is rapidly increased by the number of CNs of an MN when the MN moves around between MAPs. In this paper, we propose a new location management scheme in MIPv6 that reduces the total signaling cost in the location update based on HMIPv6 Here the MN does not send the BU to CNs and HA when it moves around between MAPs. Instead it informs the current location by transferring the modified BU to the previous MAP. According to the results of the performance analysis, we determine the upper bound for the number of forwarding links allowed among adjacent MAPs without the BU information.
