Contrast-Enhanced Magnetic Resonance Imaging at True End-Diastole to Quantify Reproducible Transmural Extent of Myocardial Hyperenhancement

  • To determine feasibility of contrast-enhanced MRI (co-MRI) at true end-diastole (ED) free from limitation of time for inversion-recovery and trigger window for quantifying transmural extent of infarction. 대상 및 방법: MRI was performed in 18 patients with myocardial infarction. Cine imaging and co-MRI with same registered slices in short axis were peformed. To allow true ED co-MRI, ECG synchronization should use two RR-intervals for one acquisition of a segment of k-space by setting the heart rate to half that of the true heart rate. Trigger delay time was adjusted to the RR-interval for imaging at ED and to the sum of RR-interval plus the time between R-wave and the end-systole (ES) determined in cine images for imaging at ES.

양말 소재별 인체생리적 반응 및 주관적 감각평가 (Evaluation of Physiological Responses and Subjective Sensation in Different Sock Materials)

  • The purpose of this study was to determine the physiological responses and subjective sensation according to fiber content of socks. Six female students participated in the wear trial test which was conducted in controled environmental chamber with 26${\pm}$1$^{\circ}C$ and 60${\times}$3%. R.H.SAS program was used for statistical analysis. The results of this study were as follows. Mean skin temperature was significantly different among three different socks. AWNP socks had the highest mean skin temperature and instep temperature. Also relative humidity in the microclimate of socks as well as heart rate were influenced by fiber content of socks, However, test of subjective sensation such as thermal, humid, comfort sensation showed that there was no significant difference among three different socks thermal sensation during the exercise and recovery period.

Outcome of Video-assisted Thoracoscopic Surgery for Spontaneous Secondary Pneumothorax

  • Background: Conventional treatment (i.e. chest tube insertion and chemical pleurodesis) still remains standard for patients with secondary spontaneous pneumothorax because the risk of surgical bullectomy is deemed high in this subset. However, it has been suggested that surgical treatment using thoracoscopy may expedite postoperative recovery and, thus, may reduce hospital stay. Materials and Methods: Retrospective review of 61 patients with secondary spontaneous pneumothorax, who underwent conventional treatment (n=39) or video-assisted thoracoscopic surgery (VATS) (n=22) between January 2007 and December 2009, was performed. Talc was used for chemical pleurodesis in both groups. Results: Hospital stay of conventional treatment group and VATS group was $14.2{\pm}14.2$ days (4~58 days) and $10.6{\pm}5.8$ days (5~32 days), respectively, with statistically significant difference (p=0.033). Recurrence rate of conventional treatment group was also significantly higher (12/39, 30%) compared to VATS group (1/22, 4.5%) (p=0.016). Conclusion: In selected patients with secondary spontaneous pneumothorax with continuous air leak or inadequate lung expansion, thoracoscopic surgery with chemical pleurodesis using talc results in shorter hospital stay and lower recurrence rate compared to conventional approach.

Chlorpromazine 이 과분극 정지심장의 재관류 손상에 미치는 보호효과 (Protective Effect of Chlorpromazine for the Isolated Rat Heart from Reperfusion Injury)

  • This study was designed to investigate the protective effect of chlorpromazine against the reperfusion injury of myocardium after high potassium cardioplegic arrest. Langendorff`s preparations of rat heart were infused with high potassium cardioplegic solution[St. Thomas Hospital Solution] at 25oC. Chlorpromazine [10-7M] increased the recovery of myocardial contractility[dp/dt], left ventricular pressure[LVP], and coronary flow rate of the reperfused heart. Both in control and experimental groups, the restoration of myocardial activity could not reach to the level of preplegic control. These results suggest that the etiologic factors of the reperfusion injury include the influence of high potassium cardioplegic solution and/or reperfusion itself, and that chlorpromazine protects myocardium from the reperfusion injury.

사체중 MaIathion의 각 장기조직별 분석 및 정량에 관한 연구 (Study on the Accumulative Distribution of Malation and itns Determination form the Human Tissue.)

  • An experimental study was conducted to determine the quantity of contamination of organophosphrous pesticides accumulated in each human tissues. The samples used for this experiment were spleen, lung, heart, liver and kindney and those tissues were homogenized by a blender. The homogenized materials was extracted with mixed solvent, acetone/benzene (1:1) and cleaned up on a activated carbon column and determined by gas chromatography using AFID supported on 5% QF-1. The average recovery rate was 94% and the results obtained are summarized as follows. 1) The quantities of Malathin accumulated in each tissues were 0.53 ppm in spleen, 0.42 ppm in lung, 0.34 ppm in kidney, 0.19 ppm in heart and 0.19 ppm in liver. 2) Residues of pesticides in chronic or acute intoxicated tissues were highest in the spleen, decreasing in order of the lung, kidney, heart, and liver. 3) According to the above resuk we can conclude that the most proper material in detecting the pesticide is the spleen.

  • The Cox-Maze procedure was developed as a cure for atrial fibrillation. The recovery rate of both atrial contractility is reported low in the atrial fibrillation associated with mitral valvular heart disease than that of loan atrial fibrillation. We performed the Cox-Maze procedure (Maze III) in three cases who suffered from non-mitral heart diseases associated with atrial fibrillation: A ruptured sinus of Valsalva aneurysm, a ventricular septal defect, and an aortic stenoinsufficiency. The Cox-Maze procedure was performed concomitantly with correction of the underlying heart disease. Conversion to sinus rhythm was achieved in all three patients, and both right and left atrial mechanical activities could be identified echocardiographically after three postoperative months.

개에서 Xylazine 전투여가 Propofol 마취에 미치는 영향 (The Effects of Xylazine Premedication on the Propofol Anesthesia in the Dog)

  • To invstigate the available dosage and effects of xylazine as preanesthetics on the propofol anesthesia in the dog, the experimental animals were randomly divided into 3 groups (xylazine 0.55 mg/kg (group 1), 1 mg/kg (group 2) and 2 mg/kg (group 3) were premedicated) and, monitored analgesic and anesthetic effect, body temperature, respiratory rate (breaths/minute), heart rate (beats/minutes). Also, hematological and serum chemical changes were monitored. In all experimental groups, the animals were recumbent just after propofol injection and time difference was not detected. Except vomitting after xylazine injection and insignificant ataxia during recovery, no significant side effects were observed. In group 2, loss of toe-web needle prick response time was slightly longer than group 1 but the response in group 2 and group 3 were similar, In group 2 and 3, the duration of anesthesia was longer than group 1 (2 folds) but there was no difference between group 2 and 3. Recovery time was prolonged in proportion to administration dosage of xylazine. In all experimental groups, the body temperature of animals was decresed gradually according to experimental time but no significant changes were monitored. The heart rate and respiratory rate were significantly (p<0.01, p<0.05) decreased after propofol injection Hematologically, no significant changes were monitored in total leukocye numbers, total erythrocye numbers, MCV, MCH, MCHC, serum GOT and GPT values Significant changes in all groups were not observed except significant increase in BUN, total-protein and abumin values of group 3. On the basis of these result, premedication of xylazine can be helpful in decresing some side effects and the dosage of propofol. 1 mg/kg of xylazine as preanesthetics on the propofol anesthesia in the dog is considered to be available.

개의 Isoflurane 흡입마취 시 Thiopental, Propofol, Etomidate 및 Diazepam-Ketamine 병용투여의 영향 (The Effects of Thiopental, Propofol, Etomidate and Diazepam-Ketamine Combination on Isoflurane Anesthesia in Dogs)

  • This study was performed to compare propofol, thiopental, etomidate and diazepam plus ketamin as induction agents for the isoflurane anesthesia in dogs. Experimental groups were divided into low groups (thiopental group: thiopental 15 mg/kg IV, propofol group: propofol mg/kg IV, etomidate group: etomidate 1.5 mg/kg IV, DZP+KET group: diazepam 0.5 mg/kg + ketamine 5 mg/kg, IV) and each group was consisted of 4 dogs. Cardiorespiratory changes (heart rate, $SpO_2$, respiratory rate, End-tidal $CO_2$ and body temperature), blood serum chemistry values (alkaline phosphatase, alanine aminotransforase, and total protein), and recovery and walking time were measured. The end tidal carbon dioxide level was significantly increased in the thiopental group (P<0.05). Heart rate and respiratory rate higher in the DZP+KET groups. There was hypothermia in all groups and significant decrease in body temperature was showed in thiopental group (p<0.05). Mean arousal time and mean walking time were significantly longer in thiopental group (P<0.05). Cardiovascular stimulating effects were minimal in etomidate group. Etomidate provides uneventful and rapid recovery.

Verapamil [anticalcium agent]의 심근 보호작용Langendorff씨 장치하의 심근 보호 작용 (Effects of Myocardial Protection of Verapamil in Serum: Under Langendorff Apparatus in Ischemic Arrest Heart)

  • This study was evaluated the metabolic, physiologic and histologic effects of myocardial protection of verapamil[isoveratril]on isolated rat hearts to 90 minutes of ischemic arrest. Heart was perfused with a modified Kreb’s Henseleit bicarbonate buffer with glucose and arrested with retrograde coronary perfusion by glucose insulin[GI], potassium and verapamil. Mean aortic systolic pressure, heart rate, coronary flows were measured and morphologic changes were examined during working heart perfusion. Perfusion and arrest were controlled four groups subjected 60 isolated rat hearts. Four groups hearts reperfused during 40 minutes after 90 minutes global ischemia for physiologic recovery. 15 hearts of four groups were assayed to histological morphologic changes. GI treated hearts recovered less than 28% of function and changed more than 80% of mitochondria of control group. Verapamil hearts[0.2, 0.1 gm/kg] recovered more than 88% of function and permitted the maintenance of continuous cellular level of Serum Glutamic Oxalaxetate Transaminase[SGOT], but declined 28% of Phosphate Kinase[CP], GI treated heart showed widespread evidence of extensive damage of mitochondria. The damage was that interstitial huge edema are present and there was contraction band formation within the swollen cells. The verapamil and potassium group were not found morphologic change compared with control group. Their functions were shown that metabolic and physiologic action of verapamil-group lasted 20 minutes longer than potassium group.
