Implant overdenture treatment using Locator attachment system on edentulous patient (무치악 환자에서 로케이터 부착장치를 이용한 임플란트 피개의치 수복 증례)

  • Severely absorbed edentulous ridge cannot bear mechanical stress, causes undesired transformation of oral environment and makes patients difficult to adapt to dentures. Nowadays implant overdenture can be a treatment of choice in order to relieve patients' discomfort and improve stability and retention of the denture. Placement of implant on maxilla is difficult because of its bone quality and anatomic structure. It also has wide supportive tissue and convenience of border sealing, which provides sufficient support and stabilization with conventional complete denture. Mandible, on the other hand, is difficult to obtain sufficient support, retention and stabilization with conventional complete denture. Therefore, implant overdenture is recommended on mandible. Locator attachment has been improved for convenience of use and male parts of various retention enabled it to replace ball type attachment clinically. In this study, we restored maxillary arch with conventional denture, and mandibular arch with implant and tissue-supported overdenture and Locator attachment system.

Case Report of Autonomic Dysreflexia in a Pressure Sore Patient (욕창 환자에서 자율신경성 반사부전증의 경험례)

  • Purpose: Plastic surgeons are responsible for the management of spinal cord injury patients with upper and lower extremity reconstruction, pressure sore, and wounds. Derailment of autonomic nervous systems caused by injury to the spinal cord may result in fatal autonomic dysreflexia. Autonomic dysreflexia is a syndrome of massive imbalance of reflex sympathetic discharge occurring in patients with spinal cord lesion above the splanchnic outflow(T6). It is characterized by a sudden onset and severe increase in blood pressure and is potentially life threatening. The other classic symptoms are headache, chest pain, sweating, and bradycardia. In order to lower the blood pressure, it is important to remove the noxious stimulus for autonomic dysreflexia. If such symptoms last for more than 15 minutes despite conservative interventions, antihypertension drugs are recommended. Methods: In this case study, we report an autonomic dysreflexia case that developed in a 45 year-old tetraplegia patient with sacral pressure sore. When he got bladder irrigation, his blood pressure went up very high and his mentality became stuporous. He was sent to ICU for his blood pressure and mental care. ICU care made his vital sign stabilized and his mentality alert. Results: After the patient underwent proper treatment like inotropic agent, he was transferred to the general ward and his pressure sore on sacral area was coveraged with gluteus maximus myocutaneous advancement flap. Conclusion: If treatment is not effective, the patients have to undergo sudden, severe hypertension, which can cause stroke or death. To provide safe and effective care, plastic surgeons should be able to recognize and treat autonomic dysreflexia.

Efficacy of Chlorine for Reducing Bacterial Populations and Bacteriological Contamination on Carcass and Treatment Water at Different Stage of Poultry Processing (도계처리 단계별 도체와 처리수의 세균오염 및 염소처리 효과)

  • This study was carried out to assess the effect of the chlorine treatment into water for processing chicken products in each stage of slaughtering, with a special viewpoint related with reducing the viable number of microorganisms by which the water and the chicken body were contaminated. The mean bacterial number on chicken samples after picking process was log5.37$\pm$0.20~5.84$\pm$0.160CFU/$\textrm{cm}^2$. When assessed by standard plate count method, it was the higher one than any other processing stage in which eviscerating, pinning, packaging, and chilling was followed in order of the mean bacterial number. The coliform bacterial numbers on carcasses after sampling from different processing stages were log2.11$\pm$0.63~2.88$\pm$0.25MPN/$\textrm{cm}^2, which show almost similar numbers in each processing stage. But, after chilling process the number was decreased slightly. The bacterial counts in the water for scalding and chilling showed log3.43 $\pm$ 0.59~5.06$\pm$0.21 and log4.30$\pm$0.21~6.62$\pm$0.33CFU/$m\ell$, respectively. In the coliform counts for the water taken out from the 2nd chilling tank, the number was log1.97$\pm$0.35~2.91$\pm$0.22MPN/$m\ell$ which showed higher than those of the 1st and the 3rd chilling tank water. The effect of chlorination in reducing the bacterial numbers was accepted at the residual chlorine concentration of 1$m\ell$/$\ell$by showing the reduction from $10^8$ to $10^4$CFU level and the numbers were decreased less than 10CFU at the concentration of 5mg/$\ell$, when assessed by viable cell counts. In conclusion, these results suggested that chlorination In chilling water with final concentration of 5mg/$\ell$was strongly recommended to reduce the bacterial numbers on final chicken products.

  • Temporomandibular joint ankylosis is the movemental obstacle of mandible which depend on proliferation of bony or fibrous tissue in temporomandibular joint structure by various causes. In order to treat this, various surgical methods have been performed, but no operative methods have been produced consistently successful results. This research has been performed to the patients who had been operated due to temporomandibular joint ankylosis by studying classification, cause, onset, duration, anesthesia and treatment method, symptom, change of mouth opening, complication through medical record, X-ray, follow-up for being a help to proper selection of treatment method and evaluation of prognosis. The author obtained the following results by analyzing 44 cases among patients who had been operated due to temporomandibular joint ankylosis during 8 year hospitalization from 1986 to 1993 in Dept. of Oral & Maxillofacial Surgery of Seoul National University Hospital. 1. The occurrence was in the order of below 10, 20's, 10's, 30's. The average of occurrence was 12.95. Illness period was 50.0% within 10 years and 50% beyond 10 years. The average period of illness was 13.33 years. 2. Trauma occupied 54.5% of causes and inflammation occupied 45.5%. Men had more occurrences due to trauma and there was no difference in case of inflammation. 3. In nasotracheal intubations for general anesthesia, the cases of using fiberoptic laryngoscope occupied 40.9%, direct or blind nasotracheal intubation occupied 40.9% and the cases of using tracheostomy occupied 18.2%. 4. In operative approaching methods, submandibular & preauricular approach were mainly applied, and in operative methods, high condylectomy(Group I) occupied 11.4%, arthroplasty without interpositional material following condylectomy or gap ostectomy(Group II) occupied 11.4%, with interpositional material following high condylectomy (Group III) occupied 40.9%, and using condylar reconstruction following condylectomy or gap ostectomy(Group IV) occupied 36.6%. 5. In change of mouth opening reformed after surgery, Group III showed the best result of average 23.5mm, Group IV showed 16.3mm, Group I showed 14.9mm and Group II showed 10.2mm of reformation. Summarizing the results as written above, it is considered that early treatment is important as soon as possible in Temporomandibular joint ankylosis. It is recommended in surgical method what can lead to postoperative early movement maintaining anatomaical & functional form, and then the development of various surgical methods will be requested.

Growth of Plant and Changes in Phosphorus Availability in Phosphorus Accumulated Soils (인 축적 토양에서 작물의 생장과 토양 유효인 함량의 경시적 변화)

  • Pot experiments were conducted to evaluate the phosphorus (P) availability with cropping in soils where P were highly accumulated. Bray 1-P contents of the used three soils were 584, 695 and $1043P\;mg\;kg^{-1}$, respectively. Corn ${\rightarrow}$ Chinese cabbage ${\rightarrow}$ Chinese cabbage ${\rightarrow}$ corn were sequentially grown from 1996 to 1998. P fertilizer was applied at three levels of 0 (P0), recommended application rate (P1), and double of recommended application rate (P2). At the end of each crop growth, available P content was determined by methods of Bray 1-P, Olsen-P and Lancaster-P. The growth of crops were not significantly affected by the rates of P applications. The relative yields of PO treatment were more than 88% of P1 treatment. The recoveries of added phosphorus were relatively low due to the high content of available phosphorus in soils. Although available phosphorus contents decreased through cropping, the concentration of soil available phosphorus was maintained high level even after the final cropping. In the case of P0 treatment in the three soils, the residual concentration was in the range of $410{\sim}610mg\;kg^{-1}$ for Bray 1-P, $284{\sim}410mg\;kg^{-1}$ for Olsen-P and $368{\sim}524mg\;kg^{-1}$ for Lancaster-P. The amount of soil available phosphorus decreased during the experiments was linearly regressed with high significance to the amount of P taken up by crops. The regressions of soil 1 as follow, Bray 1-P : y=149.7x=102.7, Lancaster-P : y=209.2x-140.2, Olsen-P: y=60.8x=19.9. The decrease rate of available phosphorus in the P0 treatment with cropping was described by an equation of first-order chemical reaction. The equation of soil1 was as follow: Bray 1-P: In(C)= -0.12N + 6.96 r=-0.991, Lancaster-P: In(C)= -0.14N = 6.88 r= -0.938, Olsen-P: In(C)= -0.07N = 6.37 r= -0.959. The rate constants seemed to be affected by ply, sand and silt content, and exchangeable $Ca^{2+}$ concentration. The times of cropping needed to reduce the content of available P to half of the initial or to the index level could be predicted by using that equation.

Study on the effect of different temperatures on the main productive characters of the silkworm larvae, Bombyx mori under Egyptian agro-climatic conditions (에집트 기후조건 특히 온도의 변화가 원원잠품종의 주요 유전형질에 미치는 영향)

  • Eleven inbred lines of silkworm, Bombyx mori L. were exposed to two main different temperature (23C and 25C) during the larval period in order to elucidate the effect of the temperature differences on their main characters under the, prevailing in Egyptial agro-climactic conditions. The results show that the temperature differences did not affect significantly the pupation ratio and cocoon shell ratio. However, they affected their larval duration to become shorter with the high temperature treatment ($27^{\circ}C$) and their cocoon shell weight and cocoon yield per box of silkworm eggs to be higher in the lower temperature($23^{\circ}C$). It was recommended that some inbred lines should be bred at the 23C throughout the whole larval period, particularly in being reared with high nutritional leaves in order to maximize the hybridization of cocoon yields.

The Stress of Patients in the Coronary Care Unit (심장내과 중한자실 입원 환자의 스트레스)

  • Purpose: This study was conducted to investigate degree of stress level in CCU patients, and to give a baseline data in developing nursing intervention program to reduce the stress. Method: Subjects of this study were the 100 hospitalized patients of C University Hospital in Seoul City from February to December 2003. The collected data was analyzed by using the SAS program: t-test, Scheff$\acute{e}$ test, and ANOVA analysis were employed to find differences in stress level. Result: The highest stress factor was 'Concern', and next were 'Dependency & limitation', 'Uncertainty', 'Treatment procedure', 'Sleep disturbance', 'Environment' in order. The perceived stress level of CCU patients was 1.65 scores; the highest scored item was 'have to void urine and feces on the bed', next were 'have to use the toilet bowel lying on the bed' in order. The stress level according to characteristics of the subjects was significantly different with the marital status, the presence of religion, the hospitalized days, the economic status, and the admission experience. Conclusion: The stress level of patients in the coronary care unit was high in concern of a disease, family and economic state, in limitation and dependency, and in uncertainty about prognosis of a disease. Further researches are necessary to develop and apply the intervention method of reducing stress level of CCU patients. In addition studies on family members of CCU patients are also recommended.

CEPHALOMETRIC AND NASOPHARYNGEAL ENDOSCOPIC STUDY IN PATIENTS WITH OBSTRUCTIVE SLEEP APNEA (폐쇄성 수면 무호흡증 환자에 있어서 두부방사선 계측 분석 및 인후 내시경적 연구)

  • The pathomechanism of obstructive sleep apnea(OSA) is not clearly elucidated. The possible mechanisms are pathologic reduction of pharyngeal muscular tonus during sleep, abnormal anatomical stenosis of nasopharyx or a combination of the above two mechanisms. It is very important to find the cause(anatomical location or pathologic dynamic change) of OSA in order to treat it. Cephalometric analysis in patients with obstructive sleep apnea is a good method for evaluating anatomical morphologic change but it cannot give any information about the dynamic changes occurring during sleep. On the contrary, nasopharyngeal endoscopy offer 3 dimensional image and information about the dynamic changes. Accordingly, these two diagnostic tools can be utilize in the diagnosis and treatment planning of OSA Cephalometric analysis of craniofacial skeletal and soft tissue morphology in 53 patients with OSA and 43 controls was performed and cephalometric analysis and nasopharygeal endoscopy were performed in 9 patients with OSA in order to come up with individualized therapy plans. Following results were obtained ; Patients with OSA showed 1. body weight gain 2. clockwise mandibular rotation 3. increased anterior lower facial height 4. inferiorly positioned hyoid bone 5. increased length of soft palate 6. decreased sagittal dimension of nasopharyx 7. increased vertical length of inferior collapsable nasopharyx 8. increased length of tongue Through cephalometric analysis and nasopharygeal endoscopy(mutually cooperative in diagnosis), 9. one can find the possible origin of OSA and make a adequate individualized therapy plan and predict accurate prognosis. Cephalometric analysis and nasopharygeal endoscopy are highly recommended as a diagnostic aid in OSA patients

Study on Simple Repetitive Work and the Risks of Musculoskeletal Injuries (단순반복 작업유형별 근골격계 질환 발생특성과 예방 대책에 관한 연구)

  • According to the results of this study, we found through preliminary research that most of the workers studied suffered from some sort of musculoskeletal injury, but did not obtain hospital treatment, which shows that although their work was the source of injury, they did not realize the significance of their injury. In addition, there were some subjects that did not even know what musculoskeletal injuries were and in order to solve such problems, companies need to educate their workers about musculoskeletal injuries and show the significance of the injuries. Also, there was a difference in rates of musculoskeletal injuries in different occupations, with higher rates of injury in esthetics, driving and packaging positions. This calls for the use of automated machinery and alteration of the working environment to make it more ergonomic according to the recommended improvement plan, and continuous effort and support on the part of the company in order to reduce the risk of musculoskeletal injuries.

Effects of Planting Density on Growth Characteristics, Dry Matter Yield and Feed Value of Teosinte New Variety, "Geukdong 6" [Zea mays L. subsp. mexicana (Schrad.) H. H. lltis]

  • This study was carried out to investigate the effects of planting densities on the growth characteristics, dry matter yield, and feed value of "Geukdong 6" (a new variety of corn for feed). The experimental design was arranged in a randomized block design with three replications. Treatments consisted of six planting densities, 60 cm × 25 cm (T1), 60 cm × 30 cm (T2), 70 cm × 25 cm (T3), 70 cm × 30 cm (T4), 80 cm × 25 cm (T5) and 80 cm × 30 cm (T6). All treatments were sown on May 14, 2021, and the harvest was on October 3 (late flowering). Plant length and the number of tillers were the highest in T5 (p<0.05), but the number of leaves and stem diameter were the highest in T6 than in the other treatments (p<0.05). Leaf length, leaf width, and dead leaf were not significantly different among the treatments. Organic matter was highest in T6, and crude protein was highest in T5 (p<0.05). The ether extract was not significantly different among the treatments. Crude fiber, NDF, and ADF were highest in T2 with relatively higher planting density (p<0.05). Calcium and phosphorus were not significantly different among the treatments. TDN content was the highest in T3 (p<0.05). Sugar degree (Brix), fructose, glucose, dextran, isomerose, and inverted sugar were not significantly different among the treatment. Fresh yield, dry matter yield and TDN yield were higher in order of T6 > T5 > T4 > T3 > T2 > T1 (p<0.05). Relatively feed value was higher in order of T3 > T6 > T5 > T1 > T4 > T2 (p<0.05). Based on the above results, planting density could be recommended from 80 cm × 30 cm for efficient production of "Geukdong 6".
