Major Reforms and Issues of the Medical Licensing Examination Systems in Korea (의사면허 필기시험 제도의 성과와 과제)

  • Since the establishment of the national medical licensing examination board in 1992, the medical licensing examination system has changed enormously and this has had a number of impacts on examination services. All those reforms were aimed at improving the relevance and reliability of the test. Several attempts of the testing system have appeared in the new examination service, and which have also brought about the changes in the medical school curriculum such as introducing integrated courses instead of traditional subjects, using test scores as a reference to the post-graduation selection test. Some examples of changes in the examination system are as follows: 1) choosing three integrated test subjects and outlines of their reference content instead of 15 academic subjects, 2) adjusting the ratio of multiple choice question items to focus more on the problem solving level, 3) introduction of 'one-best answer' single set and 'extended matching type items, 4) item construction based on real clinical cases and real clinical materials. Recently, a clinical skill test system has been introduced to measure examinees' basic clinical skills competencies. Despite continuing efforts, the examination system still has many issues remaining to be solved. These problems include the differential weighting of test items, appropriate threshold for passing, and practicality of pre-testing to stabilize the passing rate and avoid the hazards of newness and undesirably difficult test items.

Comparative Volume Measuring Methods According to the Tumor Characters in PET/CT (PET/CT 검사에서 종양의 특성에 따른 체적 측정 방법 비교)

  • Purpose Recent retrospective studies are being actively conducted to analyze the survival of patients with SUVmax, MTV, TLG, such as information from a variety of PET originating. However, there is no clear way is difficult to accurately measure the volume of the tumor may be the difference between the caster is raised. In this study, to evaluate compare the volume measuring methods according to the characteristics of the tumor. Materials and Methods 18F-saline to fill the NEMA IEC Body Phantom insert the volume of balance and imbalance in phantom were acquired to the Biograph truepoint 40 (Siemens medical system, Germany) PET/CT scanner. The ratio of the volume and Background was acquired as 3.0, 5.0, 8.0, 18, 40. Clinical patients were randomly selected 120 people in staging patients with cancer of the digestive system from the year 2010 until the year 2014. Measurement methods were used a 40% threshold, 50% threshold and gradient segmentation technique, i.e. PET EDGE. Five years of experience of the two radio-technologist and one doctor was measured by repeated three times. Analysis methods were Intraclass correlation coefficient and Pearson correlation. Results In Phantoms, the 40% threshold method gave the best concordance between measured and actual volumes (r = 0.992, 0.997). In clinical patient outcome agreement between observers EDGE it is as high as 0.999 (CI: 0.998-0.999). And there were no statistical significance of the difference between the measurements (P = 0.620). 40% threshold method showed the best correlation between the measurements (r = 0.953). Increasing the ratio of tumor to background decreased the influence of a measuring method. Conclusion How to measure volume of the tumor in the patient was clinically most useful is 50% and the lowest impact on the characteristics of the tumor. Therefore, to reduce the background of the patients in PET/CT scan, it should be required research and effort.

  • In this study, CT specimens were prepared hem ASTM SA516 which was used for pressure vessel plates for room and low temperature service. And we got the following characteristics from fatigue crack growth test carried out in the environment of room and low temperature at $25^{\circ}C$, -3$0^{\circ}C$, -6$0^{\circ}C$, -8$0^{\circ}C$, -l$0^{\circ}C$ and -l2$0^{\circ}C$ and in the range of stress ratio of 0.1, 0.3 by means of opening mode displacement. At the constant stress ratio, the threshold stress intensity factor range ΔK$_{th}$ in the early stage of fatigue crack growth ( Region I ) and stress intensity factor range ΔK in the stable of fatigue crack growth ( Region II) was increased in proportion to descend temperature. It assumed that the fatigue resistance characteristics and fracture strength at low temperature is considerable higher than that of room temperature in the early stage and stable of fatigue crack growth region. The straight line slope relation of logarithm da/dN - ΔK in Region II, that is, the fatigue crack growth exponent m increased with descending temperature at the constant stress ratio. It assumed that the fatigue crack growth rate da/dN is rapid in proportion to descend temperature in Region H and the cryogenic-brittleness greatly affect a material with decreasing temperature.e.greatly affect a material with decreasing temperature.

Hearing Ability of Sharp Toothed Eel Muraenesox cinereus caught in the Southern Korean Waters (남해 연안에서 어획된 갯장어의 청각 능력)

  • The auditory thresholds for 7 specimens of the sharp toothed eel Muraensox cinerus were measured at 5 frequencies by heartbeat conditioning method using pure tones coupled with a delayed electric shock. The audible range of the sharp toothed eel extended from 80Hz to 300Hz with the best sensitivity around 80Hz and 100Hz. In addition, the auditory thresholds over 200Hz increased rapidly. The mean auditory thresholds of the sharp toothed eel at the test frequencies of 80Hz, 100Hz, 200Hz and 300Hz were 87dB, 86dB, 105dB and 126dB, respectively. Auditory masking was determined for the sharp toothed eel by using masking stimuli with the spectrum level range of about 70~80dB (0dB re $1{\mu}Pa/\sqrt{Hz}$). According to white noise level, the auditory thresholds increased as compared with thresholds in a quiet background noise. The noise spectrum level at the start of masking was distributed at the range of about 64dB within 80~100Hz. Critical ratio ranged from minimum 24dB to maximum 40dB at test frequencies of 80Hz~200Hz.

The Evaluation on In-Situ Adaptability of Mono-layer Landfill Final Cover System (단층형 매립지 최종복토시스템의 현장 적용성 평가)

  • The mono-layer cover system is composed of soils only as a filling material and various plants are planted on the surface to control the water balance in the cover system. In this paper, the mono-layer cover system was considered as an alternative landfill final cover system and developed a model that could utilize industrial by-product (especially, coal ash & phosphogypsum) as additive filling materials. The mixture of granite soil, coal ash, and phosphogypsum was placed as a cover material in a box constructed with cement. Laboratory tests were carried out to investigate the environmental effect on the utilization of coal ash & phosphogypsum and to determine the mxing ratio of each materials. In the leaching test, all materials showed lower heavy metal concentration than the threshold values of regulation. The optimum mixing ratio of materials which was applied to field model test was determined to soil (4) : coal ash (1) : phosphogypsum (1) on the volume base. Field model tests were continued from February to July, 2004 in the soil box that was constructed with cement block. It was verified that coal ash and phospogypsum mixed with soil was to be safe environmentally and the water balance of mono-layer cover system was reasonable.

  • Pulse electric field induced electron emission from ferroelectrics has been studied with Pb$(Zr_xTi_{1-x})O_3$ ceramics with varying Zr/Ti ratio from 35/65 to 65/35, Electron emission was proved to be concentrated on the electrode edge by emission profile test and emission capture photographs. The 65/35 composition showed largest emission charge in lowest field and lowest emission threshold field. The emission characteristics are closely dependent on their ferroelectric properties in hysteresis curve. Electron emission charge increases with the polarization charge and emission threshold voltage is dependent on coercive field regardless of their composition. But dielectric constant has little relation with emission property. Electron emission charge increases exponentially with pulse electric field irrespective of composition. On the assumption that the surface potential is linear with the pulse electric field, electron emission can be regarded as a field emission at the electrode edge using Fowler-Nordheim plot of ln$(Q_e/E_{fe})$ to $1/E_{fe}$.

Effect of Latex on Corrosion Resistance of Steel Rebar in Concrete (콘크리트 내에서 보강철근의 부식저항성에 미치는 라텍스의 효과)

  • This study were evaluated the corrosion threshold reached at steel reinforcement in latex modified concrete(LMC). Accelerated testing was accomplished to the evalate the diffusion coefficient of LMC mix, and the time dependent constants of diffusion. Also, average chloride diffusion coefficient was estimated. From the average chloride ion diffusion coefficient, the time which critical chloride contents at depth of reinforcement steel was estimated. Test results indicated that the corrosion threshold reached at reinforcement in LMC are effected on the mix proportion factor including latex content, and water-cement ratio.

  • In this paper, we apply a likelihood ratio test (LRT) to a non-negative matrix factorization (NMF) based voice activity detection (VAD) to find optimal threshold. In our approach, the NMF based VAD is expressed as Euclidean distance between noise basis vector and input basis vector which are extracted through NMF. The optimal threshold each of noise environments depend on NMF results distribution in noise region which is estimated statistical model-based VAD. According to the experimental results, the proposed approach is found to be effective for statistical model-based VAD using LRT.

  • GPS(Global Positioning System) signal structure uses spread spectrum and the received power is relatively lower than the receiver noise figure. Therefore, it is vulnerable to the RF interferences and it could restrict on the safety navigation. The objective of this paper is to research on the spectral domain GPS interference rejection algorithm using proposed K-median filtering threshold setting method. In the performance test, the proposed algorithm has a relatively higher ISR(interference to signal ratio) compared with the conventional temporal domain technique in case of time variant interference signals.

  • Voice activity detectors (VAD) are important in wireless communication and speech signal processing, In the conventional VAD methods, an expression for the likelihood ratio test (LRT) based on statistical models is derived in discrete Fourier transform (DFT) domain, Then, speech or noise is decided by comparing the value of the expression with a threshold, This paper presents a new statistical VAD method based on a signal subspace approach, The probabilistic principal component analysis (PPCA) is employed to obtain a signal subspace model that incorporates probabilistic model of noisy signal to the signal subspace method, The proposed approach provides a novel decision rule based on LRT in the signal subspace domain, Experimental results show that the proposed signal subspace model based VAD method outperforms those based on the widely used Gaussian distribution in DFT domain.
