Morphological variation of the velum in children and adults using magnetic resonance imaging

  • Purpose: The purpose of this study was to investigate variations in velar shape according to age, sex, and race using magnetic resonance imaging (MRI). Materials and Methods: The study sample consisted of 170 participants (85 children, 85 adults) between 4 and 34 years of age. Velar morphology was visually classified using midsagittal MRI scans for each participant by 2 independent raters. Inter- and intra-rater reliability was assessed. Statistical analyses were performed to identify correlations of velar shape with sex, age, and race. Results: The most frequent velar shape was "butt" for both adults(41%) and children (58%) in this study. The least common shapes for adults were "leaf" and "S." The children did not exhibit any "leaf" or "straight" velar shapes. A statistically significant difference was noted for age with respect to velar shape (P=0.014). Sex and race were found to have no significant impact on velar shape in this study. Conclusion: When using MRI to evaluate velar morphology, the "butt" shape was most common in both children and adults. Velar shape varied significantly with age, while race and sex did not have a significant impact.

요양시설 노인을 위한 구강건강 평가 도구의 타당도와 신뢰도 (Validity and Reliability of an Oral Health Assessment Tool for Elderly in Long-Term Care Facilities)

  • Background: The elderly in long-term care facilities tend to have a diminished oral health status, with a high prevalence of dental caries and periodontal disease, as reduced cognitive function, joint mobility, and gait ability hinder the performance of oral hygiene. To improve the oral health of the elderly at long-term care facilities, it is necessary to have precise guidelines for oral health care and assessment; however, such guidelines are not readily available. Therefore, the present study aimed to develop an oral care assessment tool with verified reliability and validity. Methods: The participants in this study were 100 elderly patients at a care facilities and 10 clinical dental hygienists. Collected data were analyzed using the descriptive statistics, content validity index, and inter-rater reliability, as well as the analyses of intra-class correlation coefficient. Results: After a review of relevant literature, a preliminary questionnaire comprised of seven questions related to the evaluation of oral health was formed. After revising and supplementing the questions through a content validity test, a total of nine questions were selected. Conclusion: The novel assessment tool developed for the present study is anticipated to allow analyses of the level of problems related to oral health care before routine and professional care. Moreover, regular oral health status check-ups will enable the early diagnosis and treatment of diseases.

Predicting CEFR Levels in L2 Oral Speech, Based on Lexical and Syntactic Complexity

  • With the wide spread of the Common European Framework of Reference (CEFR) scales, many studies attempt to apply them in routine teaching and rater training, while more evidence regarding criterial features at different CEFR levels are still urgently needed. The current study aims to explore complexity features that distinguish and predict CEFR proficiency levels in oral performance. Using a quantitative/corpus-based approach, this research analyzed lexical and syntactic complexity features over 80 transcriptions (includes A1, A2, B1 CEFR levels, and native speakers), based on an interview test, Standard Speaking Test (SST). ANOVA and correlation analysis were conducted to exclude insignificant complexity indices before the discriminant analysis. In the result, distinctive differences in complexity between CEFR speaking levels were observed, and with a combination of six major complexity features as predictors, 78.8% of the oral transcriptions were classified into the appropriate CEFR proficiency levels. It further confirms the possibility of predicting CEFR level of L2 learners based on their objective linguistic features. This study can be helpful as an empirical reference in language pedagogy, especially for L2 learners' self-assessment and teachers' prediction of students' proficiency levels. Also, it offers implications for the validation of the rating criteria, and improvement of rating system.

Perception of discrepancy in the upper midline position in conjunction with the gingival display according to various occupations in Iran

  • Objective: This study evaluated the influence of various gingival displays on the esthetic perception in the presence of upper dental midline discrepancy. Methods: A smiling image of a male subject was altered digitally to produce five image series: normal smile (series A), decreased tooth show (series B), increased gingival show (series C), maxillary cant (series D), and asymmetric upper lip elevation (series E). In each image series, the midline was deviated to the right and left incrementally. A total of 210 raters (four professional groups and laypersons, n = 42 in each group) determined the midline deviation threshold and the attractiveness of midline position in each series. Results: The right and left thresholds were statistically similar for the symmetrical series (A, B, and C), while for series D, the right threshold was significantly lower. In most rater groups, the mean threshold order was: B > A > E > C > D. In all the series, the raters selected the coincident midline as the most attractive series except for series D, for which 1-2-mm deviations to the left were selected as the most attractive by almost all the groups. Conclusions: It is crucial to establish the coincident midline position in a symmetrical smile, especially when a gummy smile exists. In the asymmetrical gingival show, a coincident midline might not be the most esthetic midline position.

뇌졸중 환자에서 '환자 주도적 총체적 영양사정' 도구의 신뢰도 및 타당도 평가 (The Reliability and Validity of Patient-Generated Subjective Global Assessment (PG-SGA) in Stroke Patients)

  • Purpose: This study was to examine the reliability and validity of Patient-Generated Subjective Global Assessment (PG-SGA) as a nutritional measurement for stroke patients. Methods: This was a methodological study performed from May 6 to June 10, 2009 at a tertiary university hospital in Seoul. For reliability of PG-SGA, inter-rater reliability was used for statistics. For concurrent validity, BMI and biomarkers were compared between PG-SGA 0 ~ 8 and ${\geq}$ 9. In addition, sensitivity, specificity, and predictive value of PG-SGA compared with SGA were calculated using a contingency table. For predictive validity, hospital day, complications, and readmission within 1-month after discharge were compared between PG-SGA 0 ~ 8 and ${\geq}$ 9. Results: Correlation of PG-SGA score between two observers was 0.83, and kappa value for the agreement of severe malnutrition was 0.78(all $p_s$ < .001). The scored PG-SGA showed high sensitivity and specificity (100% and 96.7%, respectively). Severe undernourished patients (PG-SGA ${\geq}$ 9) had significantly low TLC, protein, albumin, and prealbumin (all $p_s$ < .01) compared with non-undernourished patients (PG-SGA 0 ~ 8). Also, in severe undernourished patients, complications and readmission (all $p_s$ = 0.01) were more often represented, and hospital days (p = .013) were significantly delayed. Conclusion: PG-SGA is a reliable and valid measurement to assess nutritional status for stroke patients.

Reliability of Measured Popliteal Angle by Traditional and Stabilized Active-Knee-Extension Test

  • The active-knee-extension (AKE) test has been used to measure hamstring muscle length. The traditional AKE test measures the popliteal angle to the point of resistance with a 90-degree flexion of the hip fixed by straps, while the stabilized AKE test measures the popliteal angle to the point of resistance with a 90-degree flexion of the hip stabilized using a pressure biofeedback unit providing lumbopelvic stabilization. The purpose of this study was to determine test-retest reliability of the traditional AKE test and stabilized AKE test. Twenty healthy adults participated in the study. The popliteal angles were measured with a digital inclinometer during each test. To assess the test-retest reliability between the 2 test sessions, intraclass correlation coefficients (ICCs) were calculated. The intrasubject coefficient of variation ($CV_{intra}$) was also calculated. To compare the traditional and stabilized AKE tests for changes in pressure, paired t-tests were applied. The results of this study were as follows: 1) ICCs(3,1) value for test-retest reliability was .96 in the traditional AKE test, and was .98 in the stabilized AKE test. 2) The maximal $CV_{intra}$ was 33.7% in the traditional AKE test and 15.7% in the stabilized AKE test. 3) Differences of $6.1{\pm}2.1$ mmHg in pressure were measured in the traditional AKE test, and differences of $1.2{\pm}1.0$ mmHg in pressure were measured in the stabilized AKE test. The results show the traditional and stabilized AKE test to be highly reliable, with test-retest reliability. However, the stabilized AKE test represented less variation and more stabilization than the traditional AKE test. Further study is needed to measure the inter-rater reliability of the stabilized AKE test for generalization and clinical application.

Reliability and Validity of the Postural Balance Application Program Using the Movement Accelerometer Principles in Healthy Young Adults

  • The purpose of this study was to determine the reliability and validity of the postural balance program which uses the movement accelerating field principles of posture balance training and evaluation equipment and smartphone movement accelerometer program (SMAP) in healthy young adults. A total of 34 people were appointed as the subject among the healthy young adults. By using Biodex stability system (BSS) and SMAP on the subject, the posture balance capability was evaluated. For the test-retest reliability, SMAP showed the intra-class correlation (ICC: .62~.91) and standard error measurement (SEM: .01~.08). BSS showed the moderate to high reliability of ICC (.88~.93) and SEM (.02~.20). In the reliability of inter-rater, ICC (.59~.73) as to SMAP, showed the reliability of moderate in eyes open stability all (EOSA), eyes open stability anterior posterior (EOSAP), eyes open stability medial lateral (EOSML) and eyes open dinamic all (EODA), eyes open danamic anterior posterior (EODAP), and eyes open danamic medial lateral (EODML). However, ICC showed reliability which was as low as .59 less than in other movements. In addition, BSS showed the reliability of high as ICC (.70~.75). It showed reliability which was as low as ICC (.59 less than) in other movements. In correlation to the balance by attitudes between SMAP and BSS, EOSML (r=.62), EODA (r=.75), EODML (r=.72), ECDAP (r=.64), and ECDML (r=.69) shown differ significantly (p<.05). However, the correlation noted in other movements did not differ significantly. Therefore, SMAP and BSS can be usefully used in the posture balance assessment of the static and dynamic condition with eyes opened and closed.

Home exercise program adherence strategies in vestibular rehabilitation: a systematic review

  • Objective: The aim of this systematic review was to investigate for effective strategies to improve home exercise program (HEP) adherence in vestibular rehabilitation (VR). Design: Systematic review. Methods: A systematic review was conducted to identify effective strategies used to improve HEP adherence of patients in VR. Six databases, Academic Search Premier, Cochrane Library, CINAHL, PUBMED, PsycINFO, and Web of Science were searched from their inception to December 31, 2015. The keywords used for search were 'home program', 'home intervention', 'compliance', 'adherence', 'vestibular rehabilitation', 'motion sickness', and 'motion sensitivity'. Results: A total of eight studies were selected to be included in the review. There was 95.2% agreement between the two reviewers who reviewed the studies using a quality assessment tool. The overall inter-rater agreement (${\kappa}$=0.73) showed good agreement between the reviewers. Strong evidence was identified for 3 major categories of effective HEP adherence strategies, 1) providing patient with written summary of HEP; 2) asking patient to maintain a record of HEP and symptoms; and 3) providing tele-rehabilitation in form of email and/or telephone support along with in person treatment sessions. Also, based on strong evidence, computerized technology was not found to be superior to other strategies for improving patients' HEP adherence in VR. Conclusions: The effective strategies for improving HEP in VR include written summary of exercise, maintenance of log of HEP and symptoms and tele-rehabilitation along with in person treatment sessions.

아동학회지에 게재된 측정도구 개발 및 타당화 연구 고찰: 1984-2016년 (A Review of Published Instruments for Children and their Families in the Korean Journal of Child Studies)

  • Objective: This study aimed to review scales and tests developed for or validated with children and their families that were published in the Korean Journal of Child Studies from 1984 to 2016. Specifically, the purpose of the present study was to analyze the contents and format of the selected instruments, as well as to evaluate their psychometric properties. Methods: Using several databases and journal archives from the Korean Journal of Child Studies, searches were implemented using the key terms: instrument, scale, development, and validation. Instruments from 76 selected studies were reviewed based on several characteristics, such as assessment areas, contents, respondents, responding types, and psychometric properties. Results and Conclusion: First, a majority of the reviewed instruments were developed for infants and children, whereas only one was developed for adolescents. With regard to their specific measurement domains, many instruments focused on social emotional development among children. Second, with a few exceptions, the selected studies provided appropriate evidence for the reliability of the instrument, including its internal consistency, inter-rater reliability, and spilt-half reliability. Many studies also reported on the criterion-related or construct validity of the instrument to establish its validity. Future studies need to develop instruments across diverse developmental areas that collect information from multiple sources and raters. In addition, more evidence on the reliability and validity of the reviewed instruments should be provided to demonstrate their psychometric qualities.

Changes of Soil Physical Properties by Manured Sorghum Residues Incorporation

  • Although sorghum residue production was estimated to be $8{\sim}10Mg\;ha^{-1}$, most sorghum straw was used to be burnt or removed after harvest. This experiment was conducted to evaluate the effect of the incorporation of manured sorghum residues on soil physical properties from 2010 to 2012 in the converted paddy field. Five treatment with 3 replication consisted of control, inorganic fertilizer (NPK), manured residues, sorghum stover and sawdust manure. The incorporation level of organic source was $10Mg\;ha^{-1}$ without inorganic fertilizer NPK. The investigated physical parameters were bulk density (BD), porosity, water stability aggregate (WSA), water infiltration rater (WIR), penetration resistance (PR) and soil water retention characteristics (WRC) with soil depth. The incorporation of manured sorghum residues improved WIR significantly over inorganic fertilizer (NPK), sorghum residues and sawdust manure. The BD by incorporating with manured residues on sorghum was reduced significantly with crop residue over plot of NPK and sawdust. Significant increase in WSA was observed with stubble incorporation alone or manured sorghum residues. Soil WRC were significantly enhanced with manured sorghum residue incorporated without NPK. The average PR at 0~15 cm was 0.54 MPa for manured sorghum residues. For sawdust manure plot it was 0.42 MPa, for Sawdust manure plot 0.39 MPa and for NPK plot 0.54 MPa.
