Validation of Nursing-Sensitive Patient Outcomes: Focused on Self Care Outcomes (자가간호결과에 대한 타당성 검증 -간호결과분류(NOC)에 기초하여-)

  • The purpose of this study was to validate self care outcomes included Nursing Outcomes Classification(NOC) developed by Johnson and Maas at the University of Iowa. A sample of 103 nurse experts working in university affiliated hospitals participated in this study. They were asked to rate indicators that examplified the outcomes on a scale of 1(indicator is not at all characteristic) to 5(indicator is very characteristic). A questionnaire with an adaptation of Fehring's methodology was used to establish the content validity of outcomes. The results were as follows : 1. All indicators were considered to be 'supporting' and no indicators were considered to be 'nonsupporting'. 2. 'Self care: Toileting' attained an OCV score of 0.884 and was the highest OCV score among self care outcomes. 3. 'Self care: Oral Hygiene' attained an OCV score of 0.756 and was the lowest OCV score among self care outcomes. 4. 'Self-care: Activities of Daily Living (ADL)' attained an OCV score of 0.845 and the highest indicator was 'eating'. 5. 'Self-care: Bathing' attained an OCV score of 0.810 and the highest indicator was 'washes body'. 6. 'Self-care: Dressing' attained an OCV score of 0.831 and the highest indicator was 'buttons clothing'. 7. 'Self-care: Eating' attained an OCV score of 0.815 and the highest indicator was 'chews food'. 8. 'Self-care: Grooming' attained an OCV score of 0.833 and the highest indicator was 'combs or brushes hair'. 9. 'Self-care: Hygiene' attained an OCV score of 0.823 and the highest indicator was 'washes hands'. 10. 'Self-care: Insrumental Activities of Daily Living(IADL)' attained an OCV score of 0.776 and the highest indicator was 'uses telephones'. 11. 'Self-care: Non-Parenteral Medication' attained an OCV score of 0.796 and the highest indicator was 'identifies medication'. 12. 'Self-care: Parenteral Medication attained an OCV score of 0.810 and the highest indicator were 'identifies medication' and 'administers medication correctly'. More outcomes need to be validated and outcomes sensitive to Korean culture need to be developed.

A Highly Expandable Forwarded-Clock Receiver with Ultra-Slim Data Lane using Skew Calibration by Multi-Phase Edge Monitoring

  • A source-synchronous receiver based on a delay-locked loop is presented. It employs a shared global calibration control between channels, yet achieves channel expandability for high aggregate I/O bandwidth. The global calibration control accomplishes skew calibration, equalizer adaptation, and phase lock of all the channels in a calibration period, resulting in the reduced hardware overhead and area of each data lane. In addition, the weight-adjusted dual-interpolating delay cell, which is used in the multiphase DLL, guarantees sufficient phase linearity without using dummy delay cells, while offering a high-frequency operation. The proposed receiver is designed in the 90-nm CMOS technology, and achieves error-free eye openings of more than 0.5 UI across 9-28 inch Nelco4000-6 microstrips at 4-7 Gb/s and more than 0.42 UI at data rates of up to 9 Gb/s. The data lane occupies only $0.152mm^2$ and consumes 69.8 mW, while the rest of the receiver occupies $0.297mm^2$ and consumes 56.0 mW at the 7- Gb/s data-rate and supply voltage of 1.35 V.

Activation of autophagy at cerebral cortex and apoptosis at brainstem are differential responses to 835 MHz RF-EMF exposure

  • With the explosive increase in exposure to radiofrequency electromagnetic fields (RF-EMF) emitted by mobile phones, public concerns have grown over the last few decades with regard to the potential effects of EMF exposure on the nervous system in the brain. Many researchers have suggested that RF-EMFs can effect diverse neuronal alterations in the brain, thereby affecting neuronal functions as well as behavior. Previously, we showed that long-term exposure to 835 MHz RF-EMF induces autophagy in the mice brain. In this study, we explore whether shortterm exposure to RF-EMF leads to the autophagy pathway in the cerebral cortex and brainstem at 835 MHz with a specific absorption rate (SAR) of 4.0 W/kg for 4 weeks. Increased levels of autophagy genes and proteins such as LC3B-II and Beclin1 were demonstrated and the accumulation of autophagosomes and autolysosomes was observed in cortical neurons whereas apoptosis pathways were up-regulated in the brainstem but not in the cortex following 4 weeks of RF exposure. Taken together, the present study indicates that monthly exposure to RF-EMF induces autophagy in the cerebral cortex and suggests that autophagic degradation in cortical neurons against a stress of 835 MHz RF during 4 weeks could correspond to adaptation to the RF stress environment. However, activation of apoptosis rather than autophagy in the brainstem is suggesting the differential responses to the RF-EMF stresses in the brain system.

  • The fisheries in East Asia are reviewed in conjunction with climate change and social-economic developments in the 20th century. About one third of the human population resides in this region, producing a large share of the world's fisheries products, consuming them, and contributing significantly to the international trade of the products. Ongoing local and global climate changes, as well as ocean warming and acidification, are anticipated to have significant impacts on fisheries. Frequent typhoons have brought untold calamities and miseries to coastal communities. The rate of environmental change is outpacing our ability to respond effectively. The science must now move beyond identifying issues and toward providing sound bases for the development of innovative solutions, including effective adaptation and mitigation strategies. Fisheries management plans must be made to consider both changes in climate and social systems. It seems logical that an international forum should be made available to coordinate scientific research, management, and conservation of the region's fishery resources.

Performance Analysis on Coexistence of Contention-based Heterogeneous Wireless Networks (경쟁기반 이기종 무선 통신망의 공존 성능 분석)

  • This paper analyzes the fairness and efficiency of channel sharing when heterogeneous wireless networks that have different transmission power and/or coverage coexist with the contention-based channel access protocol. First, we show that the existing CSMA (carrier sensing multiple access) protocol, that is a prevailing contention-based mechanism, results in significant unfairness of channel access because of (1) the asymmetric capability of carrier sensing and (2) the blindness of binary exponential backoff and link adaptation mechanisms to the interference-driven transmission failures. Next, we derive the feasible region of carrier sensing thresholds that assures spatial reuse and fair channel sharing simultaneously. Moreover, we establish an analytical model for per-system throughput and investigate the effect of contention window size and transmission rate on the fairness and efficiency of channel sharing. Finally, we compare the performance of several approaches for fair channel sharing via simulations under various network configurations.

A Clinical Study on the Social Environmental Factors of Postpartum blues and depression (사회환경적 인자와 관련된 산후우울의 임상적 연구)

  • Objective : Doctors who treat women in childbed have to pay attention to postpartum blues and depression which women in childbed can suffer from, as well as recovery of physical function. Methods : Subjects were 107 females who admitted in Woosuk Hospital of Oriental Medicine from September, 2000 to October, 2001 and made out the question paper within 10 days after delivery. The paper included EPDS(Edinburgh Postnatal Depression Scale) and many items known to be the factors related to postpartum blues and depression. Results : The rate of postpartum depression assessed by EPDS was 16.8%. As the result of analysis, there were significant statistic corelations between each group assessed by EPDS and age parity relation with husband yes or no living with parents-in-law. But there were no significant corelations between each group assessed by EPDS and education religion yes or no occupation delivery method sex of infant marriage type yes or no rearing the infant. Conclusions : We recognized that insufficiency- of delivery experience and stress due to bad adaptation after delivery are possible to be risk factors of postpartum blues and depression. More research should be taken on the corelation between postpartum depression and yes or no living with parents-in-law, because this result is opposite to the trouble between mother-in-law and daughter-in-law. In addition, more research is needed on corelation between physical condition, oriental-diagnosis of women in childbed and postpartum depression.

Evaluation of Soil Loss with Surface Covering Methods Using Strip Tillage Seeding Device

  • Most fields in highland areas are covered with rye or hairy vetch for conservation during a fallow. However, using cover crops needs an effort to sow, and this makes top soil more vulnerable to loss due to surface disturbances. The aims of this study were to develop an automatic seeding-regulator device using a low-price, extensive-use GPS sensors and a DC motors and to evaluate a working efficiency of it after adaptation to partial tillage machine for reducing seeding effort. The amount of runoff water and soil loss was evaluated with partial tillage and simultaneous-seeding after harvesting soybean, in 17% slope lysimeters. In results, the seeding amount with the machine speed was stable between $0.5{\sim}0.8m\;s^{-1}$ of working sections. The automatic control device of seeding-rate could be enough to solve the slip problems of power selecting supply system or five four-wheel drive device. In partial tillage and simultaneous seeding, runoff water was 11.6% ($1.8m^3ha^{-1}$) of the scatter-seeding control ($15.5m^3ha^{-1}$) and soil loss was 13.2% ($7kg\;ha^{-1}$) of the scatter-seeding control ($53kg\;ha^{-1}$). These results suggest that partial tillage and simultaneous-seeding methods are very effective in decreasing work effort and soil loss of sloped land.

Effects of Sowing Density and Thinning Treatment on Growth Characteristics and Yield of 3-Year-Old Ginseng Cultivated in a Greenhouse (인삼 하우스 직파재배 시 파종밀도 및 솎음처리가 3 년근 인삼의 생육과 수량에 미치는 영향)

  • Background: The cultivation of ginseng (Panax ginseng C. A. Meyer) in greenhouses could reduce the use of pesticides and result in higher yield; however, construction costs are problematic. The adaptation of direct-sowing culture in greenhouses could reduce the cost of ginseng production. Methods and Results: To improve seedling establishment in direct-sowing culture, effects of sowing density (SD), number of seeds sown per hole (SN), and thinning (TH) treatment on the root yield were investigated after 3 years of seeding. The emergence rate was significantly influenced by SD, but not by SN or TH. Damping-off and rusty roots increased with an increase in SN with diminishing effects of SN on seedling establishment. Root weight and diameter were affected by SD, SN, and TH, however, there were no statistical significances. The total number of roots harvested per unit area increased with increasing SD and SN, and the weight of roots was affected by SD, but not by SN or TH. Conclusions: Multi-seed sowing per hole and/or thinning might not be an efficient method for the direct-sowing culture of ginseng. The SD for direct seeding culture in greenhouses should be approximately $33-42seeds/m^2$ for an optimum yield of 3-year-old ginseng.

Concentration Dependent Effect of Heavy Metals on Soil Carbon Mineralization

  • The present laboratory investigation was conducted to assess the effect of heavy metals on carbon mineralization. Soil was treated with three concentrations (50, 100 and $150{\mu}mol\;g^{-1}$ soil) of two heavy metals (Cd and Zn) in a factorial combination of treatments replicated four times. Determination of carbon mineralization was carried out at 3, 7, 14, 21, 28, 42 and 56 days after metal treatments.. The amount of $CO_2$-C released from heavy metal treated soils was found to be decreased at an increasing rate during the first 28 days, followed by slow release as incubation progressed. The total amounts of $CO_2$-C released were 448, 382 and $348mg\;kg^{-1}$ soil respectively for soils treated with 50, 100 and $150{\mu}mol\;g^{-1}$ soil of Zn. The corresponding figures for Cd treated soils were 406, 354 and $282mg\;kg^{-1}$ soil implying that dose-dependent reduction in cumulative $CO_2$-C released from soils. The inhibition of carbon mineralization was found to be high in Cd treated soils than that of Zn treated. Therefore, tolerance and adaptation of the microbial community is likely to be related to the concentration and the type of metal. According to the results, carbon mineralization can be considered as possible indicator of soil pollution by means of heavy metals.

No Adverse Outcomes of Video-Assisted Thoracoscopic Surgery Resection of cT2 Non-Small Cell Lung Cancer during the Learning Curve Period

  • Background: Video-assisted thoracoscopic surgery (VATS) anatomic lung resections are gradually becoming the standard surgical approach in early-stage non-small cell lung cancer (NSCLC). The technique is being applied in cases of larger tumors depending on the experience of the surgical team. The objective of this study was to compare early surgical and survival outcomes in patients undergoing anatomic pulmonary resections using VATS and thoracotomy techniques for clinical T2 NSCLC during the adaptation period of the surgical team to the VATS approach. Methods: The data of all patients who underwent anatomic pulmonary resection for NSCLC using VATS and open techniques since April 2012 were recorded to create a prospective lung cancer database. Clinical T2 NSCLC patients who underwent VATS anatomic lung resection were identified and compared with cT2 patients who underwent open resection. Results: Between April 2012 and August 2014, 269 anatomical resections for NSCLC were performed (80 VATS and 189 thoracotomy). Thirty-four VATS patients who had clinical T2 disease were identified and stage-matched to thoracotomy patients. The average tumor diameter was comparable ($34.2{\pm}11.1{\times}29.8{\pm}10.1mm$ vs. $32.3{\pm}9.8{\times}32.5{\pm}12.2mm$, p=0.4). Major complications were higher in the thoracotomy group (n=0 vs. n=5, p=0.053). There was no 30-day mortality, and the 2-year survival rate was 91% for VATS and 82% for thoracotomy patients (p=0.4). Conclusion: VATS anatomic resections in clinical T2 NSCLC tumors are safe and have perioperative and pathologic outcomes similar to those of thoracotomy, while remaining within the learning curve.
