동결견 환자에서 관절강내 스테로이드 주사 후 관절가동범위의 변화 (Changes in Range of Motion after Intra-Articular Corticosteroid Injection in Frozen Shoulder: A Retrospective 3-Month Follow-Up Study)

  • Objective: To examine (1) the degree of reduction of passive range of motion (PROM) on the affected side compared to that on the unaffected side and (2) the degree of increase in PROM following intra-articular corticosteroid injection (IACI) in patients with frozen shoulder. Method: The medical records of 120 patients with frozen shoulder were retrospectively reviewed. PROM of the unaffected and affected shoulder (flexion, extension, abduction, internal rotation, external rotation) was compared, and changes in PROM of the affected shoulder after a single IACI (triamcinolone 20 mg) were evaluated after 12 weeks. Results: At the time of diagnosis, PROM of the affected shoulder was most limited in external rotation, followed by internal rotation, abduction, extension, and flexion, compared to that of the unaffected shoulder. Compared to before IACI, PROM of external rotation demonstrated the greatest increase compared to all the other movements after IACI. Conclusion: Limitation in PROM of the frozen shoulder at the time of diagnosis was greatest for external rotation. Moreover, external rotation experienced the greatest improvement after IACI. Our findings should help to further clarify the clinical characteristics of frozen shoulder, aid in its diagnosis, and allow the prediction of the effects of IACI.

진동을 동반한 슬링 운동이 어깨 손상 환자의 관절가동범위, 근력, 통증, 기능장애 수준에 미치는 영향 (Effects of Sling Exercise With Vibration on Range of Motion, Muscle Strength, Pain, Disability in Patients With Shoulder Injuries)

  • Background: Sling exercises are frequently used for the rehabilitation process of patients with shoulder joint injuries, but research on the significant frequency intensity and appropriate treatment duration for sling exercises with local vibration stimulation is lacking. Objects: The aim of this study was to investigate the effects of sling exercise with vibration on shoulder range of motion (ROM), muscle strength, pain, and dysfunction in patients with a medical diagnosis of shoulder joint injury. Methods: Twenty-two patients were randomly assigned to the experiment and control groups. Six sling exercises with and without 50 Hz vibrations were applied in the experiment and control groups, respectively. Each exercise consisted of 3 sets of 5 repetitions performed for 6 weeks. The assessment tools used included shoulder joint range of motion, muscle strength, pain level, and shoulder pain and disability index for functional disability. We conducted re-evaluations before and 3 and 6 weeks after intervention. The changes in the measurement variables were analyzed and compared between the two groups. Results: The ROM of the external rotation of the shoulder joint had a significant interaction between the group and the measurement point (F=3.652, p<.05). In both groups, we found a significant increase in external rotation angle between the measurement points (p<.05). The flexor strength of the shoulder joint significant interaction between the group and the measurement point (F=4.247, p<.05). Both the experiment (p<.01) and control groups (p<.05) showed a significant increase in shoulder flexor strength at the measurement points. After 6 weeks of the interventions, both the groups showed significantly improved VAS (p<.01), SPADI (p<.01), and orthopedic tests (p<.01). However, there was no significant difference between the group and the measurement point in terms of the clinical outcomes observed. Conclusion: The sling exercise with local vibration of 50 Hz affected the external rotation of the shoulder range of motion and improved shoulder flexor strength in the patients with shoulder injuries. Therefore, we propose the use of the sling exercise intervention with vibration in the exercise rehabilitation of patients with shoulder joint injuries.

어깨재활운동 융합프로그램이 어깨수술 후 통증, 관절운동범위 및 운동자기효능감에 미치는 효과 (Effects of Shoulder Rehabilitation Exercise Convergence Program on Pain, Range of Motion and Exercise Self-efficacy After Shoulder Surgery)

  • 본 연구목적은 어깨재활운동 융합프로그램의 효과를 확인하기 위함이다. 비동등성 대조군 전후설계에 따라 어깨수술환자 58명을 연구대상으로 하였다. 실험군에게는 동영상교육, 어깨재활운동 및 운동모니터링으로 구성한 어깨재활운동 융합프로그램을 적용하였다. 자료는 구조화된 질문지와 어깨관절운동범위를 측정하여 수집하였으며 기술통계, ${\chi}^2-test$, t-test 및 ANCOVA로 분석하였다. 연구결과 실험군의 어깨통증은 대조군보다 유의하게 낮았으며(p<.001), 어깨관절운동범위와 운동자기효능감은 대조군보다 유의하게 높았다(p=.038; p<.001). 그러므로 어깨재활운동 융합프로그램은 어깨수술환자를 위한 효과적인 재활중재로 활용될 수 있으며 다양한 임상실무영역에서 지속적인 융합중재개발의 확장이 요구된다.

배구선수들의 극하근 근위축이 어깨 관절 회전근력과 운동범위에 미치는 영향 (The Effect of Atrophy of the Inpraspinatus on Strength and ROM in Shoulder Joint of Male Volleyball Players)

  • 본 연구의 목적은 극하근 근위축이 배구선수들의 등속성 최대토크와 운동범위에 어떠한 영향을 미치는지 알아보고자, 극하근의 근위축 소견이 있는 선수 3명, 근위축 소견이 없는 선수 5명을 대상으로 등속성 근력측정기기인 Biodex와 동작분석기기 Simi-Motion을 이용하여 측정분석 하였다. 본 연구의 결과, 극하근의 근위축이 있는 그룹과 없는 그룹 사이에 어깨 관절의 외회전 등속성 최대토크, 내회전에 대한 외회전 등속성 최대토크 비율 그리고 내회전 회전 운동범위가 유의한 차이가 있는 것으로 나타났다. 배구선수들의 극하근의 근위축을 예방하기 위해서는 어깨 관절의 기능을 고려한 외회전근의 근력강화 훈련이 중점적으로 시행되어야 하며, 이와 더불어 어깨 관절 후부 조직에 대한 유연성 훈련도 병행되어야 할 것으로 사료된다.

  • Purpose : The purpose of this study was to determine the initial effects of deltoid inhibition taping to Pain, Function, Strength, ROM in shoulder impingement syndrome (SIS). Methods : This study is 28 patients(male 16, female 12) with shoulder impingement syndrome(SIS).The experimental group received deltoid inhibition taping and the control group had sham taping. Outcome variables measured degree of pain, disability, strength, and range of motion at pre-post intervention. The changes between pre-post interventions are analyzed by a repeated measure ANOVA test. Results : Pain and disability index significantly decreased (p<.05), and the rate of change in pain and disability level of the experimental group increased significantly more than control group (p<.05). Strength and range of motion significantly increased (p<.05), but the rate of change of the two groups showed no significant difference (p>.05). Conclusion : These results suggest that deltoid inhibition taping was initial effective in decreasing pain and disability in SIS patients.

Effect of Chuna (Shoulder Traction) Treatment on Frozen Shoulder During Korean Medical Treatment

  • Objectives : Through an analysis of the previous studies, It is estimated that Chuna(shoulder traction) is effective in reducing the pain and increasing motion range of shoulder joint of the patient. So this study is to investigate the effects of Chuna(shoulder traction) on frozen shoulder. Methods : After treated with acupuncture and electro stimulating therapy, the subjects were measured list of measurement. And then treated with Chuna(shoulder traction) therapy, the subjects were measured list of measurement again. All treatment was performed by the same doctor who is a highly qualified about Chuna(shoulder traction). Chuna(shoulder traction) treatment takes 15 minutes. Three kind of Chuna(shoulder traction) therapy were performed for 5 minutes each. Depending on the degree of the subject's pain, treatment strength was adjusted. Results : - Chuna simultaneous treatment group, shoulder joint range of flexion, extension, abduction, adduction, external rotation and internal rotation movement improved statistically significant. - Chuna simultaneous and acupuncture treatment group, visual analogue scale(VAS) scores of two groups decreased statistically significant. Conclusions : The result suggests that Chuna(shoulder traction) can be effective to recover range of motion and reduce pain on patients with frozen shoulder.

A Comparison of Shoulder Muscle Activities on Sitting Posture and Shoulder Angle

  • Background: Sitting posture influences movements of scapulothoracic and glenohumeral joints and changes the shoulder muscle activities. The development and maintenance of correct sitting posture is important for the fundamental treatment of shoulder pain during rehabilitation. Objects: The purpose of this study was to investigate the effects of the sitting postures and the shoulder movements on shoulder muscle activities for both male and female. Methods: Twenty-eight subjects without shoulder-related diseases participated in this experiment. The subjects had randomly adopted three different sitting postures (upright posture, preferred posture, maximum slouched posture) and shoulder flexion angles in scapular plane ($30^{\circ}$, $90^{\circ}$, $120^{\circ}$). Surface electrodes were collected from upper trapezius (UT), anterior deltoid (AD), and posterior deltoid (PD) and the active shoulder range of motion was measured in each sitting posture and shoulder flexion angle. Results: The active range of motions of the shoulder external rotation and the flexion in the scapular plane decreased from the upright posture to the maximum slouched posture (p<.05, mixed-effect linear regression with random intercept, Tukey post-hoc analysis). All muscles showed the highest EMG activities at $120^{\circ}$ shoulder flexion with the maximum slouched posture and did not show the gender differences. Conclusion: Increased shoulder muscle activities may become the potential risk factor for the shoulder impairment and pain if people continuously maintain the maximum slouched posture. Therefore, an upright position is necessary during shoulder exercises, as well as in activities of daily living, including motions involving lifting the arms.

동결견 환자에 대한 키네시오 테이프 적용이 관절가동범위 및 통증에 미치는 영향 (Effects of Kinesiotape on Range of Motion and Pain in Frozen Shoulder Patients)

  • This study was performed to assess the efficacy of Kinesiotape for the frozen shoulder patients. The subjects of this study were 17 patients with frozen shoulder who visited the out-patient department of the physical therapy, Pyongchon Sacret Heart Hospital, Hallim University, from July. 3, 2000 through August 12, 2000. To find out the effect of kinesiotape therapy, we sampled 8 patients treated with electrotherapy and therapeutic exercise (control group), and 9 patients treated with electrotherapy and therapeutic exercise with kinesiotape(experimental group). All patients were treated 3 days a week for 5 weeks. The results after 5-week treatment, compared with before treatment, were as follows: 1. The increase in range of motion in the electrotherapy and therapeutic exercise with kinesiotape after 5-week treatment was very significant(p<0.01). 2 The increase in range of motion in the electrotherapy and therapeutic exercise after 5-week treatment was significant (p<0.05). 3. The pain of decrease in the electrotherapy and therapeutic exercise with kinesiotape after 5-week treatment was very significant (p<0.01). 4. The pain of decrease in the electrotherapy and therapeutic exercise with kinesiotape 5-week treatment was significant(p<0.05). 5. The electrotherapy and therapeutic exercise with kinesiotape was more effective in increasing the range of motion on shoulder than the electrotherapy and therapeutic exercise after 5-week treatment(p<0.01). 6. The pain of decrease in between electrotherapy. therapeutic exercise with kinesiotape electrotherapy and therapeutic exercise after 5-week treatment was no significant.

Comparison of Extracorporeal Shock Wave Therapy Versus Injection Therapy for Shoulder Disorders: A Systematic Review and Meta-Analysis of Randomized Controlled Trials

  • Objective: The purpose of this study is to compare the effects of extracorporeal shock wave therapy (ESWT) and injection therapy through qualitative and quantitative analysis by synthesizing randomized controlled trials (RCTs) conducted on patients with various shoulder diseases. Design: A systematic review and meta-analysis of randomized controlled trials Methods: This review conducted a literature search through international electronic databases in January 2023 to compare the effects of ESWT and injection therapy. Qualitative analysis was performed as a risk of bias tool, and quantitative analysis was synthesized with a random effect model to show the effect size as a standardized mean difference (SMD). Results: Five RCTs involving 298 individuals with shoulder disorders were included in this systematic review and meta-analysis. ESWT showed a moderate effect on pain (SMD=-0.44; 95% CI, -0.95 to 0.06) and a large effect on shoulder function (SMD =-0.81; 95% CI, -1.70 to 0.07) than injection therapy. A significant difference was found in the shoulder range of motion, showing a large effect size (SMD=1.50; 95% CI, 0.58 to 2.43). Conclusions: When considering treatment options for the management of patients complaining of shoulder disorders, it is appropriate to recommend ESWT first rather than injection therapy to increase the range of motion of the joint. In addition, ESWT is safe for pain control and shoulder function improvement, and a positive prognosis could be expected.

고유수용성신경근촉진법 패턴 운동과 스크램블러 치료가 유방암 환자의 통증과 가동범위 및 어깨기능장애에 미치는 영향 (Effect of the Proprioceptive Neuromuscular Facilitation Pattern Exercise and Scrambler Treatment on Pain, Range of Motion, and Shoulder Dysfunction in Breast Cancer Patients)

  • Purpose: The study aims to examine the effects of the proprioceptive neuromuscular facilitation pattern exercise and scrambler therapy on pain, range of motion (ROM), and shoulder dysfunction in breast cancer patients. Methods: In total, 30 breast cancer patients were recruited and randomized to group I (n = 10), group II (n = 10), and group III (n = 10). Pain was measured using a visual analogue scale, ROM was measured using a goniometer, and shoulder dysfunction was measured using a shoulder pain and disability index. Group I practiced the proprioceptive neuromuscular facilitation pattern exercise and underwent scrambler therapy, group II underwent scrambler therapy only, and group III practiced the proprioceptive neuromuscular facilitation pattern exercise only. Results: A within-groups comparison showed that all groups demonstrated significant differences in pain, ROM, and shoulder dysfunction after the experiment (p<0.05). Further, according to a comparison of the three groups, group I showed a more significant difference in effectiveness than groups II and III in terms of pain, ROM, and shoulder dysfunction before and after the experiment (p<0.05). Conclusion: This study showed that the proprioceptive neuromuscular facilitation pattern exercise and scrambler therapy are effective in treating pain, ROM issues, and shoulder dysfunction in breast cancer patients.
