Subtalar Distraction Bone Block Arthrodesis (Five Cases) (거골하 신연 골편 관절 유합술)

  • Purpose: To determine the efficacy of the SDBBA (Subtalar distraction bone block arthrodesis) procedure on patients with late complication of intra-articular calcaneal fractures including subtalar joint arthritis and anterior ankle impingement syndrome. Materials and Methods: Five cases in which the SDBBA procedure was implemented were followed for more than one year. All five patients were male with an average age of 56. Clinically, the American Orthopedic Foot and Ankle Society (AOFAS) ankle-hindfoot score and the pain score were assessed. Radiographically, the talocalcaneal height and the talar inclination angle were determined. Results: All five patients achieved subtalar joint fusion. The average pre-operative AOFAS score was 22.8 scores (range, 8-32 scores). At last follow-up, these scores improved to an average of 72.4 scores (range, 64-82 scores). The average pre-operative pain score was 8.2 scores (range, 7-10 scores). At last follow-up, these scores improved to an average of 13.2 (range, 12-15 scores). The average pre-operative talocalcaneal height was 72.8 mm (range, 70-77 mm), average post-operative talocalcaneal height improved to 79.8 mm (range, 78-84 mm). At final follow-up, these measurement was slightly decreased to average 78.6 mm (range, 74-83 mm). The average pre-operative talar inclination angle was 13.2 degrees (range, 12-15 degrees), average post-operative talar inclination angle improvedto 19.2 degrees (range, 15-24 degrees). At final follow-up, these measurement was slightly decreased to average 18.6 degrees (range, 12-24 degrees). Four patients achieved successful outcomes. One patient developed a wound infection with subsequent sural neuropathy as well as collapse of the bone graft. Conclusion: This study shows that the SDBBA procedure successfully restores the talocalcaneal height and tibio-talar relationship. This procedure is useful in surgically managed patients with talo-calcaneal height loss and anterior ankle impingement syndrome due to the late complications of calcaneal intra-articular fractures.

  • In order to determine the content levels of trace metals in cow's milk, 79 market milk samples were collected from markets in major cities of Korea from July to August, 1985 and analyzed for content levels of Cu, Fe, Zn, Mn, Pb, Cd. Treatment for Hallux Valgus with Chevron Metatarsal Osteotomy in Patients over 60 Years Old (60세 이상의 고령에서의 중족골 절골술을 이용한 무지 외반증 치료)

  • Purpose: To treat hallux valgus in old age patients with chevron metatarsal osteotomy and to see the subsequent clinical and radiological outcomes. Materials and Methods: 23 cases of 18 hallux valgus patients of age 60 years or older who received proximal or distal corrective osteotomy from April 2007 to August 2009 and were followed up for at least 1 year were included in the study. The mean age at operation was 65 years (range, 60~81 years), and the mean follow-up period was 2 years and 6 months (range, 1 year~3 years 6 months). Clinical outcome was assessed with the American Orthopaedic Foot and Ankle Society (AOFAS) score, complications, satisfaction rate, as well as measurements and comparison of pre- and postoperative hallux valgus angles, the 1st~2nd intermetatarsal angle, and the position of hallucal medial sesamoid bone. Results: The AOFAS score was improved from preoperative average of 35.1 (range, 13-47) to average 85.1 at last follow-up (range, 75-100). Patients were satisfied about the operation in 21 cases (91.3%). Preoperative hallux valgus angle was $31.7^{\circ}$ on average (range, $19.1^{\circ}-48.9^{\circ}$), and $4.9^{\circ}$ on average at last follow-up (range, $0.3^{\circ}-21.2^{\circ}$). The 1st~2nd intermetatarsal angle was $14.4^{\circ}$on average (range, $8.7^{\circ}-25.7^{\circ}$) and $3.1^{\circ}$ on average at last follow-up (range, $0.6^{\circ}-7.5^{\circ}$). The hallucal medial sesamoid bone position was improved from preoperative average 3.5 (range, 3-4) to postoperative average 1.0 (range, 0-2). Conclusion: Proximal and distal metatarsal osteotomy treatment yielded good clinical and radiological outcomes in old age hallux valgus patients.

A Study on the Curriculum of Emergency Medical Technology in Korea (한국의 응급구조와 교과과정)

  • Purpose : This study is to provide the basic data for developing the curriculum standardization of emergency medical technology by analyzing the three-year and four-year period curriculum and high-advanced major course to bring up highly qualified paramedics. Method : This study was analyzed, divided in 3 sections, majors, teaching profession subjects and liberal arts of 18 three-year curriculum universities and 7 four-year curriculum universities. Majors were analyzed, departmentalized in the national examination for license, the clinical and on-the-job training(OJT) and other major-related subjects. It is descriptive study, analyzed by dividing high-advanced major course into majors and liberal arts. Result : 1) The average number of subjects established in three-year-course college were 58.1. The completion credit was 130.1 in average. Also, the average number of established subjects at four-year-course were 61.1. The average completion credit was 141. 2) Three-year-course college professors give lecture in theory for 95.4 hours on average, which account for 59.9% of overall lecture, and 63.8 hours for practical training that takes 40.1% of the hours lectures are given. Meanwhile, four-year-course give lecture in theory for 111 hours on average, taking a part of 59.5%, and 76.5 hours for practical training, 40.5%. 3) In regard to liberal arts, at three-year-course college, the average credit was 16 but the proportion that liberal arts charge among the completion credit was 11.6. Meanwhile, at four-year-course universities, the average credit was 28.4 and the percentage that the liberal arts took part in the completion credit was 20.1. 4) Looking over national examination subjects among majors at three-year-course college, basic medicine's average credit was 17.2, introduction to advanced emergency care's average credit was 15.5, Special advanced emergency care's average credit was 28, emergency patient management's average credit was 9.2 and emergency medicine statute's average credit was 3.6. Meanwhile, in case of four-year-course universities, basic medicine's average credit was 14.3, introduction to advanced emergency care's average credit was 12.7, special advanced emergency care's average credit was 31, emergency patient management's average credit was 8.3 and emergency medicine statute's average credit was 2.9. 5) Among national examination subjects, in case of three-year-course, the range of basic medicine credits was 6 to 23, the average credit showed 17.2. The range of introduction to advanced emergency care credits was 9 to 21, the average credit showed 15.5. The range of special advanced emergency care credits was 19 to 36, the average credit showed 28. The range of emergency patient management credits was 5 to 12, the average credit was 9.2. The range of legal medicine credits was 3 to 6, the average was 3.6 credit. In case of four-year-course the range of basic medicine was 12 to 17, the average of the credit was 14.3. The range of introduction to advanced emergency care was 9 to 19, and the average of the credit was 12.7. The range of special advanced emergency care was 18 to 41, and the average of the credit was 31. The range of emergency patient management was 7 to 12, and the average of the credit was 8.3. The range of legal medicine was 3 to 4, and the average of the credit was 2.9. So except special advanced emergency care, all the other subjects had higher score in three-year-course than four-year-course. 6) About the other major-related subjects, the completion credit was 23 on average at three-year-course and 13.5 at four-year-course. 7) The clinical and on-the-job training(OJT), In the case of three-year-course, the average completion credit was 13.5 and 13.5 at four-year-course. 8) The teaching profession subject, In the case of three-year-course, the average credit was 3 and the percentage that the teaching profession subject took part in the completion credit was 2.3% on average. Four-year-course established the subject only at one university. 9) In high-advanced major process, the entire established subjects on average were 12.8 and the average completion credit was 36.3. In liberal arts, the average established subjects were 6.5 and the completion credit was 19.5. The number of majors were 9.5 on average and the credit was 26.5 Conclusion : According to the aforemention results, the curriculum for emergency medical technology needs to be developed and standardized.

Outcomes of Internal Fixation with Compression Hip Screws in Lateral Decubitus Position for Treatment of Femoral Intertrochanteric Fractures

  • Purpose: Internal fixation using compression hip screws (CHS) and traction tables placing patients in the supine position is a gold standard option for treating intertrochanteric fractures; however, at our institution, we approach this treatment with patients in a lateral decubitus position. Here, the results of 100 consecutive elderly (i.e., ${\geq}45$ years of age) patients who underwent internal fixation with CHS in lateral decubitus position are analyzed. Materials and Methods: Between March 2009 and May 2011, 100 consecutive elderly patients who underwent internal fixation with CHS for femoral intertrochanteric fracture were retrospectively reviewed. Clinical outcomes (i.e., Koval score, Harris hip score [HHS]) and radiographic outcomes (i.e., bone union time, amount of sliding of lag screw, tip-apex distance [TAD]) were evaluated. Results: Clinical assessments revealed that the average postoperative Koval score decreased from 1.4 to 2.6 (range, 0-5; P<0.05); HHS was 85 (range, 72-90); and mean bone union time was 5.0 (range, 2.0-8.2) months. Radiographic assessments revealed that anteroposterior average TAD was 6.95 (range, 1.27-14.63) mm; lateral average TAD was 7.26 (range, 1.20-18.43) mm; total average TAD was 14.21 (range, 2.47-28.66) mm; average lag screw sliding was 4.63 (range, 0-44.81) mm; and average angulation was varus $0.72^{\circ}$(range, $-7.6^{\circ}-12.7^{\circ}$). There were no cases of screw tip migration or nonunion, however, there were four cases of excessive screw sliding and six cases of varus angulation at more than $5^{\circ}$. Conclusion: CHS fixation in lateral decubitus position provides favorable clinical and radiological outcomes. This technique is advisable for regular CHS fixation of intertrochanteric fractures.

The Content of Minerals in Algae (해조류(海藻類)의 무기성분(無機成分))

  • Sixteen species of algae (4 species of green algae, 5 species of brown algae and 7 species of red algae) were collected from the coast of Chungmu, Gyeongnam, from June to October, 1976. The content of minerals in algae were analyzed by Atometic Absorption Spectrophotometer. The results were as followed 1) The content of Iron and Nickel in green algae were abundant, and those of Lead, Nickel and Manganese in brown algae were low. 2) The content of Cadmium were in the range of 0.58-1.04ppm (average: 0.85ppm) in green algae, 0.32-2.10ppm (average: 1.08ppm) in brown algae and 0.54-1.70ppm (average: 1.04ppm) in red algae. The content of Cadmium were in the range of 0.3-0.6ppm in laver, Porphyra tenera, sea mustard, Undaria pinnatifida, and tangle, Laminaria japonica, but its content was lower than those expected. 3) The content of Lead were in the range of 0.67-1.40ppm (average: 1.03ppm) in green algae, 0.60-1.00ppm (average: 0.82ppm) in brown algae, 0.56-2.40ppm (average: 1.28ppm) in red algae and its content in algae were lower than in fish and shellfish. 4) The content of Copper were in the range of 10.8-24.2ppm (average: 18.95ppm) in green algae, 7.4-24.6ppm (average: 18.16ppm) in brown algae, 6.4-31.2ppm (average: 19.94ppm) in red algae and those content were considerably abundant except for some algae. 5) The content of Nickel were in the range of 5.4-16.6ppm (average: 9.1ppm) in green algae, 1.0-4.4ppm (average: 2.32ppm) in brown algae and 0.7-4.6ppm (average: 2.59ppm) in red algae. 6) The content of Iron were in the range of 686.4-1159.0ppm (average: 916.5ppm) in green algae, 131.0-499.2ppm (average: 310.16ppm) in brown algae and 156.0-530.4ppm (average: 248.2ppm) in red algae. Especially, that of Iron in green algae showed higher value than in any other. 7) The content of Manganese were in the range of 48-221ppm (average: 157.25ppm) in green algae, 12-65ppm (average: 41ppm) in brown algae and 72-162ppm (average: 121ppm) in red algae. Especially, that of Manganese in brown algae showed lower value than in any other. 8) The content of Zinc were in the range of 191.3-451.1ppm (average: 290.05ppm) in green algae, 89.9-374.2ppm (average: 202.64ppm) in brown algae and 106.4-281.4ppm (average: 188.93ppm) in red algae. 9) The content of Magnesium were in the range of 0.48-1.83% (average: 1.27%) in green algae, 1.04-1.71% (average: 1.21%) in brown algae and 0.42-1.24% (average: 0.097%) in red algae. 10) The content of Fluorine were in the range of 29.2-92.7ppm (average: 53.03ppm) in green algae, 33.3-43.5ppm (average: 39.18ppm) in brown algae and 32.4-59.0ppm (average: 44.84ppm) in red algae.

A Study on the Character of Observation Time in order to Draw the Features of Spatial Primacy Effect - Focus on the Public space - (공간의 초두효과 특성 도출을 위한 주시시간특성에 관한 연구 - 공공공간의 로비를 대상으로 -)

  • This study introduced the concept of Primacy Effect for the analysis of the observation characters by time so as to analyze the characters of information which the users observing the lobby in a public space get when they encounter the space. From the viewpoint that the first impression on any space can motivate the judgement of space and continuous activities, the time range with the observation characters which can be regarded as Primacy Effect was analyzed in the aspects of continuity and saccade. First, with the case of alignment by time on three times of continuous observation, the perception attributes of subjects were shown to be very different among one another depending on which time range was applied for alignment. Second, the subjects with low average value of continuous observation times were found to be relatively stable in "ii~iii time ranges" and the time range of "0~5.10 seconds" was known to be the time range of relatively stable analysis for the characters of subjects' primacy effect to be analyzed. Third, the change of the amount of acquired information could be examined concretely through the analysis of observation attributes by time range, and the time range of "iii time range" showed more stable changing characters than other time ranges. Fourth, in the "iii time range" set up as a proper time range for the analysis of primacy effect had higher average value of 3.9 times of observation frequency compared with 6 times of continuous observation. As a grand average, 6 times(19.3) of average observation value was between 3 times(21.3) and 9 times(16.9), which enabled to set up 6 times of continuous observation as the number of observation times for stable analysis of primacy effect.

The Bioenergy Conversion Characteristics of Feedlot Manure Discharging from Beef Cattle Barn

  • This study was carried out to assess bioenergy conversion efficiency by biogas and solid fuel production in the cattle feedlot manure discharged from beef cattle barn. Feedlot manure was sampled from the cattle farmhouse located in Yong-in, Gyeonggi during the mid-fattening stage, periodically. The chemical characteristics, BMP (Biochemical methane potential) and HV (Heating values) of feedlot cattle manures were analyzed. Total solid contents of cattle feedlot manure were in the range of 29.98~44.28%, and volatile solid contents were in the range of 23.53~24.47%. In the anaerobic digestion of cattle feedlot manure, the methane production potential has increased from 0.141 to $0.187Nm^3kg^{-1}-VS_{added}$. The methane production of fresh cattle feedlot manure showed the range $0.141{\sim}0.187Nm^3kg^{-1}$-Manure (average $0.047Nm^3kg^{-1}$-Manure), the LHVs (lower heating values) of the produced methane were in the range of $316{\sim}560kcalkg^{-1}$-Manure (average $400kcalkg^{-1}$-Manure). In the direct combustion of fresh cattle feedlot manure, the LHVs were measured in the range of $747{\sim}1,271kcalkg^{-1}$-Manure (average $916kcalkg^{-1}$-Manure), and LHVs of solid fuel which have the water content of 20% were in the range of $2,694{\sim}2,876kcalkg^{-1}$-Manure (average $2,791kcalkg^{-1}$-Manure). Then, the drying energy of average $443kcalkg^{-1}$-Manure was consumed in the production of solid fuel which has a water content of 20%. Therefore, the direct combustion of cattle feedlot manure showed about 2.3 times higher LHV than the LHV of methane produced by anaerobic digestion. And LHV of solid fuel was about 6.0 times higher than the LHV of methane produced by anaerobic digestion. Then, the production of solid fuel presented more bioenergy conversion efficiency than the biogas production in the bioenergy use of cattle feedlot manure.

Korean & Native Speakers' High -low Range Differences in F0 and its Role in Pronunciation Assessment (한국인과 원어민 영어 발화의 F0 고저 범위 차이와 발음 평가에 있어서 그 역할)

  • This study examines the high-low pitch range differences between Korean and native English speakers' utterances of 36 English sentences, and explores the role of the pitch fluctuation range in the pronunciation assessment. Findings are: i) Korean speakers implement half as much pitch fluctuation as the native speakers with the achievement rate of, on an average, 47%. Especially, pitch fluctuation range is below the average in the sentence types of exclamation, yes-no & wh-questions, ii) the reason why Korean speakers' pitch fluctuation range is only half the range of the native speakers is that Korean speakers do not raise the pitch at certain sentence positions so much as the native speakers, and iii) the range of pitch fluctuation plays an important role in distinguishing the utterances of the native speakers from those of Korean speakers, but does not in rating the English utterances among Korean speakers.

A Study on Exposure Among Asbestos Textile Workers and Estimation of their Historical Exposures (석면방직업 근로자의 석면노출 실태와 과거농도 추정에 관한 연구)

  • From July 8 to September 2 1994, asbestos exposure level among asbestos textile workers was surveyed. Six plants out of plants in Korea were selected for this study. In addition to the exposure level, the relationship between the level of exposure and some factors affecting exposure were studied. Also, using historical data of asbestos concentrations in asbestos textile plants plus current data, trend of asbestos exposure level could be introduced. Historical exposure level was estimated on the basis of these data. The main results of this study are follows. 1. Average concentration of all six plants surveyed was 1.54 f/cc, and range of those concentrations was 0.03 - 11.58 f/cc. The minimum average concentration was 0.32 f/cc and the maximum was 8.04 f/cc which is four times higher than the Korean standard. A wide difference of exposure level among the workers of different plants was observed. In three plants, the half of all the plants surveyed, their average concentrations exceeded the Korean standard, and those in all the plants exceeded the ACGIH TLV. 2. Among total 56 samples, 22 samples(39%) were in excess of the Korean standard, and 53 samples(95%) were above the ACGIH TLV. Among 32 personal samples, 15 samples(47%) exceeded the Korean standard, and 30 samples(94%) exceeded the ACGIH TLV. Among 24 area samples excluding a few samples collected in office area, seven samples exceeded the Korean standard, and 23 samples( 96%) exceeded the ACGIH TLV. 3. Distributions of concentrations were observed by processes. In weaving, the highest, average concentration was 4.29 f/cc, and range was 2.61 - 11.58 f/cc. In spinning, average concentration was 2.22 f/cc, and range was 0.41 - 8.93 f/cc. In carding, average concentration was 1.98 f/cc, and range was 0.23 - 10.93 f/cc, In twisting, average concentration was 1.65 f/cc, and range was 0.21 - 9.83 f/cc. In mixing, the lowest, average concentration was 0.48 f/cc, and range was 0.22 - 1.20 f/cc. 4. All the samples from basic processes of asbestos textile plants were above the ACGIH TLV. Nineteen samples(45%) out of all these 42 samples exceeded Korean standard. Fourteen samples(58%) of total 24 personal samples, and five samples(28%) of total 18 area samples exceeded the Korean standard. Considering processes, all the samples in weaving process exceeded the Korean standard and 50 did 54% of those in spinning, 40% in carding, and 27% in twisting. 5. Trend of decreasing asbestos concentrations in asbestos textile plants was observed by time. 6. Asbestos concentrations in asbestos textile plant in 1975 were estimated to be 11.0 - 92.4 f/cc.
