Safety can be defined as being maintained or reduced to a level below which the possibility of human or physical harm can be tolerated through continuous identification of risks and safety risk management. FAA, EASA, IATA and Boeing, major organizations that conduct research and analysis for aviation safety around the world, report that about 70 percent of aviation accidents are caused by human factors, which have led to a surge in interest in human factors-induced accident prevention activities around the world. As part of this purpose, the FAA in the U.S. is raising awareness among aviation workers by publicizing the 12 human errors (Boeing, 2016), which account for the largest part of aviation accidents under the theme of Dirty Dozen, to prevent aviation accidents. Therefore, based on the domestic helicopter accidents reported to the Air Railroad Accident Investigation Committee from 2007 until recently, this study aims to use HFACS to extract human factors for the six recent helicopter accidents in Korea, analyze the extracted human factors in conjunction with the Dirty Dozen concept, and then present measures to prevent accidents by item.
본 연구에서는 서울메트로 2 호선 상왕십리역에서 발생한 전동열차 충돌사고에 대하여 사고재현 시뮬레이션을 수행하고 사고 발생 시 피해를 최소화할 수 있는 개선방안을 제시한다. 사고재현을 위해 상용 충돌해석 소프트웨어인 LS-DYNA를 사용하여 1 차원 및 3 차원 시뮬레이션을 수행하였다. 1 차원 시뮬레이션을 통해 각 차량 연결기의 하중, 변위, 충격흡수 에너지 및 차량의 가속도를 분석하여 사고 시 충돌안전도를 평가하였으며, 3 차원 시뮬레이션을 통해 차량의 변형 및 타고오름 현상을 분석하였다. 또한 사고 전동차의 충돌안전도 향상을 위한 개선방안들을 제시하고, 그 중 선두차량에 고용량 완충장치를 적용하는 방안에 대하여 1 차원 시뮬레이션을 통해 충돌안전도의 개선 여부를 확인하였다.
In this study, the causes of human error were identified through the survey of the drivers of the three organizations: Seoul Metro, Seoul Metropolitan Rapid Transit Corporation, and Korail. It was started with the aim of finding and eliciting causes in various directions including human factors, job factors, and environmental factors. The Cronbach alpha value was 0.95 for the reliability significance of the stress-induced factors in the operational area. The significance probability for organisational factors was shown to be 0.82, and the significance of the sub-accident experience and the driving skill factors in operation was 0.81 In addition, the analysis results showed that stress-induced in the field of driving is higher than the human factors in the reliability analysis. The results of the analysis confirmed that the reliability of the organizational and operational stress-induced factors was higher than other causes. In order to reduce urban railroad accidents, this paper suggests a method for operating safe urban railroad through the minimization human errors.
Turnout is one of railroad track components. The turnout is used to change train routes to near other sections when keeping the train speed. There are many accidents in the section including the turnout. The number of accidents and errors is 363 for ten years in the section including the turnout and annual average is 36.3. To ensure safety of the turnout, it is necessary to install monitoring systems. By using the monitoring systems, it is able to accumulate related information, develop maintenance and take a step for accidents in advance. Hence, The number of accidents and errors is decreased in the section including the turnout. So, when new turnout is installed, it is needed to use monitoring systems. Therefore, combined turnout management system is needed in Korea. It is possible to perform efficient maintenance because real-time monitoring system by using sensors and data-base management system. In this paper, accidents in the section including domestic turnout are analyzed and foreign combined turnout management system is presented. Hence, articles for installing the combined turnout management system in Korea are proposed.
This study proposed and examined a model in which perceived organizational support and leader-member exchange are linked to safety communication safety commitment, and accidents. In addition, a moderating effect of role overload in the relationship between safety communication and safety commitment and that of performance monitoring in the relationship between safety commitment to accidents. Data were collected from 193 workers in a railroad company. The results indicated that the goodness of fitness of the proposed model was acceptable. More specifically, both perceived organizational support and leader-member exchange were significantly related to safety communication and safety communication was, in turn, significantly related to safety commitment which was ultimately related to accidents. Also, the moderating effects of both role overload and performance monitoring were found. Implications of these findings for safety and the possible roles of variables in the organizational level are discussed.
Purpose - This thesis is to develop a management plan and checklist by analyzing the prevention of accidents in advance by presenting the management plan for the major causes of railway accidents. Research design, data and methodology - In recent 5 years, we have analyzed the cases of railway accident, presented the management plan for the accident, and made a practical safety checklist focusing on the main measures according to the management plan. Results - The analysis of the cases of near-railway accidents suggests more concrete and practical safety management measures because the similar accidents are continuously occurring due to formal safety management. Conclusions - It is more valuable to apply to the adjacent construction of the railway line by creating a detailed checklist based on cases rather than the existing checklist. This study is written only as a human factor. For future real - time safety management, it is necessary to study more precisely cause analysis and safety equipment as a big data - based safety control system for more systematic safety management.
The purpose of this study is to investigate the influence of perceived job insecurity, organizational commitment, and social climate on safety behavior and accidents. The research on this subject was conducted in the Korean National Railroad. The cross-level model was hypothesized to study causal relations among these variables. The cross level model combines individual level approach with group level approach. In this model supervisors(group level) affect organizational commitment(individual level) and at the same time safety climate(group level) influences safety behavior(individual level) positively. Traditionally operators have been blamed as accident causer. This study, however, shows that organizational and systematic factors are as much critical factors determining safety behavior and accidents.
Railway works on or near the line, or trackside works, typically have high potential of accidents, as the works have to be frequently performed during the train operation. In order to reduce the risks associated with the railway trackside works effectively, a systematic risk assessment must be performed considering the hazards that contribute to the accidents. In this study, hazards are identified for railway trackside works by analysing data from Korea Railroad for accidents that have occurred between the year of 2000 and 2006. The hazard list produced in this study is expected to be effectively used for the risk assessment of the trackside works.
Accidents at level crossing have large portion on train accidents, and causes economical loss by train delay and operational interruption. Various safety equipments are employed to reduce the accident at level crossing, but existing warning device, and crossing barrier are simple train-oriented protection equipments. In this paper, intelligent railway level crossing system is proposed to prevent and reduce accidents. For train driver's prompt action, image of level crossing and obstacle warning message are continuously provided to train driver through wireless communication in level crossing control zone. Obstacle warning messages, which are extracted by computer vision processing of captured image at level crossing, are recognized by train driver through message color, flickering and warning sound. It helps train driver to decide how to take an action. Meanwhile, for vehicle driver's attention, location and speed of approaching train are given to roadside equipments. We identified the effect of proposed system through test installation at Sea train and Airport level crossing of Yeong-dong line.
Even though the existing train route controlling method, using track circuits, ensures the sufficient number of operation, it still has problems such as discordance between train numbers which was planned for operating and train numbers being operated on the track, and allowing only one train entering for one route. To solve those problems, we study and develop the Smart train route controlling system which uses the real-time informations of train positions. This system enables improve the coefficient of utilization in a certain train route controlling section, and the safety level of train route controlling, but we should ensure satisfying reliability of data about tracks operated by trains. In addition, there is a need to protect accidents caused by erroneous information of train position, by reflecting changes of tracks, for example maintenance, improvement, expansion of tracks. In this paper, we describes data structure of a developed program which required to change CAD files to wiring diagrams and to generate them to data of tracks. And we show the result that the simulator, using the data structure, controls speed and route of trains without problems.
