한약의 도핑 안전성에 대한 고찰 (A Review on Safety of Herbal Medicines for Doping)

  • Objectives: This study aims to review the herbal medicines containing substances banned by World Anti-Doping Agency(WADA), and to consider criteria for the using of herbal medicines. Methods: Using foreign(Pubmed, Cochrane, Embase, Google Scholar, WANFANG, CiNii) and domestic databases (NDSL, OASIS, RISS), we analyzed the content and pharmacokinetics of the prohibited substances in herbal medicines. Results: Herbal medicines containing the prohibited substances proposed in Korea, China, and Japan are Ephedrae Herba, Cannabis Semen, Strychni Semen, Strychni Ignatii Semen, Pinelliae Tuber, Rhizoma Pinelliae, Chelidonii Herba, Papaveris Fructus Deseminatus, Liriopis Tuber, Rehmanniae Radix Recens, Cistanchis Herba, Ponciri Fructus Immaturus, Aurantii Fructus Immaturus, Moschus, Testudinis Plastrum, Otariae Testis et Penis, and Hominis Placenta. Of these, when using Ephedrae Herba, Cannabis Semen, Strychni Semen, Moschus, Strychni Ignatii Semen, and Otariae Testis et Penis, caution is needed in relation to the doping test. Other herbal medicines are considered safe for the doping test. In addition, by analyzing data related to higenamine added to the WADA's prohibited list of doping since 2017, we don't recommend using Nelumbinis Plumula. And in the case of the other herbal medicines containing higenamine, it is considered that care should be taken in doping test depending on the amount of usage. Conclusions: As a result of analyzing the herbal medicines which are in prohibited list about doping, we were able to know the criteria and precautions to use when prescribing the herbal medicines. Further research will be needed about common used herbal medicines and the amount of detection depending on extraction and boiling method.

신선육자환 에탄올 추출물의 항산화 활성 (Antioxidant activities of ethanol extract of Shinsun-yukza-hwan, a Korean medicinal recipe)

  • Objective : The Sinseon-yukza-hwan (SSY), a Korean medicinal formula which includes Radix Rehmanniae Preparata and other medicinal herbs, has long been used for treatment of alopecia and gray hair through oral administration. This study is designed to enhance the utilization of natural materials in hair and scalp-related cosmetics. Possibility of SSY as an antioxidant was examined from its 50% ethanol extract. Methods : The antioxidative capacities were evaluated by determining total phenolic and flavonoid contents, 1,1-diphenyl-2-picryl hydrazyl (DPPH), 2'-azino-bis (3-ethylbenzothiazoline-6-sulfonic acid) (ABTS), reducing power and hydroxyl radical scavenging activity. Results : Total polyphenol and flavonoid contents of SSY were 25.53 mg TE, tannic acid equivalent/g and 18.90 mg RE, rutin equivalent/g, respectively, which correlated strongly its antioxidative activity. The DPPH and ABTS free radical scavenging activities of SSY at $0.1mg/m{\ell}{\sim}5mg/ml$ were ranged from 20% to 85% and 10% to 58%, respectively. Also the hydroxyl radical scavenger activity and reducing power increased in SSY-treated group, which were significantly lower in SSY-compared to BHA-treated group. But the highest reducing power was shown as 79% from SSY-treated group, which was higher value than 65% from BHA-treatment. These results showed that SSY extract effectively inhibited the generation of free radicals in the all assay system with dose-dependent manners. Conclusions : Thus, the present study provide preclinical data to support the expanded application of SSY, which could be potential candidates for natural antioxidants.

항암화학요법 유발 말초신경병증의 한의학적 치료에 대한 국내 연구동향 고찰 (A Review of Domestic Research on Traditional Korean Medicine for Chemotherapy-Induced Peripheral Neuropathy)

  • Objectives: This study was performed to research Korean medical treatment methods for chemotherapy-induced peripheral neuropathy by summarizing domestic studies. Methods: All domestic research papers on the Korean medical treatment of chemotherapy-induced peripheral neuropathy were selected from the literature published until April 2024 in four domestic databases. In total, 26 domestic papers were selected. Results: Eighteen clinical studies and eight experimental studies were published by April 2024. Most of the studied patients were in their fifties, and 93.4% of the subjects were women. Uchashinki-hwan (牛車腎氣丸) was the most frequently prescribed herbal formula, and Rehmanniae Radix Preparata was the most commonly used herb. All studies showed significant effects of Korean medical treatment, and any adverse events were not reported. Conclusion: This study included all domestic research papers on chemotherapy-induced peripheral neuropathy to identify research trends in the Korean medicine community. Based on this, it is meaningful to confirm areas that need to be supplemented in future research plans. However, large-scale studies are needed.

증액탕(增液湯) 물추출물이 LPS로 유도된 RAW 264.7 cell에서의 Nitric Oxide 및 Cytokine에 미치는 영향 (Inhibitory Effect of Jeungaektang Water Extract on Nitric Oxide and Cytokine Production in Lipopolysaccharide - activated RAW 264.7 Cells)

  • Jeungaektang (JAT) is the herbal formula, has the effect of moistening the dryness by activating lung Qi and by nourishing Yin, has being used for dryness syndromes. Generally the herbal formulae for moistening dryness are used for exogenous or endogenous dryness syndromes. JAT has been clinically used for the treatment of endogenous dryness syndromes. It is composed of Scrophulariae Radix. Rehmanniae Radix and Liriopis Tuber. Recent studies showed that JAT has a protective effect against $CCl_{4}-induced$ hepatotoxicity and anti-inflammatory effects against ear swelling of mouse induced by Crotonis Fructus. However, the effect of JAT on the immunological activity was rarely studied. Therefore, this study evaluated the effects of JAT the regulatory mechanism of nitric oxide (NO) and cytokines in the lipopolysaccharide (LPS)-stimulated RAW 264.7 cells. After the treatment of JAT water extract, cell viability was measured by MTT assay, NO production was monitored by measuring the nitrite content in culture medium. Cyclooxygenase-2 (COX -2) and inducible nitric oxide synthase (iNOS) were determined by immunoblot analysis, and levels of cytokine were analyzed by sandwich immunoassays. Results provided evidence that JAT inhibited the production of nitrite and nitrate ($0.1{\sim}1.0$ mg/ml), iNOS ($0.1{\sim}1.0$ mg/ml), $interleukin-1{\beta}$ ( $0.1{\sim}1.0$ mg/ml) and tumor necrosis $factor-{\alpha}$ ($0.1{\sim}1.0$ mg/ml) in RAW 264.7 cells activated with LPS. Furthermore, JAT inhibited the expression of COX-2 expression and production of prostagladin E2 ($0.1{\sim}1.0$ mg/ml). These findings suggest that JAT can produce anti-inflammatory effect, which may play a role in adjunctive therapy in Gram-negative bacterial infections.

수종 한약제의 위암세포에 대한 항암작용 효능 검색 및 약리작용에 관한 분자생물학적 연구 (Molecular analysis on the pharmacological actions of oriental medicines in stomach cancer cells)

  • Traditional oriental medicines have been used for treatment of various kinds of human cancers for long times and some of them proven to be effective clinically. However, the pharmacological actions and mechanisms related to cancer treatment are generally unknown. In an effort to clarify the action mechanisms of several oriental medicines used for cancer treatments, we planned this experimental procedures. We selected Cordyceps sinensis (冬蟲下草), Punellae Herba (夏枯草), Rehmanniae Radix (熟地黃), Paeoniae Radix (白芍藥), Oldenlandiae Herba (白花蛇舌草), Partulaceae Herba (馬齒? ), Scdopendra subspinipes mutilans (蜈蚣), Mylabris Phalerara (班蟄), Phellinus igniarius(桑黃), Ganodermae Lignum(靈芝) for evaluation, which have been used for patients of gastric cancers. The twenty grams of medicines were boiled in 100ml of water for 1 hour and filtered with $0.2\;{\mu}m$ pore-sized filter unit to remove insoluble particles. Initially we evaluated the effects oriental medicines on growth inhibition in stomach cancer cells. The gastric cancer cell line, AGS, was cultured in RPMI 1640 supplemented with l0% heat-inactivated fetal bovine serum and treated with $10{\mu}l$ oriental medicines per 1ml of medium up to 48 hours. The specimens were subjected to MTT assay for evaluation of growth inhibition. We found mat Mylabris phalerata (班蟄) markedly suppressed the growth of cancer cells as shown in results. Next, we checked the effects of oriental medicines on cancer on cell cycles and apptosis. Mylabrls phalerata (班蟄) inhibited cell cycle progression of cancer cells a compared with control cells and cells treated with other medicines. In addition, Mylabri phalerata (班蟄) induced apoptosis in 30-40% of stomch cancer cells within 24 hours. Other oriental medicines used for this experiments did not show apoptosis-inducing effects on cancer cells. Finally, we determined the expression levels of genes associated with cell cycle and apoptosis. The expressions of Bcl-2 and bcl-XL were downregulated by the treatment of Mylabris phalerata (班蟄). However, the expression levels of genes related to cell cycles were not altered significantly. In conclusion, we found that Mylabris phalerata (班蟄) has in vivo gowth-inhibiting and apptosis-inducing effects on stomach cancer cells. However, we think that at least animal experiments are necessay for evduations.

  • In this thesis, I intend to study the translational and clinical interpretation through the syndrom of "Pyong-Yeol-Byong", and reached the following conclusions. 1. Eum-Yang-Kyo (陰陽交)' 1) Meaning: "Eum" means "Essential and vital energy" "Yang" means "Evil factor affecting health" and "Kyo" means "cross-struggle." 2) Location of disease: Heat evil enter Hyeol-Bun (血分) 3) Pathogenesis: Heat evil invade Eum-Bun (陰分) and struggles with Health energy, therefore Eum-Chung (陰精) is exhausted and Heat-evil doesn't disapper, it damage Eum and exhaust fluid. Reach fever, rapid pulse raving and unable to take meal, not controled by sweating and sceach death. 4) Particularity of Syndrome: Heat enter Hyol-Bun, and Evil factor is enough and Health energy is insufficient, so that reveal the symptoms of high fever, delirium with coma, unable to take meals. 5) Therapy: It clears Gi-Bun heat evil (氣分熱邪) by Gypsum, Rhizoma Anemarrhenae, Flos Lonicerae, Fructus Forsythiae, Fructus Gardeniae, Radix Scutellanae Rhizoma Coptidis, and cools Blood by Cornu Rhinoceri Asiatici, Radix Rehmanniae, Cortex Moutan Radicis, Dae-Chung-Yob (大靑葉) Radix Arnebiae Seu Lithospermi. 2. Poong Gweol (風厥) 1) Meaning: Poong means wind-evil, Gweol means reversing up. 2) Location of disease: Disease complexes with TaeYang (太陽) in outer part, and with So-Eum (少陰) in inner part. 3) Pathogenesis: Tae-Yang-Gyeong (太陽經) accept wind-evil and So-Eum-Gyeong (少陽經) Kidney Energy reverse up so that fidgetiness not resolves by sweating. 4) Particularity of Syndrome: There are outer symptoms of fever, hydrosis with inner symptoms of fidgetinessis. 5) Therapy: Reduce Jok-Tae-Yang (足太陽) and Supply Jok-So-Eum (足少陰) by accupuncture, so cure Poong Gweol and make balance between Yeong (營) and Wi (衛). 3. Scrofula coused by wind-evil (勞風) 1) Meaning: It means accepting wind evil rest less. 2) Location of Disease: It locates lung 3) Pathogenesis: Because of accepting wind-evil restless, he take scrofula with damaging lung. 4) Particularity of disease: It is lung disease of aversion to wind and shiver, nape-stiffiness, dim eyesight, cough, disphea, vomitting sputum, if one camnot vomit sputum, he died by damage of lung. 5) Therapy: The period of therapy is different by age or strength of health energy, so I think must prevent Eum deficiency and clear fever no reduced in lung. 4. Shin-Poong (腎風) 1) Meaning: It means taking edema by accepting wind-evil, because the kidney controls water. 2) Location of Disease: It is that wind-evil envade kidney. 3) Pathogenesis : Water evil of kidney with wind-heat rises up to face, reach edema, puffines s of the lower eyelid, floating pulse, bombus, yellowish urine, hydrosis and hand-heating, drymouth and excessive thirsty, walkless by heaviness, menstrual disfunction, restless and unable to take meals, unable to lie flat, heavy cough if lie flat, and accepting wind-evil by deficiency of kidney function, so the function of dredging the water passage is not smooth, symptom of water and symptom of wind reveal together. 4) Therapy: Remove wind-heat, promote diuresis to eliminate tile wetness-evil, supplement the dificiency of kidney's Eum. Finally, we can know that later Fever Disease Medicime (溫病學) is affected to the theory of "Pyong Yeol Byong" in 33th Chapter of SoMoon (素問).

한약제제(액제)의 안전성 평가 (Safety Assessment of Liquid-Type Herbal Preparations)

  • The safety of liquid-type herbal preparations (47 samples) and beverages containing herbal medicines (18 samples), which are distributed in Gyeonggi province was studied by analyzing heavy metals, benzo(a)pyrene, preservatives and sugar content and to explain the differences between medicine and food based on the labeling standards. The herbal preparation is regarded as medicine and can be differentiated from the beverages containing herbal medicine, which is food, based on the fact that the herbal preparation should be labeled with usage, dosage, efficiency and effect. The mean concentrations of Pb, As, Cd, Hg were found to be 0.014 mg/kg, 0.113 mg/kg, 0.004 mg/kg and 0.003 mg/kg in herbal preparation and 0.009 mg/kg, 0.122 mg/kg, 0.003 mg/kg and 0.002 mg/kg in beverages containing herbal medicines, respectively. In 10 cases of herbal preparations and 6 cases of beverages containing Rehmanniae Radix Preparata (Sukjihwang), benzo(a)pyrene was not found. According to preservative test on herbal medicine, dehydroacetic acid was found less than the indicated content in 6 cases out of 37 cases, which labeled with preservatives, and benzoic acid, dehydroacetic acid and methylparaben were contained with 164.0~198.0 mg/kg, 149.8~272.5 mg/kg and 88.4 mg/kg, respectively, in 3 cases out of 10 cases, which was not labeled with preservatives. Among 16 cases of beverages containing herbal medicines, 3 cases of preservative-labeled beverages had the preservatives, and the rest of all, which was not labeled with preservatives, did not show that they contain any preservative, therefore, it was suitable for the labeling standards. As a result of sugar content test, preservative-labeled products were not appreciably different from the others.

흑삼을 함유한 한방소재 화장품의 주름개선 효과 (Anti-wrinkle Effect of Oriental Medicine Cosmetics Containing Black Ginseng)

  • 맥문동, 차가버섯, 감초, 보기보혈(백출, 백복령, 당귀, 숙지황, 천궁, 작약)의 한방약재에 ginsenoside Rg3가 강화된 흑삼을 첨가한 토너와 로션을 제조하여 주름개선 효과를 평가하였다. 평균연령 42.5세의 한국인 중년여성 6명을 대상으로 토너사용 후 로션사용으로 매일 2회씩(아침, 저녁) 4주간 연속 사용하도록 한 다음, 실험 4주 후에 눈가 주름의 모사판을 제작하여 영상 분석하였다. 또한 연구책임자가 시료의 피부 안전성을 관찰하기 위해 육안 검사도 함께 수행하였다. 눈꼬리 주름 모사판 영상분석 결과, 토너와 로션 사용 4주 후 주름 R1은 $3.15{\pm}0.35$에서 $2.28{\pm}0.22$, R2는 $2.99{\pm}0.37$에서 $2.13{\pm}0.21$, R3는 $2.09{\pm}0.31$에서 $1.56{\pm}0.18$, R4는$0.37{\pm}0.05$에서 $0.29{\pm}0.03$, R5는 $0.89{\pm}0.1$에서 $0.75{\pm}0.06$로 유의하게 감소하여(P<0.05), 본 시료는 주름개선에 효과 있는 것으로 확인되었다.

한방화장품의 눈가 주름 개선 효과 (The effect of enhancing eye-wrinkle applying traditional herb medicine cosmetics)

  • 맥문동, 차가버섯, 감초, 흑삼, 백출, 백복령, 당귀, 숙지황, 천궁, 작약의 추출물을 이용한 한방소재로 에센스와 크림을 제조하여 눈가의 주름개선 효과를 평가하였다. 평균연령 44.8세의 한국인 중년여성 6명을 대상으로 에센스 사용 후 크림 사용으로 매일 2회씩(아침, 저녁), $1cm^2$당 2mg씩, 4주간 연속 사용하도록 한 다음, 4주 경과 후 눈가 주름의 모사판을 제작하여 영상 분석하였다. 눈꼬리 주름 모사판 영상분석 결과, 에센스와 크림 사용 4주 후 주름 R1은 $3.22{\pm}0.85$에서 $2.38{\pm}0.32$, R2는 $3.08{\pm}0.83$에서 $2.22{\pm}0.32$, R3는 $2.29{\pm}0.64$에서 $2.29{\pm}0.64$, R4는 $0.44{\pm}0.14$에서 $0.44{\pm}0.14$, R5는 $0.98{\pm}0.21$에서 $0.98{\pm}0.21$로 유의하게 감소하여(P<0.05), 본 시료는 눈가주름개선에 효과 있는 것으로 평가되었다.

아토피 피부염의 한약 치료에 대한 무작위대조군연구 중심의 연구 동향 - CNKI를 중심으로 (Research Trends of Randomized Controlled Trials on Herbal Medicine Treatment for Atopic Dermatitis-Focusing on CNKI)

  • The purpose of this study was to analyze the research trends of randomized controlled trials on herbal medicine treatment for atopic dermatitis in China for the last 5 years. We searched for randomized controlled trials with the intervention of herbal medicine for the treatment of atopic dermatitis in the CNKI (China National Knowledge Infrastructure) from January 2014 to December 2018. For the screening of the paper, we used '特应性皮炎' and '异位性皮炎' which mean atopic dermatitis and search was limited to three areas within Medicine & Public Health: Traditional Chinese Medicine, Traditional Chinese Medicinal Herbs, Combination of Traditional Chinese Medicine with Western Medicine. Among the 136 searched studies, we selected a total of 34 studies and analyzed a year of publication, subject characteristics, study design and intervention, prescribed herbal medicine and herbs, pattern identification, evaluation criteria, and outcomes. Longmu decoction (龍牡湯) and Polia Sclerotium (茯笭) was the most frequently prescribed medicine and herb. The most commonly used pattern identification was Blood deficiency and Wind-dryness (血虛風燥), and among them, the most frequently prescribed herb is Rehmanniae Radix (生地黃). In most studies using the total effectiveness and SCORAD index as an outcome measure, the herbal medicine treatment group showed statistically better results than the control group. As a result of the safety assessment, the herbal medicine treatment group was reported having significantly fewer side effects compared to the control group. Hence, it was confirmed that the intervention including herbal medicine had a significant effect on atopic dermatitis. This study would be able to provide the basis of clinical research on atopic dermatitis and applied to the treatment of atopic dermatitis.
