Assessment of panoramic radiography as a national oral examination tool: review of the literature

  • Purpose : The purpose of this review is to evaluate the possibility of panoramic radiography as a national oral examination tool. Materials and Methods : This report was carried out by review of the literatures. Results : Panoramic radiography has sufficient diagnostic accuracy in dental caries, periodontal diseases, and other lesions. Also, the effective dose of panoramic radiography is lower than traditional full-mouth periapical radiography. Conclusion : Panoramic radiography will improve the efficacy of dental examination in national oral examination. However, more studies are required to evaluate the benefit, financial cost, and operation time and also to make selection criteria and quality management program.

  • Spatial resolution is determined by the performance of x-ray optics used in the x-ray imaging system. A zone plate was designed for obtaining a high spatial resolution image at x-ray energy of 8.5keV. A spatial resolution of 80 nm was estimated by the ray tracing when an x-ray tube of tungsten targe was used instead of synchrotron radiation. The designed zone plate of outermost zone width of 40nm was successfully fabricated by the electron-beam lithography.

  • The neutron capture spectrum for the light nuclide was very useful to study the nuclear structure. In the present study, the capture gamma-ray from the 27-keV resonance of $^{19}F(n,g)^{20}F$ reaction were measured with an anti-Compton NaI(Tl) spectrometer and the 3-MV Pelletron accelerator of the Research Laboratory for Nuclear Reactors at the Tokyo institute of technology. A neutron Time-of-Flight method was adopted with a 1.5 ns pulsed neutron source by the $^7Li(p,n)^7Be$ reaction. In the present experiment, a Teflon(($CF_2$)n) sample was used The sample was disk with a diameter of 90mm. The thickness of sample was determined so that reasonable counting rates could be obtained and the correction was not so large for the self-shielding and multiple scattering of neutrons in the sample, and was 5mm. The primary gamma-ray transitions were compared with previous measurement of Kenny.

The angulation and the position change of the planned implant after tomographic imaging (단층촬영상에서 계획된 임플랜트 매식 각도 및 위치에 대한 스텐트 핀의 상대적 각도 및 위치에 대한 평가)

  • Purpose: To measure the differences of the splint pin angulation and the position of the planned implant site after conventional tomographic analysis. Materials and Methods: The angulation and the location of the metal splint pin retained in acrylic stent were compared with the corrected angulation and the location of the implant fixture on the 331 tomographic images. Results: The stent pins were located buccal in 40%, lingual in 10% to the corrected implant site after analysis of the conventional tomographic image. The angle and the location of the maxillary splint pin were mainly directed buccal on incisor and canine regions. The angle and the location of the splint pins in premolar and molar regions needed less corrections in both maxilla and mandible. Conclusions: This study demonstrated that the use of tomographs was essential for successful dental implant planning.

  • In case of a CT examinations, there is a difference in the distribution of radiation dose from that of general X-ray equipments, and it has been known to cause a great radiation exposure during the examinations. However, owing to its high reliability on the accuracy of a examinations result, its use has increased continuously. In consideration of such a circumstance, the CT equipment, radiation dose during CT examinations, diagnostic reference level, and solutions to reduce radiation dose were mentioned on the basis of previously reported data.

Reliability of spiral tomography on the alveolar crest (나선형 단층 방사선사진에서 치조정 판독 신뢰도)

  • Purpose: To evaluate the reliability of measurements in spiral tomography through assessing the visibility of the alveolar crest and the measurements between the alveolar crest and other anatomic structures. Materials and Methods: 110 spiral tomograms of the jaws were taken by Scanora X-ray unit from the patients. The visibility of the alveolar crests was estimated by 3 observers and classified as clearly visible, questionable visibility, or not visible. 3 observers measured the distance between the alveolar crest and the reference points of anatomic structures. The measurements were repeated 2 weeks later. Results: 52.9% of alveolar crests on upper jaws and 61.5% of alveolar crests on lower jaws were visible. The interobserver and intraobserver agreements on the visibility were low. The mean ranges of the measurements were 1.39 mm (SD = 1.37 mm) on maxilla and 1.03 mm (SD = 1.01 mm) on mandible in the interobserver evaluation. The interobserver variance was greater than the intraobserver variance in the measurements of distance. Conclusion: Spiral tomography showed a relatively low reliability in the visibility and measurements of the alveolar crest.

Review of nomenclature revision of fibro-ossous lesions in the maxillofacial region (악안면부의 섬유골성 병소 명칭에 대한 고찰)

  • Fibro-osseous lesions are composed of connective tissue and varying amount of mineralized substances, which may be bony or cementum-like structures. It is necessary for oral radiologist to differentiate due to the tendency of these fibro-osseous lesions to show similar histopathologic appearances, while the management of each lesion is different. However we often encounter a little difficulty in judgement because there are some overlaps between concept of each lesions. So recently I suggest, we face a need to review basic concept and classification of several fibro-osseous jaw lesions. In this article, several fibre-osseous lesions, such as fibrous dysplasia, cemento-ossifying fibroma and cemento-osseous dysplasia, will be discussed basing on the review of literature. particular emphasis will be made on the nomenclature revision of WHO's classification in 1992.

Dental PACS development in Korea

  • Picture archiving and communication system (PACS) is an image information technology system for the transmission and storage of medical images. In Korea the first full PACS was installed at Samsung Medical Center in 1994, but, the rate of distribution was very slow. The government's approval for the medical insurance reimbursement for full PACS examinations in November 1999 became the turning point. Thereafter the number of hospitals with full PACS has steeply increased. In September of this year, PACS was installed at 906 medical institutes, including most of university hospitals and general hospitals. The first full dental PACS was installed at Wonkwang University Dental Hospital in 2002. Now ten out of eleven university dental hospitals implemented full dental PACS. The current status and technological factors of dental PACS in Korean university dental hospitals and the future perspectives of dental PACS are described. Multilocular developmental salivary gland defect

  • Developmental salivary gland defect is a bone depression on the lingual surface of the mandible containing salivary gland or fatty soft tissue. The most common location is within the submandibular gland fossa and often close to the inferior border of the mandible. This defect is asymptomatic and generally discovered only incidentally during radiographic examination of the area. This defect also appears as a well-defined, corticated, unilocular radiolucency below the mandibular canal. Although it is not uncommon for this defect to appear as a round or ovoid radiolucency, multilocular radiolucency of these defects is relatively rare. This report presents a case of a developmental salivary gland defect with multilocular radiolucency in a male patient.

MRI findings of a huge cystadenocarcinoma of the palate (구개에 발생한 거대 낭샘암종의 MRI 소견)

  • Cystadenocarcinoma of the salivary glands is a very rare, slow growing, and low-grade malignant neoplasm. It is characterized by predominantly cystic growth with or without the intraluminal papillary component. However, it lacks of any additional specific histopathologic features that characterize other types of salivary carcinomas showing cystic growth. Therefore, definite diagnosis of the cystadenocarcinoma is difficult and it is often misdiagnosed. It is conceptually the malignant counterpart of the benign cystadenoma. We present a cystadenocarcinoma on the palate of a 49-year-old man with special emphasis on magnetic resonance imaging.
