Analysis on the Entrance Surface Dose and Contrast Medium Dose at Computed Tomography and Angiography in Cardiovascular Examination (심장혈관검사에서 전산화단층검사와 혈관조영검사의 입사표면선량 및 조영제 사용량에 관한 분석)

  • This study aimed to identify dose reduction measures by retrospectively analyzing the entrance surface dose at computed tomography and angiography in cardiovascular examination and to contribute the patients with renal impairmend and a high probability of side effects to determine the inspection's direction by measuring the contrast usages actually to active actions for the dose by actually measuring the contrast medium dose. The CTDIvol value and air kerma value, which are the entrance surface doses of the two examinations, and the contrast medium dose depending on the number of slides were compared and analyzed. This study was conducted in 21 subjects (11 males; 10 females) who underwent Cardiac Computed Tomographic Angiography (CCTA) and Coronary Angiography (CAG) in this hospital during the period from May 2014 to May 2016. The subject's age was 48~85 years old (mean $65{\pm}10$ years old), and the weight was 37.6~83.3 kg (mean $63{\pm}6kg$). Dose reduction could be expected in the cardiovascular examination using CCTA rather than in the examination using CAG. In terms of contrast medium dose, CAG used a smaller dose than CCTA. In particular, as the number of slides increases at CAG, the contrast medium dose increases. Therefore, in order to reduce the contrast medium dose, the number of slides suitable for the scan range must be selected.

Correction of Spondylolisthesis by Lateral Lumbar Interbody Fusion Compared with Transforaminal Lumbar Interbody Fusion at L4-5

  • Objective : In an aging society, the number of patients with symptomatic degenerative spondylolisthesis (DS) is increasing and there is an emerging need for fusion surgery. However, few studies have compared transforaminal lumbar interbody fusion (TLIF) and lateral lumbar interbody fusion (LLIF) for the treatment of patients with DS. The purpose of this study was to investigate the clinical and radiological outcomes between TLIF and LLIF in DS. Methods : We enrolled patients with symptomatic DS at L4-5 who underwent TLIF with open pedicle screw fixation (TLIF group, n=41) or minimally invasive LLIF with percutaneous pedicle screw fixation (LLIF group, n=39) and were followed-up for more than one year. Clinical (visual analog scale and Oswestry disability index) and radiological outcomes (spondylolisthesis rate, segmental sagittal angle [SSA], mean disc height [MDH], intervertebral foramen height [FH], cage subsidence, and fusion rate) were assessed. And we assessed the changes in radiological parameters between the postoperative and the last follow-up periods. Results : Preoperative radiological parameters were not significantly different between the two groups. LLIF was significantly superior to TLIF in immediate postoperative radiological results, including reduction of spondylolisthesis rate (3.8% and 7.2%), increase in MDH (13.9 mm and 10.3 mm) and FH (21.9 mm and 19.4 mm), and correction of SSA ($18.9^{\circ}$ and $15.6^{\circ}$) (p<0.01), and the changes were more stable from the postoperative period to the last follow-up (p<0.01). Cage subsidence was observed significantly less in LLIF (n=6) than TLIF (n=21). Fusion rate was not different between the two groups. The clinical outcomes did not differ significantly at any time point between the two groups. Complications were not statistically significant. However, TLIF showed chronic mechanical problems with screw loosening in four patients and LLIF showed temporary symptoms associated with the surgical approach, such as psoas and ileus muscle symptoms in three and two cases, respectively. Conclusion : LLIF was more effective than TLIF for spondylolisthesis reduction, likely due to the higher profile cage and ligamentotactic effect. In addition, LLIF showed mechanical stability of the reduction level by using a cage with a larger footprint. Therefore, LLIF should be considered a surgical option before TLIF for patients with unstable DS.

  • This study compared the radiation transmission and image quality of polymethylmethacrylate (PMMA), polycarbonate (PC), and carbon, which are common components of the compression plates currently used during breast imaging. In addition to measuring the transmitted dose and the intensity without the use of a compression paddle, the four different compression paddles were evaluated according to the material and thickness of each paddle. Radiation transmittance, maximum intensity, and plot profile type w ere all evaluated for each material, and for each factor evaluated the follow ing order w as noted, from best to w orst: carbon 4 mm, PMMA 3 mm, PMMA 4 mm, and PC 4 mm. It is necessary to study a variety of materials and thicknesses in order to find the optimal combination of material and thickness, because not only does the material have a large influence in reducing the radiation exposure during mammography, but the thickness of the compression plate also has a great influence.

  • The conventional types of X-ray generators were bulky in physical size and heavy in weight, and the control accuracies of the output voltages were not always satisfactory. The high frequency switching inverter and converter technology on power conversion and control systems have been greatly closed up introducing power semiconductor devices. To decreasing the volume and the weight of high voltage transformer, and to stabilize ripple. In this paper, the newly developed x-ray generator in a low cost using duty modulation PWM inverter. This system verify improved performance by stabilize ripple of X-ray tube voltage and compared the reproducibility, linearity and dose in single phase, three phase and PSU.

  • This study intended to investigate the optimum conditions for lumbar test that has the highest level of irradiation conditions among general test sites. To to this, the most widely used irradiation conditions in terms of statistics were set as standards; test groups applied with DMF were selected; tests groups suitable for clinical trials were selected by using suggested patient dose. Blind tests were conducted by 10 specialists and radiologists. The results suggested that under the optimum conditions, the radiation dose reduction of 2.09 mGy, 4.42 mGy and 3.65 mGy can be achieved in forward-backward test, lateral test and 4-direction test, respectively. There is a need of further studies on the optimization of irradiation conditions in accordance with the conditions of patients.

Radon Removal Efficiency of Activated Carbon Filter from Coconut (코코넛 기반 활성탄 필터의 라돈 제거 효율)

  • The Korea Institute of Radiological and Medical Sciences plans to produce 225Ac, a therapeutic radio-pharmaceutical for precision oncology, such as prostate cancer. Radon, a radioactive gas, is generated by radium, the target material for producing 225Ac. The radon concentration is expected to be about 2000 Bq·m-3. High-concentration radon-generating facilities must meet radioactive isotope emission standards by lowering the radon concentration. However, most existing studies concerning radon removal using activated carbon filters measured radon levels at concentrations lower than 1000 Bq·m-3. This study measured 222Rn removal of coconut-based activated carbon filter under a high radon concentration of about 2000 Bq·m-3. The 222Rn removal efficiency of activated carbon impregnated with triethylenediamine was also measured. As a result, the 222Rn removal amount of the activated carbon filter showed sufficient removal efficiency in a 222Rn concentration environment of about 2000 Bq·m-3. In addition, despite an expectation of low radon reduction efficiency of Triethylenediamine-impregnated activated carbon, it was difficult to confirm a significant difference in the results. Therefore, it is considered that activated carbon can be used as a radioisotope exhaust filter regardless of whether or not Triethylenediamine is impregnated. The results of this study are expected to be used as primary data when building an air purification system for radiation safety management in facilities with radon concentrations of about 2000 Bq·m-3.

Clinical and Radiological Analysis and Results after Anatomical Reduction and Bone Graft for Symptomatic Os Subfibulare (증상을 동반한 비골하 부 골에 대한 임상적 방사선학적 분석과 해부학적 정복술 및 골 이식술 후 결과)

  • Purpose: To analyze the clinical and radiological feature of Os subfibulare and to evaluate the results after anatomical reduction and internal fixation with bone graft for Os subfibulare. Materials and Methods: Forty-two cases, which underwent anatomic reduction and bone graft for Os subfibulare from October 1998 to September 2004 were reviewed. We analysed preopertive symptoms and onset of symptoms and radiologically measured the size and amounts of displacement of Os subfibulare under the inversion stress. Postoperatively we evaluated the clinical results measured by Hasegawa method and evidence of union. Results: Preoperatively there were only pain around the lateral malleolus in 16 cases, only instability of ankle joint in 3 cases, and pain and instability in 23 cases. The age of symptom onset averaged 23 years(range, 13-38 years). Radiographically Os sufibulare anteriorly located from lateral malleolus were in 40 cases, posteriorly situated in 2 cases. The size of Os subfibulare ranged from $1{\times}4\;mm$ to $8{\times}17\;mm$. In 22 cases of inversion stress view, displacement of the Os sbufibulare averaged $1.5{\pm}1.1\;mm$ (0 to 5 mm). The postoperative clinical results were excellent in 41 cases, poor in 1 case. There were complications of 1 case of irritation of sural nerve, 1 case of nonunion. Conclusion: Anatomic reduction and bone graft is effective treatment method for symptomatic Os subfibulare.

Performance Evaluation of U-net Deep Learning Model for Noise Reduction according to Various Hyper Parameters in Lung CT Images (폐 CT 영상에서의 노이즈 감소를 위한 U-net 딥러닝 모델의 다양한 학습 파라미터 적용에 따른 성능 평가)

  • In this study, the performance evaluation of image quality for noise reduction was implemented using the U-net deep learning architecture in computed tomography (CT) images. In order to generate input data, the Gaussian noise was applied to ground truth (GT) data, and datasets were consisted of 8:1:1 ratio of train, validation, and test sets among 1300 CT images. The Adagrad, Adam, and AdamW were used as optimizer function, and 10, 50 and 100 times for number of epochs were applied. In addition, learning rates of 0.01, 0.001, and 0.0001 were applied using the U-net deep learning model to compare the output image quality. To analyze the quantitative values, the peak signal to noise ratio (PSNR) and coefficient of variation (COV) were calculated. Based on the results, deep learning model was useful for noise reduction. We suggested that optimized hyper parameters for noise reduction in CT images were AdamW optimizer function, 100 times number of epochs and 0.0001 learning rates.

Study on Effective Shielding of Secondary Radiation Generated by High Energy Proton Accelerator (고 에너지 양성자 가속기에서 생성되는 2차 방사선의 효과적인 차폐에 관한 연구)

  • High-energy proton accelerators continue to be increasingly used in medical, research and industrial settings. However, due to the high energy of protons, a large number of secondary radiation occurs. Among them, neutrons are accompanied by difficulties of shielding due to various energy distribution and permeability. So In this study, we propose a shielding method that can shield neutrons most efficiently by using multiple-shielding material used as a decelerating agent or absorbent as well as a single concrete shielding. The flux of secondary neutrons showed a greater decrease in the flux rate when heavy concrete was used than in the case of ordinary concrete, and the maximum flux reduction was observed at the front position when using multiple shields. Multiple shielding can increase shielding efficiency more than single shielding however, As the thickness of the multiple shielding materials increased, the decline in flux was saturated. The mixture material showed higher shielding results than the polyethylene when using boron carbonate.

Effect of the Number of Projected Images on the Noise Characteristics in Tomosynthesis Imaging

  • Purpose: In this study, we investigated the relationship between the noise characteristics and the number of projected images in tomosynthesis using a digital phantom. Methods: The digital phantom consisted of a columnar phantom in the center of the image and a spherical phantom with a diameter of 80 pixels. A virtual scan was performed, and 128 projected images (Tomo_w/o) of the phantoms were obtained. The image noise according to the Poisson distribution was added to the projected images (Tomo_×1). Furthermore, another projected image with additional noise was prepared (Tomo_×1/2). For each dataset, we created datasets with 64 (half) and 32 (quarter) projections by removing the even-numbered images twice from the 128 (fully) projected images. Tomosynthesis images were reconstructed by filtered back projection (FBP). The modulation transfer function (MTF) was estimated using the sphere method, and the noise power spectrum (NPS) was estimated using the two-dimensional Fourier transform method. Results: The MTFs did not change between datasets, and the NPSs improved as the number of projected images increased. The noise characteristics of the Tomo_×1_half images were the same as those of the Tomo_×1/2_full. Conclusions: To achieve a reduction in the patient dose in tomosynthesis acquisition, we recommend reducing the number of projected images rather than reducing the dose per projection.
