An Herbal Medicine Mixture (HM-10) Induces Longitudinal Bone Growth and Growth Hormone Release in Rats

  • To investigate the growth promoting effects of an herbal medicine formulation (HM-10), Sprague Dawley (SD) male rats (3 weeks old) were divided into 3 groups (8 rats/group). The control group was given a daily oral administration of saline, and the treatment groups, HM-1 and HM-2, were given daily administrations of HM-10 (500 and 1,000 mg/kg BW, respectively). The cumulative tibial bone growth of the HM-1 and HM-2 groups (22.5 and 20.8 mm, respectively), and their cumulative femur bone growth (19.4 and 18.2 mm, respectively), were significantly different compared to the control group (7.5 mm of tibial growth and 7.7 mm of femur growth) (p<0.05). Lastly, the growth hormone levels of the HM-1 and HM-2 groups (1.70 and 1.79 ng/mL, respectively), as well as their insulin like growth factor 1 (IGF-1) levels (165.1 and 171.7 ng/mL, respectively) showed significant differences compared to the control (0.93 ng/mL of growth hormone and 125.6 ng/mL of IGF-1) (p<0.05).

Radiologic study of mandibular foramen of mandibular prognathism by three-dimensional computed tomography (3차원 전산화단층영상을 이용한 턱나옴증 환자의 하악공의 방사선학적 연구)

  • Purpose : This study is aimed to evaluate the position of mandibular foramen of mandibula prognathism patients using 3-dimensional CT images in order to reduce the chance of an anesthetic failure of the mandibular nerve and to prevent the damage to the inferior alveolar nerve during the orthognathic surgery. Materials and Methods : The control group consist of 30 patients with class I occlusion. The experimental group consist of 44 patients with class III malocclusion. Three-dimensional computed tomography was used to evaluate the position of the mandibular foramina. Results : The distance between mandibular plane and mandibular foramen, class I was 25.385 mm, class III was 23.628 mm. About the distance between occlusal plane and mandibular foramen, class I was 1.478 mm, class III was 5.144 mm. The distance between posterior border plan of mandibular ramus and mandibular foramen had not statistically significant. About the distance between sagittal plane of mandible and mandibular foramen did not also showed statistically significant. Conclusion : The result of this study could help the clinicians to apprehend more accurate anatomical locations of the foramina on the mandible with various facial skeletal types. thereby to perform more accurate block anesthesia of the mandibular nerve and osteotomy with minimal nerve damage. In addition, this study could provide fundamental data for any related researches about the location of the mandibular foramina for other purposes.

Effect of Field Size on the Clinical Exposure Index for Lumbar Spine X-ray Examination (노출지수를 이용한 요추 X선 촬영의 조사야 유효성 평가)

  • The field size of the lumbar spine X-ray examination, which belongs to the most frequent examination in general radiography, is 5 times wider than the width of the lumbar spine. Exposure index (EI) as per International Electrotechnical Commission has a proportional relationship with the dose incident on the image receptor for clinical protocols in addition to RQA5, which is a calibration beam quality. In this study, the effectiveness of the set field size was evaluated through the change of EI according to the size of field during lumbar spine X-ray examinations. Lumbar anterior-posterior and lateral examinations was performed using a whole-body phantom, and the national average exposure conditions of Korea investigated in 2017 were introduced for the X-ray exposure. As a result of comparing the EI displayed on the console of digital radiography system for the three field size in ① 18 × 36 cm2 ② 25 × 36 cm2 ③ 36 × 36 cm2, the EI values showed a tendency to increase as the field size increased. Since the patient dose, such as organ dose around the lumbar spine, increases as the field size becomes larger, thus, if the EI obtained from the field size at a level that does not interfere with diagnosis is set as a reference, the effectiveness of the field size can be evaluated through the EI displayed on the console when the lumbar spine X-ray examination is conducted.

  • As the number of Coronavirus Disease-19 (COVID-19) patients increases in a global pandemic situation, the usefulness of mobile computed tomography (CT) is gaining attention. Currently, mobile CT follows the basic safety and essential performance evaluation criteria of whole-body or limited-view X-ray CT in order to obtain device approval and evaluation in the Republic of Korea. Unlike stationary CT, mobile CT is not operated in shielded areas but rather areas such as intensive care units, operating rooms, or isolation rooms. Therefore, it requires a different basic safety and essential performance evaluation standard than stationary CT. In this study, four derived basic safety evaluation criteria related to electrical, mechanical, and radiation safety were included (dose indication test, protection against stray radiation, safety measures against excessive X-rays, half-value layer measurement); and seven essential performance evaluation criteria were included (tube voltage accuracy, mAs accuracy, radiation dose reproducibility, CT number of water, noise, uniformity, and spatial resolution); total eleven basic safety and essential performance evaluation criteria were selected. This study aims to establish appropriate basic safety and essential performance evaluation criteria for simultaneously obtaining images with diagnostic value and reducing the exposure of nearby patients, medical staff, and radiologic technologists during the use of mobile CT.

Restructuring Enrollment Quotas of College for Radiological Technologists (우리나라 방사선사 과잉공급 해결방안)

  • This study presented a mediation strategy of admittance quota of four year and three year colleges raising radiology technicians by investigating the demand and supply related literature for Radiological Technologists, a type of health and medical services personnel. Oh Yeong-ho et al.'s study on mid and long term supply system for health and medical services personnel resources, excess supply of 1,134~2,052 Radiological Technologists in 2015 was predicted and excess supply of 12,632~14,196 Radiological Technologists are to be expected in 2030. To settle excess supply of Radiological Technologists, Supernumerary quota over the entrance students of institutions raising Radiological Technologists should be less than 5% to reduce applicants for Radiological Technologists to be 308 people, reduce 387 applicants for radiology technicians every year by unifying three year colleges raising Radiological Technologists with four year colleges into education period unification, evaluate education conditions of colleges to restrict those colleges lacking the minimum requirements from getting eligibility for take the examination for Radiological Technologists by establishing tentatively 'Korean Institute of Radiologic Education and Evaluation.' For this, it is necessary to obtain cooperation from health and medical services personnel organizations in pertinent fields as well as related organizations of Radiological Technologists.

Radiologic study of intraosseous path of the inferior alveolar cantal, mental canal, and mental foramen for endosseous implants (인공치아 매식술을 위한 하악관, 이관, 이공의 골내 주행에 관한 방사선학적 연구)

  • The purpose of this study was to examine the anatomic structures of the mandible-inferior alveolar canal, mental foramen, mental canal-with panoramic radiography and conventional tomography and to compare both radiographic techniques in conjunction with endosseous implants. In this study 14 adult dentulous mandibles -27 cases of right and left side of mandibles- were examined and the results were as follows. 1. The distance between superior border of the inferior alveolar canal and the alveolar ridge crest showed a decreasing tendency from the mental foramen to 4cm posterior to the mental foramen. 2. The mean diameter of the inferior alveolar canal was $4.11{\pm}0.50mm$ with panoramic radiography and $3.29{\pm}0.59mm$ with conventional tomography. 3. The inferior border of the inferior alveolar canal and inferior border of the mandible was closest at 2cm posterior to the mental foramen but it was not statistically significant. the mean distance was $1l.64{\pm}2.95mm$ in panoramic radiography and $1l.68{\pm} 2.91mm$ in conventional tomography. 4. The inferior alveolar canal located lingually in bucco-lingual direction 16%(mental foramen), 54%(lcm posterior to the mental foramen), 68%(2cm posterior to the mental foramen), 50%(3cm posterior to mental foramen), 55%(4cm posterior to the mental foramen). 5. Mean length of the anterior loop of the mental canal was 2.73mm, and the loop below 2mm was 35% and 15% of mental canal was invisible in panoramic radiography. 6. The minimum interforaminal distance was 56.7mm, the maximum distance was 73.2mm and the mean distance was 66.42mm in panoramic radiography. 7. The mean distance between midpoint of the mental canal and alveolar ridge crest was 16.24mm and the mean buccolingual angulation of the mental canal was $52.98^{\circ}$ in conventional tomography. 8. In comparison of panoramic radiography and conventional tomography, inferior alveolar canal is better visualized with conventional tomography than panoramic radiography from the mental foramen to the 2cm posterior to the mental foramen, while visiblity of conventional tomography prominently decreased in 4cm posterior to the mental foramen and alveolar ridge crest is better visualized with panoramic radiography than conventional radiography at the mental foramen and at 4cm posterior to the mental foramen. In radiologic examination of anatomic structures of the mandible for endosseous implants, panoramic radiography and conventional tomography can be effectively used when it is used to overcome the anatomic limitations.

Coracoclavicular Ligament Augmentation Using Tight-Rope® for Acute Acromioclavicular Joint Dislocation - Preliminary Report - (Tight-Rope®을 이용한 급성 견봉 쇄골 관절 탈구의 치료 - 예비 보고 -)

  • Purpose: The purpose of this study is to analyze the results of acute acromioclavicular joint dislocation treatment with coracoclavicular ligament augmentation using Tight-Rope$^{(R)}$ (Arthrex). Materials and Methods: From October 2009 to March 2011, 30 patients with acute acromioclavicular joint dislocation underwent coracoclavicular ligament augmentation using Tight-Rope$^{(R)}$ and were followed up for at least 12 months after surgery. The radiologic results were qualified according to serial plain radiographs, and the clinical results according to University of California - Los Angeles (UCLA) Shoulder Scale, Constant score, and VAS pain score. Results: Using the UCLA scoring system, excellent results were observed in 22 cases (73%), good results in five cases (17%), fair results in two cases (7%), and a poor result in one case (3%). The average Constant score was $92.5{\pm}7.5$. According to radiologic results, anatomical reduction was achieved in 26 cases, and two cases showed a moderate loss of reduction, and two cases showed complete re-dislocation. Clinical results for patients with re-dislocation were unsatisfactory and reoperation was required. Conclusion: Coracoclavicular ligament augmentation using Tight-Rope$^{(R)}$ is a good option providing reliable functional results in patients with acute acromioclavicular joint dislocation.

AN IMMUNOHISTOCHEMICAL STUDY ON CALCIFYING TISSUES OF THE FLORID CEMENTO-OSSEOUS DYSPLASIA EASILY INVOLVING CHRONIC DIFFUSE SCLEROSING OSTEOMYELITIS (만성 미만성 경화성 골수염을 동반한 개화성 백악질-골 이형성증의 면역조직화학적 연구)

  • Florid cemento-osseous dysplasia (FCOD) is a benign, non-neoplastic lesion characterized by multiple sclerosing masses only within jawbones. It is frequently confused with chronic diffuse sclerosing osteomyelitis (CDSO) in previous literatures. In our study, two cases of FCOD were examined to know the characteristics of their calcifying tissues. The first case was non-infected, while the second case was severely infected, displaying the typical features of CDSO in clinico-radiologic findings. The infected FCOD case showed a lot of bacterial colonies in the main lesion with relatively rare inflammatory reaction. The globular cementum-like materials of FCOD showed woven bone pattern and was positive for Alcian blue stain, and also positive for the antibodies of ameloblastin, bone morphogenetic protein (BMP) -2 and -4. On the other hands, in the immunostains of matrix metalloproteinase (MMP) -3, -9, -10, and $TNF-{\alpha}$, macrophage infiltrated in the FCOD lesion was rarely observed. These data suggest that the cementum-like materials of FCOD contain various matrix proteins, and that the cementum-like materials are relevant to the overgrowth of the bacterial colonies by inhibition of the regional inflammatory reactions.

The Effect of X-ray Tube Potential on the Image Quality of Digital Chest Radiography with an Amorphus Silicon Flat Panel Detectors (비정질 평판형 측정기를 이용한 디지털 흉부 방사선 영상에서의 효과적인 관전압 선택)

  • The rapid development in digital acquisition technology in radiography has not been accompanied by information regarding optimum radiolographic technique for use with an amorphus silicon flat panel detector. The purpose of our study was to compared image quality and radiation dose of an amorphus silicon flat panel detectors for digital chest radiography. All examinations were performed by using an amorphus silicon flat panel detector. Chest radiographs of an chest phantom were obtained with peak kilovoltage values of $60{\sim}150kVp$. Published data on the effect of x-ray beam energy on image quality and patient dose when using an amorphus silicon flat panel detector. It is important that radiographers are aware of optimum kVp selection for an amorphus silicon flat panel detector system, particularly for the commonly performed chest examination.

A Case of Scar Sarcoidosis (피부반흔에서 발생한 유육종증 1례)

  • Purpose: Sarcoidosis is a systemic noncaseating granulomatous disease of an unknown origin, and can involve any organ including the skin. The infiltration of sarcoid granuloma in an old cutaneous scar is an uncommon cutaneous manifestation of sarcoidosis. This paper reports a 35-year old female who presented with cutanesous nodules in previous facial scars. Methods: A 35-year-old female presented with cutaneous nodules for 2 months in previous scars of the forehead and lower lip that she had acquired in the childhood. An excisional biopsy of the lower lip mass, serologic examinations and radiologic studies were performed. Results: The excisional biopsy revealed noncaseating granulomas consistent with sarcoidosis. Chest CT revealed both hilar and paratracheal lymphadenopathy with nodular densities in both lung fields. Routine laboratory tests, serologic tests, serum angiotensin converting enzyme level, sputum for acid-fast bacilli, ophthalmoscopic examination, TB-PCR and NTM-PCR showed normal findings. Therefore, the patient was diagnosed with sarcoidosis. The patient refused fiberoptic bronchoscopy and medication with oral steroid. Currently, the clinical manifestation and progress are being monitored closely, and treatment is expected to start with oral steroid according to the progress. Conclusion: For the proper management of cutaneous sarcoidosis, particularly scar sarcoidosis, plastic surgeons should be aware of the many clinical and histopathological features of sarcoidosis and recommend a systemic evaluation for early diagnosis and proper treatment.
