The geometrically standardized intraoral radiographs using 5 occlusal registration materials were taken serially from immediate, 1 day, 2, 4, 8, 12, and 16 weeks after making the bite blocks. And the resultant images were digitally subtracted using the immediately taken film as reference images. The qualities of those subtracted images were evaluated to check the degree of reproducibility of each impression material. The results were as follows: 1. The standard deviations of the grey scales of the overall subtracted images were 4.9 for Exal1ex, 7.2 for Pattern resin, 9.0 for Tooth Shade Acrylic, 12.2 for XCP only, 14.8 for Impregum. the lesser the standard deviation, the better the quality of the subtracted images. 2. The standard deviation of the grey scales of the overall subtracted images were grossly related to those of the localized horizontal line of interest. 3. Exaflex which showed the best subtracted image quality had 15 cases of straight, 14 cases of wave, 1 case of canyon shape. Impregum which showed the worst subtracted image quality had 4 cases of straight, 8 cases of wave, 18 cases of canyon shape respectively.

The purpose of this study was to investigate whether a radiographic estimate of osseous fractal dimension is useful in the characterization of structural changes in bone. Ten specimens of bone were progressively decalcified in fresh 50 ml solutions of 0.1 N hydrochloric acid solution at cummulative timed periods of 5, 10, 20, 30, 60 and 90 minutes, and radiographed from 0 degree projection angle controlled by intraoral parelleling device. The test set of 70 radiographs was digitized and digitally filtered to reduce film -grain noise. I performed one-dimensional variance and fractal analysis of bony profiles or scan lines. Correlation analysis quantified the relationship between variance and fractal dimension. The obtained results were as follow. 1. After the first stage of decalcification variance and fractal dimension of scan line pixel intensities generally decreased with a range of 57.94 to 12.64 and 1.59 to 1.36. 2. Correlation coefficient(r) relating variances to fractal dimensions was consistantly excellent(range r=0.90 to 0.98). 3. Variance and fractal dimension were much alike in ability to discriminate, at leat on a group basis, between control and decalcified specimens.
In spite of relatively low level of radiation dose used at dental clinics, long term exposure may be harmful, so radiation workers at dental clinics must be well aware of its danger. This study was radiation safety management by dental hygienists in order to take preventive measures for dental hygienists and suggest ideas to develop radiation safety training programs. For this, we contacted dental hygienists working at the local dental clinics for 4 months from December of 2003 to march of 2004 and obtained the following findings. 1. Regarding the intraoral radiographic method, the average daily photographing frequency of standard films stood at one to five pieces (47.5%), and the average weekly photographing frequency of digital radiation medicine stood at less than one piece (69.8%), and the average weekly photographing frequency of bitewing films stood at less than one piece (67.7%), and and the average weekly photographing frequency of occlusal films stood at less than one piece (95.5%), and the dentistries whose average weekly photographing frequency of pediatric films stood at one to five pieces accounted for 47.1 percent. 2. Regarding the extraoral radiographic method, the average weekly photographing frequency of panorama film stood at one to five pieces (63.7%), and less than one piece (20.9%), the average weekly photographing frequency of cephalometric film stood at less than one piece (72.3%), and one to five pieces (20.1%). 3. Concerning the radiation safety management training program, only 18.7% of total 278 surveyed attended the training progra., Attendance tendency of the training program by general characteristics showed statistically significant difference according to age (p<0.01), working experience (p<0.001), and marital status (p<0.01). 4. When asked about the protective equipments against radiation exposure, 40.6% of them said "modest", and 71.1% appeared equipped with led apron as a protective tool.
This study was performed to evaluate the sensitivity of conventional film-screen radiography (CFSR) and direct digital radiography (DDR) for detection of various amounts of free peritoneal fluid. Ten adult male healthy beagles were used in this study. Radiographic examinations were performed in the right lateral and ventrodorsal positions. Fluid was injected in increments of 2.0 ml/kg of body weight up to 20.0 ml/kg of body weight. The images of CFSR and DDR were evaluated by two veterinary radiologists for evidence of abdominal fluid without knowledge of injected fluid volume. Data were evaluated by using the receiver operation curve (ROC) analysis and the area under the curve (AUC). There was no significant difference in detection of peritoneal fluid between DDR and CFSR in the ROC analysis. The accuracy of CFSR (0.805) was relatively higher than that of DDR (0.733), based on the ROC analysis and AUC. AUC of CFSR was higher in most injection doses. These results suggest that CFSR is more accurate than DDR for the detecting peritoneal fluid. Therefore, for situation in which digital radiographs are equivocal or small amount of fluid is suspected, other imaging modalities, such as ultrasonography would be helpful for determining the presence of fluids.

TMJ Osteoarthritis is a degenerative disease caused by overload on joint tissue, and often accompany with local tenderness on the joint, crepitus by mandibular movement, restriction of mandibular movement, and anterior openbite. In general, panoramic radiography, TMJ panoramic radiography, and transcranial radiography are conducted to diagnose osteoarthritis after clinical examination, however, these radiographic evaluations are limited in detecting minute bony changes of early pathologic lesion. The aim of this pilot study was to evaluate the limitation and usefulness of several TMJ imaging techniques, so we compare the findings from clinical examination, plain film radiographs, tomograph, and bone scan from 81 patients with temporomandibular joint disorder. The proportion of patients showing same findings in plain film radiographs, TMJ tomograph, and bone scan was high, however, it is desirable that clinician should conduct phased imaging examinations according to the clinical findings due to the possibilities of false negative findings in diagnosis of osteoarthritis.

Facial asymmetry is the most frequent disease in craniofacial deformities. And the primary causing area of that is mostly placing in mandible. That is to say, it is known that primarily, mandible grows excessively or deficiently, and other facial region involving maxilla undergoes compensatory growth secondarily, so asymmetric face develops. In facial asymmetry, the surgical correction of undergrowth is more difficult than that of overgrowth and the reason of it is the postoperative relapse caused by stress of surrounding soft tissues. It means the stress of surrounding soft tissues occurring after bone lengthening and reducing above stress is the same meaning with reducing postoperative relapse. Among various areas, mandibular ramus is the most difficult area to lengthen vertically and maintain its length. The reason of it is considered by many authors as the stress of surrounding pterygomasseteric sling which is enveloping lower border of mandible and interrupting elongation of ramal height. So we applied two different surgical procedures in which pterygomasseteric slings have different stress respectively to monkeys which have similar masticatory function and anatomy to human being and compared relapse by radiographic film and observed periodically the histochemical change of masseteric muscle fiber. So we could see the following results. The relapse was less in EVRO group in which we separated pterygomasseric sling in inferior border and didn't approximate muscle sling after vertical lengthening to minimize the stress of soft tissues than IVRO group in which we elongated ramal height preserving pterygomassetric sling. Of course, we could see a problem in EVRO group such as bone resorption in inferior border caused by uncovering the periosteum of inferior border. But we expect that such problem will be solved by developing periosteum substitutes for covering the exposed bone and minimizing the surgical trauma. In histochemical study of masseteric muscle fiber, the fiber constituents of EVRO group in which we minimized soft tissue stress was changed immediately after operation and maintained it for 1 year, whereas that of IVRO group in which we preserved soft tissue stress was changed in more portion after operation and recovered it by 1 year. By the histochemical results, we can see that the recovery of fiber constituents reflect the recovery of muscle stress and it is closely related with relapse phenomenon.
The purpose of this paper is to explain the making procedure and the usage of receiver operating characteristic (ROC) curve for interpretation of radiographic images. The conventional radiograms obtained after the creation of the lesions in the acrylic plates and were enhanced in color. The observer were informed of which tooth to examine, the 'a priori' probability of a lesion present and the approximate diameter of the lesions. The two groups of films were interpreted separately by the same observer using the same rating scale. The following rating scale was used: A; definitely no lesion, B; probably no lesion, C; not sure, D; probably a lesion, and E; definitely a lesion. In analysis, for each observer the diagnostic results in terms of true positive (TP) and false positive (FP) decisions were plotted on a graph. The lowest point on the graph represents the TP and FP when only decisions designated as E according to the rating scale are included. The next point shows the TP and FP values when diagnoses designated as D are added and so forth. By connecting such plot points, a receiver operating characteristic (ROC) curves is obtained. The area under the curve represents the diagnostic accuracy resulting from a diagnostic performance at pure chance level and a value of 1.0 at perfect performance. This method has been known as an useful method to detect the minute difference for each radiographic technic, each observer and for the different lesion depths.
The purpose of this study was first to analyze the utilization of dental examination through questionnaire to develop a diagnostic reference level of patient doses for dental radiography in korea. 77 dental institutions were classified into three groups: A group for the dental hospitals of the college of dentistry (11 institutions), B group for dental hospitals (30 institutions) and C group for dental clinics (36 institutions). The results were as follows : The mean numbers of unit chairs and medical staffs were 140.2, 15.3 and 5.8 sets, 112.6, 7.3 and 1.7 dentists, 3.1, 0.5 and no one radiologic technologists, and 19.7, 12.5 and 3.3 dental hygienists in A, B and C groups, respectively. The mean numbers of dental X-ray equipments were 14.64, 3.21 and 2.19 in A, B and C groups, respectively. Intraoral dental X-ray unit was used the most, the following equipments were panoramic, cephalometric, and cone-beam CT units. The most used X-ray imaging system was also digital system (above 50%) in all three groups. Insight dental film (Kodak, USA) having high sensitivity was routinely used for periapical radiography. The automatic processor was not used in many dental institutions, but the film-holding device was used in many dental institutions. The utilization rates of PACS in A, B and C groups were 90.9%, 83.3% and 16.7% respectively, and the PACS software program was used the most PiView STAR (Infinitt, Korea). The annual mean number of radiographic cases in one dental institution in 2008 for A group was 6.8 times and 21.2 times more than those for B and C groups, and periapical and panoramic radiographs were taken mostly. Tube voltage (kVp) and tube current (mA) for periapical radiography were similar in all three groups, but exposure time in C group was 12.0 times and 3.5 times longer than those in B and C groups. The amount of radiation exposure in C group, in which dental hygienists take dental radiographs, was more than those in other groups. The exposure parameters for panoramic radiography were similar in all three groups. In conclusion, the exposure parameters in dental radiography should be determined with reference level, not past experiences. Use of automatic processor and film-holding devices reduces the radiation exposure in film system. The quality assurance of dental equipments are necessary for the reduction of the patient dose and the improvement of X-ray image quality.
We described two cases of odontogenic keratocysts of the maxilla. Odontogenic keratocysts (OKCs) are jaw cysts with a proclivity for local invasion and recurrence. Clinically, OKCs are characterized by aggressive, local growth. The diagnosis of mandibular OKC is based on physical examination and plain film radiographic findings. However, histopathological confirmation is required to make the diagnosis with certainty. The molar regions of the mandible and maxilla are the principal primary locations. The maxillary antrum is also a common site. Initial therapy is typically enucleation with or without extraction of the associated teeth. Long-term follow-up is necessary because of the aggressive nature and recurrence rate of OKCs.

Objectives: Patient dose verification is clinically the most important parts in the treatment delivery of radiation therapy. The three dimensional(3D) reconstruction of dose distribution delivered to target volume helps to verify patient dose and determine the physical characteristics of beams used in intensity modulated radiation therapy(IMRT). We present Beam Intensity Scanner(BInS) system for the pre treatment dosimetric verification of two dimensional photon intensity. The BInS is a radiation detector with a custom made software for relative dose conversion of fluorescence signals from scintillator. Methods: This scintillator is fabricated by phosphor Gadolinium Oxysulphide and is used to produce fluorescence from the irradiation of 6MV photons on a Varian Clinac 21EX. The digitized fluoroscopic signals obtained by digital video camera will be processed by our custom made software to reproduce 3D relative dose distribution. For the intensity modulated beam(IMB), the BInS calculates absorbed dose in absolute beam fluence, which are used for the patient dose distribution. Results: Using BInS, we performed various measurements related to IMRT and found the followings: (1) The 3D dose profiles of the IMBs measured by the BInS demonstrate good agreement with radiographic film, pin type ionization chamber and Monte Carlo simulation. (2) The delivered beam intensity is altered by the mechanical and dosimetric properties of the collimating of dynamic and/or static MLC system. This is mostly due to leaf transmission, leaf penumbra, scattered photons from the round edges of leaves, and geometry of leaf. (3) The delivered dose depends on the operational detail of how to make multileaf opening. Conclusions: These phenomena result in a fluence distribution that can be substantially different from the initial and calculative intensity modulation and therefore, should be taken into account by the treatment planing for accurate dose calculations delivered to the target volume in IMRT.
